The medial clearspace is a risk factor for secondary dislocation following cast immobilization after closed reduction in closed ankle fracture dislocations.

PURPOSE: Ankle fractures represent about 10% of all adult fractures, with increasing incidence. Dislocated ankle fractures often require delayed open reduction and internal fixation due to swelling, necessitating temporary stabilization using a cast or an external fixator. This study aims to assess risk factors for insufficient preliminary reduction immobilized by a cast, focusing on medial clearspace and posterior malleolus fragment size, to identify fractures that would benefit from initial stabilization with an external fixator. METHODS: Patients treated for dislocated ankle fractures at o

Ultrasound-guided closed reduction of distal radius Salter-Harris I and II fracture in children.

BACKGROUND: Distal radial physeal fractures are the most common type of physeal fracture. However, few studies have been dedicated to the role of ultrasound-guided closed reduction in these fractures. This study aimed to investigate the utility of ultrasound-guided closed reduction in paediatric distal radial physeal fractures. METHODS: Consecutive patients undergoing ultrasound-guided closed reduction of fractures in our department between November 2017 and October 2019 were included. The adequacy of realignment according to the ultrasound and radiography was recorded. RESULTS: A total of 5

Closed reduction of glenohumeral dislocations with associated tuberosity fracture in the emergency department is safe.

AIM: A fracture of the tuberosity is associated with 16% of glenohumeral dislocations. Extension of the fracture into the humeral neck can occur during closed manipulation, leading some to suggest that all such injures should be managed under general anaesthesia in the operating theatre. The purpose of this study was to establish the safety of reduction of glenohumeral dislocations with tuberosity fractures in the emergency department (ED). PATIENTS AND METHODS: We reviewed 188 consecutive glenohumeral dislocations with associated tuberosity fractures identified from a prospective orthopaedic

Closed reduction of laterally displaced fracture/dislocation of the mandibular condylar process in a child.

This is a unique case of isolated lateral fracture/dislocation of the condylar process in a 3-year-old child injured on an escalator. Immediate closed reduction under general anesthesia successfully restored condylar height and premorbid dental occlusion. Maxillomandibular fixation was not used because mandibular mobilization was encouraged. Clinical follow-up confirmed maintenance of normal dental occlusion and temporomandibular joint function.

Closed Reduction Nailing or Open Reduction Plating in Unstable Paediatric Forearm Fractures: A Case of Paediatric Forearm Fracture Implant Failure.

A significant amount of all paediatric fractures are forearm fractures involving the radius, ulnar shaft, or both. As surgical stabilisation lowers the likelihood of re-displacement, surgical intervention is currently recommended over conservative treatment of such fractures involving significant displacement and angulation. Open reduction and plating can better anatomically repair the majority of fractures. Bracing is necessary for the first six to eight weeks after nailing since nailing does not give a rigid fixation. External bracing is generally not necessary for plating. In our facility,

Skin laceration during closed reduction of distal radius fracture in the elderly: an underappreciated complication.

BACKGROUND: Distal radius fractures are common osteoporotic injuries in elderly patients. Dermatoporosis, an age-related cutaneous insufficiency often associated with corticosteroid use and malnutrition, is frequently overlooked in these cases despite its clinical relevance during fracture management. MATERIALS AND METHODS: A 72-year-old underweight woman with chronic obstructive pulmonary disease and long-term corticosteroid use presented with an extra-articular Colles-type distal radius fracture after a trivial fall. Closed reduction under hematoma block was attempted. During disimpaction,

Risk factors for displacement after closed reduction of greater tuberosity fracture associated with shoulder dislocation.

BACKGROUND: Greater tuberosity (GT) fractures associated with shoulder dislocation carry a high risk of secondary displacement, even when initial postreduction alignment appears acceptable. However, the specific factors predicting such displacement remain unclear. The aim of the study was to investigate risk factors associated with displacement of GT fragments following anterior shoulder dislocation. METHODS: This retrospective study included 52 patients (20 males, 32 females; mean age, 71.5 years) with anterior shoulder dislocation and concomitant GT fractures. Following closed reduction, pa

Early Septoplasty Versus Closed Reduction for Acute Nasoseptal Fracture: A Systematic Review and Meta-analysis.

