Do pulmonary artery catheters alter outcome in trauma patients?

OBJECTIVE: To review the literature addressing the use of the pulmonary artery catheter (PAC) in victims of blunt and penetrating trauma and examine the available evidence that supports or refutes the claim that PAC use alters outcome in this patient population. Furthermore, to determine what additional research should be done in this area. DATA SOURCE: All pertinent English language articles dealing with pulmonary artery catheterization in trauma patients were retrieved from 1979 through 1996. STUDY SELECTION: Clinical studies were considered if PACs were used to establish a cardiopulmonary

[A case of the old injured vena cava superior found during the operation of lung cancer].

We operated a 78-year-old male with lung cancer and unstable angina pectoris. During the operation of right upper lobe, a mass, with size of 2 cm in diameter was detected on the right postero-lateral side of the superior vena cava, and was resected. Postoperative pathological examination suggested that the mass resulted from an injury made by a Swan-Ganz catheter used before operation. Thus, as insertion of a Swan-Ganz catheter through the right subclavian vein may involve risk, so we concluded that the right internal jugular vein or femoral vein may be preferable as a route of first choice fo

Validation of a noninvasive device for measuring filling pressure against invasive pulmonary capillary wedge pressure.

BACKGROUND: Intravascular congestion, particularly elevated cardiac filling pressures, is a key component of heart failure and often precedes symptom onset. Early intervention is critical, yet non-invasive tools to detect congestion before symptoms appear are limited. OBJECTIVES: This study evaluates the accuracy of a non-invasive filling pressure (NIFP) device in assessing cardiac filling pressure, compared to pulmonary capillary wedge pressure (PCWP) obtained via Swan-Ganz catheterization. METHODS: Ninety-eight patients undergoing cardiac surgery had NIFP device measurements immediately be

The efficacy of pulmonary artery catheters in reducing mortality in acute heart failure cardiogenic shock: A systematic review.

BACKGROUND: Cardiogenic shock (CS), a complex and life-threatening medical condition, has an astounding hospital mortality rate spanning from 40 % to 59 %. Frequently, CS requires the use of pulmonary artery catheters (PACs) for management. OBJECTIVE: This literature review aims to investigate the relationship between PAC utilization in CS patients and in-hospital 30-day mortality rates compared to noninvasive vital sign monitoring alone. METHODS: An integrative literature search was conducted from January 1, 2003, until August 1, 2023. The review focused on patients with acute decompensated

Transvenous Retrieval of a Pulmonary Artery Catheter Knot Around Pacing Leads: A Case Report.

BACKGROUND: Pulmonary Artery Catheter (PAC) knotting is a rare complication of PAC insertion. In patients with dilated right heart chambers, blind insertion of PAC significantly increases the risk of catheter knotting. We demonstrate a safe and successful approach to resolving a PAC knot around pacing leads of a cardiac resynchronization device. CASE PRESENTATION: A 63-year-old African American male with dilated cardiomyopathy and a cardiac resynchronization therapy (CRT) device for severe left ventricular systolic dysfunction required PAC insertion for hemodynamic management of acute heart f

Severe Tricuspid Regurgitation After Percutaneous Removal of a Swan-Ganz Catheter Caught by Suture.

Swan-Ganz catheter entrapment in the heart, vena cava, or pulmonary artery is rare but may lead to life-threatening events. We report a case of severe tricuspid regurgitation resulting from papillary muscle rupture during percutaneous removal of a Swan-Ganz catheter (SCG) caught by a surgical suture. We performed urgent tricuspid valve repair by suturing the ruptured papillary muscle. This report highlights preventive measures to avoid suture-related entrapment and raises a word of caution regarding percutaneous removal of accidentally entrapped SCGs.

Rupture of the chordae of the tricuspid valve after knotting of the pulmonary artery catheter.

A case is presented in which the insertion of a pulmonary artery catheter was complicated by the formation of a knot around the chordae tendineae of the tricuspid valve. The catheter was pulled out under fluoroscopic guidance using a guidewire inserted through the femoral vein.

Advanced haemodynamic monitoring in cardiogenic shock.

PURPOSE OF REVIEW: To describe haemodynamic monitoring methods that may be used in cardiogenic shock including the information they provide, their prognostic value, how they may be used for risk classification and guiding management. RECENT FINDINGS: Haemodynamic monitoring in cardiogenic shock encompasses multimodal techniques that provide complementary information to enable identification of different cardiogenic shock phenotypes. Isolated right ventricular failure, biventricular dysfunction, and mixed cardiogenic shock are associated with poor outcomes and require different management stra

Estimation of Right Atrial Pressure by Ultrasound-Assessed Jugular Vein Distensibility in Patients With Heart Failure.

