Experience of Ceftazidime/avibactam in a UK tertiary cardiopulmonary specialist center.

OBJECTIVES: Antimicrobial resistance is a major threat to public health. New drugs such as Ceftazidime/avibactam have been developed for the treatment of Multi-Drug resistant (MDR) pathogens. Susceptibility can be variable and inappropriate use can add a financial strain on the National Health Service (NHS). There is a pressing need to ensure these new and invaluable antimicrobials are preserved and used effectively. METHODS: We undertook a retrospective observational study to assess the use of Ceftazidime/avibactam and evaluated prescribing against applied standards. RESULTS: Between August

Pharmacokinetic and Pharmacodynamic Analysis of Ceftazidime/Avibactam in Critically Ill Patients.

BACKGROUND: The pharmacokinetics, especially the volume of distribution (Vd), of ß-lactam antibiotics can be altered in critically ill patients. This can lead to decreased serum concentrations and a reduction in clinical cures. Ceftazidime/avibactam (CZA) is a new antimicrobial agent utilized in critically ill patients although its pharmacokinetics has not been well defined in these patients. PATIENTS AND METHODS: In this study, the serum concentrations of CZA from adult patients treated in an intensive care unit (ICU) with standard dosing regimens were measured and both pharmacokinetic and p

Comparison of the in vitro and in vivo susceptibilities of Burkholderia mallei to Ceftazidime and Levofloxacin.

BACKGROUND: Burkholderia mallei is a zoonotic Gram negative bacterium which primarily infects solipeds but can cause lethal disease in humans if left untreated. The effect of two antibiotics with different modes of action on Burkholderia mallei strain ATCC23344 was investigated by using in vitro and in vivo studies. RESULTS: Determination of minimal inhibitory concentrations (MICs) in vitro was done by the agar diffusion method and the dilution method. The MICs of levofloxacin and ceftazidime were in the similar range, 2.5 and 5.0 microg/ml, respectively. Intracellular susceptibility of the b

The effect of diabetes and severe ischaemia on the penetration of ceftazidime into tissues of the limb.

AIMS: To determine the effect of diabetes and of different degrees of ischaemia on the penetration of ceftazidime into different tissues. METHODS: Sixteen patients (10 with diabetes mellitus) undergoing lower extremity amputation for severe ischaemia (in 12 in combination with infection), received 2000 mg ceftazidime intravenously as a bolus 30 min prior to the operation. Skin perfusion was determined by transcutaneous oxygen pressure measurements (TcPO2) on the dorsal side of the midfoot. After amputation bone, skin and muscle samples were obtained from the forefoot, midfoot and proximal tib

Neurotoxicity associated with ceftazidime therapy in geriatric patients with renal dysfunction.

Ceftazidime, a beta-lactamase-stable, third-generation cephalosporin, is widely used for the treatment of serious gram-negative infections. Neurotoxicity has rarely been associated with the drug; however, two of our patients developed ceftazidime-induced neurotoxicity that produced confusion, disorientation, agitation, generalized weakness, and myoclonus. In both patients these symptoms cleared with either discontinuation or reduction of the dosage of ceftazidime. This emphasizes the importance of adjusting the dosage of ceftazidime in patients with renal insufficiency.

Emergence of resistance to ceftazidime during therapy for Enterobacter cloacae infections.

The mechanism of resistance to ceftazidime in two clinical isolates of Enterobacter cloacae that emerged during therapy with broad-spectrum beta-lactam antibiotics was studied. Both isolates acquired broad resistance to advanced-spectrum beta-lactam drugs other than imipenem. Biotyping confirmed strain identity in both cases, and no new plasmids were detected in the resistant isolates. Both resistant isolates produced beta-lactamase constitutively. Slow but definite hydrolysis of ceftazidime was demonstrated by using purified beta-lactamase in a spectrophotometric assay. Further evidence that

Liposomal tobramycin and ceftazidime as advanced nanocarriers against Pseudomonas aeruginosa infections.

The worrisome decline of antibiotic efficacy against Pseudomonas aeruginosa infections emphasizes the urgent need for new antibiotic delivery techniques. Liposomal formulations of tobramycin and ceftazidime were developedand comprehensively studied utilizing an active loading approach. Both formulations showed nanoscale size (<120 nm), low polydispersity index (<0.3), and spherical morphology, as validated by TEM examination. Tobramycin and ceftazidime encapsulation efficiency was in the rank of 20 %. Drug entrapment exploits both temperature change and electrostatic interaction between the ch

Klebsiella pneumoniae Carbapenemase Variants Resistant to Ceftazidime-Avibactam: an Evolutionary Overview.

