Stiripentol in the Management of Epilepsy.

Stiripentol is a structurally unique antiepileptic drug that has several possible mechanisms of action, including diverse effects on the gamma-aminobutyric acid (GABA)-A receptor and novel inhibition of lactate dehydrogenase. Because of its inhibition of several cytochrome P450 enzymes, it has extensive pharmacokinetic interactions, which often necessitates reduction in doses of certain co-therapies, particularly clobazam. Stiripentol also has a neuroprotective action, by reducing calcium-mediated neurotoxicity. Evidence of its efficacy is most robust for Dravet syndrome, where stiripentol add

Real-world data on cannabidiol treatment of various epilepsy subtypes: A retrospective, multicenter study.

OBJECTIVE: Cannabidiol (CBD) is approved for treatment of Dravet syndrome (DS), Lennox-Gastaut syndrome (LGS), and tuberous sclerosis complex (TSC). Several studies suggest antiseizure effects also beyond these three epilepsy syndromes. METHODS: In a retrospective multicenter study, we analyzed the efficacy and tolerability of CBD in patients with epilepsy at 16 epilepsy centers. RESULTS: The study cohort comprised 311 patients with epilepsy with a median age of 11.3 (0-72) years (235 children and adolescents, 76 adults). Therapy with CBD was off-label in 91.3% of cases due to age, epilepsy

[Electrical status epilepticus during sleep: a retrospective multi-centre study of 29 cases].

INTRODUCTION: Electrical status epilepticus during sleep (ESES) is an epileptic syndrome characterised by the presence of very persistent slow spike-wave-type epileptic discharges during non-REM sleep. The management of this pathology, today, is heterogeneous and no controlled studies have been conducted with the treatments employed; similarly, whether or not they improve patients' cognitive development or not has still to be determined. PATIENTS AND METHODS: A review was carried out of the patients diagnosed with ESES at four hospitals over a period of 15 years; data concerning their clinica

Cannabidiol inhibits phenytoin clearance and can result in clinical changes: Two cases.

PURPOSE: Cannabidiol has well described drug interactions including inhibition of clobazam metabolism. Phenytoin is known to induce metabolism of cannabidiol. Cannabidiol inhibition of phenytoin metabolism is predicted but not well described. METHODS: We present the cases of two individuals with Lennox Gastaut Syndrome which demonstrate the clinical relevance of cannabidiol inhibition of phenytoin metabolism. RESULTS: In the first case, the addition of cannabidiol led to increased levels of phenytoin and clobazam which resulted in freedom from previously refractory tonic-clonic seizures. Thi

Clinical efficacy and safety of cannabidiol for pediatric refractory epilepsy indications: A systematic review and meta-analysis.

Antiseizure medications (ASMs) are the mainstay for the treatment of seizure disorders. However, about one-third of people with epilepsy remain refractory to current ASMs. Cannabidiol (CBD) has recently been approved as ASM for three refractory epilepsy syndrome indications in children and adults. In this study, we evaluated the overall clinical potential of an oral CBD to treat refractory epilepsy in patients with Dravet syndrome (DS), Lennox-Gastaut syndrome (LGS), and tuberous sclerosis complex (TSC) through a systematic review and meta-analysis. A comprehensive search of databases was cond

Evaluating Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis Associated With Newer Antiseizure Medications Using FAERS and JADER Combined With Bioinformatics Exploration.

PURPOSE: Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) are rare but life-threatening cutaneous adverse reactions associated with certain antiseizure medications (ASMs). While risks for first- and second-generation ASMs are well-established, the safety profile of third-generation ASMs across different populations remains unclear. This study aimed to systematically evaluate SJS/TEN signals associated with four third-generation ASMs (lacosamide, clobazam, perampanel, and rufinamide) using the Food and Drug Administration Adverse Event Reporting System (FAERS) and Japanese Adverse

Newer Antiseizure Medications and Suicidality: Analysis of the Food and Drug Administration Adverse Event Reporting System (FAERS) Database.

BACKGROUND: The association between antiseizure medications (ASMs) and suicidality remains controversial. Analyses of additional datasets are needed to further elucidate the complex relationship between antiseizure medications and suicidality. OBJECTIVE: The aim of this study was to compare the safety profile of newer ASMs with older ASMs through an analysis of the US Food and Drug Administration Adverse Event Reporting System (FAERS) database, with a focus on suicidality. METHODS: We queried over 17 million reports in the FAERS database from 2012 to 2021 and identified cases involving ASMs.

What does data on adverse reactions reveal about benzodiazepines and suicide-related risk?: A disproportionality analysis using FDA FAERS pharmacovigilance data.

Benzodiazepines (BZDs) are widely used in the treatment of psychiatric disorders, but their association with suicidal/self-injurious behavior is conflicting. This study investigated the relationship between BZDs and suicidal and self-injurious behavior, by analyzing data from the FDA Adverse Event Reporting System. We analyzed FDA Adverse Event Reporting System data from January 2004 and March 2025, to analyze adverse events (AEs) associated with suicidal and self-injurious behavior in BZDs. Thirty-eight BZDs were initially identified via the WHO Anatomical Therapeutic Chemical Code System. Af

Use of Newer Anticonvulsants for the Treatment of Status Epilepticus.

Status epilepticus (SE) has a high mortality rate and is one of the most common neurologic emergencies. Fast progression of this neurologic emergency and lack of response to traditional antiepileptic drugs (AEDs) in most cases has challenged clinicians to use new agents. This article evaluates the efficacy and safety of AEDs released to the market after 2000 for SE, refractory status epilepticus (RSE), and super-refractory status epilepticus (SRSE). The PubMed database was searched for clinical trials published between January 2000 and July 2018 using the search terms status epilepticus, refra

Binding energy analysis and molecular dynamic simulation studies of the designed orally active, non-toxic GABARAP modulators.

