In vivo kinematics of the scaphoid, lunate, capitate, and third metacarpal in extreme wrist flexion and extension.

PURPOSE: Insights into the complexity of active in vivo carpal motion have recently been gained using 3-dimensional imaging; however, kinematics during extremes of motion has not been elucidated. The purpose of this study was to determine motion of the carpus during extremes of wrist flexion and extension. METHODS: We obtained computed tomography scans of 12 healthy wrists in neutral grip, extreme loaded flexion, and extreme loaded extension. We obtained 3-dimensional bone surfaces and 6-degree-of-freedom kinematics for the radius and carpals. The flexion and extension rotation from neutral g

[Occult fractures of the wrist joint: high resolution image magnification roentgen versus MRI].

PURPOSE: To compare the diagnostic value of high definition macroradiography and MRI in patients with the suspicion of occult wrist fractures. MATERIALS AND METHODS: In a prospective study, 23 patients with clinically suspected wrist fractures and normal initial plain radiographs underwent high resolution macroradiography and MR imaging shortly after trauma. Macroradiographs were taken with a microfocus tube using an anode of 0.03-0.3 mm providing a 4x magnification of the wrist, which was obtained in 4 projections. MR images were performed on a 1.0 T MR unit in coronal planes using T1 weight

Kienbock's disease: diagnosis and treatment.

Kienbock's disease, or osteonecrosis of the lunate, can lead to chronic, debilitating wrist pain. Etiologic factors include vascular and skeletal variations combined with trauma or repetitive loading. In stage I Kienbock's disease, plain radiographs appear normal, and bone scintigraphy or magnetic resonance imaging is required for diagnosis. Initial treatment is nonoperative. In stage II, sclerosis of the lunate, compression fracture, and/or early collapse of the radial border of the lunate may appear. In stage IIIA, there is more severe lunate collapse. Because the remainder of the carpus is

Aneurysmal bone cyst involving the lunate.

Aneurysmal bone cysts are rare lesions which even more rarely involve the bones of the hand. We report a case of a cyst involving the lunate which was initially mistaken for interosseous ganglion. Due to delay in diagnosis, marked destruction of the lunate occurred, necessitating excision and scapho-capitate arthrodesis. Although rare, aneurysmal bone cysts should be included in the differential diagnosis and ruled out prior to embarking on a course of treatment.

Management of Neglected Developmental Dysplasia of the Left Hip in a 14-Year-Old Child with Staged Osteotomies Planned with 3D-Printed Model - A Case Report.

INTRODUCTION: Giant cell tumors of tendon sheath (GCTTS) are benign soft-tissue lesions commonly affecting the digits, which occasionally cause pressure atrophy of an adjoining bone; but perforating the cortex to expand into the medullary canal is quite uncommon. We report such a case of suspected recurrent ganglion cyst with eventually manifested as a GCTTS with an intra-osseous involvement of the capitate and hamate bone. CASE PRESENTATION: A 28-year-old lady had been diagnosed as a case of recurrent ganglion cyst of the dorsum of the left wrist - 6 years ago and 4 years ago - both of which

Avascular necrosis of the hamate treated with capitohamate and lunatohamate intercarpal fusion.

This article describes a case of a 58-year-old man with no significant underlying disease who had chronic pain due to osteonecrosis of the hamate. Following physical examination, imaging, and laboratory findings, he underwent surgical exploration via a limited dorsal approach. The hamate bone did not have a normal anatomical appearance or structure. It was marble-like in appearance, soft, friable, and lacking physiologic contour and bone strength. We removed the dorsal aspect of the hamate piecemeal. The articular surfaces of the adjacent carpal bones (capitate and lunate) were excised, and th

Centralization of ulna with wrist fusion for failure of reconstructed distal radius by allograft bone or prosthesis: A case report and literature review.

RATIONALE: Centralization of the ulna is commonly used in the treatment of radius developmental deformity in children. The secondary distal radius deficiency in adults is different from the developmental deformity of the radius in children. There is no report on the ulna centralization with wrist fusion for the failure of the reconstructed distal radius by allograft bone or prosthesis for osteosarcoma in adults. PATIENT CONCERNS: 2 patients with a bone tumor on the distal radius underwent tumor resection and distal radius reconstruction by allograft bone or prosthesis and suffered distal radi

Prospective investigation of the relationship between dorsal tilt, carpal malalignment, and capitate shift in distal radial fractures.

AIMS: Carpal malalignment after a distal radial fracture occurs due to loss of volar tilt. Several studies have shown that this has an adverse influence on function. We aimed to investigate the magnitude of dorsal tilt that leads to carpal malalignment, whether reduction of dorsal tilt will correct carpal malalignment, and which measure of carpal malalignment is the most useful. METHODS: Radiographs of patients with a distal radial fracture were prospectively collected and reviewed. Measurements of carpal malalignment were recorded on the initial radiograph, the radiograph following reduction

Multicentric giant cell reparative granuloma of the small bones of the hand. A case report and review of the literature.

