Surgical management of cervical cord injury with ossification of the cervical posterior longitudinal ligament.

OBJECTIVE: To identify an appropriate surgical approach for the management of cervical cord injury with ossification of the posterior longitudinal ligament. METHODS: A retrospective study of 25 cases of cervical cord injury with ossification of the posterior longitudinal ligament was performed. Two cases were classified as Frankel grade A, three as grade B, fourteen as grade C, and six as grade D. Treatment procedures consisted of anterior decompression with instrumentation (twelve patients), posterior decompression (eight patients), and combined anterior and posterior decompression (five pat

Trigemino-cervical reflex in pathology of the brain stem and of the first cervical cord segments.

The trigemino-cervical reflex (CR) sensitivity and specificity in the assessment of cervico-bulbar lesions was evaluated in eleven selected subjects (4 multiple sclerosis, 3 tumoural processes and 4 cervical cord spondilogenetic myelopathies). Results were compared with those obtained from others neurophysiological investigations (EMG and blink-reflex) and with neuroimaging findings. Significant abnormal responses of CR were found in all patients varying from absent responses, to delayed or reduced amplitude patterns. These data stress the utility of CR in the check of cervico-bulbar lesions,

Effects of cervical rotatory manipulation on the cervical spinal cord: a finite element study.

BACKGROUND: Little information is available concerning the biomechanism involved in the spinal cord injury after cervical rotatory manipulation (CRM). The primary purpose of this study was to explore the biomechanical and kinematic effects of CRM on a healthy spinal cord. METHODS: A finite element (FE) model of the basilaris cranii, C1-C7 vertebral bodies, nerve root complex and vertebral canal contents was constructed and validated against in vivo and in vitro published data. The FE model simulated CRM in the flexion, extension and neutral positions. The stress distribution, forma and relati

The role of decompression for acute incomplete cervical spinal cord injury in cervical spondylosis.

STUDY DESIGN: A prospective study was conducted in 37 patients with cervical spondylosis with incomplete cord injury to assess the role of decompression in these patients. OBJECTIVES: To evaluate surgical and nonsurgical outcomes in patients with pre-existing cervical spondylosis of the spine, in whom an incomplete spinal cord syndrome developed after a minor neck injury. SUMMARY OF BACKGROUND DATA: The benefits of surgical treatment of incomplete cord injury with cervical spondylosis is controversial but remains a treatment option. The results of this study clarified the benefits of surgery

Cervical Cord-Canal Mismatch: A New Method for Identifying Predisposition to Spinal Cord Injury.

The risk for spinal cord injuries (SCIs) ranging from devastating traumatic injuries, compression because of degenerative pathology, and neurapraxia is increased in patients with congenital spinal stenosis. Classical diagnostic criteria include an absolute anteroposterior diameter of <12-13 mm or a Torg-Pavlov ratio of <0.80-0.82; however, these factors do not take into account the size of the spinal cord, which varies across patients, independent of canal size. Recent large magnetic resonance imaging studies of population cohorts have allowed newer methods to emerge that account for both cord

Characteristics of the cervical spine and cervical cord injuries in older adults with cervical ossification of the posterior longitudinal ligament.

Although the incidence of cervical spinal cord injury (CSCI) with ossification of the posterior longitudinal ligament (OPLL) has increased in older adults, its etiology and neurological outcomes remain unknown. We identified OPLL characteristics and determined whether they influence neurological severity and improvement of CSCI in older patients. This multicenter retrospective cohort study identified 1512 patients aged ≥ 65 years diagnosed with CSCI on admission during 2010-2020. We analyzed CSCI etiology in OPLL patients. We performed propensity score-adjusted analyses to compare neurological

Cervical spinal cord injury following near-hanging without associated cervical fracture: a case report.

BACKGROUND: Hanging injuries are primarily associated with hypoxic-ischemic brain damage due to vascular compression, rather than airway obstruction. Spinal cord injury in this context is exceedingly rare, particularly when no bony abnormality is present. Only a limited number of cases of cervical spinal cord injury after hanging have been reported, most without radiographic abnormalities resembling spinal cord injury without radiographic abnormality (SCIWORA). We present an additional rare case of cervical spinal cord injury following near-hanging in an elderly patient, focusing on diagnostic

[Cervical Cord/Root Decompression:Posterior Approach/Variation of Surgical Procedure].

