A decade of catheter ablation of cardiac arrhythmias in Sweden: ablation practices and outcomes.

AIMS: Catheter ablation is considered the treatment of choice for many tachyarrhythmias, but convincing 'real-world' data on efficacy and safety are lacking. Using Swedish national registry data, the ablation spectrum, procedural characteristics, as well as ablation efficacy and reported adverse events are reported. METHODS AND RESULTS: Consecutive patients (≥18 years of age) undergoing catheter ablation in Sweden between 01 January 2006 and 31 December 2015 were included in the study. Follow-up (repeat ablation and vital status) was collected through 31 December 2016. A total of 26 642 patie

Catheter ablation for ventricular tachycardia in patients with cardiac sarcoidosis: a systematic review.

AIMS: Cardiac sarcoidosis (CS) is associated with a poor prognosis. Important features of CS include heart failure, conduction abnormalities, and ventricular arrhythmias. Ventricular tachycardia (VT) is often refractory to antiarrhythmic drugs (AAD) and immunosuppression. Catheter ablation has emerged as a treatment option for recurrent VT. However, data on the efficacy and outcomes of VT ablation in this context are sparse. METHODS AND RESULTS: A systematic search was performed on PubMed, EMBASE, and Cochrane database (from inception to September 2016) with included studies providing a minim

Cardiac tamponade: a rare but preventable complication of central venous catheter in neonates.

Pericardial effusion with cardiac tamponade is a rare and life-threatening complication of peripherally inserted central catheter (PICC) in a neonate. We report a 33-week preterm neonate who had sudden clinical deterioration at day seven of total parenteral nutrition regime via PICC. Recognition of pericardial effusion with cardiac tamponade in neonates with a PICC requires a high index of suspicion and steps in prevention include proper catheter tip placement and continuous monitoring of line position and function.

Permanent catheter drainage system for palliation of diuretic-resistant cardiac ascites.

We report the case of a 69-year-old man with dilated cardiomyopathy treated with a permanent catheter drainage system for diuretic resistant cardiac ascites. At 1 year follow-up, the patient had no heart failure related hospitalisations, displayed improved quality of life measures and had incurred no complications related to the catheter. Permanent tunnelled catheters are widely used to treat malignant ascites but may also be considered for palliation of cardiac ascites.

[Catheter-induced Pulmonary Artery Injury and Pseudoaneurysm after Cardiac Surgery].

The pulmonary artery catheter( PAC) has been used for hemodynamic monitoring in patients undergoing cardiac surgery. Pulmonary artery injury and pseudoaneurysm formation are the most serious and fatal complications of PAC. A 73-year-old female with aortic stenosis and angina underwent aortic valve replacement and coronary artery bypass grafting. PAC was inserted without difficulty before cardiac surgery in the operating room. The operation proceeded uneventfully. On the 1st postoperative day, massive hemoptysis suddenly occurred in the intensive care unit. The patient was treated with mechanic

Conceptus radiation dose and risk from cardiac catheter ablation procedures.

BACKGROUND: The aim of the current study was to estimate the conceptus radiation dose and risk associated with fluoroscopic imaging during a catheter ablation procedure for supraventricular tachycardia performed on the expectant mother. METHODS AND RESULTS: Exposure parameters and fluoroscopy times for each projection of the cardiac ablation procedure performed in 20 female patients of childbearing age were recorded. Radiation doses for a potential conceptus were estimated by using dose data obtained in anthropomorphic phantoms simulating pregnancy at the first, second, and third trimesters.

Cardiac catheter guided surgical closure of an apical ventricular septal defect.

Surgical closure of muscular ventricular septal defects can be difficult. We report a technique in which a preoperatively placed cardiac catheter allowed accurate localization of a residual apical muscular defect at operation. The approach not only allowed improved visualization of the defect from the right side of the heart, but also helped to minimize the size of the left ventriculotomy required for satisfactory closure.

Delayed time response of the continuous cardiac output pulmonary artery catheter.

Previous studies of the accuracy of pulmonary artery catheters (PAC) which provide continuous cardiac output (CCO) monitoring have investigated the performance during steady-state conditions. We compared the response time to hemodynamic change using a CCO PAC and an ultrasonic flow probe (UFP). In five sheep, a CCO PAC was inserted, and an UFP for measurement of CCO was placed around the pulmonary artery via a left thoracotomy. Six interventions which rapidly alter cardiac output were studied: crystalloid bolus, balloon inflation in the inferior vena cava (IVC), IVC balloon deflation, dobutami

Intracardiac echocardiography during radiofrequency catheter ablation of cardiac arrhythmias in humans.

