Kissing choroidal sign: A case report.

Choroidal detachment (CD) is a rare and potentially vision-threatening complication of glaucoma surgery. Inflammation and prolonged ocular hypotony can promote fluid accumulation between the choroid and sclera. Risk factors include trauma, advanced age, use of anticoagulant medications, systemic hypertension, atherosclerosis, and diabetes. CD ultrasound findings will show 2 layers, detaching as far anteriorly as the ciliary bodies, that protrude convexly into the vitreous without extending to the optic disc, often described as the appositional or kissing choroidal sign. In contrast, retinal de

Two cases of uveal effusion syndrome.

PURPOSE: To report a case of uveal effusion syndrome associated with hypotony and a case of uveal effusion syndrome in nanophthalmos. METHODS: The first case was a 25-year-old man who presented with decreased visual acuity in the left eye and hypotony. Fundus examination revealed choroidal effusion and retinal detachment with a thickened eyeball. Partial thickness sclerotomy and sclerectomy were performed. The second case was a 13-year-old boy who had uveal effusion syndrome with a nanophthalmic eye. RESULTS: In the patient with hypotony, intraocular pressure was well maintained following pa

Anterior Chamber Sulfur Hexafluoride Injection for Hypotony Management: Case Report and Literature Review.

INTRODUCTION: Hypotony is a potentially vision-threatening condition that may persist despite conservative management, often necessitating surgical or interventional treatment. We present a case where a minimally invasive injection of 100% sulfur hexafluoride (SF6) into the anterior chamber (AC) successfully reversed refractory hypotony secondary to ciliary body shut-down. CASE PRESENTATION: A 72-year-old male with advanced pseudoexfoliation glaucoma and a nonfunctioning trabeculectomy presented with hypotony, decreased vision, a deep AC, and 360° serous choroidal effusions. Choroidal detachment with exudative retinal detachment following Ahmed valve implantation in Sturge-Weber syndrome.

Ahmed glaucoma valve implant appears to be a relatively useful drainage device in eyes with glaucoma secondary to Sturge-Weber syndrome (SWS). However, early postoperative choroidal and exudative retinal detachment may occur from a rapid expansion of the choroidal hemangioma with effusion of fluid into the suprachoroidal and subretinal spaces. We report the case of a ten-year-old boy who had SWS with choroidal haemangioma and secondary glaucoma. He had Ahmed glaucoma valve implantation on account of the secondary glaucoma which had been refractory to both conventional medical and surgical mana

[Uveal effusion syndrome--case report].

The history and clinical findings of 60-year old man with effusion syndrome are presented. The uveal effusion syndrome typically affects healthy middle age men and causes recurrent, spontaneous, serous retinal and cilio-choroidal detachments which, often results in significant visual impairment. In addition, to the general clinical examinations, indirect ophthalnoscopy, fundus fluorescein angiography, ultrasonography and MRI are used to make definitive diagnosis. Annular cilio-choroidal detachment, shifting non-rheumatogenous retinal detachment, unremarkable inflammation in the anterior segmen

Ciliary body detachment caused by capsule contraction.

A 74-year-old woman developed capsule contraction associated with hypotony and choroidal effusion 18 months after uneventful phacoemulsification with 3-piece poly(methyl methacrylate) intraocular lens implantation. Ultrasound biomicroscopy revealed ciliary body detachment and stretched zonules. A radial neodymium: YAG anterior capsulotomy was performed, resulting in the resolution of the ciliary body detachment and choroidal effusion as well as in normal intraocular pressure over 4 days.

Surgical Management of Choroidal Diseases.

Accumulation of serous fluid in the suprachoroidal space, known as uveal effusion, and choroidal or suprachoroidal haemorrhage (SCH) following rupture of ciliary blood vessels are considered rare, but serious, events with extremely poor functional prognosis. As a result, uveal effusion, and expulsive suprachoroidal haemorrhage in particular, continue to be considered as more or less fatal complications. However, clinical experience demonstrates that both clinical entities can be managed by conservative as well as surgical strategies, depending on their severity and localisation, with sometimes

[Choroidal detachment--pathogenesis, etiology and clinical features].

Serous choroidal detachment is characterized by exudative detachment of the retina and choroid following leakage of fluid from the choriocapillaris into suprachoroidal space. This accumulation of fluid has been known to be a complication of various intraocular surgeries (cataract, glaucoma and retinal detachment surgery), where hypotony is combined with postoperative inflammation. Among other non surgical conditions associated with uveal effusion are idiopathic (uveal effusion syndrome, microphthalmia) and inflammatory diseases (scleritis, sympathic ophthalmia, pars planitis, Harada's disease)

Kissing Choroidals Despite Normal Intraocular Pressure Following Ahmed Clear Path Implantation in a Vitrectomized Eye.

BACKGROUND Choroidal effusion is a known complication of glaucoma drainage devices. It is typically associated with hypotony. The Ahmed Clear Path (ACP) implant, a novel valveless glaucoma drainage device with an intraluminal ripcord, is designed to reduce such risks. However, eyes with altered anatomy - such as those previously vitrectomized - may still be prone to atypical postoperative outcomes. We report the first known case of "kissing" serous choroidals following ACP implantation in a vitrectomized eye, despite normal intraocular pressure (IOP). CASE REPORT A 68-year-old monocular man wi

Orbital lymphoma presenting with choroidal detachments.

This report describes a case of orbital B-cell lymphoma of extranodal marginal cell type with atypical clinical and radiographic features. This patient presented with choroidal effusion and optic disk swelling of the right eye. A CT scan revealed an intraconal mass surrounding the optic nerve and indenting the globe. A right orbitotomy with biopsy of the mass was performed. Histologic and immunohistochemical analysis confirmed a diagnosis of extranodal marginal zone B-cell lymphoma. This is the first case of primary orbital lymphoma presenting with choroidal effusion described in the literatur

Morbidity following prolonged postoperative hypotony after trabeculectomy.

The postoperative course of a series of 54 patients that had undergone primary trabeculectomy for open-angle glaucoma is reviewed. Almost one third of the eyes had prolonged postoperative hypotony with an intraocular pressure less than 8mm Hg for more than 2 weeks. The possible significance of prolonged hypotony and choroidal effusions is discussed in relation to the final intraocular pressure and the effect on visual acuity.

Rapid resolution of severe exudation in uveal effusion syndrome with anti-vascular endothelial growth factor alone in a case of bilateral nanophthalmos: a case report.

BACKGROUND: Uveal effusion syndrome is a rare disease characterized by exudative detachments of the choroid, ciliary body, and retina. Various surgical procedures and nonsurgical strategies have been described to treat uveal effusion syndrome with limited success. The treatment for uveal effusion syndrome remains a serious challenge for clinicians. To the best of our knowledge, no previous report has described a severe uveal effusion syndrome patient with nanophthalmos treated by using an anti-vascular endothelial growth factor agent alone. We report here one such case with unexpected positive

Novel technique for traumatic cyclodialysis repair with phacoemulsification and Cionni's ring implantation.

A 17-years-old boy presented with cataract, 360° choroidal effusion and disc oedema secondary to chronic hypotony. Gonioscopy and AS-OCT revealed 2 clock hours of cyclodialysis in the superonasal quadrant. The case was successively managed with phacoemulsification with foldable IOL in the bag and Cionni's ring sutured in the superonasal quadrant. Post surgery, the BCVA improved from counting finger 1 meter to 20/40. IOP returned to normal and there was resolution of choroidal effusion and fundus signs. The case highlights the use of Cionni's ring in management of small cyclodialysis cleft by p