BACKGROUND: Clinical evaluation of central venous pressure is difficult, depends on experience, and is often inaccurate in patients with chronic advanced heart failure. We assessed the ultrasound-assessed internal jugular vein (JV) distensibility by ultrasound as a noninvasive tool to identify patients with normal right atrial pressure (RAP ≤7 mm Hg) in this population. METHODS: We measured JV distensibility as the Valsalva-to-rest ratio of the vein diameter in a calibration cohort (N=100) and a validation cohort (N=101) of consecutive patients with chronic heart failure with reduced ejection

[Changes in hemodynamics and volemic status during intraoperative blood exfusion in patients with chronic heart failure].

Forty-five patients with dilated cardiomyopathy, NYHA Functional Classes III-IV chronic heart failure with left ventricular ejection fraction 26.98+/-7%, who had undergone surgical left ventricular remodeling using an extracardiac conduit in combination with mitral or tricuspid valve correction and blood exfusion, were examined. Central hemodynamic monitoring was performed, by using a Swan-Ganz catheter in all the patients at surgery. In 11 patients, transpulmonary thermodilution in combination with pulmonary artery catheterization with a Swan-Ganz catheter (PICCO-Plus + VOLEF, Pulsion, German

Cardiorenal Syndrome Type 1 Complicated by Uremic Pericarditis and a Small Pericardial Effusion Causing Cardiac Tamponade.

Cardiac tamponade is typically associated with large pericardial effusions; however, even small effusions can result in significant hemodynamic compromise, particularly in the setting of rapid fluid accumulation and underlying cardiac pathology. This case highlights the diagnostic challenge of identifying low-volume tamponade in a patient with elevated baseline intracardiac pressures. An elderly man was admitted for acute heart failure complicated by cardiorenal syndrome. Following initial diuresis, he developed paradoxically worsening hypotension and pleuritic pain. The absence of a pericardi

Right heart catheterization via hemiazygos-azygos collaterals in the setting of inferior vena cava stenosis.

BACKGROUND: In cases of inferior vena cava (IVC) obstruction, venous collaterals involving the hemiazygos and azygos veins can provide venous return to the right atrium. This can lead to procedural challenges in right heart catheterization (RHC). CASE PRESENTATION: A 54-year-old male with end stage renal disease and severe aortic regurgitation underwent RHC for valve replacement planning. Swan Ganz catheter, via left femoral vein, would not advance beyond the distal IVC despite an inflated balloon. It was advanced over a V18 wire, taking a tortuous path to the right ventricle. [Refractory pulmonary hypertension following mitral valve repair necessitating percutaneous cardiopulmonary support].

A 37-year-old woman underwent semi-emergency mitral valve plasty for severe mitral regurgitation and congestive heart failure. Although repair was successfully performed and the left ventricular function was maintained, refractory pulmonary hypertension precluded cessation of cardiopulmonary bypass. Inhalation of nitric oxide was ineffective, and assist circulation was required. All data obtained from the Swan-Ganz catheter and other routine monitors were devastating, and the patient showed no clinical recovery in the early postoperative period. However, transesophageal echocardiography (TEE)

Doppler echocardiography in critically ill cardiac patients.

E-D is a powerful technique that can provide invaluable information in the acutely ill cardiac patient. It can help in the early diagnosis of conditions causing acute chest pain such as AMI and aortic dissection. E-D can help diagnose the causes of underlying hemodynamic instability such as hypotension and pulmonary edema, and it can help determine the initial management strategy. In this regard, E-D can replace Swan-Ganz catheterization in most patients. In patients with AMI who may appear to be hemodynamically stable, it can provide information regarding the following: size of risk area; sta

[Ischemic myocardial disorder in acute phase subarachnoid hemorrhage: clinical study of 52 patients].

Fifty-two cases of acute phase subarachnoid hemorrhage were studied by brain CT scanning to determine the presence and incidence of ischemic myocardial disorder, the relationship between ischemic change and severity, disease prognosis, and the relationship between acute phase circulatory dynamics and so-called neurogenic pulmonary edema. In all cases, ECGs were carried out and CPK-MB determined. Some of the patients underwent Tl myocardial scintigraphy, echocardiography, cardiac catheterization, as well as circulatory dynamic investigation (by Swan-Ganz catheter) and arterial blood gas analysi
