Thumb Metacarpophalangeal Joint Kinematics and Elongation of the Ulnar Collateral Ligament.

PURPOSE: The purpose of the study was to determine if the sphericity of the thumb metacarpophalangeal (MCP) joint influences the joint's rotational axis (RA) and elongation patterns of the ulnar collateral ligament (UCL). METHODS: Ulnar collateral ligament origins and attachments of 28 fresh-frozen cadaveric thumbs were marked with a radiopaque marker. Lateral radiographs were obtained in neutral and 20°, 40°, and 60° of flexion. The dorsal and palmar joint gap and the length of four different UCL portions were digitally measured in all flexion states. The RA was determined by the intersectio

Primary Repair of Medial Collateral Ligament Tears With Suture Augmentation.

BACKGROUND: The medial collateral ligament (MCL) is considered to have superior healing and has classically been treated nonoperatively in low-grade MCL injuries. The MCL injury in the setting of concomitant anterior cruciate ligament (ACL) injury is usually treated with a delay of surgery to allow for nonoperative MCL treatment. Recent studies have shown that even with grade II MCL injury there may be residual laxity which may affect place greater strain on an ACL graft in the multi-ligamentous setting and can be corrected for with early primary repair of the MCL which also allows for early d

[Recognition of ankle sprain: not only lateral collateral ligament injury].

Ankle sprains are a common cause of morbidity in the general and athletic population. The majority of sprains involve the lateral aspect of the ankle, affecting the anterior talofibular ligament. However, the extent and categories of injury will depend upon the mechanism of injury. The ankle injury caused by ankle sprain is not only the injury of lateral collateral ligaments, but also chondral or osteochondral lesions of the talus, anterior ankle impingement, posterior ankle impingement and so on. In addition to the involvement of ankle joint, ankle sprain often leads to the injury of subtalar

Relaxation of the Medial Collateral Ligament to Facilitate Pediatric Meniscal Surgery.

Meniscal injuries are becoming increasingly common in the pediatric population. During arthroscopic evaluation, pediatric patients typically have pristine articular cartilage and tight medial joint spaces. Therefore, when an arthroscope enters the medial compartment, iatrogenic damage to the articular cartilage may occur. Additionally, proper visualization of the medial meniscus (posterior horn or meniscocapsular junction) can prove to be difficult, and potential pathology may be missed. Proposed methods of increasing medial compartment visualization include the use of a 70° arthroscope placed

Biomechanical Analysis of Thumb Ulnar Collateral Ligament Tear Kinematics.

Background: Thumb ulnar collateral ligament (UCL) injuries are common, but the kinematics of these injuries have not been comprehensively described, especially regarding kinematic changes with progressive UCL injury. Methods: Eleven cadaveric thumbs underwent kinematic testing under 4 conditions: intact, partial tear (50%) of the proper UCL, full tear of the proper UCL, and complete tear of both the proper and accessory UCL. Kinematic testing parameters included varus/valgus stress, pronation/supination, and volar/dorsal translation at -10 degree, 0 degree, 15 degree, and 30 degree of metacarp

Impact of Ulnar Collateral Ligament Tear on Posteromedial Elbow Biomechanics.

Ulnar collateral ligament insufficiency has been shown to result in changes in contact pressure and contact area in the posteromedial elbow. This study used new digital technology to assess the effect of a complete ulnar collateral ligament tear on ulnohumeral contact area, contact pressure, and valgus laxity throughout the throwing motion. Nine elbow cadaveric specimens were tested at 90° and 30° of elbow flexion to simulate the late cocking/early acceleration and deceleration phases of throwing, respectively. A digital sensor was placed in the posteromedial elbow. Each specimen was tested wi

Open Anatomic Reconstruction of the Medial Collateral Ligament and Posteromedial Corner.

Injuries to the medial collateral ligament (MCL) and posteromedial corner can occur in isolation or in the setting of multiligamentous knee injuries. Reconstruction of the MCL and posteromedial corner is indicated in the setting of a multiligamentous knee injury. Isolated cases failing nonoperative treatment may also undergo surgical treatment. Our preferred technique for anatomic medial-sided knee reconstruction is an open anatomic MCL reconstruction using an Achilles tendon allograft along with direct repair of all associated medial and posteromedial structures.

Optimal management of ulnar collateral ligament injury in baseball pitchers.

