Evolution of continuous renal replacement therapy scenarios and the rise of anticoagulant-free continuous veno-venous hemodiafiltration.

PURPOSE OF REVIEW: Continuous renal replacement therapy (CRRT) is an essential support modality for patients with acute kidney injury (AKI) and hemodynamic instability. Circuit clotting remains a major limitation to efficacy. The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend regional citrate anticoagulation (RCA) as the preferred method of anticoagulation in CRRT for patients without contraindications. The guidelines also emphasize tailoring anticoagulation strategies to individual patient needs and institutional capabilities. RECENT FINDINGS: Strategies to prevent CR

Monitoring and dose adjustment of regional citrate anticoagulation for continuous renal replacement therapy.

PURPOSE OF REVIEW: Regional citrate anticoagulation (RCA) is the preferred anticoagulation strategy for many patients receiving continuous renal replacement therapy (CRRT). Safe implementation requires a coordinated prescription integrating citrate delivery, citrate removal within the extracorporeal circuit, individualized calcium replacement, CRRT fluid composition, and structured biochemical monitoring. This review summarizes practical approaches to RCA prescription, monitoring, and dose adjustment, emphasizing recent advances in citrate kinetics, calcium homeostasis, impaired citrate metabo

Continuous Renal Replacement Therapy in Acute Liver Failure: Rationale, Evidence, and Evolving Practice.

Acute liver failure (ALF) is a life-threatening condition hallmarked by an acute onset of liver dysfunction, coagulopathy, and encephalopathy in individuals without pre-existing liver disease. Advances in intensive care, coupled with increased access to liver transplant (LT), have substantially improved outcomes of individuals with ALF. Aggressive goal-directed intensive care focusing on individual organs has become the cornerstone of ALF management. In this context, continuous renal replacement therapy (CRRT) has emerged as a critical component in the management of ALF. While traditionally ce

Population Pharmacokinetics of Tigecycline for Critically Ill Patients Undergoing Continuous Renal Replacement Therapy.

BACKGROUND: Tigecycline is considered one of the last resorts for treating infections caused by multidrug-resistant bacteria. Continuous renal replacement therapy (CRRT) is widely used in critically ill patients, especially those with acute kidney injury or severe infections. However, pharmacokinetic data for tigecycline in patients receiving CRRT are limited. METHODS: This was a single-center prospective clinical study with intensive sampling that included critically ill patients who received tigecycline and CRRT. A population pharmacokinetic (PPK) model was developed and evaluated by goodne

Severe vitamin C deficiency associated with continuous renal replacement therapy: A case report.

Hypovitaminosis C is prevalent in critically ill patients. Continuous renal replacement therapy (CRRT) clears vitamin C, increasing the risk for vitamin C deficiency. However, recommendations for vitamin C supplementation in critically ill patients receiving CRRT vary widely, from 250 mg/day to 12 g/day. This case report describes a patient who developed a severe vitamin C deficiency after prolonged CRRT despite receiving ascorbic acid (450 mg/day) supplementation in her parenteral nutrition. This report summarizes recent research investigating vitamin C status in critically ill patients recei

Complications of Regional Citrate Anticoagulation for Continuous Renal Replacement Therapy: An Observational Study.

INTRODUCTION: Regional citrate anticoagulation (RCA) is the recommended anticoagulation modality for continuous renal replacement therapy (CRRT). RCA was associated with a low rate of complications in randomized controlled trials. However, little is known about the type and rate of complications in real life. We sought to describe complications associated with RCA in comparison with those associated with heparin anticoagulation. METHODS: In our institution, RCA has been the default anticoagulation modality for CRRT in all patients without contraindications since 2013. We have retrospectively

Calorie provision from citrate anticoagulation in continuous renal replacement therapy in critical care.

BACKGROUND: Citrate is used as a regional anticoagulant for continuous veno-venous haemofiltration and provides 0.59 kcal/mmol. Previous studies hypothesised continuous veno-venous haemofiltration can provide 200-1300 kcal/day dependent on the anticoagulant and replacement solutions used. The aim of this study was to calculate the calorie load from citrate in our patient group. METHODS: An equation derived from a paper by Oudemans-van Straaten was used to estimate calorie provision from citrate. Citrate calorie load was defined as the difference between the citrate in the filter circuit and t

Pharmacological interventions for preventing clotting of extracorporeal circuits during continuous renal replacement therapy.

