Computed tomographic laryngotracheal dimensions in adult domestic rabbits (Oryctolagus cuniculus) are positively associated with body weight and the laryngotracheal lumen is narrowest at the level of the thyroid cartilage.

OBJECTIVE: This retrospective study aimed to measure rabbit laryngotracheal dimensions at different locations on computed tomography (CT), assess the relationship of these measurements with rabbit body weight, determine the most common narrowest measurement and assess its relationship with endotracheal tube (ETT) size and body weight. ANIMALS: 66 adult domestic rabbits (Oryctolagus cuniculus) of different breeds and body weights. PROCEDURES: CT laryngotracheal luminal height, width, and cross-sectional area measurements were made at the rostral thyroid cartilage at the level of the arytenoid

Morphology and computed tomography of the larynx in the white-eared opossum (Didelphis albiventris).

The white-eared opossum (Didelphis albiventris) is a Neotropical marsupial that occurs in the Brazilian territory. The larynx is an important organ of vocalization in mammals, although, other laryngeal functions are more fundamental for survival of mammals than phonation. The anatomical knowledge of respiratory structures is pivotal for a better understanding of the species. Thus, this study aimed to examine the larynx of the white-eared opossum by gross anatomy, computed tomography and histological description. For this, 10 adult (six females and four males) white-eared opossums (D. albiventr

Chronic subglottic stenosis.

Severe subglottic stenosis or stenosis that persists after a number of repairs have failed can be treated by an operation to widen the entire cricoid. This is accomplished by splitting the cricoid arch anteriorly and the posterior cricoid lamina posteriorly, both in the vertical midline. These vertical splits are held apart by using thyroid laminae cartilage grafts with periosteum intact on both sides. The posterior cricoid lamina split is covered by using muscle fascia or buccal mucous membrane. The subglottis is stented with a silicone conforming laryngeal stent or a silicone tracheal T tube

Autologous cricoid graft reconstruction of complete transglottic laryngeal atresia: Case report.

INTRODUCTION: Improved survival in congenital high airway obstruction syndrome (CHAOS) has shifted management toward definitive airway reconstruction and functional airway independence. However, severe cartilaginous laryngeal atresia may distort normal laryngotracheal anatomy and limit conventional reconstructive approaches. We describe an anatomy-specific reconstruction using obstructing dysplastic cricoid cartilage as an autologous posterior graft. CLINICAL FINDINGS: A male infant with CHAOS underwent EXIT-to-tracheostomy at 32 weeks' gestation. At age three, airway evaluation demonstrated

Does the cricoid resection increase the risk of complications in the surgical treatment of benign laryngotracheal stenoses?

PURPOSE: The most of post-intubation stenoses occurs in the cricotracheal segment and require open surgery. Resection of the cricoid arch is a part of the surgical technique that carries certain difficulties. Resection of the cricoid arch, sometimes also the posterior cricoids plate, is part of a surgical technique that carries potential risks of complications, due to the proximity of the recurrent nerve and the need to preserve the airway lumen, in which the cricoids cartilage plays a central role. Purpose of this study is to evaluate whether resection of the cricoid cartilage is associated w

Laryngeal chondroma: a benign process with long-term clinical implications.

Chondroma of the laryngeal cartilage is a rare, benign neoplasm which can manifest as a neck mass or, if situated within the airway, as slowly progressive obstruction, hoarseness or dyspnea. The most common location for chondroma is the posterior lamina of the cricoid cartilage; the next most common locations are the thyroid, arytenoid and epiglottic cartilages. Chondroma and low-grade chondrosarcoma are difficult to distinguish from one another histologically. Although chondrosarcoma reportedly recurs, local surgical excision without radical margins and with long-term clinical follow-up is re

Radiographic Evaluation of the Reliability of Neck Anatomic Structures as Anterior Cervical Surgical Landmarks.

BACKGROUND: Accurate location of the skin incision is helpful to decrease the technical difficulty and save the operative time in anterior cervical spine surgery. Spine surgeons usually use the traditional neck anatomic structures (the hyoid bone, thyroid cartilage, and cricoid cartilage) as landmarks during the surgery. However, the reliability of these landmarks has not been validated in actual practice. OBJECTIVE: To find out which landmark is the most accurate for identifying the cervical levels in anterior cervical spine surgery. METHODS: The lateral flexion and extension radiographs of

High Tracheal Resection With Intralaryngeal Extension as an Alternative to Cricotracheal Resection for Treatment of Subglottic Stenosis.

OBJECTIVE: Some patients with subglottic stenosis (SGS) require open airway reconstruction, which traditionally involves resection of the anterior cricoid cartilage. As an alternative, we present a novel technique: cricoid-sparing high tracheal resection with excision of subglottic stenotic tissue from below. A novel set of posterior circumferential cricoid sutures is used to reline the exposed cricoid plate. STUDY DESIGN: Retrospective chart review. SETTING: Single tertiary care center. METHODS: The surgical technique is described and illustrated. A chart review was performed for all patie

Optimal Arytenoid Position in Laryngeal Framework Surgery: An Anatomic Human Larynx Study.

OBJECTIVES: The purpose of this study was to better understand the effects of stitch placement on arytenoid medialization by measuring normative cricoarytenoid joint anatomy and changes in arytenoid position when varying arytenopexy stitch configuration. METHODS: This adult human larynx study was done in two parts. First, measurements of the cricoid and arytenoid cartilage anatomy relevant to cricoarytenoid joint function were made in 45 preserved larynges (26 male (M), 19 female (F)) using digital calipers. Second, the arytenoids of six fresh larynges ( three M, three F) were sutured to the

Subglottic region: normal topography and possible clinical implications.

