An unusual intracerebral connective tissue mass.

A sharply circumscribed, vascular, connective tissue mass which replaced the cortex of several adjoining cerebral gyri is described. The histology of the lesion differs from that of previously reported central nervous system maldevelopments and neoplasms. However, there are a number of similarities to the pathology found in late phases of syphilitic "colloid" or "hyaline" degeneration of the brain.

Epidermal naevus and segmental hypermelanosis associated with an intraspinal mass: overlap between different mosaic neuroectodermal syndromes.

UNLABELLED: The epidermal naevus syndrome is a neurocutaneous syndrome characterised by the association of epidermal naevi with central nervous system, skeletal, ocular or cardiovascular abnormalities. We report on a 1.75-year-old boy who presented with spastic diparesis, a partial paresis of the left forearm and macrocephaly. He had a large epidermal naevus along the cervical spine and a segmental hypermelanosis. MRI studies revealed a large intraspinal mass extending from the lower cervical to the upper thoracic spine. The condition of our patient demonstrates the overlap of the epidermal na

[Primary leiomyosarcoma of the pineal gland--a case report].

Primary sarcoma in the central nervous system is rare. From 1954 to 1986, 1417 neoplasms of the central nervous system were treated in our hospital and only one case of primary leiomyosarcoma of the pineal gland was seen. The patient, a forty-seven year old man, had diplopia for 2 months, headache and ataxia for 2 weeks before admission. The CT scan showed a space occupying lesion in the pineal gland. During operation, a mass, 3 x 2.5 x 2 cm in size, was found in the pineal and subtotal resection was performed. The pathological diagnosis of primary leiomyosarcoma of the pineal gland was establ

Neurologic disorders associated with AIDS retroviral infection.

Neurologic disease occurs frequently in patients infected with the human immunodeficiency virus (HIV), and a large body of literature now exists detailing the various infections, neoplasms, and other conditions that can affect the central nervous system (CNS) or the peripheral nervous system in children and adults with AIDS, persistent generalized lymphadenopathy, or (in some cases) only serologic evidence of retroviral infection. Although certain opportunistic infections (toxoplasmosis, cryptococcosis, progressive multifocal leukoencephalopathy, and herpesviral infections) and CNS lymphomas o

P14.02 Proton radiation therapy combined with immunomodulatory drugs for central nervous system invasive multiple myeloma: case report and review of the literature

Abstract Central nervous system invasion of multiple myeloma is a rare extra-medullary form of the disease, presenting as intraparenchymal lesions, leptomeningeal disease, or via direct dural invasion. Survival of leptomeningeal myelomatosis in historical series is considered to be extremely poor, but seems to be improving in the era of novel therapy, namely proteosome inhibitors and immunomodulatory class drugs. Here we present the case of a 48 year-old male with isolated central nervous system relapse of multiple myeloma who had multiple rapidly progressive intraparenchymal myeloma lesions,

Intradural Carcinoid Tumor Found in a Patient with No History of Cancer.

BACKGROUND: Carcinoid tumors are rare neoplasms that often arise from the gastrointestinal or respiratory tracts. They often metastasize to bone tissue and pancreatic and hepatic sites. The central nervous system and most specifically the spinal cord are rarely involved. Primary carcinoid tumors of the central nervous system are even rarer. CASE DESCRIPTION: A 58-year-old man presented with progressive bilateral lower extremity weakness and a negative history of cancer. Imaging revealed an intradural mass at the L1 and L2 spinal levels. Surgical resection of the tumor via laminectomy was perf

Multiple Sclerosis and the Choroid Plexus: Emerging Concepts of Disease Immunopathophysiology.

BACKGROUND: The coexistence of multiple sclerosis and intracranial neoplasms is very rare, and whether this occurrence can be explained by a causal relationship or by coincidence remains a matter of debate. Possible roles of the choroid plexus as a site of tumor cell invasion and lymphocyte infiltration into the central nervous system have been hypothesized in recent studies. METHODS: We describe a 13-year-old boy with concurrent multiple sclerosis and choroid plexus papilloma, then review the published literature with a focus on the pathophysiologic mechanisms of neuroinflammation in multipl

[Presenting signs and symptoms of central nervous system tumors according to age].

INTRODUCTION: Central nervous system tumors are the second most frequent group of neoplasms in children and are the main cause of cancer-related deaths in the pediatric population. There are no specific clinical findings. Consequently clinical data that would aid diagnosis are required. OBJECTIVE: To establish the presenting signs and symptoms of brain tumors in childhood according to age and identify their influence on diagnostic delay. PATIENTS AND METHODS: We retrospectively studied 69 patients with central nervous system tumors diagnosed between January 1997 and December 2002 in the Virg

Central nervous system and head and neck imaging findings of MYC/BCL2 "double hit" B cell lymphoma.

MYC/BCL2 double hit lymphoma (DHL) is a rare, recently recognised and highly aggressive subtype of non-Hodgkin lymphoma, with an affinity to involve the central nervous system and the head and neck either at initial presentation or during relapse. We present a case of 43-year-old woman with MYC/BCL2 DHL relapse in the nasopharynx with extensive spread to the neck, skull base, and the central nervous system. To our knowledge, this is the first reported case in the literature describing the MRI and CT scan findings and the profound pattern of disease involvement of this rare neoplasm.

