Limited clinical utility of mutation-based circulating tumor DNA detection in pseudomyxoma peritonei.

Pseudomyxoma peritonei (PMP) is a rare abdominal cancer where curative treatment involves cytoreductive surgery and hyperthermic intraperitoneal chemotherapy. Postoperative surveillance relies on radiological imaging and blood tumor markers, which lack sensitivity and specificity. Circulating tumor DNA (ctDNA) analysis could offer a non-invasive alternative, but evidence in PMP is limited. Plasma samples from 95 PMP patients carrying KRAS and/or GNAS tumor mutations were analyzed using droplet digital PCR. Clinicopathological parameters and outcome were assessed, and disease-free survival (DFS

Post-treatment circulating tumor DNA clinical features, prognosis in HPV-positive oropharynx cancer.

INTRODUCTION: Treatment de-escalation has been studied to reduce treatment toxicity in patients with HPV-associated oropharyngeal cancer(OPSCC). Circulating tumor DNA(ctDNA) is a potential non-invasive tool to assist treatment decision-making and recurrence detection. This study investigates the association between ctDNA clearance at timepoints during and after treatment with clinicopathological characteristics and outcomes. METHODS: In this retrospective cohort study conducted between April 2020-August 2025 at a tertiary care center in a large city, 132 patients treated for HPV-associated OP

Diagnostic Performance of a Multiplex Circulating Tumor DNA Assay for Lung Cancer Detection.

Lung cancer remains the leading cause of cancer-related death worldwide, largely because of late-stage diagnosis. Although low-dose computed tomography screening can reduce mortality, it is limited by high false-positive rates, radiation exposure, and low adherence. Thus, there is an urgent need for precise, accessible, and noninvasive diagnostic alternatives. This study evaluated the diagnostic accuracy of a novel multiplexed digital droplet PCR (ddPCR) assay targeting hypermethylated circulating tumor DNA (ctDNA) markers (HOXA9, OTX1, MCIDAS, SP9, and TFAP2B) in plasma for lung cancer detect

Active enrichment of circulating tumor DNA by supramolecular-functionalized micromotors for ultrasensitive quantification.

Liquid biopsy of pancreatic cancer via circulating tumor DNA (ctDNA) is challenged by the extreme rarity of targets, a limitation that conventional passive biosensors cannot overcome. Here, we engineered an intelligent micromotor that actively captured, transported, and electrochemically reported ctDNA. The asymmetric flask-shaped micromotor (HPCMF) was constructed from carbonaceous micro-flasks (CMF) decorated with platinum nanoparticles (PtNPs) and supramolecular (HP5) stabilized PtNPs. The HP5 moieties served as docking sites for methylene blue-labeled assist-DNA (MB-aDNA) via host-guest in

Circulating tumor DNA in lung cancer immunotherapy: prognostic marker today, predictive tool tomorrow?

Circulating tumor DNA (ctDNA) is emerging as a minimally invasive biomarker for risk stratification and treatment-response assessment in lung cancer immunotherapy, but its routine use for treatment selection has not been established. This review critically evaluates evidence across neoadjuvant, adjuvant, consolidation after definitive chemoradiotherapy, and advanced/metastatic settings, with emphasis on serial sampling and on the distinction among prognostic, response-associated, and predictive roles. Across disease stages, baseline or post-definitive-treatment ctDNA detectability consistently

Development of a circulating tumor DNA methylation biomarker panel for hepatocellular carcinoma detection.

INTRODUCTION: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality worldwide. Early detection is crucial for improving the overall survival rates in patients diagnosed with HCC. MATERIALS AND METHODS: In this study, we developed a novel methylation-based gene panel for HCC detection using a large cohort of 149 patients with HCC and 270 controls, which included 109 healthy individuals, 102 patients with benign liver diseases, and 59 patients with non-hepatic malignancies. The diagnostic performance of the panel was assessed by generating receiver operating characterist

Circulating Tumor DNA as a Biomarker for Disease Surveillance in Anaplastic Thyroid Cancer.

PURPOSE: Anaplastic thyroid cancer (ATC) is a highly lethal malignancy in which early detection of progression could allow timely treatment modifications and potentially improve outcomes. Novel circulating tumor DNA (ctDNA) assays allow the detection of molecular residual disease. We aimed to investigate the utility of ctDNA as a biomarker in ATC. MATERIALS AND METHODS: Retrospective cohort study of patients with ATC and at least one ctDNA drawn before systemic therapy start (referred to as baseline) between February 2021 and August 2024. ctDNA was correlated with concomitant imaging and/or p

Prediction model for mutation detection via circulating tumor DNA analysis in pancreatic cancer.

BACKGROUND: Comprehensive genomic profiling (CGP) is essential for precision medicine in pancreatic cancer (PC) patients. In unresectable cases, obtaining sufficient tissue is challenging, making plasma-based liquid CGP a common choice. The successful detection of KRAS mutations, which are present in more than 90 % of pancreatic cancers, indicates that liquid CGP has successfully detected circulating tumor DNA. This study aimed to develop a prediction model for detecting KRAS mutations in liquid CGP among patients with PC using the Japanese National Database. METHODS: We evaluated the detecti

Clinical Implications of Circulating Tumor DNA in Multiple Myeloma and Its Precursor Diseases.