OBJECTIVES: The purpose of this study was to describe our preliminary experience using catheter-based intracardiac echocardiography as an adjunct to biplane fluoroscopy for guiding radiofrequency catheter ablation of atrial arrhythmias in the right side of the heart. BACKGROUND: Catheter ablation requires precise positioning and stable ablation electrode-endocardial contact. This procedure is currently guided by an analysis of intracardiac electrograms and fluoroscopy. However, the use of fluoroscopy does not allow the endocardium and certain anatomic landmarks to be identified and is associa

Perioperative cardiac pacing using an atrioventricular pacing pulmonary artery catheter.

An atrioventricular pacing thermodilution pulmonary artery catheter was evaluated in 40 patients undergoing cardiac surgery. The catheter was inserted in all study patients in a timely fashion without difficulty and functioned well during the perioperative period. Before the start of cardiopulmonary bypass, atrial capture was achieved in 98% of the patients (threshold mean 4.9 mA), ventricular capture in 100% (threshold mean 3.0 mA) and atrioventricular sequential (AVS) pacing in 98%. After cardiopulmonary bypass, atrial capture was achieved in 95% of the patients (threshold mean 5.2 mA), vent

Cardiac tamponade caused by caval catheter--a radiological demonstration of an unusual complication.

An adult male developed shock some hours after the start of intravenous alimentation. The caval catheter, which had been inserted through the right subclavian vein, was later radiographically demonstrated to be in the pericardial sac. We reviewed the literature on this and other complications of caval catheters. From this information and our own experience we offer some suggestions for prevention and management of some of the complications of caval catheters.

Automatic image-based tracking of gadolinium-filled balloon wedge catheters for MRI-guided cardiac catheterization using deep learning.

INTRODUCTION: Magnetic Resonance Imaging (MRI) is a promising alternative to standard x-ray fluoroscopy for the guidance of cardiac catheterization procedures as it enables soft tissue visualization, avoids ionizing radiation and provides improved hemodynamic data. MRI-guided cardiac catheterization procedures currently require frequent manual tracking of the imaging plane during navigation to follow the tip of a gadolinium-filled balloon wedge catheter, which unnecessarily prolongs and complicates the procedures. Therefore, real-time automatic image-based detection of the catheter balloon has

Radio-Telemetric Assessment of Cardiac Variables and Locomotion With Experimentally Induced Hypermagnesemia in Horses Using Chronically Implanted Catheters.

The objective of this study was to characterize the pharmacokinetics and pharmacodynamics of intravenous administration of magnesium sulfate to horses using a novel radio-telemetry system for physiologic signal capture. Five Horses were surgically implanted with a radio-telemetric carotid catheter. Implants were paired with a non-invasive telemetric unit which acquired a six lead ECG and 3-axis acceleration to assess activity acquired wirelessly in real-time for future analysis. Horses were exposed to a new stall environment before (baseline) and after 60 mg/kg (30 mL) of magnesium sulfate (Mg

A comparison of two pulmonary artery mixed venous oxygen saturation catheters during the changing conditions of cardiac surgery.

Continuous mixed venous oxygen saturation (SvO2), using modified pulmonary artery (PA) catheters, can assist in the management of cardiac surgery patients. Two FDA-approved catheters are available for SvO2 monitoring. One system uses two wavelengths of light and the other is a three-wavelength system. The present study was designed to determine the accuracy of the two catheters during cardiac surgery. Sixty-five consecutive patients were assigned to one of the two catheter groups. Data were collected pre- and post-induction, after sternotomy, and after extracorporeal circulation. An updated he

Intra-cardiac motion detection catheter for the early identification of acute pericardial tamponade during invasive cardiac procedures.

OBJECTIVES: To develop and test an intra-cardiac catheter fitted with accelerometers to detect acute pericardial effusion prior to the onset of hemodynamic compromise. BACKGROUND: Early detection of an evolving pericardial effusion is critical in ensuring timely treatment. We hypothesized that the reduction in movement of the lateral heart border present in developing pericardial effusions could be quantified by positioning an accelerometer in a lateral cardiac structure. METHODS: A "motion detection" catheter was created by implanting a 3-axis accelerometer at the distal tip of a cardiac ca
