Incidence of congenital clubfoot in Sweden.

BACKGROUND: Idiopathic clubfoot is one of the most common congenital orthopedic problems. Nationwide studies of the incidence are scarce. We performed a prospective multicenter study in order to assess the cumulative incidence in Sweden over 2 consecutive years. PATIENTS AND METHODS: 44 clinics identified as treating clubfoot reported new cases prospectively. The medical records of 280 children with clubfoot born during 1995- 1996 were collected and analyzed with special reference to gender, regional distribution and seasonal variation. RESULTS: The average cumulative incidence of clubfoot d

[The primary observation of congenital clubfoot].

OBJECTIVE: To explore the possible correlations between clinical and experimental pathological changes of congenital clubfoot and the pathodynamic developmental procedure. METHODS: Eighty-three female Wistar rats were administered with retinoic acid on the 10th day after pregnancy. And from February 2001 to February 2004, 48 patients were analyzed with electropysiological examination. RESULTS: There was clubfoot-like deformity in 53.7% of the experimental fetuses. Persistence of the embryonic position of the talus and tibia in fetuses was observed. Poor overlapping between talus and calcaneu

Joint laxity and the risk of clubfoot.

BACKGROUND: Clubfoot is a relatively common and potentially disabling birth defect. The etiology of clubfoot (talipes equinovarus [TEV]) is not known. Joint laxity, or hypermobility, is a suspected risk factor for congenital hip dislocation and clubfoot. METHODS: We conducted a case-control study of TEV in five western Washington counties. A total of 239 cases of TEV were identified from hospital and outpatient sources from 1986 to 1994. Controls (n = 356) were identified by random-digit dialing (RDD). Parents were interviewed over the telephone, and medical records were abstracted. In-person

Association of fibular hemimelia and clubfoot.

This study was designed to determine the incidence of clubfoot in our fibular hemimelia population. A retrospective review of fibular hemimelia patients treated at our institution over the past seventy years was conducted. We identified twenty-three cases of clubfoot in 121 patients with 147 involved limbs. Nineteen of twenty-three limbs retained the foot, four were converted to Syme's amputation because a plantigrade functional foot could not be obtained. Hindfoot coalitions were present in eighteen limbs and nineteen had one or more missing lateral rays. In summary, the association of clubfo

The neglected clubfoot: a salvage procedure.

The study included 18 patients (28 feet), between 4 and 14 years of age. Complete homogeneous correction of the neglected clubfoot was achieved by performing a trapezoid resection osteotomy from the cuboid bone after performing an extensive posteromedial soft-tissue release. The procedure did not sacrifice any articular cartilage and allowed realignment of the forefoot. Tibialis anterior tendon transfer to the cuboid was performed in all feet to eliminate its deforming action and to help maintain the desired degree of correction. A Dwyer calcaneal lateral closing wedge osteotomy was needed in

Management of clubfoot deformity in amyoplasia.

Forty-one clubfeet in 22 patients with amyoplasia were studied retrospectively at a mean duration after surgery of 118 months (range, 45-253). The clubfeet were managed by a regimen including initial stretching casts, posteromedial release, and postoperative splinting at night. The mean age at the time of surgery was 7.3 months. Correction of deformity without recurrence was achieved in 11 (27%). Recurrent deformity was corrected by serial casting in eight feet and required secondary operative procedures in 20 feet. In the feet without recurrence of deformity, the duration of splinting at nigh

Update on pathologic anatomy of clubfoot.

Serial histological sections in three planes (frontal, sagittal, and transverse) in four cases of clubfoot in fetuses aborted at 16-20 weeks were studied and compared to identical sections obtained in three normal feet. The talus was deformed, with its neck medially angulated and its head dome shaped. The body of the calcaneus was medially bowed and was tilted and rotated medially underneath the talus, and both the talus and the calcaneum were in plantar flexion. The tilting of the talus and the medial tilting and rotation of the calcaneus accounted for the varus deformity of the hindfoot. The

Conservative and surgical treatment of clubfoot.

One hundred twenty-four patients (174 feet) were treated for clubfoot deformity, with an average follow-up of 7 years. All patients were initially treated by manipulation and serial casting. Early soft tissue surgeries were performed for resistant cases (73 feet). In 33 feet with residual hindfoot deformity only, posterior release was performed with 85% good results. In the remainder, where there was forefoot as well as hindfoot deformity, full posteromedial release was necessary, giving good results in 90% of cases. All clubfeet should initially be treated conservatively. However, resistant c

A genetic analysis of clubfoot in Hawaii.

The roles of major genes and multifactorial inheritance in the etiology of clubfoot (talipes equinovarus) were investigated based on 365 nuclear families consisting of three major racial groups of Hawaiians, Caucasians, and Orientals in Hawaii. Complex segregation analysis was employed using the mixed model with four parameters: major gene displacement (t), degree of dominance (d), gene frequency (q), and heritability (H). Heterogeneity was evident among the racial groups in the pattern of segregation of clubfoot. The most plausible genetic model is the presence of major gene effects with the

Treatment of clubfoot by posterior capsulectomy.

Posterior capsulectomy is a complete excision of the posterior tibiotalar and talocalcaneal capsule with a release of the distal tibiofibular connection. The operation is proposed for a resistant or relapsed clubfoot, where remaining equinus is the major problem. A follow-up of 29 operations showed that the average talocalcaneal angle became normal in the AP and lateral views. The best results were obtained with long postoperative cast immobilization, i.e. 6 months.

Heritability of clubfoot: a twin study.

INTRODUCTION: The aetiology of congenital clubfoot is unclear. Although studies on populations, families, and twins suggest a genetic component to the aetiology, other studies have identified environmental factors. The purpose of this study was to calculate heritability in order to determine to what extent genetic and/or environmental factors contribute to the aetiology of congenital clubfoot and to asses whether there was a change in the prevalence over time. MATERIALS AND METHODS: The Odense based Danish Twin Registry is unique as it contains data on all the approximately 85,000 twin pairs

Clubfoot--advances in diagnosis and management.

BACKGROUND: Congenital talipes equinovarus--or clubfoot--is a common paediatric condition of unknown cause. OBJECTIVE: This article presents two case studies and outlines the diagnosis, treatment, referral pathways and expected outcomes in patients with clubfoot. DISCUSSION: A significant change in treatment has occurred over the past decades, both in Australia and internationally, with the focus shifting from surgical to conservative management. Education of general practitioners in this area has the potential to facilitate early referral and help allay parental anxiety.

Multistate study of the epidemiology of clubfoot.

BACKGROUND: Although clubfoot is a common birth defect, with a prevalence of approximately 1 per 1000 livebirths, the etiology of clubfoot remains largely unknown. Studies of the prevalence and risk factors for clubfoot in the United States have previously been limited to specific states. The purpose of this study was to pool data from several birth defects surveillance programs to better estimate the prevalence of clubfoot and investigate its risk factors. METHODS: The 10 population-based birth defects surveillance programs that participated in this study ascertained 6139 cases of clubfoot f

A novel orthosis for the treatment of clubfoot.

We introduce a novel orthosis used with serial manipulations for the treatment of 14 congenital clubfeet (six boys and three girls). The average age at the time of initial treatment was 3.5 months (range, 2-6 months). The average length of treatment was 4.9 months (range, 3-7 months). All patients traveled at least 480 kilometres for their clinic visits. According to the clubfoot classification of Dimeglio et al., 12 clubfeet (85%) were severe and two clubfeet (15%) were moderate. At the end of treatment, all clubfeet were improved by at least one level of grading. Although all clubfeet went o