First variants of the Klebsiella pneumoniae carbapenemase (KPC), KPC-2 and KPC-3, have encountered a worldwide success, particularly in K. pneumoniae isolates. These beta-lactamases conferred resistance to most beta-lactams including carbapenems but remained susceptible to new beta-lactam/beta-lactamase inhibitors, such as ceftazidime-avibactam. After the marketing of ceftazidime-avibactam, numerous variants of KPC resistant to this association have been described among isolates recovered from clinical samples or derived from experimental studies. In KPC variants resistant to ceftazidime-aviba

Burkholderia pseudomallei acquired ceftazidime resistance due to gene duplication and amplification.

Ceftazidime (CAZ) is the antibiotic of choice for the treatment of Burkholderia pseudomallei infection (melioidosis). The chromosomally-encoded PenA β-lactamase possesses weak cephalosporinase activity. The wild-type penA gene confers clinically significant CAZ resistance only when overexpressed due to a promoter mutation, transcriptional antitermination or by gene duplication and amplification (GDA). Here we characterise a reversible 33-kb GDA event involving wild-type penA in a CAZ-resistant B. pseudomallei clinical isolate from Thailand. We show that duplication arises from exchanges betwee

Successful Treatment of Persistent Burkholderia cepacia Complex Bacteremia with Ceftazidime-Avibactam.

We report our clinical experience treating a 2-month-old infant with congenital diaphragmatic hernia who experienced prolonged bacteremia with Burkholderia cepacia complex (Bcc) despite conventional antibiotic therapy and appropriate source control measures. The infection resolved after initiation of ceftazidime-avibactam. Whole-genome sequencing revealed that the isolate most closely resembled B. contaminans and identified the mechanism of resistance that likely contributed to clinical cure with this agent. Ceftazidime-avibactam should be considered salvage therapy for Bcc infections if other

Solving the degradation problem of continuous ceftazidime infusions-the Cooltaz study.

Ceftazidime is an effective antipseudomonal antibiotic, used commonly in cystic fibrosis. Continuous 24-hour intravenous infusion allows children to receive daily treatment at home. However, the instability of 24-h ceftazidime infusions and production of toxic pyridine has led to recommendations not to use them. We aimed to determine a suitable target temperature to limit degradation of ceftazidime in 24-h infusions and assess effectiveness of simple temperature control. For in vitro experiments, infusers with 6g ceftazidime in 0.9% saline were incubated at 4ºC, 25ºC and 33ºC and sampled over

A multifunctional nanobooster to restore ceftazidime susceptibility in mucoid Pseudomonas aeruginosa.

Mucoid Pseudomonas aeruginosa (m.PAE) presents a major clinical challenge due to its high resistance to ceftazidime (CAZ), a cornerstone antibiotic. This resistance is orchestrated through three barriers, including the formation of an alginate-rich mucoid biofilm that limits drug penetration, upregulated efflux pumps such as MexAB-OprM that reduce intracellular CAZ accumulation, and expression of β-lactamases that hydrolyze CAZ. To address these barriers simultaneously, we engineered a multifunctional nanobooster termed A/C-pAg3PO4, which integrates a silver phosphate nanoparticle (NAg3PO4) co

Ceftazidime prescription and antibiotic resistance patterns in a tertiary care hospital.

Ceftazidime, a third-generation cephalosporin, is effective for treating infections caused by gram-negative bacteria. However, increasing antibiotic resistance, particularly in multidrug-resistant gram-negative bacilli, complicates treatment decisions and patient outcomes. This study aimed to assess the clinical epidemiology and characteristics of patients treated with intravenous ceftazidime and clarify the drug resistance patterns of the isolated pathogens. A retrospective cross-sectional study was conducted at a tertiary care center in the West Bank between June 2021 and June 2023. The stud

Update on the role of ceftazidime-avibactam in the management of carbapenemase-producing Enterobacterales.

Ceftazidime-avibactam is a novel combination of a known cephalosporin and a non-β-lactam/β-lactamase inhibitor that has been approved for the treatment of complicated intra-abdominal and urinary tract infections, hospital-acquired pneumonia as well as Gram-negative infections with limited treatment options in Europe. Since its approval, it has been used in patients with infections due to carbapenem-resistant bacteria, in many occasions as off-label indication or salvage therapy, with promising clinical and microbiological cure rates. Emergence of resistance during therapy to this new combinati
