[Cement augmentation in pertrochanteric fractures : Evidence, indication and surgical application].

Cement augmentation of cephalomedullary implants was developed to improve anchorage of the head element in osteoporotic pertrochanteric fractures and to reduce implant-related failure patterns such as cut-out or excessive sliding. The aim of this review article is to provide a clinical classification of cement augmentation in pertrochanteric fractures, with particular focus on clinical and socioeconomic aspects. Biomechanical studies consistently demonstrate improved rotational stability and cut-out resistance of augmented cephalomedullary fixations. Clinically, several prospective studies sho

What is the role of vertebral augmentation for osteoporotic fractures? A review of the recent literature.

PURPOSE: Vertebral augmentation procedures such as vertebroplasty and kyphoplasty are utilized in the treatment of vertebral compression fractures (VCFs). However, their capacity for providing analgesia, reducing disability, and improving quality of life in patients with osteoporotic VCFs remains a topic of debate. The objective of this narrative review is to summarize the latest evidence for the safety and efficacy of vertebral augmentation for osteoporotic vertebral compression fractures (VCFs). METHODS: A systematic literature search was conducted using the PubMed and Cochrane electronic d

Osteosynthesis of a cranio-osteoplasty with a biodegradable magnesium plate system in miniature pigs.

UNLABELLED: Biodegradable magnesium alloys are a new class of implant material suitable for bone surgery. The aim of this study was to investigate plates and screws made of magnesium for osteosynthesis in comparison to titanium in a cranial fracture model. Implants were used for internal fixation of a cranio-osteoplasty in nine minipigs. Computed tomography was conducted repeatedly after surgery. The implants and the adjacent tissues were harvested 10, 20 and 30weeks after surgery and investigated by micro-computed tomography and histological analysis. The surgical procedure and the inserted o

Minimally invasive (percutaneous) treatment of metastatic spinal and extraspinal disease--a review.

Metastatic tumors are the most common malignancy of bone. Many patients with spinal metastases present with pain and pathologic fractures. The advent of interventional radiology resulted in alternative and less invasive treatment of these patients. This article presents minimally invasive (percutaneous) procedures that are currently in use, i.e. vertebroplasty, kyphoplasty, osteoplasty, radiofrequency ablation, cryoablation, and transarterial embolization. Indications, contraindications, results and complications are also discussed. According to our current knowledge of the results reported in

Effectiveness of Radiofrequency Ablation in the Treatment of Painful Osseous Metastases: A Correlation Meta-Analysis with Machine Learning Cluster Identification.

A systematic review and meta-analysis of pain response after radiofrequency (RF) ablation over time for osseous metastases was conducted in 2019. Analysis used a random-effects model with GOSH plots and meta-regression. Fourteen studies comprising 426 patients, most with recalcitrant pain, were identified. Median pain reduction after RF ablation was 67% over median follow-up of 24 weeks (R2 = -.66, 95% confidence interval -0.76 to -0.55, I2 = 71.24%, fail-safe N = 875) with 44% pain reduction within 1 week. A low-heterogeneity subgroup was identified with median pain reduction after RF ablatio

Surgical therapy of bone metastases.

The treatment of patients with cancer epitomizes the importance of using a collaborative team approach to optimize patient care. Physician team members most commonly are radiation oncologists, general surgeons, surgical oncologists, thoracic surgeons, neurosurgeons, and orthopedic surgeons. When patients are receiving chemotherapy, their medical oncologist frequently takes responsibility for coordinating care among the various team members and initiating consultations with necessary providers. When patients develop bone metastases or chemotherapy-induced bone loss (CTIBL), the orthopedic surge

Pulmonary cement embolism is not associated with the cause of death in a post-mortem cohort of cement-augmented interventions in the spine.

PURPOSE: Complications of cement-augmented interventions (e.g., kyphoplasty) in the spine include local cement leakage and pulmonary cement embolisms (PCE). This study was conducted to determine their extent in a unique post-mortem cohort. METHODS: Retrospective analysis of post-mortem whole-body CT scans and review of autopsy results in 29 consecutive cases with cement-augmented interventions in the spine. PCE findings were graded based on cement deposits: grade 0 (no PCE), grade 1 (1-3 PCE), grade 2 (4-6 PCE), and grade 3 (> 6 or branch-shaped PCE). Bone and lung tissue specimens were obtai

Pain management: The rising role of interventional oncology.

