Extended Retrosigmoid Craniotomy for Resection of a Cochlear Schwannoma.

Cochlear schwannomas are a rare subset of schwannomas arising from the cochlear nerve within the cochlea or internal auditory canal.1-5 These tumors can present with posterior displacement of the facial nerve, making facial nerve preservation a more technically demanding goal during surgery.6-8 This video describes a woman in her early 50s who presented with right-sided complete hearing loss and was thought to have a vestibular schwannoma extending into the internal auditory canal. Institutional review board approval was not required for this case report. Patient consent was obtained. The pati

Awake Craniotomy for a Frontal Astrocytoma: A Case Report.

Awake craniotomy is a surgical procedure that has been gaining significance over the past decades. Neuronavigation is an intraoperative technology that locates tumors and monitors the brain cortex during awake craniotomy. The presence of cerebral low-grade gliomas in the frontal lobe creates a risk of affecting vital centers of the brain cortex during surgery. We present a clinical case of a 42-year-old male patient who entered the neurosurgery clinic with a clinical manifestation of headache for two months. MRI showed evidence of the recurrence of a left frontal glioma. Differential diagnoses

Supratentorial Craniotomies with or without Dural Closure-A Comparison.

BACKGROUND: Dural closure is a routine surgical step in neurosurgery. The benefit of suturing the dura to achieve watertight closure-with or without the use of dural substitutes-has been questioned in supratentorial craniotomy. We performed a retrospective study to examine the possible benefits and harms of suturing the dura compared with no dural closure and the occurrence of postoperative infection, cerebrospinal fluid (CSF) leak, and postcraniotomy headaches. METHODS: We performed a retrospective study to compare the incidence of CSF leak, infection, surgical site swelling, and postcraniot

Dexmedetomidine and Mannitol for Awake Craniotomy in a Pregnant Patient.

We describe the use of dexmedetomidine for an awake neurosurgical procedure in a pregnant patient and quantify the effect of mannitol on intrauterine volume. A 27-year-old woman underwent a craniotomy, with intraprocedural motor and speech mapping, at 20 weeks of gestation. Sedation was maintained with dexmedetomidine. Mannitol at 0.25 g/kg IV was administered to control brain volume during surgery. Internal uterine volume was estimated at 1092 cm before surgery and decreased to 770 and 953 cm at 9 and 48 hours, respectively, after baseline assessment. No adverse maternal or fetal effects were

Craniotomy with endoscopic assistance in the treatment of nasopharygeal fibroangioma.

BACKGROUND: Nasopharygeal fibroangioma (NPF) can be approached through lateral rhinotomy, the middle skull fossa approach and the transcranial-facial combined approach. It is complicated and thus results in more insults, and when adopted, the total resection rate of tumor is still low. The nasal endoscope is minimally invasive, the dead angles of a craniotomy, such as sphenoid sinus, maxillary sinus, and nasopharynx are easily approached by an endoscope. Lateral rhinotomy have to make facial incision and affects maxillary bone development. We combined the craniotomy and endoscopic approach int

Anesthesia for craniotomy in a patient with previous paralytic polio.

Poliomyelitis remains endemic in many developing nations. Patients may develop residual muscle weakness in one or more limbs after an attack of poliomyelitis in childhood. We report an adult patient who presented for right temporal cortical grid placement. He had childhood poliomyelitis and, while showing no evidence of postpolio syndrome, demonstrated excessive sensitivity to nondepolarizing muscle relaxants and developed prolonged muscle weakness during the postoperative period.

[Supraorbital craniotomy to approach the sellar and the parasellar regions].

We report on the experience with supraorbital minicraniotomy in a series of 36 operated cases with good results: 31 aneurysms, 4 pituitary adenomas, 1 anaplastic astrocytoma and 1 arachnoid cyst. One death occurred in this series. The technique is detailed. We conclude that the minicraniotomy is a safe method for the treatment of several parasellar pathologies. The efficacy must be evaluated in clinical trials concerning other approaches.

Morbidity and mortality of craniotomy for excision of supratentorial gliomas.

Extensive surgical resection of supratentorial gliomas increases survival. However, some reports suggest that the perioperative morbidity and mortality outweigh the potential benefit of the procedure. We examined prospectively morbidity and mortality in 104 consecutive patients who underwent surgery for supratentorial glioma, as well as other factors that might affect the short-term outcome. To determine if our experience was unusual, we compared these results with those obtained from another academic neurosurgical center by a review of the records of 109 patients also treated surgically for s

Onabotulinumtoxina (craniotomy scar combined with cranial suture line injections) for persistent post craniotomy headache: Case series with long-term follow-up.

