The prevalence of misidentification syndromes in neurodegenerative diseases.

Although misidentification syndromes (MISs) have been often described in Alzheimer disease (AD), the prevalence of these phenomena in different neurodegenerative diseases has not been systematically studied. Three hundred ninety-two individuals with probable AD, 119 patients with the behavioral variety of frontotemporal dementia (FTD-bv), 101 patients with primary progressive aphasia, 24 subjects with semantic dementia, 18 subjects with corticobasal degeneration, 8 patients with progressive supranuclear palsy, 36 individuals with probable Lewy body dementia (DLB), and 26 subjects with Parkinso

[A case of probable dementia with Lewy bodies presenting with geographic mislocation and nurturing syndrome].

A 83-year-old right handed man developed nurturing syndrome and geographical mislocation (misidentification of places) as a result of dementia with Lewy bodies. He showed parkinsonism, fluctuating cognition, repeated falls, systematic delusions (delusional jealousy with vivid feeling of witness), rapid eye movement sleep behavior disorder, and mild dementia. His brain MRI showed atrophy of bilateral temporal tips and amygdala. A FDG-PET showed decrease of glucose metabolism in right frontal lobe and left temporo-parietal areas. He showed constructional disability, frontal lobe dysfunction, mil

Delusional misidentification syndromes and dangerousness.

Dangerousness in the delusional misidentification syndromes is studied by reviewing a sample of 82 cases defined by either verbal threats or physical violence caused by misidentification delusion. Eighty cases were obtained from a review of the anglophone psychiatric literature in which the patients exhibited some degree of dangerousness, to which we added 2 previously unreported cases.

Critical Issues in the Management of Agitation, Aggression, and End-of-Life in Delusional Disorder: A Mini-Review.

Background: Compared to other psychotic disorders, there is little information about staging care in delusional disorder (DD). Unlike schizophrenia, this is a disorder that begins in middle age, a time at which chronic medical comorbidities have already begun to impact global functioning. With age, the combination of psychological and somatic conditions leads to new behaviours, e.g., agitation, aggression, and behaviours that require specific preventive and interventive measures. With further age, knowledgeable end-of-life care becomes necessary for this population. Aim: The aim of this articl

Classification of psychotic symptoms in dementia with Lewy bodies.

OBJECTIVE: The aim of this study is to determine empirically a possible classification of psychotic symptoms, and identify the frequency of such symptoms in dementia with Lewy bodies (DLB). METHODS: This was a clinical study of prospectively observed patients from the memory clinic at Shiga Medical Center. The authors conducted a factor analysis of psychotic symptoms in 96 probable DLB and 4 possible DLB patients, clinically diagnosed according to the consensus criteria, were included. RESULTS: Four factors were obtained. Factor 1 was closely akin to misidentifications, including Capgras syn

A Narrative Review of the Many Psychiatric Manifestations of Neurosyphilis: The Great Imitator.

Neurosyphilis is an infection of the central nervous system caused by the spirochete, Treponema pallidum. New syphilis infections have been increasing around the world each year. This disease was much of a concern in the pre-penicillin era, where when left untreated many cases progressed to tertiary syphilis which can commonly manifest as neurosyphilis. Of particular interest, neurosyphilis has been linked to masquerading itself as various psychiatric conditions. This narrative review focuses on exploring psychiatric manifestations of neurosyphilis as well as the importance of screening in psy

[Chronic and therapy refractory Fregoli syndrome].

We present a 46-year old lady suffering from chronic paranoid schizophrenia accompanied by a chronic Fregoli's syndrome. The patient feels persecuted by a male person and identifies this persecutor in different persons of different shape, different age and different sex. The symptoms are very refractory to treatment despite various neuroleptic therapies. Historical, psychodynamic and neuropsychological aspects of this delusional syndrome are discussed.

Sequential Cotard and Capgras delusions.

We report sequential Cotard and Capgras delusions in the same patient, KH, and offer a simple hypothesis to account for this link. The Cotard delusion occurred when KH was depressed and the Capgras delusion arose in the context of persecutory delusions. We suggest that the Cotard and Capgras delusions reflect different interpretations of similar anomalous experiences, and that the persecutory delusions and suspiciousness that are often noted in Capgras cases contribute to the patients' mistaking a change in themselves for a change in others ('they are impostors'), whereas people who are depres

Delusions of substitution and diabetes mellitus.

