White matter integrity of the cerebellar peduncles as a mediator of effects of prenatal alcohol exposure on eyeblink conditioning.

Fetal alcohol spectrum disorders (FASD) are characterized by a range of neurodevelopmental deficits that result from prenatal exposure to alcohol. These can include cognitive, behavioural, and neurological impairment, as well as structural and functional brain damage. Eyeblink conditioning (EBC) is among the most sensitive endpoints affected in FASD. The cerebellar peduncles, large bundles of myelinated nerve fibers that connect the cerebellum to the brainstem, constitute the principal white matter element of the EBC circuit. Diffusion tensor imaging (DTI) is used to assess white matter integr

Microsurgical anatomy of the cerebral aqueduct and periaqueductal region: implications for microscopic and neuroendoscopic approaches.

PURPOSE: The cerebral aqueduct (CA) is the narrowest part of the ventricular system, connecting the third and fourth ventricles. This structure is encircled by important neuroanatomical features, and with the increasing application of neuroendoscopic techniques, a comprehensive understanding of the anatomy of the CA is essential. This study aims to provide a comprehensive, multi-perspective anatomical analysis of the cerebral aqueduct and its surrounding structures, integrating microsurgical dissections with endoscopic anatomical correlations to support safer surgical planning. METHODS: Three

Structural and functional changes of the cerebellum in temporal lobe epilepsy.

AIMS: This study aimed to comprehensively explore the cerebellar structural and functional changes in temporal lobe epilepsy (TLE) and its association with clinical information. METHODS: The SUIT toolbox was utilized to perform cerebellar volume and diffusion analysis. In addition, we extracted the average diffusion values of cerebellar peduncle tracts to investigate microstructure alterations. Seed-based whole-brain analysis was used to investigate cerebellar-cerebral functional connectivity (FC). Subgroup analyses were performed to identify the cerebellar participation in TLE with/without h

Acute Marchiafava-Bignami disease with widespread callosal and cortical lesions.

Marchiafava-Bignami disease (MBD) is a rare alcohol-related disorder that results in progressive demyelination and necrosis of the corpus callosum. The process may extend to the optic chiasm and tracts, cerebellar peduncle, subcortical resion, neighboring white matter, and rarely, cortical gray matter. We report a case of MBD in which fluid-attenuated inversion recovery and diffusion magnetic resonance imaging studies revealed symmetrical hyperintense lesions in the cerebral cortex in addition to the callosal lesions.

Arm chorea secondary to an unruptured giant aneurysm.

We describe the case of a 20-year-old male who developed right-arm choreic movements secondary to a giant unruptured aneurysm impinging upon the left thalamus, putamen, globus pallidus, cerebral peduncle, midbrain, and subthalamic nucleus. The aneurysm was treated successfully with coils and a supraclinoid balloon. Abnormal movements initially failed to ameliorate, but within a few months, it was possible to discontinue symptomatic haloperidol therapy, with only mild residual abnormal movements.

Computed tomography-guided stereotactic aspiration and biopsy of a cystic metastatic adenocarcinoma of the pons.

We herein report a case of adenocarcinoma of the right main bronchus disseminated to the pons, left cerebral peduncle, and liver. Computed tomography-guided Cosman-Robert-Wells stereotactic aspiration of the cystic pontine lesion was performed through a transoccipital, transtentorial route and a catheter inserted in the cyst cavity that was connected to a subgaleal Ommaya reservoir for further aspiration and decompression. The choice of this approach to the lesion is briefly discussed.

Subacute sclerosing panencephalitis: brain stem involvement in a peculiar pattern.

The most common pattern in subacute sclerosing panencephalitis, is in the cerebral hemisphere white matter on T2-weighted images with or without atrophy. Brain-stem lesions are rare. We report brain-stem involvement in two children with subacute sclerosing panencephalitis. A peculiar pattern, with involvement of the pons with extension to both middle cerebellar peduncles and substantia nigra but sparing the pontine tegmentum, is suggested.

Locked-in syndrome after head and neck trauma.

Ten patients became locked-in after head and neck trauma. Five had a prompt onset due to either direct trauma or secondary infarction of the brainstem. A delay of 6 to 48 hours in four other patients was probably the interval between vertebral artery damage and vertebrobasilar arterial occlusion. The last patient sustained ischemia and compression of the cerebral peduncles from tentorial herniation.

A case of CLCN2-related leukoencephalopathy with bright tree appearance during aseptic meningitis.

CLCN2-related leukoencephalopathy (CC2L) is a rare autosomal recessive disorder caused by variants in CLCN2. We report a boy whose brain MRI during an episode of aseptic meningitis at the age of 6 years revealed wide areas of restriction on diffusion-weighted images (DWI) in the cerebral subcortical white matter called bright tree appearance (BTA). In addition to the BTA, high intensity signals were also observed bilaterally in the posterior limbs of the internal capsules, cerebral peduncles, middle cerebellar peduncles, cerebellar white matter, and brain stem (longitudinal pontine bundle) alo

The Kinetics of Transtentorial Brain Herniation: Kernohan-Woltman Notch Phenomenon Revisited.

