Recurrent arterial and new-onset neurogenic thoracic outlet syndrome as a complication after previously inadequately excised first and cervical ribs.

Thoracic outlet syndrome commonly presents with the neurogenic subtype and can be caused in rare circumstances by an anatomic abnormality such as a cervical rib, for which surgical excision is the mainstay of management. An inadequately excised first or cervical rib can result in recurrent symptoms. We have reported the case of a 30-year-old woman who had presented with symptoms of right recurrent arterial and neurogenic thoracic outlet syndrome. She underwent repeat right-sided first rib and cervical rib resection with brachial plexus neural lysis and right carotid-axillary bypass via a combi

Management of cervical ribs causing neurogenic thoracic outlet syndrome: a ten year experience in the neurosurgery unit, Tikur Anbessa Hospital.

Eighteen cases of cervical Ribs causing numbness, paraesthesia and wasting of hand muscles were seen during the years 1990 to 1999 in neurosurgery unit of Tikur Anbessa, a central, referral teaching hospital. There were thirteen cases of unilateral, and 5 bilateral brachialgic pains M:F = 1:2 with ages range from 15-50 yrs (mean age 32 +/- 4.8 yrs); though it is 3 times common in women specially those with neurologic symptoms. 5 cases didn't have any cervical ribs. All had respective cervical rib resections and scalenotomy done. Paraesthesia and pain were relieved in about 90% of the cases. Ce

Distal arterial reconstruction using Esmarch's bandage technique to salvage upper extremity function in thoracic outlet syndrome caused by cervical ribs: a report of two cases.

We present herein the cases of two patients with thoracic outlet syndrome (TOS) who required arterial reconstruction due to gangrene of the fingers and/or hand. In both patients, the cervical ribs had produced intimal injury of the subclavian arteries, and the successive distal arterial embolism brought about severe ischemia of the affected upper extremity. To treat the TOS, the cervical ribs were resected through a supraclavicular incision. In the first patient, arterial reconstruction was performed from the subclavian artery to the radial collateral artery, a branch of the deep brachial arte

Surgical Management of Arterial Thoracic Outlet Syndrome due to Cervical Rib in a 17-Year-Old Male With Gangrenous Ulcer on Fingertip Requiring Multiple Bypass: A Case Report.

Arterial thoracic outlet syndrome (ATOS) is a rare condition, affecting 1% to 5% of TOS cases, characterized by the compression of the subclavian artery, often due to anatomical anomalies like cervical ribs. Cervical ribs that are not a very common anatomical anomaly can give rise to arterial supply compromise needing surgical intervention to undo the compression is the highlight of this case report. A 17-year-old male presented with progressive numbness, pain, and a gangrenous ulcer on his right index finger, aggravated by exertion. Clinical examination revealed absent pulses in the right han

Treatment of upper limb arterial occlusion caused by a cervical rib.

The cervical rib syndrome occurs when the interscalene triangle is occupied by a cervical rib, displacing the brachial plexus and the subclavian artery forward, which can cause pain and muscle spasms. The objective of this study is to discuss diagnosis of the cervical rib syndrome and treatment possibilities. This therapeutic challenge describes clinical and surgical management of a 37-year-old female patient with upper limb arterial occlusion caused by a cervical rib.

Isolated cervical rib fracture.

Isolated fracture of a cervical rib is a very rare entity. Cervical rib fracture due to heavy backpack usage is a new occupational hazard for students. Cervical rib fracture usually presents as a painless swelling or as thoracic outlet syndrome. We report two patients with cervical rib fractures, one of them secondary to backpack usage, and review the available literature.

Shoulder girdle compression syndrome.

Nowadays all compression syndromes at the upper chest like the costoclavicular syndrome, the scalenus syndrome, the hyperabduction syndrome and the Page-von Schroetter syndrome are included under the term of thoracic-outlet-syndrome. Apart from a constitutional disposition (cervical rib, anomalies of the tendons, etc.), it needs special factors like professional activities, sports, trauma, etc. to develop a T.O.S. syndrome. The symptoms range from prickling paresthesia, early fatigue, pains in shoulder and neck, claudication like pains and strong tendency towards swelling and rest pain or peri

Cervical ribs with neurological sequelae in children: a case series.

INTRODUCTION: Cervical ribs may result in thoracic outlet syndrome. Neurological compromise in children with cervical ribs, however, is rare. PATIENTS AND METHODS: We report our experience with cervical ribs in a pediatric population and review this specific literature. RESULTS: In our experience with a pediatric neurosurgical population, symptomatic cervical ribs were often identified in patients with Chiari I malformation. Surgical resection of symptomatic cervical ribs in children appears to have good long-term results. CONCLUSIONS: Based on our experience, symptomatic cervical ribs shou

An historical look at the thoracic outlet compression syndrome.

