Dislocation of the mandibular condyle is one of several consequences of facial trauma that can be anticipated. The condylar neck is inherently weak and likely to fracture at the time of impact before dislocating into the middle cranial fossa. A review of the literature revealed that most cases of dislocation of the mandibular condyle into the middle cranial fossa are treated by open reduction and internal fixation via an extraoral approach or are treated conservatively with closed reduction. An intraoral approach is rare. Here we present a patient with traumatic dislocation of the mandibular c

BACKGROUND: Dislocation of the mandibular condyle into the middle cranial fossa is a rare injury that can be seen after facial trauma. Early identification of condyle dislocation into the middle cranial fossa was difficult until the development of computed tomography, and diagnosis was often significantly delayed after the initial trauma.
CASE DESCRIPTION: We present a rare case of a young woman who presents after a mechanical fall resulting in facial trauma with an avulsion condyle fracture with dislocation of the mandibular condyle into the middle cranial fossa.
CONCLUSIONS: To our knowled

INTRODUCTION: Tumor-induced osteomalacia is a paraneoplastic syndrome of hypophosphatemia. Osteomalacia causes multiple bone fractures and severe pain.
CASE PRESENTATION: We report the case of a 57-year-old Japanese man with tumor-induced osteomalacia associated with a middle cranial fossa bone tumor. The tumor was successfully resected by using a middle fossa epidural approach. His phosphate level recovered to a normal range immediately after the surgery.
CONCLUSIONS: It is rare that tumor-induced osteomalacia originates from the middle skull base. This report suggests that, if patients hav

A 6-year-old Japanese girl presented with psychomotor seizures. Magnetic resonance (MR) images disclosed a mass lesion in the left middle cranial fossa with an internal irregular-shaped area, which was hyperintense on T1-weighted images and hypointense on T2-weighted images. Gross total resection of the tumor was performed through left occipital craniotomy. The tumor was white-to-grayish hard tissue, focally showing blackish pigmentation. Histological, immunohistochemical and electron-microscopical analyses revealed that white-to-grayish hard tissue corresponded to an atypical meningioma, and

We report a case of an aneurysm rupturing into an arachnoid cyst presenting as a confined intracystic hemorrhage. A 37-year-old man with a history of sporadic headaches for several years complained of a headache of several days' duration. Brain computed tomography and magnetic resonance imaging revealed a cystic lesion in the left side of the middle cranial fossa that included a 3-cm-diameter hematoma in the Sylvian fissure without extracystic hemorrhage. Left carotid artery angiography showed an internal carotid artery-posterior communicating artery (IC-PC) aneurysm and an anterior choroidal

Dural arteriovenous malformations of the middle cranial fossa are very rare. Venous drainage flows either through superficial leptomeningeal veins or through the sphenoparietal, sphenopetrous and/or sphenobasilar sinuses. They often have an aggressive course and therefore poor outcome. It is essential to analyse and understand the angioarchitecture of the dural arteriovenous malformations in order to select and plan the correct treatments. We describe an exceptional case of intraventricular haemorrhage caused by the rupture of a dural arteriovenous malformation of the middle cranial fossa. To

The enlarged transtemporal middle fossa approach to the cerebello-pontine angle is adequate for complete removal of acoustic neuromas up to 4.0 cm in diameter (sizes B and C according to Fisch). A description of the surgical technique and the results of 63 operations are presented (13, 23 and 27 tumours of sizes A, B and C respectively). 51 tumours were completely removed, 6 had a voluntary palliative incomplete resection for medical reasons or on the only remaining hearing ear. Only 6 patients (one in the last 40 consecutive cases) had an incomplete resection due to the limited exposure of th

Among 181 inpatients suffering from pituitary adenoma, craniopharyngioma, or meningioma of the anterior or middle cranial fossa, 112 had ocular symptoms initially or during the course of the disease. Patients' histories were analyzed with regard to ophthalmological, neurological, and radiological diagnostic procedures carried out, and the conclusions drawn from these. Initial symptoms and signs were often misinterpreted and appropriate investigations consequently delayed. The mean duration of the history was 2.67 years (0-13 years) for the pituitary adenomas, 1.44 years (3 months-6 years) for

OBJECTIVES: Surgical treatments for Ménière's disease differ in efficacy. Endolymphatic duct blockage (EDB) is favored for its minimal risk and ability to preserve hearing. One of the main challenges in the technique is the difficulty in accurately identifying the endolymphatic duct (ED). The aim of the study is to see the feasibility of identifying the ED opening using a middle cranial fossa (MCF) dural plate as a reference point in a wet temporal bone and to measure the angle between the ED opening and the MCF dural plate which in the future can simplify the procedure of ED blockage.

The middle cranial fossa is one of the most complex regions in neurosurgery and otolaryngology-in fact, the practice of skull base surgery originated from the need to treat pathologies in this region. Additionally, great neurosurgeons of our present and past are remembered for their unique methods of treating diseases in the middle fossa. The following article reviews the surgical anatomy of the middle fossa. The review is divided into the anatomy of the bones, dura, vasculature, and nerves-in two parts. Emphasis is paid to their neurosurgical significance and applications in skull base surger

Traumatic intracranial intrusions of the mandibular condyle are a rare injury pattern that carries significant morbid outcomes due to potential for neurological, otological, temporomandibular joint damage, or joint ankylosis. Treatment modalities involve either closed reduction, open approaches via transcranial or subtemporal approaches, or delayed total joint replacement. This paper presents a clinical report of a traumatic intracranial intrusion of the mandibular condyle treated with an endoscopically assisted preauricular approach to reconstruct the middle cranial fossa and temporomandibula

UNLABELLED: The Black Lives Matter (BLM) movement has drawn attention to the lack of progress toward racial equity in many domains. Chief among them is the unequal treatment that Black men often face when interacting with law enforcement or within the criminal justice system, which appears heavily associated with the pernicious stereotype that Black men are distinctly aggressive and dangerous. Evidence suggests that Hispanic men are also subject to similar negative stereotypes. We contend that the consequences of this intersectional stereotype are wide-ranging and explore how it manifests and

BACKGROUND: Vidian nerve schwannoma is an extremely rare type of facial nerve schwannoma. To the best of our knowledge, only 1 case has been reported.
METHODS: We report an additional case of vidian nerve schwannoma with middle cranial fossa extension in a 49-year-old Japanese woman. The surgical approaches for infratemporal fossa schwannomas are reviewed, and the maxillary swing approach we used is described.
RESULTS: We adopted a maxillary swing approach combined with endonasal endoscopic techniques for the resection of the lesion. Gross total resection was achieved without sacrificing the

The outcome of 100 consecutive transtemporal supralabyrinthine vestibular neurectomies performed between 1982 and 1992 was analyzed retrospectively and compared with that of the first 100 operations (1969 to 1975) in order to analyze the effects of the learning curve on immediate postoperative complications. A total sensorineural hearing loss, which occurred in 2% of the cases initially, could be totally avoided by (1) opening the tegmen tympani for better identification of the superior ampulla and (2) preserving the saccular branch of the inferior division of the vestibular nerve when the ide

INTRODUCTION: Dislocation of the mandibular condyle into the middle cranial fossa after a trauma is a rare event. The lack of appropriate treatment can lead to ankylosis of the temporomandibular joint (TMJ). We report about a case of TMJ ankylosis following intracranial dislocation of the mandibular condyle through the roof of the articular fossa.
CASE REPORT: A 9-year-old patient was referred for a severe limitation of mouth opening that began progressively one year before. A history of chin injury due to an accidental fall was found. Preoperative CT scan showed a TMJ ankylosis on the right