The ulnar collateral ligament stabilizes the elbow joint from valgus stress associated with the throwing motion. During baseball pitching, this ligament is subjected to tremendous stress and injury if the force on the ulnar collateral ligament during pitching exceeds the physiological limits of the ligament. Injuries to the throwing elbow in baseball pitchers result in significant time loss and typically surgical intervention. The purpose of this paper is to provide a review of current information to sports medicine clinicians on injury epidemiology, injury mechanics, injury risk factors, inju

Operative management of ulnar collateral ligament insufficiency in adolescent athletes.

BACKGROUND: The incidence of ulnar collateral ligament (UCL) injuries of the elbow has increased in adolescents over the past decade because of widespread participation in athletics and heightened awareness among physicians. HYPOTHESIS: Ulnar collateral ligament reconstruction using the docking technique would result in a successful return to athletic activity in this age group. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Study participants were 55 skeletally mature adolescent athletes (mean age, 17.6 years; range, 15-18 years) who underwent UCL reconstruction between 2008 and

MRI spectrum of medial collateral ligament injuries and pitfalls in diagnosis.

The medial collateral ligament (MCL) is made up of different components and spans the medial aspect of the knee. With injuries the superficial or deep and posterior components may be involved. A variety of conditions including MCL bursitis, medial osteoarthritis, medial cellulitis, medial bursitis, medial meniscal cyst, meniscocapsular separation, and retinacular tear may present with high signal surrounding the MCL fibers and simulate an MCL tear.

Ulnar collateral ligament injury in the overhead athlete: diagnosis and treatment.

The ulnar collateral ligament (UCL) is more commonly injured than previously suggested. Injury to this ligament can result in secondary symptoms and problems in other parts of the elbow, including the ulnar nerve, the flexor-pronator musculotendinous unit, the radiocapitellar joint, and the posterior compartment of the elbow, in addition to being a cause of loose bodies within the elbow. This article briefly reviews the anatomy, biomechanics, and pathophysiology of injury, in addition to discussing evaluation and treatment of the athlete with an injured UCL.

MR imaging in chronic rupture of the ulnar collateral ligament of the thumb.

PURPOSE: MR imaging has been shown as the best radiologic method for verifying and classifying acute ulnar collateral ligament (UCL) ruptures of the thumb. Our aim was to analyse the usefulness of MR also in old ruptures and to establish the most useful sequences. MATERIAL AND METHODS: Ten patients with an old UCL rupture of the thumb were preoperatively imaged using 1.5 T MR. Three radiologists blinded to the findings separately analysed the MR images of these patients and of 10 age-and sex-matched voluntary controls. MR findings of the patients were compared with those of surgery. RESULTS:

Effects of reconstructed radial collateral ligament on index finger mechanics.

Twenty fresh frozen hand specimens from cadavers were studied. Physiologic levels of extrinsic muscle loads were applied to the extrinsic flexor tendons of the index finger to simulate tip pinch of the finger on a fixed plate. The acute effects of transection of the radial collateral ligament and accessory radial collateral ligament (radial collateral ligament complex) with and without transection of the dorsal capsule and volar plate on the position of the proximal phalanx with respect to the metacarpal bone of the index finger were investigated. The acute effects of reconstruction of the rad

[Skier's thumb--osseous injury and rupture of the ulnar collateral ligament].

Injuries to the ulnar collateral ligament of the thumb are common in down hill-skiing, and are thus called "skier's thumb". In complete disruption of the ligament, surgical treatment is mandatory to restore stability. The management gets more difficult if a bony lesion is present: does a non-displaced fragment represent a non-displaced bony avulsion of the ulnar collateral ligament, thus allowing conservative treatment? We reviewed sixty-three consecutive patients with an acute skier's thumb injury in order to determine the anatomical nature of injuries in thumbs that were treated surgically e

[Nonoperative approach to injuries of the lateral collateral ligament of the ankle joint].

Patients in number of 185 were included in this study. All of them were treated nonoperatively. One group (64 patients) was treated with early mobilization treatment and the other one (121 patients) with three weeks cast immobilization. Checking examination was done twice: 5 weeks and one year after injury. Chi-square test was used for data processing. Patients treated with cast had shorter recovery, but three were not statistical differences. Positive ADS test one year after the injury (I group 4.7%, and II group 2.5%) was considered as poor result and so was functional instability a year aft
