Religion and conscientious objection: a survey of pharmacists' willingness to dispense medications.

Some US states allow pharmacists to refuse to dispense medications to which they have moral objections, and federal rules for all health care providers are in development. This study examines whether demographics such as age, religion, gender influence 668 Nevada pharmacists' willingness to dispense or transfer five potentially controversial medications to patients 18 years and older: emergency contraception, medical abortifacients, erectile dysfunction medications, oral contraceptives, and infertility medications. Almost 6% of pharmacists indicated that they would refuse to dispense and refus

Community pharmacist-led point-of-care eGFR screening: early detection of chronic kidney disease in high-risk patients.

Adherence to scheduled physician screenings for renal function monitoring in patients with chronic kidney disease (CKD) or those at high risk remains suboptimal despite the endorsement of regular screenings by several clinical practice guidelines. Our study aims to assess the effectiveness of a point-of-care CKD screening program led by these pharmacists using the PICCOLO device while recognizing the unique position of community pharmacists in primary care. We conducted an 11-month prospective point-of-care interventional research study in the United Arab Emirates to evaluate the performance o

Impact of Laboratory Data Integration into Outpatient Prescriptions on Community Pharmacist Feedback and Physician Responses: A Single-Center Retrospective Study.

PURPOSE: Laboratory data with outpatient prescriptions have expanded, and their usefulness has been reported. However, the effect of a modification in laboratory data provision on the content of tracing reports (TRs) submitted by community pharmacists to provide clinical feedback to physicians and on physician responses remains unclear. This study aimed to evaluate the impact of a system-level modification in laboratory data provision on the inclusion of laboratory data-related descriptions in TRs and to assess the association between TR characteristics and physician responses. Why MASLD Trials Underselect the MENA Region Sites and What Sponsors and Regulators Must Change.

Metabolic dysfunction associated steatotic liver disease (MASLD) and metabolic dysfunction associated steatohepatitis (MASH) are accelerating global priorities, yet pivotal clinical trial activity remains concentrated in North America and Europe despite substantial burden in the Middle East and North Africa (MENA). Building on prior work quantifying MENA underrepresentation, we provide a policy and operational analysis informed by contemporary epidemiological, regulatory, and clinical trial literature and practical experience in trial delivery. We argue that underselection is driven less by sc

The Use of Prophylactic Antibiotics before Primary Palatoplasty Is Not Associated with Lower Fistula Rates: An Outcome Study Using the Pediatric Health Information System Database.

BACKGROUND: Previous attempts to study the effect of prophylactic antibiotics on the outcomes of cleft palate surgery have been hampered by the need for a very large sample size to provide adequate power to discern a potentially small therapeutic effect. This limitation can be overcome by querying large databases created by health care governing bodies. METHODS: Data from the Pediatric Health Information System database were used for this analysis. Patients, aged 6 to 18 months, who had undergone primary palatoplasty (International Classification of Diseases, Ninth Revision, code 27.62) betwe

"I still can feel the sickness": Withdrawal experiences of people on methadone maintenance treatment.

INTRODUCTION: Opioid withdrawal is a regular occurrence for many people who use illicit opioids (PWUIO) involving acute physical and psychological pain. Yet, there is very little data on the withdrawal experience of people in methadone maintenance treatment (MMT) and almost none from the patients' experience. Learning more about patients' withdrawal experiences can help to inform policies and practices that are better suited to address withdrawal and may improve patient satisfaction as well as uptake and retention. METHODS: This article is based on 29 semi-structured interviews with people wh

Monthly Headaches and Severity in Patients on Galcanezumab or Traditional Preventive Migraine Medication: A 24-Month Claims and Electronic Health Records Study.

INTRODUCTION: Migraine, affecting millions globally, imposes a significant burden on patients and healthcare systems. Calcitonin gene-related peptide monoclonal antibodies are recommended as first-line preventive treatments by international guidelines, yet real-world prospective studies comparing their year-long effectiveness to standard of care (SOC) treatments are time-consuming, resource-intense and therefore limited. This study aimed to test the utility of claims data and electronic health records (EHR) by evaluating changes in monthly headache days (MHDs) and disease severity among US pat

From classroom to the counter: exploring knowledge, attitudes, and practices toward Down syndrome among pharmacy students and pharmacists.

BACKGROUND: The worldwide prevalence of Down syndrome (DS) is increasing. Limited DS-specific information and dosing guidelines can hinder pharmacists' ability to effectively respond to DS related inquiries. As accessible healthcare providers, pharmacists are well positioned to identify and address these challenges, yet their role in optimizing DS care remains underexplored. This study aimed at evaluating community pharmacists' knowledge, confidence, and practices related to DS pharmacotherapy, and to identify training needs that can improve their role in optimizing medication management for i

Health seeking behaviour and self-treatment for common childhood symptoms in rural Guatemala.

This study was conducted in the Guatemalan highland department of Sacatepequez, in a sample of 146 rural women insured by the Social Security system. It examined their health care behaviour when their children presented common childhood symptoms such as diarrhoea, fever, cough and worms. The mothers generally sought help and treatment advice from an older woman in the family, and did so more often for diarrhoea (82%) and fever (64%) than for cough (43%) or worms (28%). Obtaining advice in a pharmacy or from a drug seller ranked second (range: 8%-38%, depending on the symptom), before the procu

Reducing sleep medication via parallel delivery of clinical pharmacist-led deprescribing and clinician-supervised asynchronous CBT for insomnia: the SEDATIVE trial.

Chronic insomnia is a prevalent disorder that negatively impacts Veterans’ health. The first-line intervention is cognitive behavioral therapy for insomnia (CBT-I); however, sleep medications, which have unreliable effectiveness and pose risks for adverse events, are the most commonly used intervention. Pairing CBT-I with deprescribing (the supervised reduction of medications) can enhance medication reduction outcomes and reduce rebound insomnia. This pilot trial tests the parallel delivery of Clinical Pharmacy Practitioner (CPP) deprescribing and a digital clinical decision support (CDS) plat

Reduction of Sleep Medications via a Combined Digital Insomnia and Pharmacist-Led Deprescribing Intervention: Protocol for a Feasibility Trial.

BACKGROUND: Chronic insomnia is one of the most common health problems among veterans and negatively impacts their health, function, and quality of life. Although cognitive behavioral therapy for insomnia (CBT-I) is the first-line recommended treatment, sedative-hypnotic medications remain the most common. Sedative-hypnotics, however, have mixed effectiveness, are frequently prescribed longer than recommended, and are associated with numerous risks and adverse effects that negatively impact veteran function. Meeting the treatment needs of veterans impacted by insomnia requires delivering gold
