Successful use of Closed Loop Stimulation pacemaker for carotid sinus syndrome due to infiltrating neck cancer.

A 64-year-old male with oropharyngeal carcinoma experienced recurrent syncope and hypotension due to neoplastic compression of the right carotid space. Despite initial treatment with atropine and fluid resuscitation, the patient continued to experience presyncope and hypotensive events. After treatment with a conventional dual-chamber pacemaker failed, the use of closed-loop stimulation (CLS) successfully resolved the syncope episodes. This case highlights the efficacy of CLS pacing in managing reflex syncope caused by extrinsic neoplastic compression of the carotid sinus. The CLS algorithm, b

Rethinking Transradial Compression Band Practice.

BACKGROUND: Transradial access (TRA) is a standard approach to cardiac catheterization. Following the procedure, air-filled compression bands are often used to achieve hemostasis at the access site. Despite research supporting early, gradual deflation to minimize complications, there is no established best practice for band deflation. At our urban academic medical center, the lack of a standardized compression band deflation protocol resulted in varied start times and amounts of air removed with each attempt, as well as inconsistent documentation. PURPOSE: The aim of this quality improvement

Atom selection strategy for signal compressed recovery based on sensing information entropy.

In greedy pursuit algorithm, atom selection is commonly a concerned topic for signal compressed recovery. To improve the recovery performance, an optimal atom selection strategy without the prior information is proposed in this paper. The sensing information entropy is defined to prune the possible false atoms in the estimated support set. Fewer iterations are required in the proposed strategy and it can also be applied in the case with high sparsity level or low signal-noise-ratio. Compared with the existing representative algorithms, the superiority of the recovery error and probability is v

Post-Processing Geometry Enhancement for G-PCC Compressed LiDAR via Cylindrical Densification.

The geometry-based point cloud compression algorithm achieves efficient compression and transmission for LiDAR point clouds with high sparsity. However, the low-bitrate mode results in severe geometry compression artifacts, which involve both point reduction and coordinate offset. To the best of our knowledge, this is the first attempt to directly enhance the geometry quality for compressed LiDAR point cloud (CLGE) in a post-processing manner. Our proposed method consists of two branches: cylindrical densification and adaptive refinement. The former adopts a multi-scale sparse convolution fram

Superimposed entrapment neuropathy in diabetes: Evidence review and algorithm for referral decisions.

Diabetic peripheral neuropathy (DPN) is common, disabling, and associated with substantial risk of foot ulceration and lower extremity complications. A subset of patients develops superimposed peripheral nerve entrapment, which can exacerbate pain, impair protective sensation, and contribute to microcirculatory dysfunction. Although surgical decompression has been studied for more than three decades, awareness of entrapment features remains low in primary care, and guidance on patient selection is limited. This review summarizes current evidence on the clinical identification of focal nerve co

Muscle force evaluation and the role of posture in human lumbar spine under compression.

Using two nonlinear finite element models of the lumbar spine, the concept of optimal posture is explored by minimizing the segmental sagittal moments required for the equilibrium of the passive lumbar spine under a total of 2800 N axial compression while varying the pelvic tilt and lumbar lordosis. The redundant active-passive system is subsequently solved for this posture using a novel kinematics-based muscle calculation algorithm along with minimization approach. Some flattening in the lumbar spine substantially reduces the required moments and internal passive shear forces under 2800 N axi

A 1024-Channel 0.8V 23.9-nW/Channel Event-Based Compute In-Memory Neural Spike Detector.

The increasing data rate has become a major issue confronting next-generation intracortical brain-machine interfaces (iBMIs). The scaling number of recording sites requires complex analog wiring and lead to huge digitization power consumption. Compressive event-based neural frontends have been used in high-density neural implants to support the simultaneous recording of more channels. Event-based frontends (EBF) convert recorded signals into asynchronous digital events via delta modulation and can inherently achieve considerable compression. But EBFs are prone to false events that do not corre

Management strategy for arterial priapism: therapeutic dilemmas.

PURPOSE: We present 7 cases of arterial high flow priapism and propose management algorithms for the condition. MATERIALS AND METHODS: We studied 2 children and 5 adults with posttraumatic arterial priapism. Blood gas analysis and color Doppler ultrasonography of the corpora cavernosa confirmed the diagnosis in 4 adults, while 1 patient had already undergone cavernous artery ligation in elsewhere. In the children perineal compression resulted in detumescence, a sign that is proposed to be indicative of the diagnosis of arterial priapism (piesis sign) complementing physical examination. Mechan

Reliable estimation of the confined compressive strength of FRP-confined circular concrete columns using an ANFIS-based model.

