[Contextual factors for understanding the heterogeneity of outcomes of a policy of exemption from payment for care in Chad].

Chad has one of the highest maternal and infant mortality rates in the world. Efforts to reduce these rates have led to the introduction of fee exemption and community involvement initiatives to further encourage the use of health services. Despite the introduction of these initiatives, inequalities in access to and use of health facilities persist. The aim of this study is to understand why and how the same action, implemented in a quasi-homogeneous way, produced contrasting results in different health centers. A multiple, contrasting case study was used to analyze the outcomes of pediatrics

Update of transmission modelling and projections of gambiense human African trypanosomiasis in the Mandoul focus, Chad.

BACKGROUND: In recent years, a programme of vector control, screening and treatment of gambiense human African trypanosomiasis (gHAT) infections led to a rapid decline in cases in the Mandoul focus of Chad. To represent the biology of transmission between humans and tsetse, we previously developed a mechanistic transmission model, fitted to data between 2000 and 2013 which suggested that transmission was interrupted by 2015. The present study outlines refinements to the model to: (1) Assess whether elimination of transmission has already been achieved despite low-level case reporting; (2) quan

Survey of animal bite injuries and their management for an estimate of human rabies deaths in N'Djaména, Chad.

OBJECTIVE: To estimate the annual human rabies incidence as a baseline prior to mass dog vaccination campaigns in N'Djaména, Chad. METHODS: Survey of animal bites, involving 50% of all healthcare providers in N'Djaména, from September 2008 to April 2009. Of 86 people exposed to a suspected rabid animal, 50% received post-exposure vaccination and a further 8% had their wound cleaned. We estimated annual incidence of bites from suspected rabid animals of 12.9/100,000 and an incidence of 0.7 human rabies deaths/100,000, resulting in 7 estimated deaths (95% confidence interval 4-10 deaths) per ye

Ischaemic cardiovascular mortality in patients with non-valvular atrial fibrillation according to CHADS₂ score.

The CHADS₂ score predicts the risk of ischaemic stroke in patients with non-valvular atrial fibrillation (NVAF). Most components of the CHADS₂ score are also risk factors of atherosclerosis, and clustering of these risk factors is associated with increased risk of cardiovascular disease, including ischaemic heart disease. The aim of this study was to investigate whether the CHADS₂ score and CHA₂DS₂-VASc score are predictive of fatal ischaemic heart disease as well as fatal ischaemic stroke. Among 5,268 stroke patients admitted between August 1994 and December 2008, 770 stroke patients with NVA

Epidemiology of Trypanosoma vivax infection in cattle in the tse-tse free area of Lake Chad.

A study was conducted in Chad to estimate the prevalence and the incidence of Trypanosoma vivax infection in herds of cattle from the Lake Chad area. The risk factors associated with disease were also identified. A random sample of 933 cattle from 17 herds was initially selected (January 1999, cold dry season). Cattle were identified by ear-tags and sampled in the rainy season (July 1999) and the cold dry season (January 2000). Each animal sampled was treated with diminazene aceturate (3.5mg/kg). Samples were examined for the presence of T. vivax using an antibody (indirect ELISA) and a parasi

Allele frequency of Hemoglobin S among patients with uncomplicated Plasmodium falciparum malaria in N'Djamena, Chad.

Protection against malaria induced by the sickle cell trait has been a major example of genetic selection for over half a century. It has been demonstrated that human populations in Africa have acquired a high frequency of haemoglobin S (HbS) allele because the heterozygous genotype (sickle cell trait) provides protection against the severe symptoms of Plasmodium falciparum infection leading to lower parasite numbers and increased clearance of infected cells by the body. However, there is scarcity of data from Chad. This study was conducted to determine the HbS allele frequency, and also to as

Valorizing Erasure: Imperial Knowledge Production in Anglo-American Coverage of Chad's 'Toyota War'.

How does imperial knowledge production operate when Western media coverage of a Global South subject is predominantly admiring instead of hostile? This article addresses that question through critical discourse analysis of sixty-three Anglo-American news articles covering the Chad-Libya conflict of the 1980s, the 'Toyota War'. Despite Chad's decisive military victory and broadly favourable coverage, systematic coding reveals that Chadian political subjectivity was structurally withheld. Chadian figures appeared as sources in 41.3 per cent of articles but were granted political or strategic int

Integrated bioinformatics analysis identifies CHAD association with osteoporosis and in vitro chondrogenic effects of Wogonin.

Osteoporosis (OP), characterized by reduced bone density and increased fracture risk. Current therapies have limitations due to side effects and variable efficacy, necessitating exploration of novel therapeutic targets and alternatives like Traditional Chinese Medicine (TCM). The molecular mechanisms driving OP remain incompletely understood. This study utilized integrated bioinformatics approaches to identify potentially dysregulated genes in OP and explored in vitro effects of TCM compounds targeting top identified genes. Differentially expressed genes (DEGs) associated with OP were identifi

Evaluating the Effectiveness of Potential Interventions for Guinea Worm Disease in Dogs in Chad Using Simulations.