Patients with early or metastatic cancer may suffer from pain of different origins. The vast majority of these patients are not adequately treated by means of systemic analgesia and radiotherapy. Percutaneous neurolysis is performed using chemical agents or thermal energy upon sympathetic nervous system plexus for pain reduction and life quality improvement. Ablation and vertebral augmentation are included in clinical guidelines for metastatic disease. As far as the peripheral skeleton is concerned bone augmentation and stabilization can be performed by means of cement injection either solely

Indirect cerebral revascularization with a temporoparietal fascial flap in pediatric moyamoya patients: a novel technique and review of current surgical options.

Moyamoya syndrome is a progressive occlusive disease of the cerebral vessels. There are a variety of surgical treatments directed at revascularizing the ischemic brain in pediatric moyamoya disease. Many reports of varying success with both direct and indirect type of procedures can be found in medical literature. We present a novel technique, encephalo-TPF-synangiosis (ETS) with a pedicled bone flap, for indirect moyamoya revascularization in pediatric patients. A three-quarters osteoplastic temporal craniotomy was created. A pedicled temporoparietal fascial flap was passed intracranially thr

Rotational osteoplasty and bioabsorbable polylactate pin fixation in Pipkin type 2 fracture with acute osteochondral defect: a case report.

Pipkin fractures are relative rare high-energy lesions characterized by an intra-articular fracture of the femoral head after posterior hip dislocation. Early anatomic reduction and stable fixation are the main goals of treatment. This case evaluates the outcome of managing Pipkin type 2 fracture with acute osteochondral defect of the femoral head using "rotational osteoplasty" and bioabsorbable polylactate pin fixation. 24-year-old male patient was involved in a motorcycle accident, suffering from a left hip fracture-dislocation, and pelvic Computed Tomography revealed a Pipkin type 2 lesion.

Comparison of Percutaneous Techniques for Stabilization of Osteolytic Lesions of the Periacetabular Region Due to Metastatic Bone Disease.

Minimally invasive techniques for metastatic disease treatment in the periacetabular region of the pelvis have grown increasingly popular. Open surgical management of osteolytic lesions in the pelvis are associated with high perioperative complication rates that approach 30-36% and often require suspension of systemic treatment. Conservative measures, such as radiation treatment, can improve functional status in select patients while avoiding the morbidity of open surgical intervention; however, conservative measures do not address the problem of mechanical instability that can lead to patholo

Surgical Intervention for Spinal Lesions Due to Multiple Myeloma: A Case Report.

Vertebral disease is a main source of morbidity (MM) in individuals with multiple myeloma. The effects of associated osteolytic lesions and vertebral fractures on severe pain, functional limits, spinal deformity, and cord compression are well recognized. Systemic therapy, radiation, cementoplasty (vertebroplasty/kyphoplasty), and radiofrequency ablation are now available therapeutic options for severe MM spinal pain. We here reported a case of a 45-year-old male who had complained of progressive symptoms of pathological spine fractures. He had been examined and investigated for the cause of ly

Inflation osteoplasty: in vitro evaluation of a new technique for reducing depressed intra-articular fractures of the tibial plateau and distal radius.

BACKGROUND: Anatomic reduction of intra-articular fractures of the tibial plateau and distal radius can be difficult to achieve. Treatment goals are centered on restoring the native anatomy and articular congruency. Several surgeons in the USA have begun using an inflatable bone tamp to reduce these fracture patterns. The concept is built on the success of the tamp in kyphoplasty of the spine, but it has yet to be tested in the lab for use in the extremities. We performed an investigation into the safety and efficacy of using an inflatable bone tamp for intra-articular fracture reduction of th

Different polymethylmethacrylate (PMMA) reinforcement strategies for long bone osteoplasty procedures: a controlled laboratory comparison using the 4-point bending test.

BACKGROUND: Cementoplasty has been successfully used for treating fractures in various parts of the human body, although the use in weight-bearing long bones is a subject of controversial debate. Strategies to improve the mechanical properties of polymethylmethacrylate-based bone cement (BC) comprise changing the chemical composition or the application of metal reinforcement strategies. In clinical practice reinforced bone cement is used despite biomechanical basic research regarding this topic being scare. OBJECTIVE: The aim of the present study was to evaluate the biomechanical properties o