BACKGROUND: Acute kidney injury (AKI) is a major comorbidity in hospitalised patients. Patients with severe AKI require continuous renal replacement therapy (CRRT) when they are haemodynamically unstable. CRRT is prescribed assuming it is delivered over 24 hours. However, it is interrupted when the extracorporeal circuits clot and the replacement is required. The interruption may impair the solute clearance as it causes under dosing of CRRT. To prevent the circuit clotting, anticoagulation drugs are frequently used. OBJECTIVES: To assess the benefits and harms of pharmacological interventions

Heart rate is associated with mortality in patients undergoing continuous renal replacement therapy.

BACKGROUND: Heart rate (HR) is an essential vital sign based on the finding that HR beyond its normal range is associated with several conditions or diseases, including high mortality in several clinical settings. Nevertheless, the clinical implications of HR remain unresolved in patients undergoing continuous renal replacement therapy (CRRT). METHODS: This retrospective cohort study included 828 patients who underwent CRRT due to acute kidney injury between 2010 and 2014. HR and other baseline parameters at the time of CRRT initiation were retrieved. The odds ratio (OR) of 30-day mortality w

Continuous renal replacement therapy rescues severe haemorrhagic fever with renal syndrome in pregnancy: a case report.

BACKGROUND: Haemorrhagic fever with renal syndrome (HFRS) is a natural epidemic disease caused by various types of viruses of the genus Hantavirus, which are mainly transmitted by contact with the infected rodents and their droppings. Pregnancy complicated with HFRS is rare; however, adverse maternal and foetal outcomes may be noted. In this report, we describe a case involving a pregnant woman with HFRS who was in a state of multiple organ dysfunction syndrome (MODS) and was successfully treated with continuous renal replacement therapy (CRRT). CASE PRESENTATION: A 32-year-old pregnant woman

Circuit Survival in Children with Hemolytic Uremic Syndrome Requiring Continuous Renal Replacement Therapy.

INTRODUCTION: Hemolytic uremic syndrome (HUS) is a significant cause of acute kidney injury (AKI) in children that is caused by Shiga toxin-producing Escherichia coli. Continuous renal replacement therapy (CRRT) is frequently used for managing AKI in critically ill pediatric patients. The filter lifespan is a crucial factor affecting its effectiveness and cost. This study aimed to investigate the impact of HUS on the lifespan of CRRT filters compared with that of other etiologies. MATERIALS AND METHODS: This retrospective longitudinal study was conducted in a pediatric intensive care unit in

Intradialytic hypotension and hemodynamic phenotypes in children following continuous renal replacement therapy initiation.

BACKGROUND: Intradialytic hypotension (IDH) leads to inadequate organ perfusion and occurs frequently after continuous renal replacement therapy (CRRT) connection. Unsupervised learning can enhance our understanding of how clinical trajectories impact outcomes. We aim to investigate the association between IDH during CRRT connection and outcomes, while also identifying hemodynamic trajectory-based phenotypes. METHODS: A single center retrospective observational study of children (<18 years) undergoing CRRT from 9/2016 to 10/2018. IDH was defined as a sustained >20% decrease in mean arterial p

Evidence of Stem Cell Entrapment in AN69 Membranes During Continuous Renal Replacement Therapy.

BACKGROUND: Continuous renal replacement therapy (CRRT) is increasingly employed in pediatric intensive care units (PICUs) for managing both renal and non-renal conditions, including sepsis through cytokine removal. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a critical treatment for inherited bone marrow syndromes but poses challenges when performed alongside extracorporeal therapies like CRRT. This case explores the interaction between hematopoietic stem cells and AN69 membranes during CRRT in a pediatric patient undergoing allo-HSCT amid septic shock. METHODS: A 10-ye

Neonatal Extracorporeal Membrane Oxygenation: Associations between Continuous Renal Replacement Therapy, Thrombocytopenia, and Outcomes.

INTRODUCTION: The incidence of thrombocytopenia in neonates receiving extracorporeal membrane oxygenation (ECMO) with and without concurrent continuous renal replacement therapy (CRRT) and associated complications have not been well described. The primary aims of the current study were to (1) characterize thrombocytopenia in neonates receiving ECMO (including those treated concurrently with CRRT) and (2) evaluate risk factors (including CRRT utilization) associated with severe thrombocytopenia. In a planned exploratory secondary aim, we explored the association of severe thrombocytopenia with
