Percutaneous cholecystostomy in acute cholecystitis and common duct obstruction.

Percutaneous cholecystostomy was performed in 36 patients, of whom 22 patients with acute cholecystitis had decompression of the gallbladder and 14 patients with common duct obstruction had biliary decompression through the gallbladder. In almost all cases of acute cholecystitis, percutaneous cholecystostomy quickly confirmed the diagnosis and brought relief. Bile leakage occurred in one patient when catheter placement was performed under ultrasonic guidance alone; subsequent procedures were performed in a single step using an "accordion" catheter under fluoroscopic control following ultrasoun

Patient outcomes after treatment with percutaneous cholecystostomy for biliary sepsis.

INTRODUCTION: Acute cholecystitis is among the most common general surgical presentations. There is a cohort of patients who develop systemic sepsis and complications of acute cholecystitis. These patients are often elderly and co-morbid. Conservative management with percutaneous cholecystostomy has been shown to be a safe and effective management option in the acute setting. However, there is currently no consensus for the further management of these patients. In particular, there is a paucity of data on readmission rates and subsequent operative or non-operative management. METHODS: A retro

Comparison of early outcome from transperitoneal versus transhepatic percutaneous cholecystostomy.

BACKGROUND/AIMS: To determine whether there is a statistically significant difference in the short-term clinical outcome in patients undergoing percutaneous cholecystostomy based on the anatomic route of gallbladder puncture that is, transhepatic versus transperitoneal. METHODOLOGY: Our population consisted of 132 patients who: 1) presented with acute cholecystitis, 2) were at high risk for surgery because of comorbid conditions, 3) underwent percutaneous cholecystostomy either using computed tomography guidance or ultrasound guidance and whose anatomic route was known: the transhepatic percu

Percutaneous transhepatic cholecystostomy for acute complicated cholecystitis in elderly patients.

We report our experience with percutaneous transhepatic cholecystostomy in 10 elderly patients with acute cholecystitis, complicated by empyema formation. Most of these patients has severe underlying disease, rendering them at high risk for surgical intervention. In all patients, the percutaneous procedure was followed by a rapid regression of clinical symptoms and of radiologic abnormalities. Six were considered inoperable. Three of these remain free of biliary symptoms, respectively 22, 10, and 7 months after percutaneous cholecystostomy. Three others died of nonbiliary disease 1-4 months af

Outcomes of Percutaneous Cholecystostomy in Older Adults with Acute Cholecystitis.

BACKGROUND: Acute cholecystitis (ACC) is one of the most common diagnoses encountered in surgical wards. A number of treatment modalities are available, and various guidelines have been developed to help decision making. Many factors influence treatment strategies, including patient age and frailty. Due to the increasing proportion of older patients, consideration into the best treatment modalities for this population are warranted. OBJECTIVES: To determine outcomes of elderly patients with ACC according to different treatment strategies. METHODS: A retrospective analysis of consecutive pati

When to remove the drainage catheter in patients with percutaneous cholecystostomy?

OBJECTIVE: The treatment for patients with acute calculous cholecystitis who have high surgical risk with percutaneous cholecystostomy instead of surgery is an appropriate alternative choice. The aim of this study was to examine the promising percutaneous cholecystostomy intervention to share our experiences about the duration of catheter that has yet to be determined. METHODS: A total of 163 patients diagnosed with acute calculous cholecystitis and treated with percutaneous cholecystostomy between January 2011 and July 2020 were reviewed retrospectively. The Tokyo Guidelines 2018 were used t

Clinical course of percutaneous cholecystostomies: A cross-sectional study.

BACKGROUND: Although cholecystectomy is the standard treatment modality, it has been shown that perioperative mortality is approaching 19% in critical and elderly patients. Percutaneous cholecystostomy (PC) can be considered as a safer option with a significantly lower complication rate in these patients. AIM: To assess the clinical course of acute cholecystitis (AC) in patients we treated with PC. METHODS: The study included 82 patients with Grade I, II or III AC according to the Tokyo Guidelines 2018 (TG18) and treated with PC. The patients' demographic and clinical features, laboratory pa

Percutaneous cholecystostomy - An option in selected patients with acute cholecystitis.

