Biliary stenting as a bridge to cholecystectomy in patients with choledocholithiasis.

BACKGROUND: Early laparoscopic cholecystectomy after endoscopic retrograde cholangiopancreatography (ERCP) is recommended for patients with choledocholithiasis to reduce the risk of recurrent biliary events (RBE). However, in clinical practice early surgery is not always feasible. The role of biliary stenting as a bridging strategy before delayed cholecystectomy remains insufficiently studied. This study evaluated outcomes of delayed cholecystectomy after ERCP with biliary stent placement compared with early surgery in a stented cohort. METHODS: We conducted a retrospective single-center stud

Obstructive Jaundice Secondary to Choledocholithiasis With Chronic Cholecystitis.

Obstructive jaundice, usually from choledocholithiasis, often coexists with chronic cholecystitis and poses higher risks in the elderly. Timely diagnosis with ultrasonography (USG)/magnetic resonance cholangiopancreatography (MRCP) and prompt intervention are critical. This case report highlights the diagnostic approach, surgical management, and outcomes in elderly patients with large common bile duct (CBD) stones. A 78-year-old man presented with jaundice, abdominal pain, and fever. MRCP revealed a large common bile duct (CBD) stone measuring 3.0 cm with features of cholecystitis, confirming

Development of a risk score for choledocholithiasis in pediatric patients.

PURPOSE: The prevalence of gallstones in children has increased over the last years. Choledocholithiasis (CD) is present in up to 30% of the cases. There is a scarcity of studies on the management of choledocholithiasis in children. The aim of this study was to develop a score that would allow predicting accurately the risk of CD in children with gallstones and reduce the number of non-therapeutic ERCP. MATERIALS AND METHODS: We conducted a retrospective study in children with gallstones and suspected CD seen between January 2010 and December 2019. The main outcome was the presence of CD conf

Liver Blood Tests in the Management of Suspected Choledocholithiasis.

OBJECTIVE: The likelihood of common bile duct (CBD) stones considers liver blood tests (LBTs) if they are markedly altered only. The aim of our study was to find a reliable tool based on LBTs to predict the presence of CBD stones. METHODS: We retrospectively considered all patients who underwent magnetic resonance cholangiopancreatography (MRCP) because of suspected CBD stones from January 2014 to June 2019. Demographic, clinical data, and LBT values were collected and analyzed. RESULTS: We selected 191 patients, 64 (33.5%) with positive MRCP and 127 (66.5%) with negative MRCP. The analysis

Management of cholelithiasis with choledocholithiasis: Endoscopic and surgical approaches.

Gallstone disease and complications from gallstones are a common clinical problem. The clinical presentation ranges between being asymptomatic and recurrent attacks of biliary pain requiring elective or emergency treatment. Bile duct stones are a frequent condition associated with cholelithiasis. Amidst the total cholecystectomies performed every year for cholelithiasis, the presence of bile duct stones is 5%-15%; another small percentage of these will develop common bile duct stones after intervention. To avoid serious complications that can occur in choledocholithiasis, these stones should b

Endoscopic management of choledocholithiasis and cholelithiasis in patients with cirrhosis.

Treatment of choledocholithiasis and cholelithiasis in patients with cirrhosis often requires diagnostic and therapeutic endoscopy such as endoscopic ultrasound (EUS) and endoscopic retrograde cholangiopancreatography (ERCP). Patients with underlying cirrhosis may have coagulopathy, hepatic encephalopathy, ascites and other comorbidities associated with cirrhosis that can make endoscopic therapy challenging and can be associated with a higher risk of adverse events. Given the unique derangements of physiologic parameters associated with cirrhosis this population requires a truly multifaceted a

Outcomes of cholecystectomy after endoscopic sphincterotomy for choledocholithiasis.

Laparoscopic cholecystectomy (LC) for treatment of symptomatic common bile duct stones (CBDS) after endoscopic sphincterotomy (ES) is associated with increased conversion and complications compared with other indications. We examined factors associated with conversion and complications of LC after ES. A retrospective study of 32 patients undergoing ES for CBDS followed by cholecystectomy was undertaken. Surgical outcomes for this group were compared with a control population of 499 LCs for all other indications. Factors associated with open cholecystectomy and complications in the ES group wer
