Validation and application of the Insulin Treatment Appraisal Scale in Cuban patients with type 2 diabetes mellitus.

INTRODUCTION: The purpose of this study was to validate the Insulin Treatment Appraisal Scale (ITAS) in the Cuban population with type 2 diabetes mellitus. MATERIAL AND METHODS: A cross-sectional, multicentre analytical study was performed in Cuba from February 2020 to April 2021; 199 patients were surveyed in a hospital institution and in primary healthcare. We used the Insulin Treatment Appraisal Scale, consisting of 20 items, with a minimum score of 20 points and a maximum of 100, where the higher the score, the worse the perception of insulin therapy. The validity of the instrument was de

Getting Histoplasmosis on the Map of International Recommendations for Patients with Advanced HIV Disease.

Progressive disseminated histoplasmosis, caused by H. capsulatum, is a life-threatening illness and is an AIDS-defining opportunistic infection. It is neglected, worryingly under-diagnosed, and often misdiagnosed as cancer or tuberculosis with fatal consequences. Globally, over 100,000 cases of disseminated histoplasmosis have been estimated. In 2017, the World Health Organization (WHO) noted that disseminated histoplasmosis is a significant cause of mortality in AIDS patients. Through the rigorous efforts of the Global Action Fund for Fungal Infections (GAFFI) and partners, in 2019, the Histo

Molecular detection of pathogens in ticks associated with domestic animals from the Colombian Caribbean region.

Tick-borne diseases constitute a problem for livestock and public health. Given the socio-economic and environmental conditions of the Colombian Caribbean, ticks are particularly abundant, in turn exposing domestic animals and people in contact with them to such diseases. This study evaluates the presence of Babesia spp., Anaplasma spp., Coxiella spp. and Borrelia spp. in domestic animal ticks (Amblyomma mixtum, A. dissimile, Dermacentor nitens, Rhipicephalus sanguineus and R. microplus) by conventional PCR. Findings show a prevalence of 12.5% of Babesia, 0% of Borrelia, 39.4% of Anaplasma and

Absence of sympathetic overactivity in Afro-Caribbean hypertensive subjects studied by heart rate variability.

Black hypertensives present a greater prevalence of left ventricular hypertrophy and an increased mortality compared to white hypertensives. Differences in sympathetic activity might contribute to explain these racial differences in hypertension. Nevertheless, previous laboratory studies did not show any increase of sympathetic activity direct to the heart in black subjects. The aim of the present study was to investigate the cardiac sympatho-vagal balance in black and white hypertensives analysing heart rate variability, during the entire 24 h. We analysed Holter recordings of 52 essential hy

How does the prevalence of alcohol use and associations with psychological distress differ across ethnic groups in England: A cross-sectional analysis of eight national surveys.

PURPOSE: Research on ethnic differences in alcohol use has relied on broad groupings, potentially obscuring patterns across specific ethnic groups. Single data sources often include few participants from minoritised ethnic groups, a meta-analytic approach across data sources can overcome this limitation. This study examined, by ethnicity, the prevalence of i) alcohol use and binge-drinking, and associations of ii) alcohol use and binge-drinking with psychological distress. METHODS: A secondary analysis of eight representative surveys in England (N=112,791) was conducted. Alcohol was assessed

Unjust treatment, unfamiliar body, and unrealistic body standards: A qualitative investigation of body image, racism, and eating during pregnancy and postpartum among Black, African American, and Afro-Caribbean individuals.

The perinatal period, which includes pregnancy and postpartum, is associated with body-image disturbance and increased risk of eating disorder (ED) onset or relapse. ED diagnoses that are most common in the perinatal period are also more prevalent among Black individuals compared to other races. However, existing ED screening tools may miss important cultural nuances within the Black population. There is little data on whether ED screening tools are effective at detecting body-image or eating issues among Black individuals, perinatal individuals, or the intersections between these groups. Cross-sectional study assessing family members of psoriatic arthritis patients affected by the same disease: differences between Caucasian, South Asian and Afro-Caribbean populations living in the same geographic region.

