Biomarkers of conversion to α-synucleinopathy in isolated rapid-eye-movement sleep behaviour disorder.

Patients with isolated rapid-eye-movement sleep behaviour disorder (RBD) are commonly regarded as being in the early stages of a progressive neurodegenerative disease involving α-synuclein pathology, such as Parkinson's disease, dementia with Lewy bodies, or multiple system atrophy. Abnormal α-synuclein deposition occurs early in the neurodegenerative process across the central and peripheral nervous systems and might precede the appearance of motor symptoms and cognitive decline by several decades. These findings provide the rationale to develop reliable biomarkers that can better predict con

Diagnostic conversion to bipolar disorder in unipolar depressed patients participating in trials on antidepressants.

OBJECTIVE: In unipolar depressed patients participating in trials on antidepressants, we investigated if illness characteristics at baseline could predict conversion to bipolar disorder. METHOD: A long-term register-based follow-up study of 290 unipolar depressed patients with a mean age of 50.8 years (SD=11.9) participating in three randomized trials on antidepressants conducted in the period 1985-1994. The independent effects of explanatory variables were examined by applying Cox regression analyses. RESULTS: The overall risk of conversion was 20.7%, with a mean follow-up time of 15.2 year

Neuroimaging evidence of structural and network disruptions in adolescents with conversion disorder with seizures.

BACKGROUND: This study investigated structural and network-level brain alterations in adolescents with conversion disorder with attacks of seizures (CD/FS), focusing on stage-specific differences across acute and persistent presentations. METHODS: Sixty adolescents with CD/FS (26 acute-stage, 34 persistent-stage) and 60 age-matched controls underwent 3 T MRI. Voxel-based morphometry (VBM), source-based morphometry (SBM), and connectogram analyses were used to assess grey and white matter (GM and WM) alterations. Correlations with dissociative symptoms (DES scores) and age were evaluated. RES

Evidence-Based Practice: Physiotherapy for Children and Adolescents with Motor Symptoms of Conversion Disorder.

Clinical Case: A boy aged 14 years with conversion disorder who presented with an ataxic gait pattern experienced complete symptom resolution after engaging in physiotherapy (PT) as part of interdisciplinary treatment. Clinical Question: Is there evidence to support the use of PT in children and adolescents with motor symptoms of conversion disorder? Evidence: All the extant paediatric literature consists of case studies and case series. Some exciting research has been published in the adult literature that suggests the potential positive benefits of PT. Limitation of the Evidence and Future R

Multidisciplinary assessment and diagnosis of conversion disorder in a patient with foreign accent syndrome.

Multiple reports have described patients with disordered articulation and prosody, often following acute aphasia, dysarthria, or apraxia of speech, which results in the perception by listeners of a foreign-like accent. These features led to the term foreign accent syndrome (FAS), a speech disorder with perceptual features that suggest an indistinct, non-native speaking accent. Also correctly known as psuedoforeign accent, the speech does not typically match a specific foreign accent, but is rather a constellation of speech features that result in the perception of a foreign accent by listeners

Sex distribution and psychiatric features of child and adolescent conversion disorder across 2 decades.

BACKGROUND: Earlier research suggested female predominance in adult conversion disorder, and the strong association between conversion disorder and experiences of being abused is well known. However, the data for child and adolescent populations are limited. In Taiwan, the dramatic increase in child abuse may have some impact on the features of child and adolescent conversion disorder. This study aimed to compare the demographic characteristics, psychiatric comorbidity, and experiences of being abused in Taiwanese children and adolescents diagnosed with conversion disorder in psychiatric consu

A diagnosis of bipolar spectrum disorder predicts diagnostic conversion from unipolar depression to bipolar disorder: a 5-year retrospective study.

BACKGROUND: The major aims of this study were to identify factors that may predict the diagnostic conversion from major depressive disorder (MDD) to bipolar disorder (BP) and to evaluate the predictive performance of the bipolar spectrum disorder (BPSD) diagnostic criteria. METHODS: The medical records of 250 patients with a diagnosis of MDD for at least 5 years were retrospectively reviewed for this study. RESULTS: The diagnostic conversion from MDD to BP was observed in 18.4% of 250 MDD patients, and the diagnostic criteria for BPSD predicted this conversion with high sensitivity (0.870) a

Personality profile and its association with conversion to neurodegenerative disorders in idiopathic REM sleep behavior disorder.

Patients with Parkinson's disease (PD) were described less extraverted and more neurotic. It remained unclear whether similar personality traits could be found in idiopathic rapid eye movement sleep behavior disorder (iRBD), a prodromal stage of PD, and could predict phenoconversion to neurodegenerative disorders. We aimed to investigate the personality profile and its association with future neurodegenerative phenoconversion in iRBD patients. One hundred and eighty-five video-polysomnography confirmed iRBD patients and 91 age- and sex-matched controls underwent personality assessment using th

Conversion disorders in childhood and adolescence: a psychiatric consultation study in a general hospital.

