Intravenous Cerebral Cone Beam Computed Tomography vs. Diagnostic Intraarterial Cerebral Angiography: A Cost Comparison.

INTRODUCTION: Whilst intra-arterial (IA) cerebral angiography is a well-established diagnostic tool, the use of intravenous (IV) cerebral cone beam computed tomography (CBCT) is a comparatively new diagnostic method that is gaining popularity in the interventional neuroradiology (NIR) community. With the cost of healthcare being central to the delivery of modern medicine, this study aimed to further understand this shift by investigating the procedural cost of intravenous cerebral CBCT with IA cerebral angiography. METHODS: Retrospective cohort study of 40 patients imaged over a four-month pe

Cerebral hemodynamics in Moyamoya disease: correlation between perfusion-weighted MR imaging and cerebral angiography.

BACKGROUND AND PURPOSE: In Moyamoya disease, the relationship between cerebral hemodynamics and angiographic findings has not been fully evaluated. The purpose of this study is to evaluate hemodynamics in Moyamoya disease with perfusion-weighted MR imaging (PWI) and cerebral angiography. METHODS: Twenty patients with Moyamoya disease were the subjects. Mean transit time (MTT) derived from PWI was calculated in the medial frontal lobes, the posterior frontal lobes, the occipital lobes, and the basal ganglia. From the angiographies, we classified the degrees of internal carotid artery (ICA) and

Optimization of image shoot timing for cerebral veins 3D-digital subtraction angiography by interventional angiography systems.

3D-digital subtraction angiography (3D-DSA) is essential for understanding the anatomical structure of cerebral veins, crucial in brain tumor surgery. 3D-DSA produces three-dimensional images of veins by adjusting the X-ray delay time after contrast agent injection, but the delineation of veins varies with the delay in X-ray timing. Our study aimed to refine the delay time using time-enhancement curve (TEC) analysis from 2D-DSA conducted before 3D-DSA imaging. We retrospectively reviewed 26 meningioma patients who underwent cerebral angiography from March 2020 to August 2021. Using 2D-DSA, we

[Capacities of angiography in the imaging of abnormal changes in the cerebral arteries].

The study was based on the angiographic examination of 233 patients with prior subarachnoidal hemorrhage. Angiographic study was performed using the Seldinger technique by contrasting both carotid and vertebral arteries. Twenty-three patients in whom arterial aneurysm had been detected by digital subtraction angiography underwent 3D angiography. The authors improved a procedure during which a contrast agent was manually injected into the internal carotid or vertebral artery, by using a 20-ml disposal syringe with controlled maximum developed pressure and flow increase rate up to 2.0 ml/sec for

Normal variants of the cerebral circulation at multidetector CT angiography.

Multidetector computed tomographic (CT) angiography is used in many institutions for initial evaluation of the cerebral circulation for acute stroke and subarachnoid hemorrhage as well as in various preoperative settings. A comprehensive CT examination that includes a review of three-dimensional and maximum intensity projection images of the intra- and extracranial arteries and axial images of the skull base (obtained with bone window settings) allows identification of most abnormalities and normal variants. Knowledge of the presence and clinical relevance of normal variants such as fenestrati

Fenestration of the middle cerebral artery detected by MR angiography.

Unlike fenestration of the posterior cerebral arterial circulation, fenestration of the anterior cerebral arterial circulation has not been well described. We investigated the location and configuration of fenestration of the middle cerebral artery (MCA) detected by magnetic resonance (MR) angiography. We found 6 fenestrations of the MCA among cranial MR angiography images obtained from about 2,000 patients during the past 9 years at our institution using either of two 1.5T imagers. All images were obtained by the three-dimensional time-of-flight technique. Maximum-intensity projection images

Role of intraoperative angiography in the surgical treatment of cerebral aneurysms.

UNLABELLED: The benefit of using intraoperative angiography (IA) during aneurysm surgery is still uncertain. OBJECT: In this prospective study, the authors evaluate the radiographically demonstrated success of surgical treatment in 151 consecutive patients harboring 173 aneurysms who selectively underwent IA examination. The authors also assess the frequency with which IA led to repositioning of the aneurysm clip. METHODS: Intraoperative angiography was used selectively in this series, based on the surgeon's concern about the potential for residual aneurysm, distal branch occlusion, or paren

Cerebral CT angiography in the diagnosis of acute subarachnoid hemorrhage.

