[Topical treatment of bullous pemphigoid with tacrolimus. Case report with brief literature review].

Bullous pemphigoid (BP) is a cutaneous autoimmune disease predominantly affecting older patients which can cause death either due to severe clinical manifestations or due to the side effects of systemic immunosuppressive treatment. Topical treatment with corticosteroids is an established alternative to systemic treatment. However, prolonged application is accompanied by side effects such as skin atrophy. Recently, the immunomodulatory calcineurin antagonists tacrolimus and pimecrolimus have been introduced for topical treatment of skin diseases. Tacrolimus has been reported to be effective in

Successful Topical Treatment of Hydroxyurea-Induced Ulcers Without Drug Discontinuation: A Case Report.

Hydroxyurea, an antineoplastic agent, is widely used to treat essential thrombocythemia and polycythemia vera. Although hydroxyurea-induced skin ulcers are uncommon, they are a recognized long-term adverse effect. Current management generally recommends discontinuation of hydroxyurea as the mainstay of treatment; however, drug withdrawal may aggravate the underlying hematologic disease. We report a case of bilateral medial malleolar ulcers associated with hydroxyurea therapy in a 65-year-old man with essential thrombocythemia, who showed marked improvement after two months of topical treatment

Balanitis Xerotica Obliterans Successfully Treated with Topical Tacrolimus: A Case Report and Literature Review.

Balanitis xerotica obliterans (BXO) is a chronic, progressive, inflammatory, and sclerosing disease of the male genitalia. If left untreated, it can lead to urinary and sexual dysfunction and reduced quality of life. We describe a 15-year-old Caucasian male with a 3-month history of progressive thickening and whitening of the prepuce and penile glans. He reported mild pruritus, embarrassment of the prepuce retraction, painful cracking of the foreskin, and painful erection. Dermatological examination showed mild erythema and hypopigmentation of the foreskin and penile glans, a sclerotic white r

Topical Steroid-Induced Cushing Syndrome in an 18-Month-Old Child: A Case Report.

Topical corticosteroids are widely used in pediatric dermatology and are generally considered safe when used appropriately; however, prolonged or inappropriate use, particularly of high-potency agents, may lead to significant systemic adverse effects. This case report describes an 18-month-old female who presented with rapid weight gain and cushingoid features following prolonged application of clobetasol propionate for diaper dermatitis. Clinical evaluation revealed generalized obesity, moon facies, facial plethora, and dorsal hypertrichosis, while laboratory investigations demonstrated marke

Iatrogenic Cushing's syndrome induced by topical steroid abuse in a patient with psoriasis: a case report.

BACKGROUND: Iatrogenic Cushing's syndrome (ICS) is frequently observed as a side effect of long-term steroid treatment. While it is most commonly linked to the administration of oral steroids, rare instances have been reported following the inadvertent use of topical steroids, which can lead to severe clinical consequences. CASE SUMMARY: This case report details the case of a psoriasis patient who exhibited Cushing-like symptoms, such as central obesity, violaceous striae, moon face, and hypertension, following the long-term use of a so-called "herbal cream" containing clobetasol propionate.

Hypnosis as an Adjunct to Topical Anesthesia for Awake Intubation in a Patient With Difficult Airway: A Case Report.

BACKGROUND: Awake intubation is often distressing for patients, particularly in cases of difficult airway anatomy. CASE PRESENTATION: We report a case of successful awake fiber-optic intubation in a 69-year-old male with limited mouth opening due to previous ENT surgeries. Hypnosis was used alongside topical anesthesia and low-dose dexmedetomidine. The patient entered a relaxed state, and the intubation proceeded without pain, discomfort, or complications. CONCLUSION: This case demonstrates the feasibility of using hypnosis as a noninvasive adjunct to improve comfort during awake intubation.

Ligneous Conjunctivitis with Plasminogen Deficiency Treated with Topical Allogeneic Serum Drops: A Case Report.

To report a case of refractory ligneous conjunctivitis (LC) in which topical allogeneic serum eye drops from a relative with proven serum plasminogen activity were effective. A 12-year-old male complained of intractable swelling and hardness of all eyelids, with woody discharge. He had been followed up with similar complaints since he was 1 year old. Until his teenage years, his membranes were relatively mild and non-progressive with topical cyclosporine (0.05%), artificial tears (single dose), antihistamines, antibiotics, and prednisolone acetate during active periods. His symptoms have been

Maintenance Acne Treatment with Topical Tazarotene after Oral Isotretinoin: Overview and Case Reports.

Acne is a chronic dermatologic disorder that can require long-term treatment. To prevent recurrence after oral treatment for severe acne, topical maintenance treatment is recommended; however, there is little guidance or research on maintenance regimens. This article briefly summarizes literature on oral isotretinoin and topical retinoids and presents a case series of patients who received tazarotene 0.045% lotion as maintenance following oral isotretinoin. While oral isotretinoin is efficacious, relapse/remission rates range from 0 to 69 percent depending on the definition of relapse/remissio

Deep Dermal Dilemma: A Case Report on Majocchi's Granuloma After Topical Steroid Use.

Majocchi's granuloma represents a unique dermatological entity characterized by the invasion of dermatophytes into the dermis and subcutaneous tissue, typically following trauma or topical corticosteroids, most commonly caused by Trichophyton rubrum. The unrestrained use of over-the-counter topical steroids has led to the rise of Majocchi's granuloma in the past few years. A 57-year-old male presented with complaints of itchy skin lesions over his right ankle for 20 days. There was a history of over-the-counter topical steroid application, following which the lesions exacerbated. Clinical exam

Vulvovaginal erosive lichen planus refractory to topical therapies: What's next? A case report.

A 60-year-old woman was referred for progressive and severe vulvovaginal pain characterized by erosions and Wickham's stria for the past 7 months. Her condition had not responded to oral fluconazole, topical estrogen cream, and topical clobetasol cream. Vulvar and vaginal biopsies were obtained under general anesthesia to verify the diagnosis of erosive lichen planus given the failed response to ultrapotent topical steroids. Tacrolimus cream was added but not tolerated. Oral and cutaneous lesions of lichen planus also developed. In the absence of evidence-based guidelines, three different syst

Keratosis pilaris rubra successfully treated with topical sirolimus: Report of a case and review of the literature.

Keratosis pilaris rubra (KPR) is a subtype of keratosis pilaris (KP) presenting with numerous "grainlike" follicular papules in a background of confluent erythema most often affecting the face and upper extremities with persistence beyond puberty. Treatment has remained challenging with inconsistent benefit from topical therapies such as emollients, keratolytics, corticosteroids, and retinoids, though case reports documenting success with pulsed dye laser therapy have been found. We present a case of KPR in a 15-year-old boy who was successfully treated with topical sirolimus 1% cream.

Topical Liposomal Ozonated Oil in Complicated Corneal Disease: A Report on Three Clinical Cases.

Infectious and traumatic corneal diseases require adequate therapy against infection and promote re-epithelialization. The appearance of multidrug-resistant bacterial strains, induced by excessive use of antibiotics in the treatment of infectious diseases, has urged researchers to study new antimicrobial formulations. Ozonated oils have demonstrated antiseptic (antibacterial, antifungal and antiviral) and reparative properties and can be proposed for the prevention and treatment of acute and chronic local infections, as an alternative to conventional topical antimicrobials. In ocular treatment