PURPOSE OF REVIEW: To critically review recent literature in understanding the pathological consequences of transtentorial brain herniation resulting from unilateral expanding supratentorial mass lesions. RECENT FINDINGS: Modern neuroimaging assists in understanding the consequences of transtentorial brain herniation with the development of the Kernohan-Woltman notch phenomenon. MRI studies in post-operative patients undergoing craniotomy and removal of expanding unilateral hemispheric mass lesions (usually an extradural or subdural hematoma) have shown striking findings in the contralateral

[Microsurgical anatomy of the anterior choroidal artery].

Microdissection of 100 hemispheres from human cadavers were performed in order to study the anatomic characteristics of the anterior choroidal artery (AChA). One AChA per hemisphere was found. In 98% of hemispheres the AChA arose from the internal carotid artery (ACI) 2.4mm distal to the origin of the posterior communicating artery (ACoP) and 4.7mm proximal to the carotid bifurcation. One or more perforating branches arose from communicating segment of ACI in 29% of hemispheres. The average calibre of the cisternal portion was 0.9mm and the plexal portion 0.7mm. The most frequent branches of t

[A case of bilateral cerebellar peduncle infarction with bilateral hearing impairment of a sudden onset].

We reported a patient with bilateral cerebellar peduncle infarcts who had an abrupt onset of bilateral hearing loss. A hypertensive 56-year-old man suddenly experienced bilateral hearing loss without other accompanying neurological deficits. He was hospitalized and treated for "idiopathic deafness". In addition, dysarthria and ataxic gait appeared two days later and he was transferred to our hospital. On neurological examination, the patient presented with diplopia, neurosensory hearing loss (approximately 70 dB) ataxic dysarthria, bilateral cerebellar ataxia and bilateral Babinski's signs. Au

Diagnosis of descending transtentorial herniation by cranial computed tomography.

Descending transtentorial herniartion (DTH) secondary to a supratentorial mass effect can be detected by CT. Early signs of uncal herniation include encroachment on the suprasellar cistern, displacement of the brain stem, enlargement of the ipsilateral crural subarachnoid space, and compression of the contralateral cerebral peduncle. Increasingly severe combined uncal and hippocampal DTH results in progressive obliteration of the suprasellar and interpeduncular cisterns, elongation or compression of the brain stem, and inferior or posterior displacement of the basilar artery.

Intracranial and dermatological cryptococcal infection in an immunocompetent man.

We report the case of a 72-year-old man with a history of Parkinson's Disease who presented with a history of falls, cognitive impairment and depressed mood. Neurological examination revealed moderate rigidity and bradykinesia. Alterations to his anti-Parkinsonian medication resulted in improvements in his mobility but no change in mental state. He was also noted to have developed multiple raised skin lesions. One of these lesions was biopsied and grew Cryptococcus neoformans. He subsequently had a cerebral MRI scan which showed enhancing lesions, in the left parietal lobe and right superior c

Neurogenic diabetes insipidus in a sheep.

A 7-month-old Suffolk-cross ram was examined because of polydipsia and polyuria of 2 months" duration. Neurogenic diabetes insipidus was diagnosed on the basis of failure to concentrate urine of low specific gravity in response to water deprivation and a positive response to antidiuretic hormone administration. Post-mortem examination of the brain revealed degeneration in the cerebral peduncles, an absence of pituitary neural tissue and evidence suggesting external pressure as the cause.

Kernohan's notch phenomenon demonstrated by diffusion tensor imaging and transcranial magnetic stimulation.

Kernohan's notch phenomenon is the ipsilateral hemiplegia caused by compression of the contralateral cerebral peduncle against the tentorial edge by a supratentorial mass. Diffusion tensor imaging (DTI) and transcranial magnetic stimulation (TMS) could be useful for exploring the state of the corticospinal tract (CST). This report attempts to demonstrate Kernohan's notch phenomenon in a patient with subdural haematoma by using DTI and TMS. One patient and six normal control subjects were recruited. The patient showed severe right hemiplegia even though the subdural haematoma was located in the

Vascular Parkinsonism: a case report and review of the literature.

Vascular Parkinsonism (VP) is characterised by sudden onset and rapid progression of clinical symptoms, absent or poor response to dopamine substitution therapy, and postural instability with shuffling gait and absence of tremor, making it a clinically distinct entity from idiopathic Parkinson's disease (IPD). Furthermore, it displays certain typical findings in neurological investigations. We report on a patient presenting features of VP associated with an intracerebral lesion not ascribed to VP to date, namely an isolated ischaemic focal lesion located in the left cerebral peduncle between t
