BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome is a rare autosomal recessive disorder caused by PRG4 mutations that impair lubricin production. Resulting noninflammatory hyperplasia produces congenital or early-onset camptodactyly and noninflammatory arthropathy, affecting large joints. Because clinical features overlap with trigger finger and juvenile idiopathic arthritis (JIA), misdiagnosis is common.
CASE PRESENTATION: We describe the second genetically confirmed Brazilian case of CACP, involving two siblings. Both showed congenital trigger fingers (later recl

Familial arthropathy associated with congenital camptodactyly has been previously recognized as a definite clinical entity in the literature. The clinical spectrum of this disease seems to be variable. The typical features of congenital camptodactyly, arthropathy, coxa vara and pericarditis (CACP syndrome) appear to be a more frequent presentation in children from the Middle East and North Africa. Musculoskeletal presentation of this rare familial form of arthropathy is unique and heterogeneous. In all previous reports, non-inflammatory pattern of arthropathy involving the peripheral joints wi

BACKGROUND/OBJECTIVES: To describe the musculoskeletal ultrasound (MSUS) findings of pediatric patients with genetically confirmed CACP syndrome and to assess their clinical, laboratory, and synovial fluid characteristics.
METHODS: This single-center, cross-sectional study included 6 pediatric patients from 2 consanguineous families with genetically confirmed PRG4 mutations. Clinical features, laboratory results, synovial fluid analysis, and MSUS examinations of the knee joints were evaluated. Synovial effusion, hypertrophy, and power Doppler activity were assessed using the OMERACT pediatric

BACKGROUND: This paper reports three children with short stature: developmental coxa vara unilateral in the first case and bilateral in the other two; somewhat squared and "ovoid" vertebral bodies in the first patient, and normal to slightly tall vertebral bodies in the third; metaphyseal changes in some long tubular bones including bone fragments similar to the corner fractures seen in child abuse in all three patients.
MATERIALS AND METHODS: The first and second patients were sisters; their mother, also quite short, had surgical procedures in early life for bilateral "coxa vara"; their brot

An autosomal recessively inherited noninflammatory arthropathy known as camptodactyly, arthropathy, coxa vara, and pericarditis (CACP) syndrome was discovered in 1999. It is distinguished by synoviocyte hyperplasia and subcapsular fibrosis of the synovial capsule, which results in a shortage of lubricin production. The resulting lack of joint lubrication induces increased mechanical stress, causing progressive deformities that become evident with weight-bearing and heightened joint activity. Animal models with a lubricin gene knock-out display similar traits, underscoring the impact of mechani

OBJECTIVES: Camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome is an inherited childhood noninflammatory arthropathy that can mimic juvenile idiopathic arthritis (JIA). It consists of congenital or early-onset camptodactyly with arthropathy, but not all patients develop hip or heart involvement. We present a compelling case report that underscores the pivotal role of synovial RNA sequencing in diagnosing and understanding hereditary arthropathies.
METHODS: We report an 18-year-old man with presumed recalcitrant JIA who had a previously undescribed mutation, p.Glu940*, resulting

Objective: We report the unorthodox use of a retrograde intramedullary nail via antegrade technique for the treatment of a subtrochanteric pathological femur fracture in the presence of significant coxa vara. No similar use has been documented previously in literature. Methods: A standard approach to the proximal right femur was performed and standard antegrade technique used to introduce a guide wire into the proximal femur after which the femoral shaft was reamed. A 38-cm long 10 mm diameter retrograde nail was attached to the retrograde insertion and targeting device as if to perform a left

BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP) is a clinically heterogenous congenital disorder caused by mutations in proteoglycan 4 (PRG4), a chondroitin sulfate proteoglycan that acts as a lubricant for the cartilage surface. Although CACP is a rare genetic disorder, several cases were described in the literature from ethnically different populations including Caucasian, Egyptian, Saudi Arabian, Pakistani, and Korean. We report CACP for the first time in United Arab Emirates.
METHODS: Direct sequencing of all the coding exons and splice sites of the PRG4 gene

