Adolescent Overdose Mortality and Child Protection System Involvement: Evidence from a California Birth Cohort, 2000-2005.

Objectives. To examine overdose mortality across 4 levels of child protection system (CPS) involvement during childhood. Methods. Using integrated vital birth, death, and CPS records of California births (2000-2005) followed through 2024 (n = 3 208 771), we identified 747 adolescent overdose decedents and matched each to 4 comparison adolescents (n = 2988). We categorized CPS exposure as no contact, referral, substantiated referral, or foster care. Using conditional Poisson regression, we estimated overdose prevalence by CPS involvement level. Results. Overall, 69.9% of decedents had at least

Assessing the impact of COVID-19 pandemic on all-cause mortality and child mortality in a population cohort of Iganga Mayuge HDSS in Eastern Uganda (2015-2021).

BACKGROUND: Efforts to track the mortality and public health impact of the coronavirus disease (COVID-19) in Uganda have been hampered by weak Civil registration and vital statistics (CRVS) system and suboptimal health seeking behaviors or patterns. Evaluating unexplained increases in all-cause mortality provides a complete picture of the impact of COVID-19 pandemic and guide public health policies and resource allocation to protect the most vulnerable populations. METHODS: The longitudinal population cohort data on demographic events and socioeconomic status collected from 2015 to 2021 withi

Housing and Inequalities in US Life Expectancy, Child and Youth Mortality, and All-Cause and Cause-Specific Mortality, 1979-2020: Results from the National Longitudinal Mortality Study and the National Vital Statistics System.

BACKGROUND: Limited research exists on the association between housing, life expectancy, and mortality disparities in the United States (US). Using longitudinal individual-level and pooled county-level mortality data from 1979 to 2020, we examine disparities in life expectancy, child and youth mortality, and all-cause and cause-specific mortality in the US by several housing variables. METHODS: Using the 1979-2011 National Longitudinal Mortality Study (N=1,313,627) and the 2011-2020 linked county-level National Mortality Database and American Community Survey, we analyzed disparities in life

Large language models and child mortality: opportunities and challenges in answering public queries on under-5 causes.

BACKGROUND: Reducing under-5 mortality remains a global health priority. Large language models (LLMs) are increasingly used by the public to access medical information. However, current evidence evaluating LLMs' performance in public-facing child health communication is scarce. METHODS: We selected the top five search terms related to each of the five leading causes of under-5 mortality (prematurity, pneumonia, birth asphyxia, malaria, and diarrhoea) using Google Trends, generating 25 representative public queries. Responses were collected from four LLMs (ChatGPT-4.0, Claude 3.5 Sonnet, Bing

Child mortality in England after national lockdowns for COVID-19: An analysis of childhood deaths, 2019-2023.

BACKGROUND: During the COVID-19 pandemic children and young people (CYP) mortality in England reduced to the lowest on record, but it is unclear if the mechanisms which facilitated a reduction in mortality had a longer lasting impact, and what impact the pandemic, and its social restrictions, have had on deaths with longer latencies (e.g., malignancies). The aim of this analysis was to quantify the relative rate, and causes, of childhood deaths in England, before, during, and after national lockdowns for COVID-19 and its social changes. METHODS AND FINDINGS: Deaths of all children (occurring

Impact Evaluation of Malaria Control Interventions on Morbidity and All-Cause Child Mortality in Rwanda, 2000-2010.

The impressive decline in child mortality that occurred in Rwanda from 1996-2000 to 2006-2010 coincided with a period of rapid increase of malaria control interventions such as indoor residual spraying (IRS); insecticide-treated net (ITN) distribution and use, and improved malaria case management. The impact of these interventions was examined through ecological correlation analysis, and robust decomposition analysis of contextual factors on all-cause child mortality. Child mortality fell 61% during the evaluation period and prevalence of severe anemia in children 6-23 months declined 71% betw

MELD, MELD 3.0, versus Child score to predict mortality after acute variceal hemorrhage: A multicenter US cohort.

BACKGROUND: Acute variceal hemorrhage is a major decompensating event in patients with cirrhosis and is associated with high 6-week mortality risk. Many prognostic models based on clinical and laboratory parameters have been developed to risk stratify patients on index bleeding presentation, including those based on the Model for End-Stage Liver Disease (MELD) and Child-Turcotte-Pugh (CTP). However, consensus on model performance remains unclear. METHODS: Using a large US multicenter cohort of hospitalized patients with cirrhosis who presented with acute variceal hemorrhage, this study evalua

Community interventions to reduce child mortality in Dhanusha, Nepal: study protocol for a cluster randomized controlled trial.

BACKGROUND: Neonatal mortality remains high in rural Nepal. Previous work suggests that local women's groups can effect significant improvement through community mobilisation. The possibility of identification and management of newborn infections by community-based workers has also arisen. METHODS/DESIGN: The objective of this trial is to evaluate the effects on newborn health of two community-based interventions involving Female Community Health Volunteers. Long-term impact of community-based participatory women's groups on child and maternal mortality and child disability: follow-up of a cluster randomised trial in rural Nepal.

