The Global Macroeconomic Burden of Spinal Cord Injury: A Dynamic Modeling Study from 2020 to 2050.

BACKGROUND: Spinal cord injury (SCI) is a debilitating condition that imposes profound and persistent socioeconomic burdens. Despite its relatively low prevalence, SCI results in lifelong disability, reduced productivity, and sustained healthcare costs, particularly among working-age populations. While clinical aspects of SCI have been extensively studied, its broader macroeconomic burden remains underexplored. METHODS: We developed a dynamic macroeconomic model to estimate the long-term economic burden of SCI across 191 countries and territories from 2020 to 2050. The model integrated epidem

An economic case study of the control of dog-mediated rabies by an animal welfare organisation in Jaipur, India.

A global strategic plan for the elimination of dog-mediated human rabies deaths by 2030 was announced in 2018. The cost-effectiveness of annual mass dog vaccination programmes, as a control and elimination method, has been advocated on many occasions. Complementary methods, such as animal birth control (ABC) activities, have received less attention. This paper provides a case-study of a programme operated by Help in Suffering (HIS) in Jaipur, India from 1994/95 until 2016/17 comprising both ABC and additional vaccination-only activities. The availability of cost data alongside information on d

The effect of different methods of remuneration on the behaviour of primary care dentists.

BACKGROUND: Methods of remuneration have been linked with the professional behaviour of primary care physicians. In dentistry, this can be exacerbated as clinicians operate their practices as businesses and take the full financial risk of the provision of services. The main methods for remunerating primary care dentists include fee-for-service, fixed salary and capitation payments. The aim of this review was to determine the impact that these remuneration mechanisms have upon primary care dentists' behaviour. OBJECTIVES: To evaluate the effects of different methods of remuneration on the leve

All hands on deck during the COVID-19 pandemic. Maintaining face-to-face medical education and clinical placements.

Medical students must have robust educational experiences, graduate and commence timely employment. Here, we describe how the Royal College of Surgeons in Ireland (RCSI) delivered clinical placements in medical programmes over the first three waves of the COVID-19 pandemic in Ireland, including a student-centred, holistic approach to students' educational, social and health needs with strong student involvement, re-organising the university's primary care team, developing COVID-19 PCR testing on site and re-focusing communications and student services. This resulted in re-arranging the student

Improving primary health care facility performance in Ghana: efficiency analysis and fiscal space implications.

BACKGROUND: Health centers in Ghana play an important role in health care delivery especially in deprived communities. They usually serve as the first line of service and meet basic health care needs. Unfortunately, these facilities are faced with inadequate resources. While health policy makers seek to increase resources committed to primary healthcare, it is important to understand the nature of inefficiencies that exist in these facilities. Therefore, the objectives of this study are threefold; (i) estimate efficiency among primary health facilities (health centers), (ii) examine the potent

Cost of provision of opioid substitution therapy provision in Tijuana, Mexico.

BACKGROUND: Mexico recently enacted drug policy reform to decriminalize possession of small amounts of illicit drugs and mandated that police refer identified substance users to drug treatment. However, the economic implications of drug treatment expansion are uncertain. We estimated the costs of opioid substitution therapy (OST) provision in Tijuana, Mexico, where opioid use and HIV are major public health concerns. METHODS: We adopted an economic health care provider perspective and applied an ingredients-based micro-costing approach to quantify the average monthly cost of OST (methadone ma

Women who sell sex in a Ugandan trading town: life histories, survival strategies and risk.

Little is known about the background of commercial sex workers in Africa. This study investigated how women in a trading town on the trans-Africa highway in southwest Uganda become involved in commercial sex work, which factors contribute to their economic success or lack of success, and what effect life trajectories and economic success have on negotiating power and risk behaviour. Over the course of two years detailed life histories of 34 women were collected through recording open, in-depth interviews, the collection of sexual and income and expenditure diaries, visits to the women's native

Development of a digital cloud imaging platform based on a "pay-for-performance" model: an empirical study on cost control and resource conservation.

INTRODUCTION: Traditional physical film-based imaging services impose a heavy financial and operational burden on public hospitals. This study aims to evaluate the actual effectiveness of a digital cloud imaging platform-deployed under a model where "service providers make upfront investments and hospitals pay based on performance"-in terms of controlling medical costs, optimizing clinical workflows, and enhancing the patient experience, thereby providing a replicable management pathway and empirical evidence for the digital transformation of imaging services. METHODS: A single-center, quasi-

Provider costs of professional COVID-19 rapid antigen testing in low-income settings.

World Health Organization recommends antigen rapid diagnostic tests (RDT) as point of care tests for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in suspected outbreaks when polymerase-chain-reaction testing is not accessible; to trace the extent of outbreaks; and in areas with widespread community transmission. Annual economic costs were estimated for professional SARS-CoV-2 testing as part of several COVID-19 testing use cases in Malawi, Nigeria and Zimbabwe. Symptom screening and antigen-based RDT was implemented as part of a multi-country, Unitaid/STAR 3ACP (Africa, Asia, A

The Gradual Establishment and Historical Implications of Hospital Ship Mobile Medical Services along Korea's Western and Southern Coasts in the 1960s-1970s.

The term muui (doctorless) refers to a state of absolute deprivation of medical care. In modern Korean history, chronic shortages and regional disparities of medical resources resulted in the widespread existence of muuichon (doctorless village). In response, the Ministry of Health and Social Affairs implemented a series of policies and initiatives, including the establishment of a public health center system and the mobilization of public doctors. Yet the very term muuichon also suggested that the problem of public health was inseparable from that of space. Above all, there was a distinct spa

Lean management of crisis response and post-pandemic outcomes in US primary care practices.

BACKGROUND: Primary care forms the basis for valued outcomes in health systems, including population health, high quality care, and reduced health expenditures. However, the primary care system in the United States (US) faces many difficulties, including financial pressures and clinician burnout that accelerated following the Covid-19 pandemic. This study examines the ability of primary care practices to adapt to post-pandemic challenges using a lean management system for quality and process improvement. METHODS: We used data from the National Survey of Healthcare Organizations and Systems II

The 4MOTHERS trial of the impact of a mobile money-based intervention on maternal and neonatal health outcomes in Madagascar: study protocol of a cluster-randomized hybrid effectiveness-implementation trial.

BACKGROUND: Mobile money-a service enabling users to receive, store, and send electronic money using mobile phones-has been widely adopted across low- and middle-income economies to pay for a variety of services, including healthcare. However, evidence on its effects on healthcare access and health outcomes are scarce and the possible implications of using mobile money for financing and payment of maternal healthcare services-which generally require large one-time out-of-pocket payments-have not yet been systematically assessed in low-resource settings. The aim of this study is to determine th

The Cost and Intermediate Cost-Effectiveness of a Randomized Control Trial Delivering U=U Messaging to Increase HIV Testing Among South African Men.

BACKGROUND: South African men have lower HIV testing uptake than their female counterparts, leading to lower antiretroviral treatment (ART) uptake and higher HIV-related mortality. A recent cluster-randomized trial in a high HIV prevalence area in Cape Town showed that relaying the "undetectable equals untransmittable" (U=U) message to South African men increased their HIV testing uptake compared to issuing standard testing invitations. We conducted an economic analysis based on this trial to inform decision-making regarding implementing U=U messaging in routine public-sector HIV care. METHOD