From March 1986 to April 1995, 418 consecutive psychiatric consultations for patients aged 18 or below were completed. Ten of these patients met the Diagnostic and Statistical Manual of Mental Disorders, 4th. edition (DSM-IV) diagnostic criteria for a conversion disorder. The medical and psychiatric consultation records of these patients were reviewed. Chief conversion symptoms included pseudoseizure (four cases), gait disturbance (two cases), abnormal movement (one case), four limbs weakness (one case), aphonia (one case) and tunnel vision (one case). The psychosocial problems of the patients

Course and outcome of conversion and somatization disorders. A four-year follow-up.

Although the two disease concepts have very different histories, many previous studies have mixed conversion disorder and somatization disorder and none has made direct comparison between them. The authors applied DSM-III criteria to inpatient and outpatient medical records and attempted to follow 98 patients who met criteria for somatization disorder or conversion disorder. Five of these patients died 4 years later and, of those who survived, 70 (75.3%) were given follow-up interviews by a rater blind to baseline diagnosis. The 32 patients with a baseline diagnosis of conversion disorder were

[Organic versus functional factors in the diagnosis of conversion disorders (author's transl)].

Conversion symptoms involving conspicous pseudoneurological signs of the motor system are very seldom today. To differentiate in these cases between organic and functional disturbances is highly difficult. Two patients are described. A 25 year old soldier with the symptomatology of a repetitive transverse lesion of the spinal cord and a 70 year old pensioner with a blepharospasm. They both showed an accompanying depression. Diagnostic criteria, psychodynamic models and especially the organic background of conversion symptoms are discussed.

Work-Related Conversational Skills Training for People with Substance Use Disorders.

OBJECTIVE: There are high rates of unemployment among individuals with substance use disorders (SUDs) and unemployment can increase the risk of relapse. Additionally, SUD relapse can increase the chance of job loss. However, there is a dearth of research related to the feasibility and acceptability of employment counseling strategies for individuals with SUDs. METHODS: This study is a secondary sub-group analysis of data attained from a pre-post trial (no control group) of the Conversing with Others intervention, a direct skills teaching (DST) approach designed to enhance confidence with work

Phenotypical Conversions of Dermal Adipocytes as Pathophysiological Steps in Inflammatory Cutaneous Disorders.

Adipocytes from the superficial layer of subcutaneous adipose tissue undergo cyclic de- and re-differentiation, which can significantly influence the development of skin inflammation under different cutaneous conditions. This inflammation can be connected with local loading of the reticular dermis with lipids released due to de-differentiation of adipocytes during the catagen phase of the hair follicle cycle. Alternatively, the inflammation parallels a widespread release of cathelicidin, which typically takes place in the anagen phase (especially in the presence of pathogens). Additionally, tr

The Cash effect: Shaping the research conversation on body image and eating disorders.

Cash and Deagle (1997) examined the associations between body image disturbance (BID) and the eating disorders (EDs) of anorexia nervosa (AN) and bulimia nervosa (BN) in a meta-analytic review. They found almost twice as many studies employing perceptual measures of body size evaluation compared to cognitive-evaluative measures of body dissatisfaction, even though effect sizes were larger for studies with cognitive-evaluative measurement. We examined 109 "influential" (i.e., well-cited) studies that cited the Cash and Deagle meta-analysis. We found a slight, continued emphasis on research usin

Plasma pTau181 and amyloid markers predict conversion to dementia in idiopathic REM sleep behaviour disorder.

Blood-based biomarkers for Alzheimer's disease (AD) pathology have been investigated intensively as markers for AD-related neurodegeneration. Comorbid AD pathology is common in dementia with Lewy bodies (DLB). Accordingly, we hypothesized that plasma biomarkers associated with AD pathology might be useful to predict DLB in a cohort of idiopathic/isolated REM sleep behaviour disorder (iRBD), an incipient synucleinopathy. The aim of this study was to determine whether plasma amyloid-β and pTau181 biomarkers can predict DLB. This longitudinal single-centre (Canada) cohort study included 158 indiv

Understanding conversion disorder: How contemporary brain imaging is shedding light on an early Freudian concept.

Conversion disorder, also called Functional Neurological Symptom Disorder is poorly understood by many in the medical profession and is associated with considerable health care costs. Sigmund Freud, in an early, pre-psychoanalytic period paper, suggested that hysterical motor paralyses arose from a "functional or dynamic lesion" which was no different from an organic one, but rather an altered expression of it. He linked this functional brain disturbance to an excess of affect, a faulty conceptualization on the part of the symptomatic individual of how the affected organ works, and elements of