Giant cell reparative granuloma (GCRG) usually occurs in the jaw, but a histologically identical lesion is occasionally seen in the small bones of the hands and feet. A 24-year-old man had GCRG involving the second metacarpal and the capitate. The involvement of a carpal bone and the presence of multiple lesions are unusual for this entity.

The Etiology and Pathogenesis of Kienböck Disease.

Kienböck disease is a condition that typically occurs in the "at-risk" patient, in the "at-risk" aspect of the proximal condyle of the "at-risk" lunate. In the active male, repetitive loading causes the stress fracture that commences in the single layer proximal subchondral bone plate. The lunate fracture commences at the point the lunate cantilevers over the edge of the distal radius, and then takes on the shape of the radius. We postulate that the stress fracture violates the parallel veins of the venous subarticular plexus-leading to localized venous hypertension and subsequent ischemia and

Spatial measurement for safe placement of screws within the scaphoid using three-dimensional analysis.

The placement of a headless screw within the scaphoid is important for successful fixation of fractured scaphoids. We ascertained the safe position of a screw using virtual collision experiments on a three-dimensionally reconstructed scaphoid bone and screw model, and applied measured angles to simple radiographs. We used images from computed tomograms of the wrists taken from 13 men to reconstruct a three-dimensional cortical image of the scaphoid. Inside the bone, we inserted a virtually reconstructed Herbert screw model and calculated the safe angle along the long axis of the scaphoid at wh

Unusual combined scaphoid and lunate fracture of the wrist: a case report.

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. The most frequent is the dorsal trans-scaphoid perilunate dislocation. In rare cases, however, these dislocations also have been associated with capitate fractures, triquetral fractures, or lunate fracture. We report a combined scaphoid and lunate fracture of the wrist that was not associated with perilunate dislocation.

Perilunate and Lunate Dislocations.

Lunate and perilunate dislocations, although somewhat uncommon, are serious injuries and can be easily missed. The mechanism of injury is hyperextension of the wrist, often associated with a fall on the outstretched hand (FOOSH) injury, falls from height, or motor vehicle crash with a high-energy traumatic injury to the wrist. Perilunate dislocations typically present with pain and swelling over both the dorsal and volar aspects of the wrist and limited range of motion of the wrist. Perilunate dislocations result in disruption of the relationship between the lunate and the capitate, whereas lu

[Arthrodesis of the central support of the carpus. Indications, technic, results].

We have used this operation for the management of pain due to osteoarthritic changes at the radio-scaphoid and capitate-lunate joints. It allows correction of the carpal height by reposition of the flexed lunate, preserves the load bearing capitate-lunate axis, and decompresses the radio-scaphoid joint. We have carried out the procedure in 25 cases. Perilunate instability with osteoarthritic changes were present in 14 wrists; the remainder were consequent to non-union of fractures of the scaphoid with osteoarthrosis, or following complex fracture dislocations. At review 10 patients were free o

Cortical bone defects at metacarpophalangeal and wrist joints in healthy individuals assessed by high-resolution peripheral quantitative computed tomography.

OBJECTIVE: We examined erosions occurring in the metacarpophalangeal (MCP) and wrist joints of healthy subjects using high-resolution peripheral quantitative computed tomography (HR-pQCT). METHODS: Twenty healthy women were included. The second and third MCP and wrist joints were scanned using HR-pQCT. The number, volume, and anatomical distribution of erosions were measured, and the correlation between erosion number and age was analysed. RESULTS: An average of 4.0 erosions were found in the MCP joints, 30.8 in the carpal bones, and 7.9 in the distal radius and ulna. Most erosions (69.3%) w

[Capitate osteotomy in Kienböck's disease in twelve cases. Clinical and radiological results at five years follow-up].

Kienböck's disease could occur pain and reduce wrist's range of motion despite of an early radiologic staging. Usual surgical procedures unload the lunate. Radial shortening is the common procedure in negative ulnar variance. For wrists with neutral or positive ulnar variance, this procedure could produce a distal radio-ulnar discrepancy and an ulnocarpal impingement. We perform, in these cases, a capitate shortening. The goal of this study is to relate the wrist functional outcome and the radiological result in 12 cases. It was a consecutive retrospective study of 12 patients (nine male, thre

Multifocal giant cell tumor of the carpus: Unusual presentation. Case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Giant cell tumors (GCTs) of bone in the carpus are rare. Carpal GCTs are usually solitary lesions; multifocal involvement is exceptional. These lesions have a higher risk of local recurrence after intralesional curettage than those in other body areas. CASE PRESENTATION: We present a case of a 28-year-old male with a six-month history of a palpable mass in the dorsal aspect of the left wrist. Physical examination revealed a 2 cm, mildly tender mass. Magnetic resonance revealed a large intermediate signal lesion involving completely hamate bone and the distal porti
