A toxigenic Corynebacterium diphtheriae biovar mitis sequence type 136 (ST136) strain was recovered from a toe infection of an unvaccinated patient recently returned from India. The isolate was resistant to clindamycin, erythromycin (ermX positive), tetracycline, and trimethoprim-sulfamethoxazole, intermediate to ceftriaxone and cefotaxime, and had high MICs for telithromycin and chloramphenicol but was sensitive to other drugs.

The use of polymerase chain reaction (UP-PCR with universal primers for the genetic typing of 85 C. diphtheriae strains, isolated from patients and carriers at the period of the epidemic, has shown that a single natural clone of biovar gravis seems to prevail in North Western Russia. UP-PCR is an effective too for the rapid intraspecific typing of strains on the molecular-genetic level and for the study of the circulation of C.diphtheriae epidemic clones.

The clinical features of 5 patients with invasive disease due to nontoxigenic Corynebacterium diphtheriae are presented. 4 patients had proven left sided endocarditis, and one had probable endocarditis of the mitral valve. 4 patients were intravenous drug abusers, and one had a girl friend who was also an iv-drug abuser. Only one patient had antibodies against human immunodeficiency virus. 2 patients with endocarditis of the mitral or the aortic valve, and one patient with endocarditis of a prosthetic aortic valve, died of septic complications. All patients or their partners frequented the sam

Diphtheria is now an uncommon disease in Britain. We describe an imported case of cutaneous diphtheria in a previously immunised adult cause by C. diphtheriae var mitis. The control measures adopted to deal with the index case and two secondary cases so as to limit further spread among household and school contacts are outlined. Molecular typing was used to study the mode of spread of the organism among contacts.

In 2015, the Clinical and Laboratory Standards Institute (CLSI) updated its breakpoints for penicillin susceptibility in Corynebacterium species from <1 mg/L to <0.12 mg/L. We assessed the effect of this change on C. diphtheriae susceptibility reported at an inner city, tertiary care center in Vancouver, British Columbia, Canada, during 2015-2018 and performed whole-genome sequencing to investigate phenotypic and genotypic resistance to penicillin. We identified 44/45 isolates that were intermediately susceptible to penicillin by the 2015 breakpoint, despite meeting previous CLSI criteria for

In September 2018, a child who had returned from Somalia to Germany presented with cutaneous diphtheria by toxigenic Corynebacterium diphtheriae biovar mitis. The child's sibling had superinfected insect bites harbouring also toxigenic C. diphtheriae. Next generation sequencing (NGS) revealed the same strain in both patients suggesting very recent human-to-human transmission. Epidemiological and NGS data suggest that the two cutaneous diphtheria cases constitute the first outbreak by toxigenic C. diphtheriae in Germany since the 1980s.

The morbidity of diphtheria reported in Bari in 1970 and in 1971 was 17.1 and 19.1 for 100000 inhabitants; in the same years the national average was respectively 1.6 and 1.3. In a epidemiological research carried out in the same period in 1017 school-children of Bari not previously selected, 45 carriers of C. diphtheriae (4.42%) were identified. Toxigen strains were isolated from 11 carriers (1.08%). The frequency of carriers was higher in the school-children 9-11 years aged (6-04%), lower in the school-children 6-8 years aged (2.99%).

We report a possible zoonotic case of severe diphtheria-like respiratory illness in Norway caused by a previously unreported toxigenic Corynebacterium ulcerans sequence type. This case highlights C. ulcerans as an emerging pathogen that can cause life-threatening disease. Clinicians should be aware of C. ulcerans infection, even in regions where diphtheria is rare.

Extrapharyngeal infections caused by Corynebacterium ulcerans have rarely been reported previously, and diphtheria toxin production has usually not been addressed. This case demonstrates that strains of C. ulcerans that produce diphtheria toxin can cause infections of the skin that completely mimic typical cutaneous diphtheria, thereby potentially providing a source of bacteria capable of causing life-threatening diseases in the patient's environment. Therefore, it is recommended to screen wound swabs for coryneform bacteria, identify all isolates, carefully assess possible toxin production, a

BACKGROUND: Respiratory diphtheria is a toxin-mediated disease caused by Corynebacterium diphtheriae. Diphtheria-like illness, clinically indistinguishable from diphtheria, is caused by Corynebacterium ulcerans, a zoonotic bacterium that can also produce diphtheria toxin. In the United States, respiratory diphtheria is nationally notifiable: specimens from suspected cases are submitted to the Centers for Disease Control and Prevention (CDC) for species and toxin confirmation, and diphtheria antitoxin (DAT) is obtained from CDC for treatment. We summarize the epidemiology of respiratory diphthe

Infective endocarditis is a rare phenomenon that may have devastating consequences. While uncommon, the spectrum of causative organisms can include Corynebacterium species. Corynebacterium diphtheriae endocarditis bears a high potential for systemic complications and overall mortality. We present a case of mitral and tricuspid native valve endocarditis caused by C. diphtheriae in an immunocompetent patient and highlight the severe manifestations of this condition.

