A 76-year-old man was admitted to our hospital for a thorough examination of a suspected cardiac tumor on transthoracic echocardiography. Transesophageal echocardiography demonstrated a 9.4×8.1 mm mobile stalk-like mass in the left ventricular outflow tract. A preoperative electrocardiogram revealed paroxysmal atrial fibrillation. Tumor resection and pulmonary vein isolation were performed to prevent embolism and confirm the diagnosis. The tumor was resected using an endoscope because it was difficult to evaluate the tumor under direct view from the aortic valve. Pathological diagnosis was car

Akinetic mutism is a rare but important clinical syndrome characterised by a profound decrease in goal-directed behaviour and speech output, yet with preservation of consciousness. This report describes the case of a 58-year-old male with a background of hypertension, alcohol use disorder, smoking, and MTHFR C677T homozygosity who experienced two ischaemic strokes within weeks of each other. The initial infarct involved the right thalamus, posterior putamen, external capsule, and subcortical anterior frontal lobe. Approximately five weeks later, he developed a second stroke in the left hemisph

Introduction: Cardiac papillary fibroelastomas (PFEs) are the most common primary benign cardiac tumors, although they are somewhat unusual in children and typically seen on the left-sided cardiac valves. Case summary: A 10-week-old patient was found to have a partial atrioventricular canal defect, with associated tricuspid and mitral regurgitation. He was medically managed until 1 year of age, when surgical correction was done. During the procedure, a PFE was found incidentally on the TV. Conclusion: This is one of the youngest patients to be reported with PFE, thus adding to the literature o

Cardiac papillary fibroelastoma are rare, benign cardiac tumors that may lead to lethal complications from embolization or valvular dysfunction if left untreated. When working up symptomatic tumors with concomitant angina, traditional diagnostic studies such as cardiac catheterization may predispose the patient to embolic complications if the mass is located in the path of the catheter. Newer, non-invasive diagnostic testing, such as cardiac magnetic resonance imaging or dynamic computed tomography angiography, may be considered in lieu of invasive approaches to avoid potentially devastating c

Cardiac papillary fibroelastomas (CPF) are benign cardiac tumors and usually discovered incidentally during echocardiography. This report describes the case of a 68-year-old man, referred to cardiology for multiple masses of the left ventricle and left atrium. The transthoracic echocardiography revealed multiple oscillating masses in the left ventricle and aortic valve, non-mobile mass in the left atrium with severe mitral stenosis and moderate aortic regurgitation. The patient underwent surgical resection of the masses with valve replacements. Histopathologic examination confirmed the diagnos

This report presents a patient with flaccid paraplegia, most probably secondary to embolization from a left atrial papillary fibroelastoma. The unique location of the tumor, attached to the ridge in the left atrium between the left atrial appendage and pulmonary vein, was well documented on transesophageal echocardiography, and the diagnosis was confirmed by histopathology. Although benign, these tumors may develop in all four cardiac chambers and result in pulmonary or systemic embolization. Paraplegia is a rare embolic complication.

Although hematological malignancies are a known complication of Down syndrome, few reports have described cases involving solid tumors. We describe the case of a 3-year-old Down syndrome girl with a primary solid cardiac tumor. Outpatient echocardiography after intracardiac repair of a ventricular septal defect at 6 months of age revealed a highly mobile pedunculated mass (8 × 9 mm) on the free wall of the right atrium. Due to potential incarceration of the mass in the tricuspid orifice, it was excised under extracorporeal circulation and cardiac arrest. Macroscopically, the tumor closely rese

A 75-year-old man was admitted for cerebral infarction. Magnetic resonance imaging revealed parietal lobe cerebral infarction. Transesophageal echo and contrast-enhanced computed tomography indicated mobile and speckled mass arising from left atrium. He was diagnosed with cardiogenic cerebral embolism. Under cardiopulmonary bypass, resection of the mass including endocardium tissue was per formed. The resected specimen showed multiple small fronds resembling a sea anemone. Microscopic examination showed multiple branching fronds of paucicellular and avascular fibroelastic tissue lined by a sin

BACKGROUND: To assess the prognosis and to develop management strategies for primary cardiac tumors all patients were included in an ongoing study.
METHOD: From Oct. 1994 until December 2003 we prospectively evaluated all patients with cardiac tumors. Follow up examinations were performed every 12 months.
RESULTS: There were a total of 77 primary cardiac tumors. Seventy-three were benign: myxoma (n=59), papillary fibroelastoma (n=11), lipoma (n=2), fibroma (n=1) and four malignant sarcoma (n=4). The myxoma group consisted of 19 males and 40 females aged 12 to 88 years. Myxomas were located i

Papillary fibroelastomas (PFEs) are rare benign cardiac tumors typically arising from the valvular endocardium, often affecting the aortic and mitral valves. They can range from asymptomatic to causing severe thromboembolic complications like stroke. This article presents a case of a tricuspid valve PFE in an 81-year-old patient with severe multi-vessel coronary artery disease. Transthoracic echocardiography revealed a mass on the tricuspid valve, confirmed by cardiac MRI. The patient underwent surgical excision, and histopathology confirmed the PFE diagnosis. The case highlights the importanc

Papillary fibroelastomas (PFEs) are primary cardiac tumors with a benign and avascular nature. Majority of the PFEs are originated from the valvular endocardium, while the most common site is aortic valve. In this case, we present a patient with multiple PFEs originating from the right ventricular outflow tract who was admitted to our clinic with exertional dyspnea. As far as we know, this is the first case of this unusual presentation of multiple PFEs and also had a history of breast cancer and permanent pacemaker reported in the literature.