Femoral neck stress fractures in middle-aged individuals are usually associated with underlying metabolic bone disease. There is increased abnormal loading on femoral neck with associated coxa-vara and obesity in a female. Bilateral presentation of such fractures is rarely reported. Their management poses significant challenges. We present a case of bilateral femoral neck stress fracture in a 58-year-old obese housewife with compression type of the left hip fracture and tension type of the right hip fracture with non-union following an operated femoral neck stress fracture. She presented two m

BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis syndrome is a rare autosomal recessive disorder caused by loss-of-function variants in the proteoglycan 4 gene, which encodes lubricin. It is frequently misdiagnosed as juvenile idiopathic arthritis, leading to prolonged immunosuppressive therapy.
CASE PRESENTATION: We report two unrelated boys born to consanguineous parents who presented with progressive large-joint swelling and were initially diagnosed with juvenile idiopathic arthritis. Both received multiple disease-modifying and biologic agents without clinical response. Inflam

We studied six patients with severe femoral head deformity treated surgically by valgus-flexion-internal-rotation femoral osteotomy plus simultaneous acetabuloplasty based on the concept of "functional retroversion" and "functional coxa vara" of the deformed femoral head in late severe Legg-Calvé-Perthes disease and epiphyseal dysplasia. The combined procedure achieves the following: (i) corrects the "functional coxa vara" and hinge abduction (valgus osteotomy); (ii) establishes a more normal articulation between the posteromedial portion of the true femoral head and the acetabulum, while movi

BACKGROUND: Post-traumatic coxa vara following femoral neck or intertrochanteric fracture nonunion or malunion remains a difficult problem in young and middle-aged patients, in whom salvage total hip arthroplasty carries excess complication risk. Proximal femoral valgus osteotomy is a joint-preserving alternative, but evidence supporting fixation with a dynamic hip screw (DHS) is limited, and is largely confined to younger cohorts.
CASE SUMMARY: We report a single-center, retrospective, consecutive case series of twelve patients (seven men, five women; mean age 52.2 ± 20.0 years, range 20-73)

The rare case of a 5‑year-old girl with autosomal dominant osteopetrosis type 2, who suffered metaphyseal fractures of the femoral neck on both sides within 6 months is described. On the right side, the diagnosis was made 3 months after the onset of symptoms, so that a coxa vara occurred. The treatment was surgically treated through a valgus osteotomy with fixation of the femoral head with K‑wires. Three months after the operation, the girl complained of a painful restriction of movement on her left side. Radiologically, a metaphyseal femoral neck fracture without coxa vara was diagnosed and i

Seventeen patients with congenital spondylo-epiphysial dysplasia from six centres in Britain have been investigated and two variants delineated. There is wide clinical and radiological variability in each group with overlap between them, but 12 of the patients had very short stature and grossly disorganised hips with severe coxa vara, and the five remaining patients were less seriously affected with height only a little below the third percentile and only mild coxa vara. Both groups can be diagnosed at birth but the two cannot be differentiated on clinical and radiological grounds until after

Cleidocranial dysplasia (CCD) is inherited as an autosomal dominant disorder characterised by failure of membranous ossification. The condition is due to a mutation of the cbfa1 gene on chromosome 6 which has a role in the development of osteoblasts from the mesenchymal cells. In their growing years, these patients have an unusual shape of the femoral head reminiscent of a 'chef's hat'. In order to confirm the consistency of this sign, we have reviewed the radiographs of 28 patients with CCD. All except three had this appearance. The sign was also seen in patients with coxa vara associated wit

Seven patients (nine hips) who suffered from coxa vara were treated by distal transfer of the greater trochanter. The operation eliminated or markedly diminished the preoperative Trendelenburg sign, thus improving gait. In three patients (four hips) operated upon at the aged of 9 and 12 years, the initial coxa vara was converted into an almost normal valgus kemoral neck. There was one poor result due to technical failure.