A male patient with chronic paranoid schizophrenia developed delusions of substitution that were of abrupt onset and short duration. This coincided with the detection of diabetes mellitus. Over the next three years, despite being on continuous moderate dose neuroleptic treatment the re-emergence and resolution of delusions of substitution varied with the control of his diabetic status. A selective review of reports on organic contributors to delusions of substitution is presented. It is suggested that the abruptly appearing, short lasting delusions of substitution may be more likely to have a

Frégoli syndrome after cerebral infarction.

A case of a rare form of delusional misidentification, the Frégoli syndrome, is described. Although usually occurring in the setting of primary or secondary schizophrenic psychoses, delusional misidentification has been reported in affective, neurological, and toxic-metabolic disorders. In this instance a diagnosis of paranoia (delusional disorder) secondary to predominantly right hemisphere pathology, rather than schizophrenia, seemed more appropriate.

Visual-perceptual abnormalities in delusional misidentification.

Delusional misidentification syndromes are often found in conjunction with visual-perceptual abnormalities. These abnormalities appear to be independent of visual hallucinations and are illusory in nature. A case of delusional misidentification with visual-perceptual abnormalities is presented. Cognitive and visual phenomenology suggests that careful recording of visual abnormalities may shed light on the relation between delusional misidentification and abnormal visual cognition.

Psychiatric phenomena in Alzheimer's disease. II: Disorders of perception.

In a sample of 178 patients with AD, visual hallucinations had been experienced by 13% and auditory hallucinations by 10%. Thirty per cent had misidentification syndromes; these were associated with a younger age and younger age at onset of illness, and proportionally more men than women were affected. There was a reduced 30-month mortality rate in this group. Subjects with hallucinations had a greater deterioration in cognitive function at 12-month follow-up, which could not be accounted for by neuroleptic medication.

The accusers in dementia.

The purpose of this retrospective study of 116 dementia patients with and without accusatory behavior was to determine its frequency and evaluate its relationship to individual characteristics, behavioral and psychological symptoms, and certain dementia-related domains, namely, cognitive impairment, stage of disease, language difficulties, and functional disability. Little more than one third (38%) of the patients studied had accusatory behavior. The average age of the patients with accusatory behavior was 74 years, and the male to female ratio was 2.6:1. Accusatory behavior was more prevalent

Cognitive neuropsychiatric analysis of an additional large Capgras delusion case series.

INTRODUCTION: Although important to cognitive neuropsychiatry and theories of delusions, Capgras delusion has largely been reported in single case studies. Bell et al. [2017. Uncovering Capgras delusion using a large scale medical records database. British Journal of Psychiatry Open, 3(4), 179-185] previously deployed computational and clinical case identification on a large-scale medical records database to report a case series of 84 individuals with Capgras delusion. We replicated this approach on a new database from a different mental health service provider while additionally examining ins

The role of the right hemisphere in the Capgras delusion.

Following a general review of the literature linking right hemisphere dysfunction to psychoses in general and Capgras delusion (CD) in particular, two studies are described that confirm the link. Study 1 revealed that, compared with matched psychiatric controls, 3 CD patients were particularly poor at matching pairs of faces shown briefly in the left visual field-which implies dysfunctioning right hemisphere performance. However there was no difference in performance when the faces appeared bilaterally, which fails to support the hypothesis that poor interhemispheric communication may give ris

Neural correlates of psychotic symptoms in dementia with Lewy bodies.

The aim of this study was to investigate the association between psychotic symptoms in dementia with Lewy bodies and brain perfusion on single photon emission tomography. Based on factor analysis in 145 patients, psychotic symptoms were classified into five symptom domains (factor 1 to 4-related symptoms and delusions). The relationship between each symptom domain and brain perfusion was assessed in 100 patients with dementia with Lewy bodies, while accounting for the effects of age, sex, dementia severity, parkinsonism and dysphoria. Factor 1 symptoms (Capgras syndrome, phantom boarder, redup

Beliefs in Right Hemisphere Syndromes: From Denial to Distortion.

Striking belief distortions may accompany various disorders of awareness that are predominantly associated with right hemispheric cerebral dysfunction. Distortions may range on a continuum of pathological severity, from the unawareness of paralysis in anosognosia for hemiplegia, to a more startling disturbance in denial of paralysis where belief may starkly conflict with reality. The patients' beliefs about their limitations typically represent attempts to make sense of limitations or to impart meaning to incongruous facts. These beliefs are often couched in recollections from past memories or
