Primary healthcare centers engagement in tuberculosis treatment in Ukraine.

INTRODUCTION: We assessed the influence of a result-based financing (RBF) model, which included incentives for Primary Healthcare facilities on TB treatment outcomes. METHODOLOGY: We compared TB patients > 17 years and their treatment outcomes among those who did and did not benefit from RBF-model in 14 districts of Odeska oblast, Ukraine in 2017. Log-binomial regression was used to examine factors associated with being included in RBF-model. RESULTS: Of 2,269 reported TB patients, 308 (14%) were included in RBF-model. Most patients in the RBF-model were from rural areas 229 (74%), unemploye

Enablers and barriers to implementing a digital logistics system: A qualitative evaluation of OpenLMIS for Nigeria's vaccine supply chain.

Logistics management information systems (LMIS) strengthen health commodity distribution and decision-making. Nigeria introduced OpenLMIS, an electronic LMIS, in August 2021 to track COVID-19 vaccines, and a routine immunisation (RI) module was deployed nationwide in 2023. This evaluation examined enablers and barriers to effective implementation and sustained use. A qualitative design combined desk reviews of LMIS documents, 42 key informants interviews, and 16 structured observational assessments with Cold Chain Officers. Data collection occurred in three states, the Federal Capital Territor

Intersectoral interventions: integration for impact on preterm birth.

PROGRESS: The last two decades have seen a growing focus on intersectoral interventions to improve maternal and newborn health and well-being outcomes, as reflected in efforts to achieve the Millennium Development Goals (MDGs) and advance the Sustainable Development Goals (SDGs). Preterm births are linked to cross-sectoral determinants that affect health outcomes and human capital across the life-course, necessitating an intersectoral approach that addresses these multifaceted challenges. PROGRAMMATIC PRIORITIES: Recognizing that social, biological and economic determinants significantly infl

The health care safety net: money matters but savvy leadership counts.

The nation's health care safety net--heavily reliant on external funding and support--is uniquely vulnerable to shifting and often adverse market and policy conditions. While adequate funding is essential to ensuring safety net providers can care for low-income people, the Center for Studying Health System Change (HSC) has identified a number of other factors key to building and maintaining viable community safety nets. Throughout the four rounds of HSC's Community Tracking Study (CTS) site visits, researchers have found that strong political and organizational leadership, community support, c

The national free delivery and caesarean policy in Senegal: evaluating process and outcomes.

This article presents the results of an evaluation of the free delivery and caesarean policy (FDCP) in Senegal. The policy was introduced into five poor regions in 2005 and in 2006 was extended at regional hospital level to all regions apart from the capital (Dakar). The evaluation was carried out in 2006-7. There were four research components, all focused on selected facilities and districts within the five FDCP regions: a financial analysis of expenditure on the policy and wider health financing in the five regions and nationally; 54 key informant interviews from national down to facility le

Supply chain management of health commodities for reducing global disease burden.

PURPOSE: Reducing global disease burden requires improving access to medicines, thus the need for efficient and effective supply chain management for medicines. The Nigerian government came up with new policies on Mega Drug Distribution Centres and National Drug Distribution Guidelines to improve access to quality medicines with pharmacists having a key role to play. However, pharmacists in Nigeria seem not to be aware and adequately equipped to handle the medicines supply chain. This article aimed at assessing the awareness and readiness of Nigerian pharmacists on supply chain management prac

When algorithms don't care: physiotherapy and the post-professional turn.

Physiotherapy has long resisted the forces that restructured manufacturing, journalism, and finance. This resistance is ending. Drawing on Gilles Deleuze's prescient analysis of societies of control, this paper argues that physiotherapy - along with the orthodox health professions more broadly - is entering a post-professional era characterized by the progressive marginalization of practitioners in the coordination and delivery of care. Three concurrent forces drive this transformation: the atomization of bodies and health into commodifiable fragments under late-stage capitalism; the systemati

Lifecycle model-based evaluation of infant 4CMenB vaccination in the UK.

OBJECTIVES: Invasive meningococcal disease, an uncommon but severe disease, imposes catastrophic health and economic burdens. Cost-utility analysis (CUA) assumes separability in lifetime health and economic variables and cannot capture the full value of preventing such burdens. We overcome these limitations with a retrospective societal perspective cost-benefit analysis (CBA) of meningococcal serogroup B vaccination (4CMenB) of one infant cohort in the United Kingdom using a health-augmented lifecycle model (HALM) incorporating health's interactions with consumption, earnings, non-market time

Multi-level determinants of comprehensive palliative care delivery for breast and cervical cancer patients in Ethiopia: using the socio-ecological model as an analytical framework.

INTRODUCTION: Comprehensive palliative care is essential for improving the quality of life of women with breast and cervical cancer in Ethiopia, yet services remain poorly organized, largely inaccessible, and concentrated in the capital. While existing studies have quantified service gaps, the multi-level determinants enabling or hindering comprehensive care remain under-explored, particularly how these factors differ between urban and rural settings. OBJECTIVE: To explore the multi-level determinants of implementing comprehensive palliative care for women with breast and cervical cancer in E

The assessment of the readiness of five countries to implement child maltreatment prevention programs on a large scale.