AIM: To determine the likelihood of an individual developing psoriatic arthritis (PsA) if they have a relative diagnosed with this disease, and to compare rates among different ethnic groups living in the same geographic region. METHOD: Family histories of patients with PsA were assessed using a semi-structured questionnaire. RESULTS: Data on family members of patients with SpA were collected for 151 patients (46.6%) of the total cohort of 324. A total of 146 patients in the SpA cohort had PsA (45%) and 88 of these patients (60.2%) also had relatives with PsA. Psoriatic arthritis was seen mo

Seroepidemiology of adult T-cell leukemia virus (HTLV-I/ATLV): origin of virus carriers in Japan.

There are two large clusters of HTLV-I/ATLV carriers in the world. One large endemic area could be Africa, but available information is not yet sufficient to prove this. The other definitely large endemic area is Japan. Much smaller endemic areas and sporadic cases of the virus-carriers have been found in many parts of the world, including the Caribbean basin and Taiwan. Where did ATL virus carriers in Japan come from? From seroepidemiological studies, it is postulated that the carriers originated among Jomon people, who were the earliest inhabitants of Japan in 300 to 10,000 B.C. or earlier.

The contribution of maternal glucose to birth weight is smaller in Uganda (sub-Saharan Africa) than in Afro-Caribbean or white ethnicity mother-child pairs from outside Africa.

INTRODUCTION: Glucose is a major determinant of fetal growth, but its relative contribution in different ethnic groups or populations is not fully understood. The Hyperglycaemia and Adverse Pregnancy Outcome (HAPO) Study established a relationship between glucose and birth weight in multiple ethnic groups. However, the HAPO Study did not include any cohorts from sub-Saharan Africa (SSA), where 17% of the world population lives. This study aims to address this in a cohort study from Uganda. METHODS: We compared the relationship between oral glucose tolerance test measures and fetal outcomes in

Sickle cell disease: implications for nursing care.

Abnormal haemoglobins produce a variety of anaemias which range in effect from being very mild to lethal. The most common of these are the sickle cell disorders and thalassaemias. In London, thalassaemia affects 3.5% of the Maltese population, 2.5% of the Italian population and 1.5% of the West Indian population respectively. Seventeen per cent of the Cypriot population carry a thalassaemia gene. Sickle cell disease occurs in 500:100,000 people in Britain's black population but is not exclusive to people of Afro-Caribbean origin. The gene is carried by 1 in 10 people. Sickle cell disease is st

The prevalence of common mental disorders across 18 ethnic groups in Britain during the COVID-19 pandemic: Evidence for Equality National Survey (EVENS).

BACKGROUND: The COVID-19 pandemic negatively impacted mental health in the general population in Britain. Ethnic minority people suffered disproportionately, in terms of health and economic outcomes, which may contribute to poorer mental health. We compare the prevalence of depression and anxiety across 18 ethnic groups in Britain during the COVID-19 pandemic. METHODS: Secondary analysis of cross-sectional data (February-November 2021) from 12,161 participants aged 18-60 years old (N with data on outcomes = 11,540 for depression & 11,825 for anxiety), obtained from the Evidence for Equality N

Medical abortion provision and quality of care: What can be learned from feminist activists?

Little is known about how feminist abortion support initiatives -born in legally restrictive settings- approach quality of care. We conducted one focus group and one semi-structured interview with activists from eight organizations operating in Latin America and the Caribbean to understand their perspectives and strategies around quality of abortion care. Activists underscore the need of evidence-based information, trained providers and accessibility for people with diverse needs and resources. Grounded on feminism, they also highlight autonomy, dignity, horizontality and a new definition of s

Small countries face specific challenges in health workforce sustainability, but policy responses are a testbed for resilience for all countries.

While countries across Europe are facing challenges in building and sustaining a health workforce, small countries (official population of under 2 million people) face specific challenges that go beyond resource constraint. Limited population size constrains training capacity, specialist care provision, and career development opportunities, making these systems highly vulnerable to workforce shortages. Experiences in Cyprus, Luxembourg, North Macedonia, and the Caribbean Netherlands illustrate these dynamics. Cyprus and Luxembourg have expanded in-country medical education and postgraduate tra