Guinea worm (GW) disease (or dracunculiasis) is currently transmitted among dogs in Chad, which presents risks for the human population. We studied how interventions implemented at different levels might reduce the spread of GW disease (geographically and over time) and what levels of interventions might accelerate elimination. We built a multiple-water-source agent-based simulation model to analyze the disease transmission among dogs in Chad, as well as in geographic district clusters, and validated it using local infection data. We considered two interventions: 1) tethering, where infected d

Epidemiology and Prevalence of Oral Candidiasis in HIV Patients From Chad in the Post-HAART Era.

Oral candidiasis remains a common problem in HIV-infected individuals, especially in sub-Saharan Africa. Here, we performed the first study in Chad on the prevalence of oral yeasts carriage and oral candidiasis in HIV-positive subjects from southern Chad and analyzed the influence of HAART, CD4+ T-cell numbers, and antimycotics in 589 patients. These patients were recruited from a specialized medical center for HIV patients in Sarh and from a rural medical health dispensary in the vicinity, including a total of 384 HIV-positive and 205 HIV-negative individuals. Yeasts obtained from oral specim

Diversity of tsetse flies and trypanosome species circulating in the area of Lake Iro in southeastern Chad.

BACKGROUND: African trypanosomiases are vector-borne diseases that affect humans and livestock in sub-Saharan Africa. Although data have been collected on tsetse fauna as well as trypanosome infections in tsetse flies and mammals in foci of sleeping sickness in Chad, the situation of tsetse fly-transmitted trypanosomes remains unknown in several tsetse-infested areas of Chad. This study was designed to fill this epidemiological knowledge gap by determining the tsetse fauna as well as the trypanosomes infecting tsetse flies in the area of Lake Iro in southeastern Chad. METHODS: Tsetse flies we

The Importance of a Participatory and Integrated One Health Approach for Rabies Control: The Case of N'Djaména, Chad.

This study compares data on animal rabies cases from the Chadian national rabies laboratory, hosted at the Insitut de Recherche en Elevage pour le Developpement (IRED), with bite case reporting from health facilities. The data collection accompanied a mass dog vaccination intervention over two years in N'Djaména, Chad. This allowed for a comparison of the dynamics of the incidence of animal rabies cases, human bite exposure incidence and post-exposure prophylaxis (PEP) demand during a dog rabies elimination attempt. Following the mass vaccination, the monthly animal rabies incidence dropped fr

Treating cattle to protect people? Impact of footbath insecticide treatment on tsetse density in Chad.

BACKGROUND: In Chad, several species of tsetse flies (Genus: Glossina) transmit African animal trypanosomoses (AAT), which represents a major obstacle to cattle rearing, and sleeping sickness, which impacts public health. After the failure of past interventions to eradicate tsetse, the government of Chad is now looking for other approaches that integrate cost-effective intervention techniques, which can be applied by the stake holders to control tsetse-transmitted trypanosomoses in a sustainable manner. The present study thus attempted to assess the efficacy of restricted application of insect

Cost-description of a pilot parenteral vaccination campaign against rabies in dogs in N'Djaména, Chad.

In the discussion about policies and strategies for rabies prevention in developing countries, intervention costs arise as a major issue. In a pilot mass vaccination campaign against rabies in N'Djaména, Chad, 3000 dogs were vaccinated. We assessed vaccination coverage and cost, showing the cost per dog vaccinated for the public sector and for society. An extrapolation to city level calculated the approximate cost of vaccinating all 23 600 dogs in N'Djaména. In the pilot mass campaign with 3000 dogs the average cost per dog was 1.69 euro. to the public and the full societal cost was 2.45 euro.

The 1990 meningococcal meningitis epidemic of Sarh (Chad): how useful was an earlier mass vaccination?

A large outbreak of meningococcal meningitis (Serogroup A) occurred in southern Chad in 1990. We describe the epidemic in the town of Sarh, where a mass vaccination against meningococcal meningitis had taken place two years before, in 1988 (estimated coverage: 66%). Early warning that an epidemic was imminent was given at the end of February, following more than 15 recorded cases per 100,000 population on 3 consecutive weeks. This threshold proved to be adequate to predict the outbreak. A total of 721 cases were recorded at Sarh hospital and at a nearby health centre. Direct agglutination test

Flubendazole reduces emerging Dracunculus medinensis in dogs in Chad: Results from a two-year study.

Since being targeted for eradication in the 1980s, global human cases of dracunculiasis (Guinea worm disease; GWD) have been reduced by over 99.9% through interventions aimed at interrupting the life cycle of the causative parasite, Dracunculus medinensis. However, the burden of GWD in dogs is higher than in humans and other monitored mammalian host species, threatening eradication of GWD. Expanding on a previous clinical trial of flubendazole (FLBZ) in dogs in Chad, we conducted a cluster-randomized controlled trial in which half of enrolled villages (N = 29) were assigned to receive FLBZ and