Posterior cervical decompression surgery is safe and effective. It was developed to safely and reliably decompress nerve tissues. Maximising the reconstruction and maintenance of the posterior neck tissue has been reported and developed. Complete reconstruction of the posterior musculoskeletal unit can minimise postoperative complications and spinal alignment changes. Here, we introduced laminoplasty which completely preserved the posterior cervical spinal tissue, and discussed its variations. Although posterior cervical decompression is a basic and safe surgical technique that can be used in

Blood-spinal cord barrier disruption in degenerative cervical myelopathy.

Degenerative cervical myelopathy (DCM) is the most prevalent cause of spinal cord dysfunction in the aging population. Significant neurological deficits may result from a delayed diagnosis as well as inadequate neurological recovery following surgical decompression. Here, we review the pathophysiology of DCM with an emphasis on how blood-spinal cord barrier (BSCB) disruption is a critical yet neglected pathological feature affecting prognosis. In patients suffering from DCM, compromise of the BSCB is evidenced by elevated cerebrospinal fluid (CSF) to serum protein ratios and abnormal contrast-

Delayed Bilateral Vocal Cord Paralysis Following Cervical Spine Trauma.

Bilateral vocal cord paralysis is a potentially life-threatening condition, depending on the position in which the vocal cords are paralyzed. When the vocal cords are fixed in adduction, patients develop respiratory distress, inspiratory stridor, aspiration, and minimal phonation deficits. This condition can result from acute injuries to the right and left recurrent laryngeal nerves, or from chronic bilateral recurrent laryngeal nerve palsy. The clinical presentation is variable with such nerve injuries. Traumatic injuries to the cervical spine are an uncommon cause of this condition. In this

Cervical spinal cord stimulator trial complicated by epidural abscess.

BACK GROUND: Spinal cord stimulation (SCS) is a growing interventional treatment modality in patients experiencing intractable pain refractory to conservative treatments. Many patients with chronic low back and leg pain that persists after surgery have found pain relief, and more evidence is suggesting that chronic upper limb and neck pain may respond just as well to this therapy. However, the placement of foreign body, for instance SCS leads, in the epidural space can become the source for deep intra-spinal infection. CASE REPORT: We present a 49-year-old robust male who underwent a temporar

Traumatic Ventral Cervical Spinal Cord Herniation: A Case Report.

Spinal cord herniation (SCH) is an uncommon traumatic event that should be considered in patients with vertebral fractures who develop an unusual constellation of autonomic and motor deficits. Herein, we describe a case of rapidly deteriorating neurological function following cervical spine fracture including sequelae such as bilateral lower-extremity weakness, loss of upper extremity motor function, and priapism. Decompression of the spinal cord allowed for the identification of the unusual herniation of the spinal cord and prevention of any further worsening of the neurological injury. Hyper

Delayed spinal cord infarction following anterior cervical surgical decompression.

Anterior cervical discectomy and fusion (ACDF) for cord compression is a safe and effective procedure with good outcomes. However, worsening of myelopathy is the most feared adverse event of the surgery. We report the case of a 36-year-old male patient who presented with an acute non-traumatic C5-6 cervical disc herniation causing incomplete quadriparesis. He underwent an uncomplicated ACDF at C5-6, and after an initial period of improvement, he developed a delayed onset of an anterior cord syndrome on day 3, without any discerning cause. We have reviewed similar cases reported in the literatu

Continuous Cervical Epidural Analgesia in Metastatic Spinal Cord Compression.

Metastatic spinal cord compression is a devastating complication of cancer. Patients may often require high doses of opioids that may cause side effects, myoclonus being one such. A 63-year-old male suffering from malignant spinal cord compression was admitted to our institution. The primary team managed him conservatively with pharmacotherapy with no relief of pain, and he experienced myoclonus and sedation as adverse effects. A continuous cervical epidural catheter with local anesthetic infusion was inserted for 5 days to control his pain. This relieved his pain, which was sustained even aft

Forelimb muscle plasticity following unilateral cervical spinal cord injury.

INTRODUCTION: Motor dysfunction and muscle atrophy are well documented in the lower extremity after spinal cord injury. However, the extent and time course of myoplastic changes in forelimb musculature is not clear. METHODS: Forelimb muscle morphology and fiber type were evaluated after high cervical hemilesion injury in rats. RESULTS: There was significant atrophy of the ipsilateral extensor carpi radialis longus (ECRL) muscle at 2 weeks postinjury, which was subsequently reversed at 8 weeks postinjury. The triceps muscle showed minimal evidence of atrophy after spinal injury. No significan