Epilepsy is a severe neurological disorder that occurs when the communication between the neurons is disturbed. Gamma-amino butyric acid-associated protein (GABARAP) plays a key role in balancing Gamma-aminobutyric acid-A (GABA(A)) receptor functions of inhibiting the neurotransmission and controlling the seizure. In this study, we introduce the derivatives of the selected anti-epileptic drugs, namely Felbamate and Clobazam, by substituting different hydrophilic and hydrophobic groups at the specified positions. Molecular docking studies between the derivatives and GABARAP were carried out usi

Antiepileptic combination therapy with Stevens-Johnson syndrome and toxic epidermal necrolysis: Analysis of a Japanese pharmacovigilance database.

OBJECTIVE: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are immune-mediated diseases characterized by an extensive loss of the epidermal skin layer, often resulting in death. SJS and TEN are often triggered by certain drugs, including antiepileptic drugs (AEDs). Epilepsy is very difficult to treat and often involves the combination of two or more AEDs. In this study, we quantified not only the risk of SJS or TEN associated with single-AED therapy but also the risk related to concomitant AED treatment using reporting-derived signals. METHODS: An analysis of the Japanese

An unusual case of a toddler with Canavan disease with frequent intractable seizures: A case report and review of the literature.

Canavan disease is a rare fetal inherited leukodystrophy, caused by accumulation of N-acetyl-aspartate in the brain. Here, we report a child presented with frequent intractable seizures and visual impairment. A 14-month-old female infant with a complaint of the absence of neck holding and generalized tonic-clonic seizures was referred to our hospital. Macrocephaly, setting sun eyes, tremor, and hypotonia were observed. Funduscopy showed optic atrophy. Our patient's flash visual evoked potential showed blindness. Her brain magnetic resonance imaging showed diffuse white matter in subcortical, b

[Anxiolytics in man: studies on sleep and performance (author's transl)].

The development of the benzodiazepin molecule has led to drugs which are specially indicated in the management of anxiety. Such drugs may be ingested overnight in which a beneficial effect on sleep is followed by an anxiolytic effect the next day, or during the day. In each case it is desirable that their day time effect is without or with only minimal impairment of performance. In the present paper studies on the effects of two anxiolytic (potassium chlorazepate and clobazam) on sleep and on performance will be reviewed. Potassium chlorazepate is usually given as a single dose (15 mg) overnig

Update on the Genetic Polymorphisms of Drug-Metabolizing Enzymes in Antiepileptic Drug Therapy.

Genetic polymorphisms in the genes that encode drug-metabolizing enzymes are implicated in the inter-individual variability in the pharmacokinetics and pharmaco-dynamics of antiepileptic drugs (AEDs). However, the clinical impact of these polymorphisms on AED therapy still remains controversial. The defective alleles of cytochrome P450 (CYP) 2C9 and/or CYP2C19 could affect not only the pharmacokinetics, but also the pharmacodynamics of phenytoin therapy. CYP2C19 deficient genotypes were associated with the higher serum concentration of an active metabolite of clobazam, N-desmethylclobazam, and

Neonatal sporadic hyperekplexia: a rare and often unrecognized entity.

We report the case of a newborn infant with transient generalized stiffness, obvious from the first days of life, increased muscle tone and repeated myoclonic jerks, who was unsuccessfully treated with phenobarbital until the diagnosis of neonatal sporadic hyperekplexia. To our knowledge this is the first case successfully treated with clobazam, a 1.5 dibenzodiazepine.

Pharmacokinetics of cenobamate as monotherapy compared with adjunctive therapy.

OBJECTIVE: Cenobamate was approved by the US Food and Drug Administration (FDA) based on studies of adjunctive therapy in patients with focal epilepsy. To support the use of cenobamate monotherapy, this pharmacokinetic (PK)-based simulation analysis evaluated the predicted PK exposure of cenobamate when used as monotherapy versus adjunctive therapy. METHODS: A population pharmacokinetic (PopPK) model of cenobamate was developed using pooled human data from eight phase 1 studies in healthy subjects or special populations, and three phase 2 and 3 studies in patients with focal seizures (N = 960

In vitro studies on GABA release.

1 Recent studies have demonstrated growing evidence for a primary action of the benzodiazepines on gabaminergic neurones which induces a facilitation of gamma-aminobutyric acid (GABA)-mediated neurotransmission. As enhancement of GABA release has been suggested to account for their activation of GABA mechanisms, the effect of diazepam and clobazam, and of several other psychotropic drugs, on stimulated GABA release have been studied. 2 Using rat brain cortex slices saturated with [3H]-GABA, the electrically stimulated overflow of GABA is reduced in a concentration-dependent manner in the prese

Extending the use of stiripentol to other epileptic syndromes: a case of PCDH19-related epilepsy.

Stiripentol is an antiepileptic drug (AED) approved by the European Medicines Agency for the treatment of Dravet Syndrome (DS) as adjunct treatment with valproate and clobazam. PCDH19-related epilepsy is an emerging epileptic syndrome characterized by the occurrence of epilepsy in female patients associated with mental retardation and autistic features in most cases. It shares many features with DS: age of onset, normal development before the onset, fever sensitivity, cognitive impairment during the time, drug-resistance. Basing on the numerous similarities between DS and PCDH19-related epilep
