Combined approach of nanoemulgel and microneedle pre-treatment as a topical anticellulite therapy.

BACKGROUND AND PURPOSE: Cellulite is caused by changes in the metabolism of the fatty tissue beneath the skin. Methylxanthines and retinoids are commonly added to the different anticellulite products. However, their topical permeation into the dermis is limited. Thus, the objective of this research is to formulate a nanoemulgel (NEG) containing a triple therapy of caffeine, aminophylline, and tretinoin as a topical anticellulite product to improve their skin permeation. Furthermore, the influence of microneedles (MNs) as skin pre-treatment on the permeation of the NEG was investigated. EXPERI

[Recurrent unilateral inflammatory exophthalmos].

The issue discusses the diagnosis difficulties concerning the case of 58-years-old woman who was watched for a recurrent exophthalmos after a mild conjunctival trauma. The debut was orbital cellulite appearance, complicated by a nonaxial exophthalmos due to a dacryoadenitis. The exophthalmos is axial in the other two recurrences. All exophthalmic episodes progressed with inflammatory features associated with optic neuritis and oculomotor nerves implication without any neurological signs. The paraclinical tests (ECHO, orbital radiography, CAT) find inflammation of orbital elements: extrinsic mu

Emerging technologies in aesthetic medicine.

Recent advances in technology have drastically improved aesthetic treatment for skin. Of particular interest is the emergence of laser- and lightbased technologies, which have offered great promise among skin-rejuvenation therapies. New laser resurfacing techniques for skin rejuvenation offer significant advantages over conventional ablative lasers, such as the CO(2) and erbiumYAG laser systems. Nonablative and fractional lasers, although not as efficacious as ablative therapies, are associated with significantly diminished complication rates and shortened recovery times. Novel devices combini

[Hand involvement in Thevenard's disease: a new "phlegmonous" form. An exceptional case report].

Thévenard's disease is a rare familial ulcero-mutilating acropathology involving the peripheral nervous system. It typically begins on the feet and only rarely spreads to the hands late in the course of the disease. It first causes cutaneous ulcerations and then secondary osseous deformations and osteoarticular destruction, due to a distal loss of temperature sensation. We report the case of an 87 year old patient who suffered from a particular clinical form, undescribed as yet in the literature and characterized by local and general superinfections predominately on the hands. We term this the

Primary congenital lymphedema involving all limbs and genitalia.

Primary congenital lymphedema is the rarest form of primary lymphedema. Lymphedema of the extremities presents at birth and rarely involves the genitalia. There has never been a reported case in Thailand. The authors herby report a case of a 6-year-old boy who presented with progressive swelling of the lower legs since birth. The edema progressed into his scrotum and his arms. There was no history of lymphangitis or cellulites. Physical examination revealed a generalized non-pitting edema of all extremities, more on the right leg than the left leg. Swelling of the scrotum and penis was also de

UNLABELLED: There is an increasing interest, including androgenic and anabolic substances (AAS). The uncritical use may be associated with severe adverse effects. We observed five patients with different patterns of adverse reaction to AAS: two females and three males, they were identified when seeking medical help and advice. The following adverse effects from of AAS have been observed: deepening of the voice due to topical use of AAS in an anti-cellulite cream; circumscribed hypertrichosis and late onset acneiform eruptions due to testosterone replacement therapy after ovariectomy; homolater

A case of cellulitis causing tissue defect during etanercept therapy.

Tumor necrosis factor-alpha (TNF-α) antagonists are employed increasingly during recent years in patients with active rheumatoid arthritis who do not respond to disease-modifying anti-rheumatic drugs. Contraindications such as infections, auto-antibody formation and hypersensitive reactions can be observed during the treatment with TNF-α antagonist drugs. Our case was a 52-year-old woman, followed by several centers for a period of 21 years with a seropositive, erosive and nodular RA diagnosis. Anti TNF-α treatment was commenced due to the failure to control the disease. During the treatment,

Severe Sepsis and Wet Gangrene Requiring Foot Amputation Caused by an Emerging Human Pathogen - Shewanella algae.

A 69-year-old woman with type 2 diabetes mellitus, peripheral vascular disease, and other comorbidities presented with recurrent syncopal episodes. Cellulitic skin changes in her right lower extremity were noted, as well as a large hemorrhagic bulla on the dorsum of her right foot. Severe sepsis was determined to be the reason for her syncopal episodes. Blood cultures and the bulla aspirate culture were positive for Shewanella algae that was pan-sensitive to antibiotics. Her clinical status was stabilized with a regimen of intravenous fluids and broad-spectrum antibiotics. However, due to the

Necrotizing fasciitis caused by Staphylococcus epidermidis in a neonate with extremely low birthweight.

Necrotizing fasciitis (NF) is a severe life-threatening soft tissue infection characterized by rapidly spreading necrosis of the fascia and the subcutaneous tissue. The initial skin presentation ranges from minimal rash to cellulites. The lesions subsequently spread rapidly. Even with appropriate medical and surgical therapy, the mortality rate in NF is more than 50%. In cases of neonates, mortality rate is higher than that ratio. High index of suspicion, prompt aggressive surgery, appropriate antibiotics and supportive care are the mainstays of management in the newborn infant with NF. Herein

Psoriasis and Post-Surgical Infections in Primary Total Knee Arthroplasty: An Analysis of 10,727 Patients.