Presumptive primary central nervous system T cell lymphoma in a deer (Capreolus capreolus).

This report describes the morphological and immunohistochemical findings in an approximately 7-year old wild European roe deer (Capreolus capreolus) with an infiltrative primary central nervous system T cell lymphoma. The roebuck had lost its timidity and was shot by a hunter. It was submitted for necropsy to exclude rabies. Gross examination revealed besides a verminous pneumonia no abnormal findings. Histologically, there was a highly cellular, not well demarcated, unencapsulated, and infiltrative neoplasm within basal parts of the brain. Tumour cells were round, up to 15 μm in diameter and

Central nervous system cancers.

Primary and metastatic tumors of the central nervous system are a heterogeneous group of neoplasms with varied outcomes and management strategies. Recently, improved survival observed in 2 randomized clinical trials established combined chemotherapy and radiation as the new standard for treating patients with pure or mixed anaplastic oligodendroglioma harboring the 1p/19q codeletion. For metastatic disease, increasing evidence supports the efficacy of stereotactic radiosurgery in treating patients with multiple metastatic lesions but low overall tumor volume. These guidelines provide recommend

Neurofilament proteins and human nervous system tumors.

Neoplasms that arise in the peripheral (e.g., carotid body tumors, neuroblastomas, pheochromocytomas) or central (gangliocytomas, medulloblastomas) nervous system express a number of neuron-specific gene products. Presumably, these tumors are derived from precursor cells that are or have the potential to develop into neurons or neuron-like cells. This report provides a critical examination of the hypothesis that cytoskeletal proteins of normal neurons, in particular the neuron-specific class of intermediate filaments (neurofilaments), are present but are abnormal in neoplasms derived from neur

Primary central nervous system histiocytic sarcoma with relapse to mediastinum: a case report and review of the literature.

Histiocytic sarcoma is a rare, malignant neoplasm of the lymphohematopoietic system that usually occurs in the skin, lymph node, and intestinal tract. Here we describe a unique case of primary central nervous system histiocytic sarcoma that initially showed an indolent clinical course following local resection and radiotherapy. However, relapse of disease within the mediastinum was noted 3 1/2 years later. Biopsies of the initial brain lesion and subsequent mediastinal recurrence each revealed an identical, diffuse proliferation of histiocytes with expression of CD45, CD68, and CD163 but not p

How I prevent and treat central nervous system disease in adults with acute lymphoblastic leukemia.

The central nervous system (CNS) is the most important site of extramedullary disease in adults with acute lymphoblastic leukemia (ALL). Although CNS disease is identified only in a minority of patients at the time of diagnosis, subsequent CNS relapses (either isolated or concurrent with other sites) occur in some patients even after the delivery of prophylactic therapy targeted to the CNS. Historically, prophylaxis against CNS disease has included intrathecal (IT) chemotherapy and radiotherapy (RT), although the latter is being used with decreasing frequency. Treatment of a CNS relapse usuall

Cavernous Malformation Associated With Arterialized Developmental Venous Anomaly: A Case Report.

BACKGROUND AND IMPORTANCE: Formation of cavernous malformations (CMs) has been recognized to be associated with developmental venous anomaly (DVA) by many authors. Hemodynamic stress due to venous outflow restriction could be hypothesized as a cause. On the other hand, a rare subgroup of DVA with an arterial component has been reported as likely to hemorrhage or be symptomatic. Cases of arterialized DVAs reported previously have not been associated with the presence of CM. CLINICAL PRESENTATION: We present herein a case report of arterialized DVA in the brainstem with repeated cerebellar hemo

Review of research advances in the cerebral lymphatic system and neurological disorders.

BACKGROUND: The identification of the central lymphatic system represents a significant advancement in contemporary neuroscientific research. As a vital conduit connecting the central nervous system to the peripheral lymphatic network, the primary components of the central lymphatic system, the glymphatic system and meningeal lymphatic vessels, maintain homeostasis within the central microenvironment through cerebrospinal fluid circulation, metabolic waste clearance, and immune cell trafficking. Recent studies affirm that structural and functional perturbations within the central lymphatic app

Paraneoplastic disorders of the central and peripheral nervous systems.

Paraneoplatic neurologic syndromes (PNS) have been seminally defined as acute or subacute neurological syndromes resulting from nervous system dysfunction that is remote from the site of a malignant neoplasm or its metastases. However, in respect to our current understanding of their pathogenesis we may redefine these disorders as cancer-related dysimmune neurologic syndromes. We first deal with the epidemiology and the pathogenesis of PNS, then the different classic PNS are reviewed with clinical features according to the associated onconeuronal antibodies. Finally, therapeutic approaches are

Analysis of False-negative Findings of the Incomparable Accuracy and Swiftness of Flow Cytometric Diagnosis of Primary Central Nervous System Lymphoma.

Primary central nervous system lymphoma (PCNSL), a relatively rare brain tumor, bears a dire prognosis. On occasion, the rapid progression of the tumor makes immediate diagnosis and initiation of therapy imperative. To achieve swift diagnosis, we adopt flow cytometry (FCM) in addition to conventional histopathology. This study aimed to reveal the utility of FCM diagnosis for PCNSL and the cause of false-negative results of FCM diagnosis. We investigated 33 patients with suspected PCNSL on neuroradiological findings and received both FCM and histological diagnosis. The patients' electronic medi
