The neural correlates of cognitive dysfunction in phantom sounds.

Tinnitus is an auditory phantom percept with a tone, hissing or buzzing sound in the absence of an objective physical sound source. It has been shown that tinnitus can lead to emotional and cognitive impairment and people with tinnitus perform worse than a control group on different cognitive tasks. The hippocampus is known to play an important role in cognitive performance, and also in the pathophysiology of tinnitus. Hippocampal deficits have been described in animal models of tinnitus and in tinnitus patients a decrease in grey matter in the hippocampus has been demonstrated. Nineteen patie

The Role of Dysfunctional Cognitions in Patients With Chronic Tinnitus.

OBJECTIVES: The present study investigates the role of dysfunctional cognitions in patients with chronic tinnitus. To explore different dimensions of tinnitus-related thoughts, a 22-item self-report measure, the "Tinnitus Cognitions Scale" (T-Cog), is presented. Furthermore, dysfunctional cognitions are examined as a possible mediator of the relation between tinnitus distress and depression. DESIGN: The present study analyzes the cross-sectional data of 373 patients with chronic tinnitus. Parallel analysis and principal axis factoring are used to identify the factor structure of the T-Cog. As

Alexithymia and cognitive dysfunctions in patients with panic disorder.

BACKGROUND: In patients with panic disorder (PD), the difficulty to identify and manage emotional experience might contribute to the enduring vulnerability to panic attacks. Such a difficulty might reflect a dysfunction of fronto-temporo-limbic circuits. The present study was designed to test the hypothesis that drug-free patients with PD, as compared with healthy subjects (HS), show a higher prevalence of alexithymia, greater difficulty in emotional stimuli processing and poorer performance on neuropsychological tests exploring the activity of fronto-temporo-limbic circuits. METHODS: Alexith

Cannabis and cognitive dysfunction: parallels with endophenotypes of schizophrenia?

Currently, there is a lot of interest in cannabis use as a risk factor for the development of schizophrenia. Cognitive dysfunction associated with long-term or heavy cannabis use is similar in many respects to the cognitive endophenotypes that have been proposed as vulnerability markers of schizophrenia. In this overview, we examine the similarities between these in the context of the neurobiology underlying cognitive dysfunction, particularly implicating the endogenous cannabinoid system, which plays a significant role in attention, learning and memory, and in general, inhibitory regulatory m

[Drug treatment of emotional and cognitive dysfunction in dementia].

The evaluation, treatment and care of patients with dementia are major tasks for the health service. This paper provides an overview of drug treatment of the emotional and cognitive disturbances of dementia. Tricyclic and the newer anti-depressive agents are effective in patients with dementia and depression. Dopamine-blocking agents are effective for some patients with dementia and psychosis, but their use is limited by frequent and severe side-effects. Atypical neuroleptics may prove useful for this patient group. Aggression and agitation may be treated with serotonergic agents, neuroleptics

NCOA3 as a Key Regulator in Diabetic Cognitive Dysfunction.

Diabetes-related cognitive dysfunction (DCD) represents a significant complication of diabetes mellitus, yet its underlying molecular mechanisms remain incompletely elucidated. In this study, we aimed to investigate the potential role of nuclear receptor coactivator 3 (NCOA3) in DCD pathogenesis using both conditional knockout (cKO) and lentivirus-mediated overexpression mouse models. Diabetes was induced through combined high-fat diet feeding and low-dose streptozotocin (STZ) administration. Comprehensive behavioral assessments, including novel object recognition test (NORT), Y-maze, and cont

Cognitive and Physical Intervention in Metals' Dysfunction and Neurodegeneration.

Metals-especially iron, copper and manganese-are important elements of brain functions and development. Metal-dysregulation homeostasis is associated with brain-structure damage to the motor, cognitive and emotional systems, and leads to neurodegenerative processes. There is more and more evidence that specialized cognitive and motor exercises can enhance brain function and attenuate neurodegeneration in mechanisms, such as improving neuroplasticity by altering the synaptic structure and function in many brain regions. Psychological and physical methods of rehabilitation are now becoming incre

Cognitive dysfunction and psychosis: expanding the phenotype of SPG7.

Spastic paraplegia type 7 (SPG7) is one of the most common forms of autosomal recessive hereditary spastic paraplegia, which can lead to a hybrid spastic-ataxic phenotype. Recently, novel complicated forms of SPG7, including cognitive and social impairment phenotypes, have been reported. We present a SPG7 case with two pathogenic variants in compound heterozygosity in the SPG7 gene, featuring a cerebellar cognitive affective syndrome with psychosis not yet described in the literature.

Dysfunctional Cognition in Individuals With an Increased Risk for Mania.

BACKGROUND: There is still a lack of knowledge about attitudes and cognitions that are related to bipolar disorder. Theoretically, it was proposed that exaggerated beliefs about the self, relationships, the need for excitement, and goal-related activities might lead to mania in vulnerable individuals, however, the few studies that examined this hypothesis provided mixed results. One of the unresolved issues is if such a cognitive style is associated with current mood symptoms or with different stages of the illness, i.e. at-risk versus diagnosed bipolar disorder. Therefore, the present study a

Background and distribution of lobar microbleeds in cognitive dysfunction.

