Common carotid artery dissection: a rare cause for cervical pain.

Spontaneous dissections of extracranial carotid arteries occur most frequently in the internal carotid artery. In contrast, common carotid artery dissection (CCA) is a rare cause of cerebral ischaemia with only a few cases having been reported. We present the case of a 59-year-old male patient who was referred to our clinic with left cervical pain. The patient was otherwise asymptomatic. Magnetic resonance imaging (MRI) showed enhancement and double lumen in the left CCA without affecting the carotid bulb, internal carotid artery or aortic arch. We discuss the diagnostic and therapeutic manage

Acute monocular blindness associated with spontaneous common carotid artery dissection.

The evaluation of patients with loss of vision is common in the emergency department. Central retinal artery occlusion (CRAO) is a potentially reversible cause of acute monocular blindness. When evaluating a patient with CRAO, the potential underlying causes should be considered while simultaneously initiating treatment. We present a case of spontaneous common carotid artery dissection manifesting as CRAO and propose that spontaneous carotid artery dissection be included in the differential diagnosis of CRAO.

Isolated complete orbital infarction: a common carotid artery occlusion syndrome.

When blood flow through the internal and external carotid arteries is completely interrupted by ipsilateral common carotid artery occlusion, the arterial orbital circulation may be more compromised than the brain supply. We studied a pure and extreme example of this situation in a patient who presented with acute orbital infarction, but no cerebral ischemia on clinical, CT and single-photon emission computerized tomography (SPECT) grounds. Ipsilateral blindness corresponded to retinal, choroidal and optic nerve infarction. The pattern of ophthalmoplegia, with relative sparing of adduction, was

Common Carotid Artery Stump Syndrome Due to Mobile Thrombus Detected by Carotid Duplex Ultrasonography.

Carotid stump syndrome is a cause of recurrent embolic stroke following occlusion of the ipsilateral internal carotid artery. The present report describes a case of recurrent cerebral embolism ipsilateral to a chronically occluded left common carotid artery (CCA), i.e., "CCA stump syndrome." Doppler color flow imaging showed anterograde flow in the left internal and external carotid arteries, which were supplied by collateral flow from the superior thyroid artery inflowing just proximal to the left carotid bifurcation. According to carotid duplex ultrasonography (CDU), a low-echoic mobile thro

[Common carotid artery intima-media thickness, carotid atherosclerosis and subtypes of ischemic cerebral disease].

INTRODUCTION: Common carotid artery intima-media thickness (CCA-IMT) measurements are widely used to study atherosclerosis. CCA-IMT is a useful outcome measure in clinical studies and intervention trials because it reflects early stages of atherosclerosis and cardiovascular risk. The present study examined the relationship between common carotid artery intima-media thickness and ischemic brain infarction. MATERIAL AND METHODS: The present study examined the association between CCA-IMT and incidence of ischemic stroke and its subtypes in 75 cases and 21 controls. Cases with internal borderzone

Synchronous carotid endarterectomy and retrograde endovascular treatment of brachiocephalic or common carotid artery stenosis.

OBJECTIVES: To retrospectively evaluate the safety and the long-term results of retrograde brachiocephalic and common carotid angioplasty and stenting (AS) performed for >70% stenosis synchronously with the carotid endarterectomy (CEA). PATIENTS: Sixteen patients operated between April 1999 and March 2002. RESULTS: 14/16 procedures were successful. There was no neurological morbidity or mortality. Per-operative angiography showed the optimal stent positioning and patency of both proximal and distal arteries in all patients. In the follow-up, all patients showed patency of the treated vessels

Adaptive common carotid arteries remodeling after unilateral internal carotid artery occlusion in adult patients.

OBJECTIVE: This study was undertaken in human adults to examine modifications in common carotid internal diameter (ID), intima-media thickness (IMT), cross-sectional area (CSA) and wall shear stress (WSS) occurring both on the ipsilateral side of an internal carotid artery (ICA) occlusion and on the non-occluded contralateral side. METHODS: Seventeen patients with unilateral ICA occlusion had repeated echo-Doppler examinations during 1 year and were compared to 12 volunteers with control non-occluded common carotid arteries (CCA). RESULTS: The cause of ICA occlusion was atherosclerosis in ni

Anomalous Origin of the Right Vertebral Artery from the Right Common Carotid Artery.

The anomalous origin of the right vertebral artery (VA) from the right common carotid artery (CCA) is a rare vascular anomaly, which is usually clinically asymptomatic and found incidentally during angiographic examinations. This anomaly is invariably associated with an aberrant right subclavian artery (RSCA). Approximately 31 cases have been reported in the literature. We present a case of a right VA originating from the right CCA in a patient with Down syndrome and discuss the imaging findings, embryological etiology of the anomaly, as well as its implications for endovascular/surgical treat

Aberrant right subclavian artery associated with a common origin of carotid arteries.

