Neonatal cholestasis is a diagnostic challenge that warrants extensive investigation as there can be serious sequalae such as liver failure, cirrhosis, or other extrahepatic complications. To differentiate the etiology of cholestasis, a distinction can be made between high and low gamma-glutamyltransferase (GGT) cholestasis. Low GGT cholestasis points towards progressive familial intrahepatic cholestasis type 1-2 and 4-6, bile acid synthesis disorders, tight-junction protein type 2 deficiency and some forms of hypopituitarism. Classic galactosemia is generally not included in the differential

A 65-year-old man with fever of unknown origin developed progressive extrahepatic cholestasis. Radiological examination documented a suprarenal abdominal aortic aneurysm. Fatal intraabdominal bleeding occurred, leading to death. Post-mortem examination revealed an earlier retroperitoneal rupture of the aortic aneurysm and massive perianeurysmal inflammation. The blood masses, reaching the porta hepatis, caused progressive obstruction of the extrahepatic bile ducts. Surgical treatment, delayed because of cholestatic and febrile complications, could have prevented the outcome.

Obstructive jaundice is a sign of intra- or posthepatic blockage of bile flow. This diagnosis has to be differentiated from various other diagnoses such as disorders of bilirubin metabolism or hepatocellular causes of jaundice. An accurate evaluation of the past medical history and clinical examination of the patient can already establish obstruction as the cause of jaundice in most cases. For prevention of a cholangitis further imaging procedures should focus on rapidly establishing the cause and the location of obstruction. Further therapeutic procedures are dependent on the type of obstruct

Inflammatory and cicatrical alterations are the morphological basis of papillary stenosis in most cases due to canalicular or lymphatic spreading of bacterial infection in gallstone disease. The consequences of papillary stenosis are elevated pressure in the ducts and reduced bile flow. At surgery you find dilatation of the bile ducts, increase of residual pressure, decrease of outflow, and--in cicatrical stenosis--stop for the probe. Only the inflammatory stenosis is reversible, but permanent alteration is not to determine. Therefore therapy is necessary, at first instrumental dilation to 4-6

If the biliary tract is well seen, but the gallbladder remains unvisualized, one may conclude an obstruction of the gallbladder duct. The author shows by means of 108 cases, confirmed by surgery, that operation is indicated if the duct of the gallbladder is obstructed: Although mechanical obstruction of the cystic duct with a calculus was seen in only 10 cases (9,2%) at the time of operation, calculi were found in 101 patients (93%) in the biliary tract system, presenting with cholecystitis and mucosal oedema. In 103 patients (95,4%), the pathological findings had its origin in the biliary tra

In the King Khalid University Hospital (Central Province) and King Fahad Hospital of the University (Eastern Province) Saudi Arabia, we identified 64 infants with cholestasis. The causes of cholestasis were idiopathic neonatal hepatitis in 29; extrahepatic biliary atresia in 17; neonatal hepatitis secondary to Rubella and Cytomegalovirus in six and four infants, respectively; paucity of intrahepatic bile ducts in six and galactosaemia in two. The diagnosis was confirmed by liver biopsy and or operative cholangiography, in all infants.

A large number of drugs may be associated with impaired bile flow. Drug-associated cholestasis presents like other forms of cholestasis with pale stools, dark urine, pruritus and jaundice. Abdominal pain may be present in some instances and can be so severe as to lead to a false diagnosis of acute cholecystitis. Biochemically, drug-associated cholestasis resembles other forms of cholestasis although the presence of eosinophilia may suggest drug involvement. Many types of drug-induced cholestasis run a benign course with resolution of signs and symptoms within 3 months but occasionally the jaun

Five cases of intrahepatic cholestasis are reported in 4 families in which 7 other children have had the same disease. Cholestasis, beginning before the age of one year, is notable for the absence of lipid retention, evolving by attacks brought on by intercurrent infections with gradual development of fibrosis and fatal cirrhosis before the age of 15. Extrahepatic and interlobular bile ducts are undamaged. The disease is sometimes accompanied by biliary and pancreatic abnormalities, perhaps due to an abnormality in the biliary acid metabolism.