OBJECTIVES: Cerebral microbleeds (CMBs) are often observed in memory clinic patients. It has been generally accepted that deep CMBs (D-CMBs) result from hypertensive vasculopathy (HV), whereas strictly lobar CMBs (SL-CMBs) result from cerebral amyloid angiopathy (CAA) which frequently coexists with Alzheimer's disease (AD). Mixed CMBs (M-CMBs) have been partially attributed to HV and also partially attributed to CAA. The aim of this study was to elucidate the differences between SL-CMBs and M-CMBs in terms of clinical features and regional distribution. MATERIALS: We examined 176 sequential p

Cognition and eye movements: assessment of cerebral dysfunction.

BACKGROUND: Many neurological disorders show deficits in ocular motor function. In the past, evaluation has been limited to assessing abnormalities largely generated by pathology of the brainstem andcerebellum. In disorders that primarily or substantially, affect the cerebral hemispheres, disruption of cognitive processes occur, often early in the clinical course. While neuropsychological testing traditionally is used to measure cognitive performance, the cerebral influences on the ocular motor system provides another quantitative paradigm. This review explores the relationship between cogniti

Cognitive behavioral therapy for sexual dysfunctions in women.

Sexual dysfunctions in women are classified into disorders of desire, arousal, orgasm, and pain (including dyspareunia and vaginismus). As the cognitive behavioral treatment (CBT) procedures differ among these sexual disorders, the treatments for each disorder are reviewed separately. The efficacy of CBT differs depending on the specific sexual dysfunction to be treated. It is concluded that only a few CBT treatments for women's sexual dysfunction have yet been empirically investigated in a methodologically sound way and little is known about which of the treatment components are most effectiv

Pathogenesis of cognitive dysfunction in phenylketonuria: review of hypotheses.

In untreated phenylketonuria (PKU), deficiency of phenylalanine hydroxylase (PAH) results in elevated blood phenylalanine (Phe) concentrations and severe mental retardation. Current dietary treatment prevents mental retardation, but cognitive outcome remains suboptimal. The mechanisms by which elevated blood Phe concentrations disturb cerebral metabolism and cognitive function have not been fully elucidated. In this review, we discuss different hypotheses on the pathogenesis of PKU, focusing on the effects of disturbed large neutral amino acid (LNAA) transport from blood to brain on cerebral n

Disconnection as a mechanism for cognitive dysfunction in multiple sclerosis.

Disconnection of cognitively important processing regions by injury to the interconnecting white matter provides a potential mechanism for cognitive dysfunction in multiple sclerosis. The contribution of tract-specific white matter injury to dysfunction in different cognitive domains in patients with multiple sclerosis has not previously been studied. We apply tract-based spatial statistics (TBSS) to diffusion tensor imaging (DTI) in a cohort of multiple sclerosis patients to identify loci where reduced white matter tract fractional anisotropy (FA) predicts impaired performance in cognitive te

Does propofol definitely improve postoperative cognitive dysfunction?-a review of propofol-related cognitive impairment.

Postoperative cognitive dysfunction (POCD) is a common brain function-related complication after surgery. In addition to old age being an independent risk factor, anesthetics are also important predisposing factors. Among them, propofol is the most commonly used intravenous anesthetic in clinical practice. It has a rapid onset, short half-life, and high recovery quality. Many studies report that propofol can attenuate surgery-induced cognitive impairment, however, some other studies reveal that propofol also induces cognitive dysfunction. Therefore, this review summarizes the effects of propof

Cognitive reactivity and dysfunctional attitudes in cognitive behavioral and psychodynamic therapy for adult depression.

OBJECTIVE: It is unclear whether cognitive reactivity (CR), an indicator of depression vulnerability, is reduced after cognitive behavioral therapy (CBT), as theorized. It is also unclear whether baseline CR and dysfunctional attitudes (DA) are associated with treatment outcome in CBT for depression. This study aims to examine these questions and assess if these effects are specific to CBT for depression. METHOD: Depressed adult outpatients (n = 159) randomized to 16 sessions CBT or short-term psychodynamic supportive therapy (SPSP) completed measures of depression severity and CR at baseline

The changeability and predictive value of dysfunctional cognitions in cognitive behavior therapy for chronic tinnitus.

BACKGROUND: Multidimensional tinnitus models describe dysfunctional cognitions as a complicating factor in the process of tinnitus habituation. However, this concept has rarely been investigated in previous research. PURPOSE: The present study investigated the effects of two cognitive-behavioral treatments on dysfunctional tinnitus-related cognitions in patients with chronic tinnitus. Furthermore, dysfunctional cognitions were examined as possible predictors of the therapeutic effect on tinnitus distress. METHOD: A total of 128 patients with chronic tinnitus were randomly assigned to either
