Discriminating cognitive motor dissociation from disorders of consciousness using structural MRI.

An accurate evaluation and detection of awareness after a severe brain injury is crucial to a patient's diagnosis, therapy, and end-of-life decisions. Misdiagnosis is frequent as behavior-based assessments often overlook subtle signs of consciousness. This study aimed to identify brain MRI characteristics of patients with residual consciousness after a severe brain injury and to develop a simple MRI-based scoring system according to the findings. We retrieved data from 128 patients and split them into a development or validation set. Structural brain MRIs were qualitatively assessed for lesion

[Acute Disorders of Consciousness and Coma - Systematics, Differential Diagnosis and Management].

Cerebral diseases such as epileptic seizures, cerebral hemorrhages or meningoencephalitis are the primary cause of approximately 50 % of non-traumatic acute disorders of consciousness. For the differential diagnosis, history and other symptoms are important such as hemiplegia, signs of brain stem dysfunction, meningism or headache. Metabolic, endocrinologic, toxicologic or electrolytic causes of coma usually can be diagnosed by laboratory examinations. Anamnestic informations, body inspection, clinical neurological examination as well as laboratory and imaging findings have to be added and cat

Intrathecal baclofen associated with improvement of consciousness disorders in spasticity patients.

OBJECTIVE: Intrathecal baclofen (ITB) pump is a therapeutic option for persistent vegetative state and minimal conscious state patients that have associated spasticity. We investigated whether this treatment modality can affect their level of consciousness. METHOD: In this prospective, open label, observational study, we implanted ITB pumps for the treatment of spasticity in eight patients with disorders of consciousness (vegetative state and minimally conscious state) and we followed them with the Coma Recovery Scale-Revised, the Eastern Cooperative Oncology Group (ECOG) performance scale, a

Affect consciousness and eating disorders. Short term stability and subgroup characteristics.

The aim of the study was to analyse differences in observer rated affect consciousness (AC) between subgroups of patients diagnosed with eating disorders (N = 44; 30 with anorexia nervosa and 14 with bulimia nervosa), and a non-clinical group (N = 40). Another aim was to study the short-term stability of AC over 10-11 weeks of treatment and its relation to self-reported eating pathology and general psychopathology. A moderate short-term stability of AC was found but the levels were not correlated with eating pathology or psychopathology. No differences between the two diagnostic categories wer

HD-tACS at 40 Hz improves consciousness and brain connectivity in prolonged disorders of consciousness: a randomized sham-controlled clinical trial.

BACKGROUND: Frequency-specific high-definition transcranial alternating current stimulation (HD-tACS) may facilitate recovery in patients with prolonged disorders of consciousness (pDoC), but predictive biomarkers and mechanistic insights are limited. OBJECTIVE: To investigate the therapeutic efficacy of 40 Hz or 4 Hz HD-tACS in pDoC. METHODS: Sixty pDoC patients were randomized into four groups receiving 40 Hz or 4 Hz HD-tACS, each with a corresponding sham condition. Behavioral recovery was assessed via Coma Recovery Scale-Revised (CRS-R), network connectivity was analyzed with electroence

Alterations in white matter microstructure and glymphatic function in post-traumatic disorders of consciousness: impact on consciousness levels and prognosis.

PURPOSE: This study aimed to investigate the impairments in white matter microstructure and glymphatic function in patients with post-traumatic brain injury (TBI) who exhibit disorders of consciousness (DoCs), as well as their relationships with levels of consciousness and clinical outcomes. METHODS: We enrolled 30 patients diagnosed with TBI who exhibited DoCs and 30 healthy controls for MRI scanning. We compared intergroup differences in diffusion tensor imaging (DTI) metrics, the DTI analysis along the perivascular space (DTI-ALPS) index, and choroid plexus volume (CPV). The Pearson correl

Prolonged disorders of consciousness: Practice described by palliative and rehabilitation physicians.

STUDY OBJECTIVE: National clinical guidelines for the management of prolonged disorders of consciousness (PDOC) following sudden-onset brain injury were updated in 2020. Since then, clinical experience has grown. This paper reports on current practice. STUDY DESIGN: A questionnaire was developed, tested and sent to rehabilitation and palliative medicine specialists, via the British Society of Physical and Rehabilitation Medicine and Association for Palliative Medicine. Anonymised responses were analysed. RESULTS: 63.5% (n = 61) of respondents were involved in the care of people with PDOC, wi

Updates in Management of Acute Disorders of Consciousness After Traumatic Injury.

Traumatic injury is one of the most common causes of disorders of consciousness (DoC) worldwide, but the management and prognosis of DoC remain enigmatic. The uncertainty surrounding the natural course of DoC, the tendency of consciousness to wax and wane, and a lack of effective treatments outside of avoiding additional insults renders trauma-associated DoC complex for both providers and patient surrogates to navigate. This review explores the acute clinical course of DoC after traumatic injury chronologically and aims to compile recommendations based on the current best practices for diagnos

Definitions, diagnostic criteria, and clinical assessment scales in disorders of consciousness.