BACKGROUND: There is no defined preventive treatment protocol for persistent post-craniotomy headache. In several small case series and individual case reports onabotulinumtoxinA injected into the craniotomy scar has shown possible efficacy. What is lacking is long term follow-up and if focusing on the cranial suture lines along with the craniotomy scar can enhance improvement and provide more sustained benefit. METHODS: Retrospective chart review with case series. RESULTS: Four patients (three women, one man) with ICHD-3 defined persistent post craniotomy headache were treated using a novel

Awake craniotomy versus craniotomy under general anesthesia without surgery adjuncts for supratentorial glioblastoma in eloquent areas: a retrospective matched case-control study.

BACKGROUND: Awake craniotomy with electrocortical and subcortical mapping (AC) has become the mainstay of surgical treatment of supratentorial low-grade gliomas in eloquent areas, but not as much for glioblastomas. OBJECTIVE: This retrospective controlled-matched study aims to determine whether AC increases gross total resections (GTR) and decreases neurological morbidity in glioblastoma patients as compared to resection under general anesthesia (GA, conventional). METHODS: Thirty-seven patients with glioblastoma undergoing AC were 1:3 controlled-matched with 111 patients undergoing GA for g

Post-Craniotomy Headache: Current Concepts and Emerging Procedural Therapies.

PURPOSE OF REVIEW: Post-craniotomy headache (PCH) is a common complication following neurosurgery, yet there remains limited consensus regarding the mechanism, classification, and optimal management. This review summarizes the current literature and highlights recent trends in procedural therapies. RECENT FINDINGS: PCH occurs in approximately 60-70% of patients following craniotomy, with nearly 25% progression to persistent PCH. Identified risk factors include younger age, female sex, pre-existing headache disorders, and longer operative duration. Some studies further characterize PCH into mi

Effect of Staphylococcal Decolonization Regime on Post-Craniotomy Meningitis.

Post craniotomy meningitis (PCM), an uncommon complication following craniotomy can be categorized as either bacterial meningitis (BM) or aseptic meningitis (AM) based on the results of CSF culture. Staph. aureus is a common causative organism. Some patients who are nasal carriers of these organisms have been shown to be at a higher risk of acquiring surgical site infections (SSI) following general or gynecological surgeries. Staphylococcal decolonization regime (SDR), using chlorhexidine gluonate (CHG) showers and application of mupirocin ointment to the anterior nares, is an attempt to reduc

Dexmedetomidine for awake craniotomy: Systematic review and meta-analysis.

INTRODUCTION: For awake craniotomy, monitored anaesthesia care (MAC) had shown relatively lower failure rates. Nevertheless, the conclusion of the appropriate anaesthetic agents, and complications, has not been proposed. Therefore, the systematic review and meta-analysis was done to compare the clinical profile, surgical outcomes, and anesthesia-related complications between dexmedetomidine-based and non-dexmedetomidine regimens during monitored anesthesia care (MAC) for this procedure. EVIDENCE ACQUISITION: Published clinical trials described MAC, including the amount of anaesthetic drugs, o

Enhanced Recovery After Craniotomy: Global Practices, Challenges, and Perspectives.

The global demand for hospital care, driven by population growth and medical advances, emphasizes the importance of optimized resource management. Enhanced Recovery After Surgery (ERAS) protocols aim to expedite patient recovery and reduce health care costs without compromising patient safety or satisfaction. Its principles have been adopted in various surgical specialties but have not fully encompassed all areas of neurosurgery, including craniotomy. ERAS for craniotomy has been shown to reduce the length of hospital stay and costs without increasing complications. ERAS protocols may also red

Intraoperative Challenge: Managing Venous Air Embolism During Sitting Craniotomy.

Venous air embolism (VAE) represents a rare yet potentially life-threatening complication encountered during neurosurgical procedures, particularly craniotomy. Here, we present a case of a 30-year-old male undergoing excision of a cerebellar abscess who developed VAE midway through the procedure. Immediate recognition and intervention were paramount in managing the embolism effectively, ensuring a favorable surgical outcome. Vigilant monitoring, prompt cessation of the procedure, and implementation of preventive measures such as oxygen therapy and venous air aspiration were pivotal in mitigati