Idiopathic neonatal hepatitis is one of the more important causes of neonatal cholestasis. It is regarded one of the clinical presentations of 'idiopathic obstructive cholangiopathy', just like extrahepatic biliary atresia. Is it not possible to discriminate between intrahepatic and extrahepatic causes of neonatal cholestasis, or between idiopathic neonatal hepatitis and metabolic, infectious, or toxic causes, by using clinical or laboratory parameters. Liver histology is slightly more helpful: giant cell formation, focal liver necrosis, and lymphocytic and neutrophilic infiltration may be fou

We describe cholestasis as a result of bile duct abnormalities in 8 children with portal vein obstruction. In a clinical, biochemical and radiological investigation of 121 children with cavernous transformation of the portal vein seen between 1986 and 2000, 8 presented with jaundice, pruritus, and/or raised serum aminotransferases and/or gamma glutamyl transpeptidase (gamma GT) activities. Each displayed dilation and narrowing of intra- and/or extrahepatic bile ducts. Surgical decompression of the portal system (portal-systemic or Rex anastomosis) resulted in the regression of the signs of cho

This article summarizes our present knowledge concerning drug-induced cholestasis by lesions of the bile ducts. Acute or chronic ductular and ductal cholestasis are induced by a few drugs. The acute form may mimic viral hepatitis or biliary tract obstruction. The outcome is generally good. The mechanism may be immunoallergic. The clinical presentation of the chronic form resembles that of primary biliary cirrhosis or sclerosing cholangitis. Its prognosis is generally good and characterized by a complete recovery. However, an evolution to biliary cirrhosis is sometimes possible. The mechanism c

The causes of cholestasis range from intrinsic hepatocellular disease to extrahepatic mechanical obstruction of the bile tree. Gallstones are the most common cause of obstructive cholestasis, but it is not seldomly associated with malignant disease. A systematic approach to diagnosing cholestatic liver disease, including thorough patient history, clinical examination, lab-panels and proper use of imaging and treatment alternatives, starting from least invasive and most cost-effective, ensures efficient patient outcomes and minimises the risk of complications as summarised in this review.

Drug-induced cholestasis may be due to impairment of hepatocellular bile secretion (pure cholestasis or cholestatic hepatitis), obstruction of ductules (cholangiolitis) or interlobular ducts (cholangitis), or extrahepatic obstruction (sclerosing cholangitis). Mechanisms of hepatocellular cholestasis are multiple and include inhibition of various transport systems, cytoskeleton poisoning, disturbed intracellular calcium homeostasis and increased permeability with regurgitation of bile constituents into plasma. Pure hepatocellular cholestasis is mostly observed with sex steroid hormones and anab

BACKGROUND: Cholestasis is known as an etiologically diverse clinical entity which requires a broad differential diagnostic workup. In the majority of patients, history, clinical examination, clinical chemical analysis, and abdominal ultrasound enable the differentiation between extrahepatic and intrahepatic cholestasis. This review summarizes our current knowledge in the diagnosis of cholestatic disorders.
METHODS: In regard to clinical practice, diagnostic tools and new developments in imaging and molecular genetics are discussed including an algorithm for the diagnostic workup of cholestat

Oxidative stress and mitochondrial dysfunction are hypothesized to contribute to the pathogenesis of chronic cholestatic liver diseases. Silent information regulator 1 (SIRT1) attenuates oxidative stress and improves mitochondrial biogenesis in numerous mitochondrial-related diseases; however, a functional role for SIRT1 in chronic liver cholestasis, characterized by increased levels of toxic bile acids, remains unknown. We show decrease in SIRT1 levels and its activity and impairment of mitochondrial biogenesis in the liver of patients with extrahepatic cholestasis. Moreover, we found that gl

We retrospectively evaluated the utility of hepatobiliary scintigraphy and various clinical factors in differentiating intrahepatic cholestasis from biliary atresia in 28 consecutive infants with neonatal cholestasis. One millicurie of technetium-labeled diisopropyliminodiacetic acid (DISIDA) was administered intravenously, and images were obtained for up to 24 hours or until gastrointestinal excretion was noted. Nine separate studies in seven infants with biliary atresia were correctly interpreted as showing no gastrointestinal excretion of radionuclide. Of the 21 patients with intrahepatic c

This case report describes a 6-year-old Holstein Friesian cow with extrahepatic cholestasis. The most prominent clinical signs were disturbed general condition and behaviour, icterus, yellowish brown urine, and bilirubinuria. An increased concentration of serum bilirubin and an increase in hepatic enzyme activity were the most important laboratory findings. Ultrasonographic examination of the liver and gall bladder revealed severe bile stasis. The intrahepatic bile ducts, the cystic duct, and the gall bladder were severely dilated. Histological examination of a liver biopsy revealed cholestasi

The natural history of cholestatic syndromes in infancy remains largely unclarified for lack of sufficient data. Newborn and premature infants are particularly vulnerable to cholestasis because of immaturities in bile-forming mechanisms. Until recently, two board categories of etiologic factors has been thought to be associated with cholestasis in early infancy: mechanical obstruction (almost always extrahepatic), and hepatocellular damage (the "neonatal hepatitis" group). Although in both groups specific etiologic factors have been identified, the majority of cases are currently of unknown et
