HYPOTHESIS: Perimodiolar intracochlear electrodes with contacts facing towards the modiolus have limited current flow towards the outer wall of the cochlea and therefore, may reduce the occurrence of facial nerve stimulation (FN) in cochlear implant subjects.
BACKGROUND: Facial nerve stimulation is a well-known complication in cochlear implant treatment especially in the group of subjects with otosclerosis. The possible explanation of this side effect is a change of the electrical properties of the otosclerotic bone leading to leakage current and resulting in facial nerve stimulation.

Post-traumatic invasion of macrophages into the cochlear nerve of the rat and measurement of how their invasion was modified by the administration of methylprednisolone were investigated for the first time by using a reproducible and quantifiable experimental model of cochlear nerve injury. Two weeks after precise cochlear nerve compression, a massive invasion of ED1 immunostained macrophages was observed at the compressed portion of the cochlear nerve, and this invasion of macrophages was markedly reduced in the rats to which methylprednisolone had been administered during the pre- and post-c

The purpose of this study was to determine whether patterns of phoneme identification error differ among listeners with cochlear and retrocochlear auditory disorder. Speech intelligibility performance was analyzed in 15 patients with confirmed eighth-nerve disorder and in 15 patients with cochlear disorders, matched to the retrocochlear group for age and audiometric configuration, using confusion matrices derived from responses to a monosyllabic word list. Results indicated that: (1) vowel errors were more prevalent in the retrocochlear group and varied directly with increasing stimulus presen

Cochlear nerve degeneration (CND), including the loss of synapses between inner hair cells and auditory nerve fibers (ANFs), has emerged as a likely contributor to "hidden hearing loss", a condition in which listeners experience speech-in-noise difficulties that cannot be fully explained by audiometric thresholds. This form of primary neural de-afferentation preferentially affects low- and medium-spontaneous rate (SR) fibers, which are critical for encoding acoustic features such as amplitude modulations, especially under challenging listening conditions such as noisy backgrounds. Although CND

OBJECTIVES: Electrical stimulation from a cochlear implant can spread beyond the auditory nerve. The aims of this study were to accurately measure facial nerve stimulation in pediatric implant users and to determine the characteristics and incidence of this unwanted activity. Part A consisted of a prospective study of a randomized sample of 44 pediatric implant users. Part B consisted of a retrospective analysis of 121 children with previously recorded electrically evoked auditory brainstem responses (EABR).
STUDY DESIGN AND METHODS: Responses were evoked by 3 electrodes along the implant arr

BACKGROUND: Microvascular compressions of the cochlear nerve can lead to hearing loss. Due to the tonotopic organization of the cochlear nerve any focal compression of the cochlear nerve will result in a frequency specific hearing loss. Decompressing the cochlear nerve could result in a frequency specific hearing improvement, without improving overall hearing.
METHOD: Thirty one patients underwent microvascular decompression operations of the vestibulocochlear nerve for vertigo or tinnitus. Preoperative audiograms were substracted from postoperative audiograms obtained 2 years after microvasc

Vestibular neurectomy (VN) results in a high success rate in the control of vertigo in Meniere's disease, although the subsequent fate of auditory function is fairly unpredictable. The present investigation reports the postoperative results obtained in a group of 30 subjects with a clinical diagnosis of Meniere's disease and vascular cross-compression of cranial nerve VIII. All subjects underwent VN using a retrosigmoid approach, and in half of them microvascular decompression (MVD) of the cochlear nerve with interposition of autogenous muscle was performed at the same time. All patients had c

This report describes a patient with sudden sensorineural hearing loss who was found to have a megadolichoectasia vertebrobasilar system that appeared to be causing compression of the ipsilateral facial and vestibulocochlear nerves. The patient was treated conservatively for 4 months, during which time no hearing returned. He then underwent microvascular decompression of the affected nerves. At surgery, marked compression of the cranial nerves VII-VIII complex and the pons was observed. Postoperatively, the patient experienced a gradual return of useful hearing. We suggest that vascular compre

The clinical interpretation of acoustic reflex test results in ears with sensorineural impairment should be based on a consideration of three characteristics of the response: the reflex threshold hearing level, the reflex threshold sensation level, and the decay of the reflex. Examination of results in 152 ears with cochlear pathology and in 152 ears with acoustic tumor indicated that in some cases the combination of characteristics may be contradictory as to site of lesion. Regarding a contradictory response pattern as questionable -- neither confirming nor denying a specific site of lesion -

Generally, cochlear implants (CIs) are effective in helping patients improve their hearing performance; however, some patients have poor hearing performance owing to facial nerve stimulation (FNS), which is often associated with cochlear anomalies. We report a case with a normal cochlea and severe and persistent FNS owing to cochlear-facial dehiscence (CFD) that affected the CI outcomes. Preoperatively, a careful review of the computed tomography images before CI surgery is necessary not only for patients with otosclerosis and inner ear malformations but also for patients with normal cochlear

1. Rate-level functions for individual cochlear nerve fibers of the alligator lizard, Gerrhonotus multicarinatus, were generated by measuring a fiber's driven discharge rate (the difference between the average discharge rates in the presence and absence of a tone burst) as a function of sound pressure level. 2. When plotted in double logarithmic coordinates, the rate-level function approaches a straight line at low sound pressure levels and saturates at high levels. Thus the rate-level function is a saturating power function of sound pressure. We developed an algorithm to estimate the exponent

Hyperbilirubinemia is a major problem in neonatal intensive care. Hearing impairment is one of its sequelae. Although lesions of the central auditory pathways are known to be associated with this disorder in both humans and homozygous Gunn rats, the presence of cochler pathology is still controversial. The purpose of this study was to examine the functional integrity of the peripheral auditory system in the Gunn rat. The Gunn rat is a mutant of the Wistar strain with congenital deficiency of the liver enzyme uridine diphosphoglucuronyl transferase which is essential for bilirubin conjugation.

Cochlear nerve deficiency (CND) is a rare cause of congenital sensorineural hearing loss, often leading to serious communication challenges if not diagnosed and treated early. We present this case of a three-year-old male child with congenital hearing loss and sensory aphasia. Despite normal cochlear anatomy on computed tomography, magnetic resonance imaging (MRI) revealed the absence of the cochlear nerve on the right side. The diagnosis of right-sided CND was confirmed, and the patient was advised to undergo cochlear implantation and speech therapy. This case underscores the importance of ti

Introduction Facial nerve stimulation (FNS) is a complication in cochlear implant (CI) when the electrical current escapes from the cochlea to the nearby facial nerve. Different management to reduce its effects are available, although changes might result in a less-than-ideal fitting for the CI user, eventually reducing speech perception. Objective To verify the etiologies that cause FNS, to identify strategies in managing FNS, and to evaluate speech recognition in patients who present FNS. Methods Retrospective study approved by the Ethical Board of the Institution. From the files of a CI