OBJECTIVES: Diphtheria remains endemic in Pakistan, with cases increasing following the COVID-19 pandemic despite ongoing vaccination programs. This study analyzes the genomic diversity, virulence, and antimicrobial resistance patterns of pharyngeal diphtheria strains collected during the Karachi outbreak.
METHODS: Corynebacterium diphtheriae isolates from a tertiary care hospital laboratory in Karachi (August 2023-October 2024) were included. Antimicrobial susceptibility testing and whole-genome sequencing of phenotypically confirmed isolates were performed. Phylogenetic and bioinformatics a

Since 2022, European countries have been facing an outbreak of mainly cutaneous diphtheria caused by toxigenic Corynebacterium diphtheriae among asylum seekers. In Belgium, between 1 March and 31 December 2022, 25 cases of toxigenic C. diphtheriae infection were confirmed among asylum seekers, mostly among young males from Afghanistan. Multi-locus sequence typing showed that most isolates belonged to sequence types 574 or 377, similar to the majority of cases in other European countries. The investigation and management of the outbreak, with many asylum seekers without shelter, required adjust

Infective endocarditis due to non-toxigenic Corynebacterium diphtheriae is uncommon. We describe the case of a 42-year-old male with a history of mitral valve replacement with prosthetic valve for 4 years. He presented with fever, weight loss, dyspnea on exertion and orthopnea. The echocardiography demonstrated large vegetation attached on the left coronary cusp of the aortic valve with moderately severe aortic regurgitation but sparing of the prosthetic mitral valve. Three separate blood cultures grew Corynebacterium species. The patient underwent aortic valve replacement due to valvular dysf

Diphtheria is under control in industrialized countries. However, single cases and outbreaks still occur and the disease is not completely understood. Forty-three individuals suspected of having diphtheria who were referred to the Infectious Disease Hospital of Arkhangelsk from December 1994 to March 1995 were included in this study. Fifteen patients were diagnosed as having diphtheria and received equine hyperimmune antidiphtheria toxin antiserum, and 28 were diagnosed as carriers, 12 with nondiphtherial tonsillitis or pharyngitis and 16 without symptoms. Serum samples were obtained on admiss

We report a case of a 69-year-old woman with aspiration pneumonia in whom Corynebacterium diphtheriae (C. diphtheriae) was initially identified by mass spectrometry but subsequently confirmed as Corynebacterium belfantii (C. belfantii) through whole-genome analysis. Since sputum culture isolation and subsequent mass spectrometry identification yielded C. diphtheriae strain 2511-21 and Pseudomonas aeruginosa (P. aeruginosa), we notified public health authorities. We implemented isolation measures in accordance with the Infectious Disease Control Law. Diphtheria Toxin production assays, polymera

Corynebacterium ulcerans is an emerging zoonotic pathogen which causes diphtheria-like infections. Although C. ulcerans is found in multiple domestic and wild animal species, most human cases are linked with pets. Our ability to decipher cross-host species transmission dynamics and to understand the emergence of clinically relevant clones (e.g., diphtheria toxin-positive) is currently hampered by a limited knowledge of C. ulcerans strain diversity and genome evolution. Here, we explore the genomic population structure and evolution of C. ulcerans with 582 isolates from diverse hosts and geogra

Diphtheria is an uncommon disease in the UK due to an effective immunization programme; consequently when cases do arise, there can be delays in diagnosis and case-fatality rates remain high. We reviewed 102 patients with infections caused by toxigenic corynebacteria (an average of four per year) reported in the UK between 1986 and 2008: 42 Corynebacterium diphtheriae, 59 C. ulcerans and one C. pseudotuberculosis, as well as 23 asymptomatic carriers. Five fatalities were reported, all in unvaccinated patients. The major risk factor for C. diphtheriae infection continued to be travel to an ende