We present a patient with a rare anomaly of the aortic arch. Angiography revealed an aberrant right subclavian artery (aRSA) originating from the middle of the aortic arch. Angiography also demonstrated an anomalous origin of the left common carotid artery sharing a common trunk with the innominate artery and a large right vertebral artery arising from the right common carotid artery. Although this particular combination of anomalies has been reported in cadaver cases, to our knowledge this is the first premortem angiographic description of a patient in which an aRSA originates from the middle

Common carotid blood flow in patients with hypertension and stenosis of the internal carotid artery.

Common carotid blood flow has been measured using pulsed Doppler techniques in hypertensive patients over 45 years of age, with and without stenosis of the internal carotid artery. In hypertensive patients without stenosis, arterial diameter remained within the normal range, whereas blood flow velocity and blood flow were significantly reduced. Vascular resistance was significantly increased but did not change markedly after administration of the vasodilating drug isosorbid dinitrate. Patients with hypertension and stenosis of the internal carotid artery were compared with two different popula

High Bifurcation of the Common Carotid Artery with Pentafurcation of the External Carotid Artery: Case Series with Review of Literature.

The bifurcation of the Common Carotid Artery (CCA) is a pivotal anatomical landmark in head and neck surgeries, particularly affecting procedures such as carotid endarterectomy. Typically occurring around the C3-C4 intervertebral disc, variations in its location can complicate surgical access and pose risks of nerve injury. In this case report, three rare bilateral high bifurcations of the CCA at the C2 and C2/3 vertebrae levels are documented. Additionally, an unusual pentafurcation of the External Carotid Artery (ECA) and bilateral superior thyroid artery are observed, arising directly from

Absence of the common carotid artery: a rare vascular anomaly.

Although absence of the common carotid artery (CCA) is a rare anomaly, the specific configuration in our case makes it extremely rare. Clinical, angiographic, ultrasonographic, and embryologic correlations of this anomaly are discussed after the case report.

Dissection and intussusception of the common carotid artery following endarterectomy.

We describe a 67-year-old man who developed a dissection of the right common carotid arterial wall with intussusception two years following endarterectomy for atherosclerosis. We suggest that the previous surgical procedure resulted in a thinned and weakened media. With a disruption of the intima, a dissection into the inner third of the media formed, and blood in the false lumen buckled the intima into the lumen of the blood vessel. Hemodynamic forces then probably played a role in the propagation of the dissection and the formation of an intussusception. Complete occlusion by thrombus follow

[A case of common carotid artery stenosis due to hanging].

We report a case of a 50-year-old woman with traumatic dissection of the right common carotid artery secondary to attempted suicidal hanging. Initial examination showed marks of strangulation on the neck, facial edema, and multiple conjunctival petechiae accompanied by difficulty in breathing and loss of consciousness. She gradually recovered except for her left upper limb weakness. Rehabilitation, with a diagnosis of left brachial plexus injury, was started. Two years later, she occasionally felt numbness of the left upper extremity. On her second admission, a bruit was heard on the right sid

Mapping the Common Carotid Artery Bifurcation Utilizing Anterior Neck Landmarks.

Background/Objectives: High common carotid artery bifurcation (CCAB) is an anatomic variant relevant to carotid endarterectomy that is associated with an increased risk of cranial nerve injury, particularly hypoglossal nerve injury. This study analyzed CCAB location relative to anterior neck landmarks as a method for categorizing its proximity to critical surgical structures. Methods: Eighty-one formalin-fixed donors were dissected, yielding 159 CCABs. CCAB height was classified relative to six transverse anterior neck planes using superficial anatomic landmarks. Distances from the CCAB to the

Differential Expression of Pax6 Following Bilateral Common Carotid Artery Occlusion.

BACKGROUND/AIM: Chronic cerebral hypoperfusion causes neuronal damage involving cognitive impairment and development of dementia. Permanent bilateral common carotid artery occlusion (BCCAO) in rat models is used to study chronic cerebral hypoperfusion. Pax6 is used as an early neurogenesis marker which affects the maturation of neuronal cells. However, the expression of PAX 6 after BCCAO is not well understood. In this study, we investigated the expression of PAX6 in the neurogenic zones after BCCAO to evaluate the effects of Pax6 on chronic hypoperfusion. MATERIALS AND METHODS: Chronic hypop