While urgent percutaneous cholecystostomy (PC) was introduced as an alternative to acute surgical treatment for acute cholecystitis (AC), the current place of PC in the treatment algorithm for AC is challenged. We evaluate demographics and outcomes of PC in routine clinical practice in a population-based cohort.Retrospective evaluation of consecutive patients treated with PC for AC between 2000 and 2015. The severity of cholecystitis was graded according to the 2013 Tokyo Guidelines. One hundred forty-nine patients were included (82; 55% males) (median age of 72.5 years; range, 21-92). The Toky

Acalculous Cholecystitis in Burn Patients: Is There a Role for Percutaneous Cholecystostomy?

Although acute acalculous cholecystitis is uncommon in burn patients, this condition can be rapidly fatal due to delays in diagnosis and treatment and should always be considered in the differential diagnosis when burn patients become septic, develop abdominal pain, or have hemodynamic instability. This article reviews the use of percutaneous cholecystostomy in burn patients as both a diagnostic and therapeutic intervention.

Systematic review of cholecystostomy as a treatment option in acute cholecystitis.

OBJECTIVES: Percutaneous cholecystostomy (PC) is an established low-mortality treatment option for elderly and critically ill patients with acute cholecystitis. The primary aim of this review is to find out if there is any evidence in the literature to recommend PC rather than cholecystectomy for acute cholecystitis in the elderly population. METHODS: In April 2007, a systematic electronic database search was performed on the subject of PC and cholecystectomy in the elderly population. After exclusions, 53 studies remained, comprising 1918 patients. Three papers described randomized controlle

[Echo-guided percutaneous cholecystostomy in the treatment of acute cholecystitis].

Ultrasound-guided cholecystostomy (UGC) is indicated for high-risk patients with acute cholecystitis (AC). The advantage of this approach is greatest for critically ill patients who develop AC while in the intensive care unit (ICU). Moreover, in ICU patients with unexplained sepsis UGC serves as a diagnostic maneuver since it may allow the identification of a biliary infection. UGC has a high therapeutic efficacy approaching 100% in patients with a well-established diagnosis of AC. Morbidity is low and almost entirely related to catheter dislodgment. Trans-catheter cholecystocholangiograms (TC

Percutaneous cholecystostomy for suspected acute cholecystitis in the hospitalized patient.

PURPOSE: The authors evaluated the outcome of 49 hospitalized patients with sepsis and possible acute cholecystitis in whom emergency percutaneous cholecystostomy was attempted on 50 occasions. PATIENTS AND METHODS: All cholecystostomy procedures were performed with ultrasound (US) guidance by using either the trocar (n = 35) or the Seldinger (n = 15) technique. Forty of the 50 cholecystostomies (80%) were attempted at the patients' bedside, and 49 of the 50 catheters (98%) were placed successfully. RESULTS: Twenty-five of these patients eventually died of other causes (51%), but there was c

[Percutaneous cholecystostomy in treating acute cholecystitis in patients at risk].

Percutaneous cholecystostomy was performed in 17 poor surgical risk patients. 16 patients developed acute acalculous cholecystitis postoperatively in the intensive care unit, 1 patient had an acute cholecystitis with calculi. Diagnostic imaging using CT and US was specific for acute cholecystitis in 58% only. Percutaneous cholecystostomy was the definitive treatment in 69% of the patients. Additional cholecystectomy was required in 3 patients with complicated cholecystitis, in 1 patient with bile leakage after catheter dislocation and in 1 patient with gallbladder calculi. 3 patients died, 2 o

Cholecystostomy in the high risk patient with biliary tract disease.

The morbidity of complications and mortality following surgical operations for nonmalignant disease of the biliary tract is greatest among those 65 years and older. The reported overall mortality regardless of age ranges upward from 0.88%. At The New York Hospital-Cornell Medical Center for a 43-year period (1932-1975) it was 1.77%. For those 65 and older it was 5.1%. For patients with acute cholecystitis it was 3.5% and of those in this category 65 and older it was 9.2%. This experience and that reported from other comparable clinics is the basis for advocating that the procedure selected be