OBJECTIVE: Accurate management of acute nasoseptal fractures can mitigate cosmetic and functional complications. There is a lack of consensus on the ideal approach to manage acute nasoseptal fractures, which are often addressed with closed nasal and/or septal reduction (CN/SR) but sometimes addressed using an open approach with septoplasty. We aimed to systematically assess and compare outcomes of CN/SR versus open approaches to treat acute nasoseptal fractures. DATA SOURCES: PubMed, Scopus, and CINAHL. REVIEW METHODS: Studies evaluating outcomes of adults with acute nasoseptal fractures tre

Clinical Outcomes of Closed Reduction and External Fixation for Isolated Second Metacarpal Base Fracture-Dislocations.

OBJECTIVE: The isolated second metacarpal base fracture-dislocations in adults are rare, and the traditional main treatments include closed reduction with the plaster or splint and open reduction with internal fixation (ORIF). However, closed reduction with the plaster or splint is not solid enough and ORIF can damage the surrounding important tissues. The purpose of this study was to explore the clinical outcomes of closed reduction and external fixation (CREF) for the treatment of isolated second metacarpal base fracture-dislocations. METHODS: Ten patients who suffered isolated second metac

[Iatrogenic femoral head fracture during a closed reduction of posterior dislocation of hip. Case report].

The fracture dislocation of the hip is a rare lesion described up to 4 to 17% of the cases and usually secondary to automobile accidents. Generally of poor prognosis and with already well documented complications such as avascular necrosis of the femoral head, osteoarthritis and heterotopic ossification. The iatrogenic fracture of the neck or head of the proximal femur is a complication even more rare and is not documented its incidence in the world literature. Having as probable causes the irreducibility of a dislocation of hip, more than one attempt of closed reduction, inadequate technique

Traumatic pediatric shoulder fracture dislocation treated with closed reduction and intramedullary nailing: A case report.

BACKGROUND: Although rare, pediatric proximal humerus fractures may coexist with glenohumeral dislocations. Most of these injuries are Salter -Harris type of injuries. The treatment usually consists of closed or open reduction and k- wires fixation. THE CASE: In this case report; a 10-year-old girl presented with a rare traumatic humeral surgical neck fracture with a posterior shoulder dislocation that was treated by closed reduction and flexible intramedullary nailing. CONCLUSION: Pediatric humerus fracture dislocations are rare. Treatment should keep in mind preservation of the growth plat

Adult Closed Distal Radius Fracture Reduction: Does Fluoroscopy Improve Alignment and Reduce Indications for Surgery?

BACKGROUND: This study compared the radiological outcomes of adult closed distal radius fractures (DRFs) reduction with and without fluoroscopy. We hypothesized that fluoroscopy-assisted reduction would not improve radiographic alignment or decrease the need for surgery. METHODS: Hospital medical records and radiographic images of all patients who presented with DRFs between April to June 2009 and April to June 2013 were reviewed. All patients underwent closed reduction and immobilization with or without fluoroscopic assistance. Reduction attempts were noted and pre- and postreduction postero

Functional and radiological outcome in patients with acute Monteggia fracture treated surgically: a comparison between closed reduction and external fixation versus closed reduction and elastic stable intramedullary nailing.

The aim of this study was to compare the clinical and radiological outcomes in children with acute Monteggia fracture treated by two different surgical techniques. Twenty-six children treated by either closed reduction and external fixation (group A) or elastic stable intramedullary nails (group B) were retrospectively reviewed. Clinical outcome was evaluated with the Quick DASH questionnaire. Overall, both surgical techniques provided similar excellent healing rates and good clinical outcomes (P > 0.05), as well as radiological outcomes (P > 0.05). Radiographically, no secondary displacement,

Comparison of Open Reduction and Internal Fixation Versus Closed Reduction and Percutaneous Fixation for Medial Malleolus Fractures.

INTRODUCTION: Surgical treatment of medial malleolus (MM) fractures can be performed through open or percutaneous approaches, although comparisons between these two approaches have not been undertaken. In this study, we compared patients with MM fractures treated with closed reduction and percutaneous fixation (CRPF) with patients treated with traditional open reduction and internal fixation (ORIF). METHODS: A group of 165 consecutive patients underwent surgical fixation of a closed MM fracture from 2011 to 2015 at a single institution. Thirty-one underwent CRPF and 134 underwent ORIF. Patien