PURPOSE: To evaluate the usefulness of CT angiography (CTA) in the detection of intracranial aneurysms in patients with acute subarachnoid hemorrhage (SAH). MATERIAL AND METHODS: In 53 patients with nontraumatic SAH a helical contrast-enhanced CTA was performed. CTA data were reconstructed with maximum intensity projection (MIP). Each patient underwent selective arteriography of the cerebral vessels (as the gold standard). CTA (axial images and MIP reconstructions) and arteriography were evaluated separately and their diagnostic information was compared. RESULTS: In 14 of the 53 patients nei

[Cerebellar infarction; the findings of CT, MRI and cerebral angiography].

The findings of CT, MRI and cerebral angiography were analysed in 18 cases with cerebellar infarction, of which 10 were men and 8 women. The areas of infarction were as follows; PICA in 9 cases, SCA in 8 cases, AICA and diffuse type in one case each. One case had two lesions. CT showed low density areas in 15 cases after 24 hours of the onset, but an abnormality was not demonstrated in one case which had an infarction of PICA area. MRI showed a high-intensity lesion on T2 weighted image during the first and tenth day after the onset, while a low-intensity lesion on T1 weighted image and high-i

[Digital subtraction angiography of the cerebral vessels by intraarterial injection].

Three hundred and fifty-seven digital subtraction angiography (DSA) were performed in 184 neurosurgical patients by intraarterial injection. Examinations consisted of 192 carotid angiography, 110 vertebral angiography, 23 aortography, 11 spinal angiography and 21 other angiography. In all examinations, visualization of the vessels was excellent and the complications were never experienced. High contrast sensitivity of DSA resulted in better visualization of tumor stains, phlebogram, and arteries in cerebral arteriovenous malformations with large shunt blood flow than conventional angiography.

A New Technique for the Assessment of Cerebral Vasodilatory Capacity as Part of Catheter-Based Cerebral Angiography.

BACKGROUND: Previous studies have demonstrated the value of cerebral vasodilatory capacity assessment for risk stratification in patients with extracranial arterial stenosis or occlusion. We describe a new method that assesses cerebral vasodilatory capacity as part of catheter-based cerebral angiography. METHODS: We assessed regional cerebral blood volume (rCBV) in the arterial distribution of interest using a controlled contrast injection through a diagnostic catheter placed in the common carotid or the subclavian artery. rCBV maps were created using predefined algorithm based on contrast di

[Sequential change of cerebral angiography in a case of cerebral angiitis following herpes zoster ophthalmicus].

Delayed neurological symptoms and signs following herpes zoster ophthalmicus (HZO) such as "Delayed contralateral hemiplegia with HZO" are supposed to be due to ipsilateral intracranial angiitis and ischemic disorder. We experienced a rare case with ipsilateral cerebral hemorrhage following HZO. Under the diagnosis of cerebral angiitis associated with HZO, we treated her conservatively and observed sequential change of angiography for four months. A 54-year-old female, who had been treated for systemic lupus erythematosus (SLE), developed HZO on left ophthalmic nerve area. Seven weeks after th

Usefulness of CT angiography for characterizing cerebral arteriovenous malformations presenting as hemorrhage: comparison with digital subtraction angiography.

OBJECTIVE: To compare the usefulness of CT angiography against the gold standard, digital subtraction angiography (DSA), in the characterization of cerebral arteriovenous malformations (AVM) that present with bleeding. MATERIAL AND METHODS: We retrospectively analyzed patients with intracranial bleeding due to an AVM who were included in a prospective database in the period comprising January 2007 through December 2012. We reviewed radiologic variables such as the characteristics of the AVM (size, location, presence of deep venous drainage), involvement of eloquent areas, and the presence of

Radial Artery Access for Cerebral Angiography: 2-Dimensional Operative Video.

A number of studies have demonstrated that the radial artery is a safer access site than the femoral artery for endovascular procedures.1-4 In the prospective randomized studies comparing transradial and transfemoral approaches for cardiac procedures, there was a 60% reduction in access site complications as well as significant decreases in all-cause mortality with the transradial approach as compared to transfemoral, which has led to the adoption of a radial first strategy.5-7 Neurointerventional studies have demonstrated similar safety benefits as well as improved patient preference.8-14 In