This study aimed to systematically assess the readiness of five countries - Brazil, the Former Yugoslav Republic of Macedonia, Malaysia, Saudi Arabia, and South Africa - to implement evidence-based child maltreatment prevention programs on a large scale. To this end, it applied a recently developed method called Readiness Assessment for the Prevention of Child Maltreatment based on two parallel 100-item instruments. The first measures the knowledge, attitudes, and beliefs concerning child maltreatment prevention of key informants; the second, completed by child maltreatment prevention experts

Effects of Interventions to Improve Access to Financial Services for Micro, Small, and Medium-Sized Enterprises in Low- and Middle-Income Countries: An Evidence and Gap Map.

Micro, small, and medium-sized enterprises (MSMEs) account for most firms in most economies, particularly in developing nations, and are key contributors to job creation and global economic development. However, the most significant impediment to MSME development in low- and middle-income countries is a lack of access to both investment and working capital financing. Due to a lack of essential track record, appropriate collateral, and credit history, MSMEs are frequently denied business loans by traditional lending institutions. In addition, MSMEs face institutional, structural, and non-financ

High schistosomiasis-related mortality in Northeast Brazil: trends and spatial patterns.

BACKGROUND: We analyzed the trends and spatial patterns of schistosomiasis-related mortality in Northeast Brazil in 2000-2019. METHODS: A mixed population-based ecological study was conducted, using information on the underlying or associated causes of death. We used Joinpoint regression analysis to calculate the trends. The spatial analysis included rates, spatial moving averages, and standardized mortality rates. The spatial dependence analysis was based on Getis-Ord's G and Gi* indices (Gi star) and local Moran's index to check for autocorrelation. RESULTS: A total of 5,814,268 deaths wer

Nigeria experience on the use of polio assets for the 2017/18 measles vaccination campaign follow-up.

BACKGROUND: The global polio eradication initiative has made giant stride by achieving a 99% reduction in Wild poliovirus (WPV) cases, with Nigeria on the verge of being declared polio-free following over 36 months without a WPV. The initiative has provided multiple resources, assets and lessons learnt that could be transitioned to other public health challenges, including improving the quality and vaccination coverage of measles campaigns in order to reduce the incidences of measles in Nigeria. We documented the polio legacy and assets used to support the national measles campaign in 2017/201

Policy frameworks for promoting public hospital development in major economies: implications for China.

BACKGROUND: As the core providers of medical and health services, public hospitals play an irreplaceable role in ensuring basic medical care, safeguarding health equity, and fulfilling social responsibilities. Their operational efficiency and institutional design not only influence the national health status but also relate to the stability and development of the entire healthcare system. Therefore, continuously optimizing support policies and management mechanisms for public hospitals has become a crucial issue for governments worldwide in advancing healthcare system reforms. METHODS: This p

Impact mechanisms of high-speed railway accessibility on urban expansion in China: An analysis based on the mediating effect model.

High-speed railway (HSR) has emerged as a transformative force in China, markedly reshaping urban development and spatial structures. Here, we examine the indirect mechanisms through which enhanced HSR connectivity influences urban land expansion. We construct national HSR networks for 2011, 2014, and 2017, and assess city-level accessibility based on network structure, direct connectivity, and service frequency. Urban expansion is measured using the average annual intensity of urban land growth during the periods 2011-2014, 2014-2017, and 2017-2020,taking data from 278 cities in China. Using

Developing a Peer-to-Peer mHealth Application to Connect Hispanic Cancer Patients.

Cancer and its treatment can significantly impact health-related quality of life (HRQOL) (J Pain Symptom Manag 9 (3):186-192, 1994; Soc Sci Med 46:1569-1584, 1998), particularly for Hispanics (Healthcare Financ Rev 29 (4):23-40, 2008; Psycho-Oncology 21 (2):115-124, 2012). Moreover, providers of cancer support for this population may encounter unique challenges. Grounded in social capital theory, this study identified Spanish-speaking, Hispanic breast cancer survivor support needs and preferences for a mHealth intervention. Order Without Intellectual Property Law: Open Science in Influenza.

Today, intellectual property (IP) scholars accept that IP as an approach to information production has serious limits. But what lies beyond IP? A new literature on "intellectual production without IP" (or "IP without IP") has emerged to explore this question, but its examples and explanations have yet to convince skeptics. This Article reorients this new literature via a study of a hard case: a global influenza virus-sharing network that has for decades produced critically important information goods, at significant expense, and in a loose-knit group--all without recourse to IP. I analyze the

Empowering sex workers in India to reduce vulnerability to HIV and sexually transmitted diseases.

The Sonagachi Project was initiated in Kolkata, India in 1992 as a STD/HIV intervention for sex workers. The project evolved to adopt strategies common to women's empowerment programs globally (i.e., community mobilization, rights-based framing, advocacy, micro-finance) to address common factors that support effective, evidence-based HIV/STD prevention. The Sonagachi model is now a broadly diffused evidence-based empowerment program. We previously demonstrated significant condom use increases among female sex workers in a 16 month replication trial of the Sonagachi empowerment intervention (n=
