Chronic idiopathic urticaria (CIU) is a common dermatologic disorder that may severely impair quality of life. Patients may suffer symptoms such as pruritus and disfigurement due to wheals for years or decades. Advances have been made in the last 10 years with the identification of an autoimmune pathogenesis in a significant proportion of patients. Despite this, treatment remains symptomatic, and antihistamines are the first choice of therapy once the diagnosis of CIU has been established. The goal of treatment is rapid, long-lasting symptom relief, and currently available antihistamines fail

Psychologic symptoms of 102 adult patients with asthma and 74 adult patients with chronic idiopathic urticaria, plus 252 normal nonclinical controls and 383 psychologically distressed ('socially isolated') but medically healthy adults were assessed by means of the Brief Symptom Inventory (BSI). On average, both asthma and urticaria patients had more psychopathology-evident in most symptom dimensions of the BSI-than healthy controls but much less than 'socially isolated' adults. Nevertheless, only about 40% of the subjects in both patient groups had above-normal BSI scores. Both men and women w

For many years, H1 antihistamines have been the primary management option for urticaria. However, undesirable side effects, particularly daytime sedation, have limited the usefulness of these classic antihistamines. A new class of peripherally acting, nonsedating antihistamines (e.g., terfenadine, astemizole, loratadine, and cetirizine) has proved to have clinical efficacy comparable with the classic antihistamines. In comparative trials between the various nonsedating agents, no significant difference in efficacy has been noted. All these agents have good safety profiles, although astemizole

In a group of patients with a syndrome consisting of recurrent episodes of urticaria, arthralgia, abdominal pain, and (rarely) glomerulonephritis, examination of skin biopsy specimens showed necrotizing venulitis. An elevated erythrocyte sedimentation rate was the most common laboratory abnormality. Analyses of serum immunoglobulins revealed random abnormalities of immunoglobulin levels, and assessment of the complement system showed two groups of patients--some with hypocomplementemia and others with a normal complement system. In those with hypocomplementemia, there were low levels of C1q, C

Most of the literature on chronic urticaria has emphasized the difficulties of identifying etiology. Employing the sequential vascular response test, the authors have had a 93% success rate in determining causative factors in 156 clinic patients and of identifying specific aggravants in 85% of 300 cases.

BACKGROUND/OBJECTIVES: Omalizumab is an effective treatment for antihistamine-refractory chronic urticaria. However, post-injection urticaria exacerbation and related adverse reactions remain inconsistently reported. This scoping review mapped evidence on clinical presentation, timing, mechanisms, management strategies, rechallenge outcomes, treatment discontinuation, and patient outcomes associated with these reactions.
METHODS: The review followed the Joanna Briggs Institute methodology and was registered prospectively in INPLASY (INPLASY202650060). PubMed, Scopus, and the Directory of Open

Chronic urticaria is a common skin disorder that substantially affects both daily functioning and quality of life. Among its subtypes, chronic inducible urticaria often presents with greater therapeutic challenges than chronic spontaneous urticaria. A comprehensive review of the available evidence indicates that β-blockers may offer a rapid-acting, well-tolerated, and effective treatment option for patients with specific subtypes of chronic inducible urticaria specifically cholinergic urticaria, adrenergic urticaria, and the aquagenic continuum. This approach introduces a readily available, no

PURPOSE OF REVIEW: To provide an update and new insights into the management of pediatric patients with chronic urticaria.
RECENT FINDINGS: Chronic urticaria in children may be more prevalent than previously thought, with a high resolution rate within two years. Patient-reported outcome measures (PROMs) are effective tools for assessing disease severity and control in pediatric populations. Children with chronic urticaria generally respond better to H1-antihistamines compared to adults. For patients refractory to antihistamines, omalizumab has shown excellent efficacy. Additionally, dupilumab

PURPOSE OF REVIEW: While aeroallergens are a well-established trigger of asthma and allergic rhinitis, their role in allergic skin diseases such as atopic dermatitis and chronic urticaria remains controversial. This paper reviews the pathophysiology and clinical evidence for aeroallergens in these allergic skin diseases and summarizes current strategies for evaluation and management.
RECENT FINDINGS: Current evidence implicates aeroallergens as triggers of cutaneous reactions in atopic dermatitis. Direct skin contact is the likely route of trigger. Aeroallergens may also trigger chronic urtic

Urticaria commonly presents with intensely pruritic wheals, sometimes with edema of the subcutaneous or interstitial tissue. It has a lifetime prevalence of about 20%. Although often self-limited and benign, it can cause significant discomfort, continue for months to years, and uncommonly represent a serious systemic disease or life-threatening allergic reaction. Urticaria is caused by immunoglobulin E- and non-immunoglobulin E-mediated release of histamine and other inflammatory mediators from mast cells and basophils. Diagnosis is made clinically; anaphylaxis must be ruled out. Chronic urtic

Omalizumab is a recombinant humanized monoclonal antibody that inhibits immunoglobulin E. It has been approved for the treatment of severe asthma and chronic spontaneous urticaria refractory to other treatments. Its use in the management of chronic inducible urticaria (a type triggered by certain stimuli) is still considered off-label, although this use has been discussed in some consensus papers. This review brings together case reports and case series describing the use of omalizumab to treat chronic inducible urticaria. We analyze the most important aspects of the cases and the outcomes rep

Developments increasing our understanding of chronic urticaria have resulted in the simplification and improvement of available treatments. Currently, many treatments target mast cell mediators, but we can now disrupt mast cell activation with the anti-IgE antibody omalizumab, which has markedly advanced the treatment landscape for patients with difficult-to-treat urticaria. Current guidelines provide a framework for the management and treatment of patients with CU but, as each patient is different, knowledge and experience of specialist dermatologists and allergists are key to effective pharm

BACKGROUND: Urticaria produces weals, which are itchy and can affect the quality of life and social functioning of an affected patient. There are several different subtypes of urticaria. This article focuses on chronic urticaria.
OBJECTIVE: This article details the approach to assessment and management of chronic urticaria.
DISCUSSION: Chronic urticaria is characterised by the occurrence of weals, which occur on a daily basis for more than 6 weeks. Clinical history and physical examination alone are sufficient to establish a diagnosis. Non-sedating histamine H1 receptor antagonists are the f

BACKGROUND: Chronic idiopathic urticaria (CIU) is a common dermatological condition. Its pathogenesis involves mainly histamine and also other mediators, including platelet-activating factor (PAF) and tumor necrosis factor-α (TNF-α). In the absence of an exact etiology, H1 -antihistaminics are the mainstay of treatment. Levocetirizine is widely prescribed for CIU. Rupatadine, a newer antihistaminic, has PAF receptor antagonist activity and has shown anti-TNF-α activity in vitro. These additional anti-inflammatory effects may improve its efficacy.
OBJECTIVES: This study was conducted to compar

Chronic urticaria is a common disease characterized by recurrent pruritic wheals with surrounding erythema for >6 weeks. It is associated with a significant health care burden and affects patient quality of life. The etiology of chronic urticaria is often difficult to elucidate; however, known etiologies include autoimmune urticaria, physical urticarias (eg, cold, cholinergic, and delayed pressure urticaria), and idiopathic urticaria. The etiology is unknown in many patients, leading to a diagnosis of chronic idiopathic urticaria. The diagnosis of chronic idiopathic urticaria can be challengin