BACKGROUND: Community-based women's groups practising participatory learning and action (PLA) can reduce maternal and neonatal mortality in low-income countries. However, it is not clear whether these reductions are associated with subsequent increased or decreased rates of childhood death and disability. We assessed the impact on child deaths and disability beyond the perinatal period among participants in the earliest trial in Nepal 2001-2003. METHODS: Household interviews were conducted with mothers or household heads. At cluster and individual levels, we analysed disability using pairwise

Evaluation and Forecasting Analysis of the Association of Conditional Cash Transfer With Child Mortality in Latin America, 2000-2030.

IMPORTANCE: Latin America has implemented the world's largest and most consolidated conditional cash transfer (CCT) programs during the last 2 decades. As a consequence of the COVID-19 pandemic, poverty rates have markedly increased, and a large number of newly low-income individuals, especially children, have been left unprotected. OBJECTIVE: To evaluate the association of CCT programs with child health in Latin American countries during the last 2 decades and forecast child mortality trends up to 2030 according to CCT alternative implementation options. Neonate, Infant, and Child Mortality by Cause in Provinces of Iran: An Analysis for the Global Burden of Disease Study 2019.

BACKGROUND: Since 1990, neonatal, infant, and child mortality has substantially decreased in Iran. However, estimates for mortality by cause at subnational scale are not available. METHODS: This study is part of the Global Burden of Diseases, Injuries, and Risk Factors study (GBD) 2019. Here we report the number and rates of neonate, infant, and child deaths by cause across provinces of Iran from 1990 to 2019. RESULTS: Between 1990 and 2019, the neonatal mortality rate per 1000 live births decreased from 31.8 (95% UI: 28.1-35.5) to 6.8 (6.1-7.4). The child mortality rates decreased from 71.2

The maternal and child mortality in the Middle East and North Africa between 2000 and 2020: the role of health financing.

BACKGROUND: Improving maternal and child health (MCH) outcomes is a critical agenda item in global development. Health financing factors play a crucial role in affecting MCH outcomes, which vary substantially in the Middle East and North Africa (MENA) region. This study aims to examine the trends in maternal mortality rate (MMR), infant mortality rate (IMR), and under-5 mortality rate (U5MR) in the MENA region and the potential impact of health financing factors on them. METHODS: We compiled data on MCH mortalities and potential determinants, including health financing factors, for all countr

Maternal ethnic group, socioeconomic status, and neonatal and child mortality: a nationwide cohort study in England and Wales.

BACKGROUND: The UK currently has one of the highest rates of child mortality in Europe. Robust population-level estimates of differences in neonatal and child mortality by ethnic and socioeconomic group are currently scarce for England and Wales. We aimed to examine variation in neonatal and child mortality by maternal ethnic group and indicators of socioeconomic status to help understand which groups are most at risk of neonatal and child mortality. METHODS: In this nationwide cohort study, we used linked population-level data for England and Wales, comprising birth registrations and notific

Leading causes of child mortality in Brazil, in 1990 and 2015: estimates from the Global Burden of Disease study.

OBJECTIVE:: To analyze under-5 mortality rates and leading causes in Brazil and states in 1990 and 2015, using the Global Burden of Disease Study (GBD) 2015 estimates. METHODS:: The main sources of data for all-causes under-5 mortality and live births estimates were the mortality information system, surveys, and censuses. Proportions and rates per 1,000 live births (LB) were calculated for total deaths and leading causes. RESULTS:: Estimates of under-5 deaths in Brazil were 191,505 in 1990, and 51,226 in 2015, 90% of which were infant deaths. The rates per 1,000 LB showed a reduction of 67.6

Trends and patterns of modern contraceptive use and relationships with high-risk births and child mortality in Burkina Faso.

BACKGROUND: In sub-Saharan Africa, few studies have stressed the importance of spatial heterogeneity analysis in modern contraceptive use and the relationships with high-risk births. OBJECTIVE: This paper aims to analyse the association between modern contraceptive use, distribution of birth risk, and under-five child mortality at both national and regional levels in Burkina Faso. DESIGN: The last three Demographic and Health Surveys - conducted in Burkina Faso in 1998, 2003, and 2010 - enabled descriptions of differentials, trends, and associations between modern contraceptive use, total fe

Patterns in coverage of maternal, newborn, and child health interventions: projections of neonatal and under-5 mortality to 2035.

BACKGROUND: Urgent calls have been made for improved understanding of changes in coverage of maternal, newborn, and child health interventions, and their country-level determinants. We examined historical trends in coverage of interventions with proven effectiveness, and used them to project rates of child and neonatal mortality in 2035 in 74 Countdown to 2015 priority countries. METHODS: We investigated coverage of all interventions for which evidence was available to suggest effective reductions in maternal and child mortality, for which indicators have been defined, and data have been obta

Mortality and morbidity in the city of Bern, Switzerland, 1805-1815 with special emphasis on infant, child and maternal deaths.

This article contributes to the research on demographics and public health of urban populations of preindustrial Europe. The key source is a burial register that contains information on the deceased, such as age and sex, residence and cause of death. This register is one of the earliest compilations of data sets of individuals with this high degree of completeness and consistency. Critical assessment of the register's origin, formation and upkeep promises high validity and reliability. Between 1805 and 1815, 4,390 deceased inhabitants were registered. Information concerning these individuals p