A precise definition of clinically important laryngeal regions and compartments is still a matter of controversy. The often neglected subglottic area was reinvestigated in plastinated serial sections of 21 human adult specimens. The superior border of the subglottic region is defined by the transition of squamous stratified epithelium covering the vocal cords into the respiratory epithelium of the caudal airways. It is found at the inferior margin of the vocal fold at variable distance from the free edge of the vocal cord and bears a high risk of carcinogenesis. The anterior border of the subg

Endoscopic laryngotracheoplasty and graft soldering with the carbon dioxide laser. An animal study.

Laryngotracheoplasty (LTP) has had considerable success for treatment of selected cases of subglottic stenosis in children. However, certain complications have been associated with an open approach. Our study explores the possibility of decreasing these complications by means of a minimally traumatic endoscopic laser cricoid split and graft transplantation technique. In an in vivo study of six dogs, LTP was performed with a carbon dioxide laser. The gap between the divided cricoid cartilage was either left with no coverage or filled by autogenous mucosa or cartilage grafts through a laser-assi

Cricoid Force: Anatomic, Physiologic, and Clinical Concepts.

The application of cricoid force remains controversial in modern practice. This review critically assesses the anatomic, physiologic, and contemporary clinical evidence of cricoid force application. There may be a sound anatomic basis to cricoid force application, involving occlusion of the postcricoid hypopharynx, but the physiologic basis is uncertain. Clinical evidence to date has not reliably determined efficacy of cricoid force application, largely because of inconsistent localization and application. Modern evidence suggests no clinically relevant implications to successful intubation wi

Surgical management of an infantile elliptical cricoid: Endoscopic posterior laryngotracheoplasty utilizing a resorbable plate.

The cricoid is a circular "ring" of cartilage in the airway. When the lateral walls of the cricoid approximate, it takes the shape of an ellipse. In severe cases, this also reduces the glottic aperture and causes respiratory distress, stridor, and failure to thrive. The elliptical cricoid has limited surgical options outside of open laryngotracheal procedures and tracheostomy. Recently, alternatives to autologous grafts have been utilized in airway reconstruction to reduce harvest site morbidity and increase operating room efficiency. Herein a case is presented that demonstrates the successful

Analysis of the functionally-narrowest portion of the pediatric upper airway in sedated children.

The narrowest portions of the pediatric larynx are the glottis and subglottic region. However, the pliable and paralyzed subglottic region, acting like a curtain, is no resistance when passing an endotracheal tube. Therefore, the 'functionally' portion of the pediatric upper airway, which may be the most vulnerable to damage during intubation, is the unyielding portion below the cricoid cartilage. We investigated the functionally-narrowest portion below the cricoid cartilage.Computed tomography (CT) was performed under deep sedation. CT images were used for measurement of dimensions and cross-

Cartilage invasion patterns in laryngeal cancer.

The cartilaginous invasion determines the T and is one of the most common sources of mistake in tumor staging. Also it is of great importance when planning any therapeutic alternative. In the latest revision of the TNM classification a clear distinction is made between infiltration of cartilage without going through it, considered a T3 recently and that would be a T4 according to the previous classification, and those going through the cartilage, classified as T4a. While this classification makes the difference in depth of infiltration, it does not emphasize the extent of invasion. This paper

Endoscopic holmium laser laryngotracheoplasty in animal models.

Subglottic stenosis remains a difficult clinical problem with varied management approaches. An accepted procedure has been anterior and posterior cricoid incisions through an external approach for treatment of severe stenoses without a tracheotomy. The holmium:yttrium-aluminum-garnet laser, 2.1 microns wavelength with a pulsed output, is transmissible through standard fibers and ablates soft tissue and cartilage with minimal surrounding damage. This study in in vitro and in vivo animal models shows that this new laser can be used to incise the anterior and posterior cricoid and tracheal cartil

Assessment of cartilage invasion in case of laryngeal cancer by means of longitudinal sectioning for histopathology - Clinical implications.

AIM: The aim of the study was to assess the accuracy of radiological diagnosis of laryngeal cartilage infiltration by histopathological examination of laryngeal specimen after total laryngectomy. BACKGROUND: Despite the development of new medical technologies and significant clinical advances allowing early diagnosis and treatment of laryngeal cancer, mortality is still on the rise. Neoplastic infiltration of the laryngeal cartilages is the most common source of error in the assessment of cancer staging. Furthermore, cartilage invasion is listed as a contraindication to partial surgical techn

Differential effects of endolaryngeal trauma upon the growth of the subglottis.

In young rabbits (4-week-old) the growth of the subglottis was studied after an endolaryngeal soft tissue trauma. The injury induced the formation of a subglottic stenosis. In contrast to earlier observations on endolaryngeal trauma extending into the cricoid cartilage, the cricoid developed normally to the adult size and shape at 24 weeks. Similar to the earlier findings, the subepithelial layer showed considerable thickening as the result of formation of scar tissue, ectopic cartilage and fatty tissue with interruption of the elastic tunica (= conus elasticus). It was concluded that after en

Cricoid regeneration using in situ tissue engineering in canine larynx for the treatment of subglottic stenosis.

The purpose of the present study was to evaluate the efficacy of cricoid regeneration via in situ tissue engineering in a canine larynx for the treatment of subglottic stenosis. As the tissue scaffold, a Marlex mesh tube coated by collagen sponge was used for a rigid airway framework and for tissue regrowth around the tube. On 5 dogs, the larynx was exposed and the anterior third of the cricoid cartilage was resected. The tube was anastomosed to the lower edge of the thyroid cartilage and to the first tracheal cartilage. By postoperative endoscopic examination at 3 to 7 months, no airway obstr