Disorders of consciousness (DoC) are neurologic conditions characterized by severe alteration in level of consciousness. Categories of DoC may include coma, unresponsive wakefulness syndrome/vegetative state, minimally conscious state (MCS; can be further categorized into MCS+ and MCS- based on the presence or absence of language-related behaviors), emergence from MCS, confusional state or delirium, and cognitive motor dissociation (CMD). CMD is a recently defined condition in which the patient fails to demonstrate observable behavioral responses on bedside assessment but demonstrates covert c

Feasibility of Non-Gaussian Diffusion Metrics in Chronic Disorders of Consciousness.

Diagnostic accuracy of different chronic disorders of consciousness (DOC) can be affected by the false negative errors in up to 40% cases. In the present study, we aimed to investigate the feasibility of a non-Gaussian diffusion approach in chronic DOC and to estimate a sensitivity of diffusion kurtosis imaging (DKI) metrics for the differentiation of vegetative state/unresponsive wakefulness syndrome (VS/UWS) and minimally conscious state (MCS) from a healthy brain state. We acquired diffusion MRI data from 18 patients in chronic DOC (11 VS/UWS, 7 MCS) and 14 healthy controls. A quantitative

Deep brain stimulation in five patients with severe disorders of consciousness.

OBJECTIVE: The efficacy of deep brain stimulation in disorders of consciousness remains inconclusive. We investigated bilateral 30-Hz low-frequency stimulation designed to overdrive neuronal activity by dual pallido-thalamic targeting, using the Coma Recovery Scale Revised (CRS-R) to assess conscious behavior. METHODS: We conducted a prospective, single center, observational 11-month pilot study comprising four phases: baseline (2 months); surgery and titration (1 month); blind, random, crossover, 1.5-month ON and OFF periods; and unblinded, 5-month stimulation ON. Five adult patients were in

Can self-relevant stimuli help assessing patients with disorders of consciousness?

Emotional and self-relevant stimuli are able to automatically attract attention and their use in patients suffering from disorders of consciousness (DOC) might help detecting otherwise hidden signs of cognition. We here recorded EEG in three Locked-in syndrome (LIS) and four Vegetative State/Unresponsive Wakefulness Syndrome (VS/UWS) patients while they listened to the voice of a family member or an unfamiliar voice during a passive. Data indicate that, in a passive listening condition, the familiar voice induces stronger alpha desynchronization than the unfamiliar one. In an active condition,

[Diagnosis and differential diagnosis of consciousness disorders from the psychiatric viewpoint].

Diagnosis and differential diagnosis of disturbance of consciousness are often difficult, especially when occurring in the course of psychiatric diseases. Apart from discord in defining the term of consciousness it is not always easy to distinguish between qualitative and quantitative alterations of the disorders. The performance of an exact interview and a thorough clinical examination allows an estimation of the degree of dimming of consciousness as well as the possible causes. Disorder of Consciousness is often found in patients carrying out substance abuse but also in the course of other p

Perspectives on consciousness in patients with disorders of consciousness from brain injury: group concept mapping study across clinic, research, and families.

BACKGROUND: An effective healthcare system depends on clinic, research, and patient/relatives interactions. Such interactions may at their core be challenged by misalignments of concepts and the practices that constitute them. The concept of consciousness and what is experienced and understood as signs of consciousness in patients with severe acquired brain injury is one of these potential areas of misalignment. Different perspectives and experiences of consciousness are challenging the delivery of care and the high-stake decision-making process on the potential withdrawal of treatment. The en

[Disorders of consciousness due to disorders of body fluid composition--case report of a female patient].

We report a female patient presenting with sepsis and multi-organ failure following eclampsia and intrauterine childdeath. In the phase of recovery, the patient developed consciousness disorder and coma characterized by fasciculation, generalized myoclonia and respiratory insufficiency. The clinical picture corresponded to that of Lance Adam's syndrome. A quick change in the composition of body fluids in the polyuric phase of renal insufficiency associated with an antidiuretic hormone deficit was a cause of that disorder. Metabolic dysfunction and hyperexcitability of neurons developed as a re

Multimodal multicentre investigation of diagnostic and prognostic markers in disorders of consciousness.

Severely brain-injured patients may enter a spectrum of conditions collectively known as disorders of consciousness. This spectrum includes clinical conditions such as unresponsive wakefulness syndrome or minimally conscious state, where the behavioural assessment of consciousness can often be deceptive. To bridge this dissociation, neuroimaging techniques are employed to identify the residual brain functions. Each neuroimaging modality imperfectly captures distinct aspects of brain preservation-functional, anatomical, or both. In this study, we adopt a comprehensive approach by integrating th

Eye-Tracking Assessment in Patients with Disorders of Consciousness: A Systematic Review.

BACKGROUND/OBJECTIVES: Disorders of consciousness (DOC), including vegetative state/unresponsive wakefulness syndrome (VS/UWS) and minimally conscious state (MCS), present significant diagnostic challenges. Misdiagnosis rates approach 40%, often due to limitations in detecting subtle behavioural responses through clinical observation alone. Eye-tracking technology offers objective, quantitative assessment of visual behaviours that may reveal covert signs of consciousness. This systematic review aimed to evaluate the diagnostic accuracy of eye-tracking technology compared to the Coma Recovery S