The story of the thoracic outlet compression syndrome begins with the identification of the anatomical abnormality of the cervical rib, and the symptoms related to it. Resection of the cervical rib, however, was soon followed by the recognition that symptoms could occur in the absence of a cervical rib. The scalenus anticus was then held to be the culprit, although several different mechanisms were advanced to explain the development of symptoms. Subsets of the thoracic outlet compression syndrome were then codified; costoclavicular compression; compression under the coracoid process during hy

Cervical ribs: identification on MRI and clinical relevance.

To determine the prevalence of cervical ribs on cervical spine MRI and clinical relevance, we reviewed 2500 studies for cervical ribs and compression of neurovascular structures and compared to CT, when available. Brachial plexus or subclavian artery contact by cervical rib was identified on MRI and/or CT in 12 cases with diagnosis of thoracic outlet syndrome in one. Cervical ribs were identified on 1.2% (25/2083) of examinations, lower than on CT (2%), but MRI may offer equivalent anatomic explanation for patient symptoms.

Surgical management of the thoracic outlet compression syndrome.

There is some disagreement about whether the first rib should be excised in the presence of a cervical rib for the relief of the thoracic outlet compression syndrome (TOCS). Over a 14-year period (1975-1988) 58 patients have undergone surgery for TOCS. Forty-four patients (76 per cent) had vascular symptoms, 28 (48 per cent) with a neurological component; 11 (19 per cent) had only neurological symptoms. Thirty-six patients (62 per cent) had the first rib excised; 19 (33 per cent) had a cervical rib excised; two (3 per cent) had a division of fibrous bands; and one patient had a large transvers

The rare case of cervical rib fusion to the second rib.

This article reports on 3 patients who presented with arterial thoracic outlet syndrome and were found to have a rare clinical entity. All 3 patients presented with a rare cervical rib, which was also found to be fused to the second rib, with no first rib at all. All underwent transaxillary cervical and second rib resections and anterior scalenectomy. After transaxillary thoracic outlet decompression, all completed physical therapy with complete resolution of symptoms.

Lower trunk brachial plexus compression neuropathy due to cervical rib in young athletes.

Compression by a cervical rib may result in neurologic and/or vascular symptoms. Two patients are reported with thoracic outlet syndrome (TOS) secondary to cervical rib. Both patients had vague shoulder pain as well as neurologic manifestations due to compression neuropathy of the lower trunk of the brachial plexus. One patient was suspected initially to have carpal tunnel syndrome.

SUPRACLAVICULAR FIRST RIB RESECTION FOR TREATMENT OF THORACIC OUTLET SYNDROME.

Twelve patients with thoracic syndrome were operated during a 15 month period. Eleven patients had features of neurogenic thoracic outlet syndrome, while one presented with arterial ischemia due to distal embolism. The diagnosis was based on the characteristic history and positive stress tests. Cervical rib was present in 7 patients. Abnormal nerve conduction studies were present in 7 out of 8 cases. Supraclavicular first rib resection was done in all patients in view of the severity of symptoms. If present, the cervical rib was also excised. There was no major operative complication. Eleven o

[Thoracic outlet syndrome in a child: a rare pediatric diagnosis].

Thoracic outlet syndrome (TOS) is a rare pediatric syndrome and few cases have been reported in the literature. The authors report the case of a 10-year-old boy with generalized arm pain and neck stiffness. The angiocomputed tomography revealed the presence of a TOS caused by an additional cervical rib and stenosis of the sub-clavian artery. The resection of this rib relieved the patient of his pain during a full year following surgery. This case reminds us that children can also be affected by TOS. When a cervical rib causes TOS, a resection is often necessary.

Isolated fracture of a cervical rib: a case report.

A case of isolated fracture of a cervical rib in a young woman is presented. The mechanism of injury was direct blunt trauma. Histological examination showed changes compatible with a previous fracture of the cervical rib. Literature concerning the incidence, complications and removal of these ribs is reviewed. Closely associated with this anomaly is the thoracic outlet syndrome, a differential diagnosis of which is given.

[Cervical ribs. Therapeutic deliberations apropos of 10 cases].

The authors report ten cases of cervical ribs operated upon in 7 patients. The predominantly neurological symptoms and signs were associated with vascular problems in 3 cases, with intermittent compression of the sub-clavian vessels. Treatment consisted of resection of the cervical rib and the first rib via an extra-pleural axillary approach using the technique described by Roos. It gave 10 good results with a follow-up of 6 months to 3 years. The authors emphasise the fact that this pathology falls within the context of compressive syndromes of the root of the upper limb or thoracic outlet sy

TOS-surgery via a single supraclavicular incision.

BACKGROUND: We report about our experiences using a single supraclavicular incision at the base of the neck for Thoracic Outlet Syndrome (TOS) surgery. METHODS: 10 patients aged between 12 and 59 years (mean 31 years) underwent 12 times a TOS procedure. Patients suffered from compression of their brachial plexus with main affection of the ulnar nerve (9 out of 12 cases). Electroneurography was positive for TOS 4 times in 3 patients, in other 3 patients additionally a distal nerve compression syndrome was evident. In 7 cases (5 patients) a cervical rib was present on X-ray. In 10 cases (8 pati