BACKGROUND: Psoriasis is a dermatologic condition characterized by erythematous plaques that may increase wound complications and deep infections following total knee arthroplasty (TKA). There is a paucity of evidence concerning the association of this disease and complications after TKA. This study aimed to determine if patients who have psoriasis vs non-psoriatic patients have differences in demographics and various comorbidities as well as post-operative infections, specifically the following: (1) wound complications; (2) cellulitic episodes; and (3) deep surgical site infections (SSIs). Management of orbital complications due to rhinosinusitis.

Emergencies in sinusitis are in 60-75% orbital complications defined by blepharedema (stage I), periostitis (stage II), subperiostal abscesses (SPA) (stage III) and orbital cellulites (stage IV). Ophthalmic complications such as diplopia, exophthalmia and reduced visual acuity are seen in stages III and IV. There is a consensus for primary conservative treatment in stage I or II and until recently for surgery in stages III and IV. The discussion concerns the decision for surgery versus conservative therapy in stage III in children. Another question is the definitive outcome of ophthalmic sympt

Product-related emphasis of skin disease information online.

BACKGROUND: In 2000, 41 million people searched for medical information online. The quality of that information is unregulated, variable, and not well studied. OBJECTIVE: To quantify and compare the numbers and types of Internet sites matched for 10 diverse skin conditions through different search systems. DESIGN: Search strategies were performed at 6-month intervals via Netscape Navigator, using 3 search engines and 1 directory. Ten skin conditions were chosen to represent common (psoriasis and eczema), cosmetic (wrinkles and cellulite), difficult-to-treat (alopecia, mastocytosis, granuloma

Cirrhosis, cellulitis and cats: a 'purrfect' combination for life-threatening spontaneous bacterial peritonitis from Pasteurella multocida.

Pasteurella multocida is a Gram-negative coccobacillus that colonises the upper airways of many animals, in particular, dogs and cats. It acts as an opportunistic infection in humans following an animal bite or scratch and is associated with soft tissue infections, septicaemia and pneumonia, particularly in patients with a compromised immune response, such as patients with liver failure. Spontaneous bacterial peritonitis (SBP) is a serious complication of cirrhosis with a death rate of 10-15%. We report a case of a 47-year-old man with cirrhosis who presented with life-threatening P multocida

Factitious thyrotoxicosis induced by mesotherapy: a case report.

CONTEXT: Mesotherapy consists of cutaneous injections of a mixture of compounds and has recently been used for cosmetic purposes to reduce local fat and cellulite. To date, several reports have described only local adverse events related to this therapy. We describe the first report of a female patient who developed thyrotoxicosis due to cosmetic mesotherapy with triiodothyroacetic acid in its formulation. Apart from mechanical rupture of the epidermal barrier, a disturbance of type III deiodinase activity or skin fibroblast paracrine function and vascular alterations related to simultaneously

Fungal infections of the eye--laboratory diagnosis and treatment.

Infections of the eye give rise to severe ocular morbidity and blindness include keratitis, orbital cellulites, endophthalmitis and dacryocystitis. Corneal blindness, in developing countries is predominantly associated with infections. In India, nearly 30-35% of all culture positive infectious keratitis are caused fungi. Laboratory diagnosis mainly depends upon proper collection and transport of clinical specimens. In fungal keratitis, corneal scraping is the ideal sample, but occasionally corneal biopsy or anterior chamber aspirate may also be needed. Corneal scraping is usually by Kimura spa

A case of Vibrio vulnificus septicaemia acquired in Victoria.

The clinical features and laboratory findings of a case of Vibrio vulnificus septicaemia are reported. The illness occurred in a previously well 68-year-old man who was accidentally spiked in the buttock by the dorsal spine of a flathead caught in Tamboon Inlet, near Mallacoota, Victoria. The clinical picture of an acute septicaemic illness with shock, associated with metastatic cellulitic lesions on the lower limbs progressing to bulla formation, skin necrosis, necrotising fasciitis and myositis, is characteristic of V. vulnificus septicaemia. It would appear that tetracyclines are the drugs

Aggressive intranasal carcinoma mimicking infection or inflammation.

In a twenty-year period we have treated five patients with intranasal squamous cell carcinomas who initially were thought to have benign cutaneous disorders of the nasal skin. Advanced squamous cell carcinoma requiring total rhinectomy, often with extensive contiguous midface excision, was found in all patients. Three of these five patients died of their disease. In one patient, a red nose resembling acne rosacea was present. In the four others, chronic cellulitic or ulcerative cutaneous involvement was caused by squamous cell carcinoma. These cases emphasize the need for intranasal examinatio

[Surgical or conservative treatment in orbital complications of sinus inflammations (author's transl)].

47 patients with orbital complications resulting from acute sinusitis have been treated at the Freiburg ENT Department during the last seven years. Acute ethmoiditis predominated in children, while pansinusitis was more common in adults. Previous sinus surgery did not prevent orbital complications arising from the same sinus. Orbital involvement was classified into periostitis, subperiosteal abscess and orbital cellulitis. According to clinical findings, 44.7% of the patients were managed conservatively. Prognosis was best in periostitis, whereas consersative treatment was unsafe in orbital ce

Orbital lymphatic malformation showing the symptoms of orbital complications of acute rhinosinusitis in children: a report of 2 cases.

Orbital lymphatic malformations are benign cystic malformations of the lymphatic system. The present report shows two cases with symptoms of orbital complications of acute rhinosinusitis with proptosis, compressive optic neuropathy, loss of vision and cellulites in children. Magnetic resonance imaging (MRI) revealed a well-demarcated intraorbital mass with heterogeneous signal conformable with lymphatic malformation in both cases. A tumor extirpation was performed via lateral orbitotomy in both cases. Postoperatively the symptoms and especially the loss of vision improved completely. Histologi