BACKGROUND: Genetic alterations play a pivotal role in multiple myeloma (MM) development and therapeutic resistance. Traditionally, the genetic profiling of MM requires invasive bone marrow (BM) procedures; however, these procedures are associated with patient discomfort and cannot fully capture the spatial and temporal heterogeneity of the disease. Therefore, we investigated the clinical implications of liquid biopsy using targeted deep sequencing. METHODS: We analyzed the genetic profiles of circulating tumor DNA (ctDNA) by targeted deep sequencing from 102 patients, including those with mo

Monitoring Response Using Circulating Tumor DNA in Undifferentiated Pleomorphic Sarcoma: A Case Report.

Circulating tumor DNA (ctDNA) can be used to assess treatment response in patients with undifferentiated pleomorphic sarcoma (UPS). The importance of this is explored in our case of a 75-year-old man who was diagnosed with UPS of the right kidney. After a right nephrectomy and tumor resection, the patient was recovering well with initially undetectable, and then slightly elevated, circulating tumor DNA. Abdominal pain started shortly before a scheduled magnetic resonance imaging (MRI) which revealed a large mass in the resection bed invading the liver. The patient was treated with gemcitabine

CRISPR/dCas9-Mediated Specific Molecular Assembly Facilitates Genotyping of Mutant Circulating Tumor DNA.

Breakthroughs in circulating tumor DNA (ctDNA) analysis are critical in tumor liquid biopsies but remain a technical challenge due to the double-stranded structure, extremely low abundance, and short half-life of ctDNA. Here, we report an electrochemical CRISPR/dCas9 sensor (E-dCas9) for sensitive and specific detection of ctDNA at a single-nucleotide resolution. The E-dCas9 design harnesses the specific capture and unzipping of target ctDNA by dCas9 to introduce a complementary reporter probe for specific molecular assembly and signal amplification. By efficient homogeneous assembly and inter

Postoperative recurrence detection using individualized circulating tumor DNA in upper tract urothelial carcinoma.

Biomarkers that could detect the postoperative recurrence of upper tract urothelial carcinoma (UTUC) have not been established. In this prospective study, we aim to evaluate the utility of individualized circulating tumor DNA (ctDNA) monitoring using digital PCR (dPCR) as a tumor recurrence biomarker for UTUC in the perioperative period. Twenty-three patients who underwent radical nephroureterectomy (RNU) were included. In each patient, whole exome sequencing by next-generation sequencing and TERT promoter sequencing of tumor DNA were carried out. Case-specific gene mutations were selected fro

Genomic approaches to cancer and minimal residual disease detection using circulating tumor DNA.

Liquid biopsies using cell-free circulating tumor DNA (ctDNA) are being used frequently in both research and clinical settings. ctDNA can be used to identify actionable mutations to personalize systemic therapy, detect post-treatment minimal residual disease (MRD), and predict responses to immunotherapy. ctDNA can also be isolated from a range of different biofluids, with the possibility of detecting locoregional MRD with increased sensitivity if sampling more proximally than blood plasma. However, ctDNA detection remains challenging in early-stage and post-treatment MRD settings where ctDNA l

Circulating Tumor DNA Profiling for Detection, Risk Stratification, and Classification of Brain Lymphomas.

PURPOSE: Clinical outcomes of patients with CNS lymphomas (CNSLs) are remarkably heterogeneous, yet identification of patients at high risk for treatment failure is challenging. Furthermore, CNSL diagnosis often remains unconfirmed because of contraindications for invasive stereotactic biopsies. Therefore, improved biomarkers are needed to better stratify patients into risk groups, predict treatment response, and noninvasively identify CNSL. PATIENTS AND METHODS: We explored the value of circulating tumor DNA (ctDNA) for early outcome prediction, measurable residual disease monitoring, and su

Prediction and monitoring of relapse in stage III melanoma using circulating tumor DNA.

BACKGROUND: The advent of effective adjuvant therapies for patients with resected melanoma has highlighted the need to stratify patients based on risk of relapse given the cost and toxicities associated with treatment. Here we assessed circulating tumor DNA (ctDNA) to predict and monitor relapse in resected stage III melanoma. PATIENTS AND METHODS: Somatic mutations were identified in 99/133 (74%) patients through tumor tissue sequencing. Personalized droplet digital PCR (ddPCR) assays were used to detect known mutations in 315 prospectively collected plasma samples from mutation-positive pat

Whole-genome Mutational Analysis for Tumor-informed Detection of Circulating Tumor DNA in Patients with Urothelial Carcinoma.

BACKGROUND AND OBJECTIVE: Circulating tumor DNA (ctDNA) can be used for sensitive detection of minimal residual disease (MRD). However, the probability of detecting ctDNA in settings of low tumor burden is limited by the number of mutations analyzed and the plasma volume available. We used a whole-genome sequencing (WGS) approach for ctDNA detection in patients with urothelial carcinoma. METHODS: We used a tumor-informed WGS approach for ctDNA-based detection of MRD and evaluation of treatment responses. We analyzed 916 longitudinally collected plasma samples from 112 patients with localized