Reliable estimation of the confined compressive strength of fiber-reinforced polymer (FRP)-wrapped concrete is essential for the safe design and assessment of strengthened structural members. This study proposes an adaptive neuro-fuzzy inference system (ANFIS) model to predict the confined compressive strength of FRP-confined circular concrete cylinders. The model is trained using the Levenberg-Marquardt backpropagation algorithm, combined with an early-stopping strategy, to enhance generalization and prevent overfitting. Four physically meaningful parameters-unconfined compressive strength, c

Smartwatch assessment of CPR performance administered to an instrumented mannequin.

Cardiac arrest is a life-threatening emergency that requires effective cardiopulmonary resuscitation (CPR) to preserve life. Smartwatches equipped with accelerometer sensors and worn close to the wrist joint may offer an accessible way to provide CPR feedback. This study determined whether accelerometer data from a smartwatch could provide valid estimates of CPR parameters, including compression, decompression, frequency, and cycle time. Nineteen medical school students performed CPR on an instrumented mannequin while wearing a smartwatch. CPR parameters were derived from the smartwatch’s acce

Designing parallel transmit head coil arrays based on radiofrequency pulse performance.

PURPOSE: A new approach to design parallel transmit (pTx) head arrays is proposed that integrates transmit radiofrequency pulse designs with electromagnetic modeling of array coil elements. THEORY AND METHODS: An approach to design pTx head arrays is proposed that finds optimal groupings of a large number of coils into a small number of channels. An algorithm is proposed to extend array-compressed parallel transmit pulse design by adding the ability to optimally select and prune coil elements, in addition to optimizing compression weights. The performance of the method is demonstrated in simu

Design of a Biorthogonal Wavelet Transform Based R-Peak Detection and Data Compression Scheme for Implantable Cardiac Pacemaker Systems.

Bradycardia can be modulated using the cardiac pacemaker, an implantable medical device which sets and balances the patient's cardiac health. The device has been widely used to detect and monitor the patient's heart rate. The data collected hence has the highest authenticity assurance and is convenient for further electric stimulation. In the pacemaker, ECG detector is one of the most important element. The device is available in its new digital form, which is more efficient and accurate in performance with the added advantage of economical power consumption platform. In this work, a joint alg

Clinical Efficacy and Safety of Long-Term Compression in Patients with Mixed Arterial and Venous Etiology Ulcers in the Leg.

Elastic compressions are standard treatment for leg ulcers of venous etiology. The effect of compressions on ulcers of mixed (arterial or venous) etiology, however, has rarely been studied. The objective of this study was to evaluate the variation in transcutaneous oxygen pressure (TcPO2) in patients with ulcers of mixed arterial or venous etiology treated with 1 month of compression. This prospective cohort study was conducted at a university hospital in France. Patient eligibility was for those attending a consultation of a work-up of a leg ulcer of mixed arterial-venous etiology lasting at

[Neurovascular compression syndrome in the upper thoracic aperture region: diagnosis and surgical treatment].

UNLABELLED: Neurovascular compression syndrome (NVCS) in the region of the upper thoracic aperture (UTA) occurs due to significant compression of the vascular-nervous bundle (VNB) in narrow anatomical spaces, which are represented by an interscalene space, costoclavicular space and area under the pectoralis minor tendon. Based on the data of traditional clinical and instrumental research methods, it is not always possible to accurately determine the nature and level of damage to the vascular-nervous bundle (VNB), that is often caused by variability of anatomical organization of the peripheral

Material properties of bovine intervertebral discs across strain rates.

The intervertebral disc (IVD) is a complex structure responsible for distributing compressive loading to adjacent vertebrae and allowing the vertebral column to bend and twist. To study the mechanical behaviour of individual components of the IVD, it is common for specimens to be dissected away from their surrounding tissues for mechanical testing. However, disrupting the continuity of the IVD to obtain material properties of each component separately may result in erroneous values. In this study, an inverse finite element (FE) modelling optimisation algorithm has been used to obtain material

A Review of Perilunate Dislocations and Concomitant Acute Carpal Tunnel Syndrome: When Should the Carpal Tunnel be Released?

Perilunate dislocations (PLDs) are severe injuries of the wrist often resulting from a high-energy trauma. Given the traumatic mechanism and complexity of the injury itself, PLDs often require urgent intervention in the form of early open reduction and surgical stabilization. One potential, but severe, complication of perilunate dislocations is median nerve compression resulting in acute carpal tunnel syndrome. The treatment of acute median nerve compression secondary to PLDs is controversial, leading to the question of when, and in what PLD cases, should the carpal tunnel be released? The pur

Solving linear systems on quantum hardware with hybrid HHL+.

The limited capabilities of current quantum hardware significantly constrain the scale of experimental demonstrations of most quantum algorithmic primitives. This makes it challenging to perform benchmarking of the current hardware using useful quantum algorithms, i.e., application-oriented benchmarking. In particular, the Harrow-Hassidim-Lloyd (HHL) algorithm is a critical quantum linear algebra primitive, but the majority of the components of HHL are far out of the reach of noisy intermediate-scale quantum devices, which has led to the proposal of hybrid classical-quantum variants. The goal