A 25-year-old woman presented with acute onset of right hemiplegia and global aphasia with National Institutes of Health Stroke Scale score of 19. Computed tomography angiography demonstrated occlusion of the left proximal middle cerebral artery. She was thrombolysed with intravenous recombinant tissue plasminogen activator (0.6 mg/kg) within 3 hours of onset of symptoms and was taken up for mechanical thrombectomy. The retrieved specimen appeared pale white and soft; histopathologic examination revealed a papillary neoplasm composed of papillae with hyalinized cores lined by endothelium, cons

An 80-year-old woman presented with dyspnoea. Laboratory test results and ECG were normal but echocardiography revealed a right atrial round mobile mass measuring about 20 mm. CT scan and cardiac MRI showed a mass appending on the right atrial free wall above the tricuspid annulus, enhanced by contrast, without any thoracic or abdominal extension, and confirmed the diagnosis of a tumour. After surgical treatment, the pathological examination revealed a cardiac papillary fibroelastoma. This small primary cardiac benign neoplasm usually occurs on cardiac valves, but can also appear, as described

Papillary fibroelastomas (PFEs) are rare, benign primary cardiac tumors. Their clinical manifestations range from asymptomatic findings to serious embolic events, including cerebrovascular accidents, myocardial infarctions, or arrhythmias. Though extremely rare, PFEs have been associated with acute coronary syndrome in only a few cases documented in medical literature. We present the case of a 39-year-old male who presented with acute-onset severe chest pain and was diagnosed with inferior myocardial infarction. Coronary angiography revealed a high thrombus burden in the right coronary artery.

BACKGROUND: Papillary fibroelastoma (PFE) rarely arises in the left atrial appendage (LAA) and is often misidentified as intracardiac thrombus.
CASE SUMMARY: A 54-year-old female presented with 2 years of intermittent chest discomfort. She had no chronic illnesses and normal physical examinations. Transthoracic echocardiography, transesophageal echocardiography, and cardiac magnetic resonance identified a mass in the LAA. She underwent surgical resection of the mass together with partial LAA tissue. Intraoperative observation revealed a yellowish lesion at the LAA orifice. Pathologic examinat

Mesothelial/monocytic incidental cardiac excrescences (MICE) are rare, reactive tumor-like lesions of the heart. They are typically incidental findings during cardiac surgery. They can mimic thrombi or neoplasms such as myxoma, papillary fibroelastoma, and metastatic carcinoma histopathologically, leading to diagnostic confusion. We report a case of atrial MICE in a 40-year-old woman with rheumatic mitral stenosis and long-standing atrial fibrillation, who underwent mitral valve replacement following a failed percutaneous transvenous mitral commissurotomy. Histopathological examination reveale

Papillary fibroelastoma (PFE) is a benign tumor that arises mostly from left-sided valves. PFE can cause stroke, and surgical resection may be needed. Lambl's excrescence (LE) is a filiform valvular lesion and is considered a possible cause of stroke. A 79-year-old man with light-headedness and left-sided hemiparesis was diagnosed with stroke. Transesophageal echocardiography (TEE) revealed a round-shaped mobile mass in the left ventricular outflow tract (LVOT), which was considered the cause of the stroke. Surgical resection was performed transaortically, and during surgery, a mass was incide

BACKGROUND: Primary cardiac tumors are rare. Myxomas are the most prevalent benign primary cardiac tumors, followed by papillary fibroelastomas. These tumors can cause local cardiac complications, but also thromboembolic events. This case report describes an aortic valve tumor, causing embolic complications at two rare locations.
CASE DESCRIPTION: A 43-year-old female patient came to the emergency department with abdominal pain and shortly after chest pain with associated ECG abnormalities. CT angiography and transesophageal echocardiography revealed a tumor located at the aortic valve. The t

BACKGROUND: Papillary fibroelastoma (PFE) is a rare primary cardiac neoplasm that is usually discovered incidentally at autopsy or during cardiac surgery. PFE combined with rheumatic heart disease (RHD) is extremely rare, and only a few cases have been reported. Additionally, the growth rate of the tumor is unknown.
CASE PRESENTATION: Here, we present a very rare case of PFE of the aortic valve combined with RHD, which were identified in a female patient who survived for 5 years without surgical intervention, and who subsequently underwent successful surgical treatment.
CONCLUSIONS: PFEs may

Cardiac tumors, although rare, carry high morbidity and mortality rates. They are commonly first identified either at echocardiography or incidentally at thoracoabdominal CT performed for noncardiac indications. Multimodality imaging often helps to determine the cause of these masses. Cardiac tumors comprise a distinct category in the World Health Organization (WHO) classification of tumors. The updated 2021 WHO classification of tumors of the heart incorporates new entities and reclassifies others. In the new classification system, papillary fibroelastoma is recognized as the most common prim
