Fibrinolysis in critically ill patients.

Impaired fibrinolysis may contribute to development of adult respiratory distress syndrome (ARDS). Pathologic increases in endogenous plasminogen activator inhibitor (PAI-1) may blunt normal fibrinolysis and unmask alternate fibrinolytic mechanisms, such as elastase-induced fibrin degradation. We measured PAI-1 and elastase-induced fibrin(ogen) degradation products in 69 critically ill patients in our medical intensive care unit (MICU) and in nine healthy volunteers. Factor VIII-related antigen protein (VIII:Ag), a reported marker of acute lung injury, and alpha-1-protease inhibitor (alpha-1-P

The fluid problem in the critically ill.

Fluid therapy in the critically ill patient must be adjusted to accommodate continuing changes in the plasma volume, interstitial space, and intracellular space. During and after hemorrhagic shock, replacement of crystalloid is needed to replenish the plasma and interstitial spaces during operation and then interstitial and intracellular spaces after operation. Severe sepsis leads to a more pronounced expansion of the interstitial space than that of hemorrhagic shock. Continuing therapy after both hemorrhage and sepsis should be directed toward maintaining effective plasma volume and levels of

Hypocalcemia in critically ill patients.

Hypocalcemia is an important metabolic problem in critical care medicine. To determine the frequency of this problem and the patient subsets at risk, a retrospective study of a large series of ICU patients was performed. During the study period, 259 patients were admitted to the ICU, of whom 210 (81%) had a serum calcium (Ca++) measured. Of these 210 patients, 135 (64%) were hypocalcemic (serum Ca++ less than 8.5 mg/dl) and 75 (36%) were normocalcemic. Serum albumin concentration was less than 3.5 g/dl in 70% of the hypocalcemic patients who hd albumin measured, suggesting that the ionized Ca+

Hypertensive crisis in the critically ill.

Hypertensive crises encompass a spectrum of clinical presentations ranging from severe asymptomatic blood pressure (BP) elevation to life-threatening hypertensive emergencies and represent a frequent and challenging situation in the critically ill. While hypertensive urgency, defined as a marked elevation in blood pressure without acute organ damage, can generally be managed in the outpatient setting, hypertensive emergencies require immediate in-hospital management. Hypertensive emergencies are defined by a BP ≥180/110 mmHg associated with acute target-organ damage. Malignant hypertension rep

Sedation and analgesia in the critically ill.

Pain management and sedation for critically ill patients remain areas of active interest and debate among intensivists. We review the foundational literature behind practices now considered the standard of care, including a systematic approach to target light sedation as well as the avoidance of benzodiazepine use. These approaches facilitate earlier liberation from mechanical ventilation, shorten intensive care unit length of stay, and reduce delirium. We also discuss the relative merits of dexmedetomidine versus propofol, analgesia-first sedation protocols, emerging medications and areas for

Constipation in critically ill adults.

Constipation is a common yet underrecognized gastrointestinal complication among critically ill adults, significantly impacting morbidity, length of stay, and overall prognosis. This narrative review explores the current understanding of constipation in the critical care setting, emphasizing the challenges in its definition and identification due to variability in clinical presentation and lack of standardized diagnostic criteria. We examine contributing factors such as immobility, opioid use, altered fluid and electrolyte balance, and the effects of critical illness itself on gastrointestinal

Magnesium in the Critically Ill Patient.

Magnesium is an electrolyte essential for life and plays a crucial role in several biochemical and physiologic processes in the body. Dysmagnesemia, defined as the presence of either hypomagnesemia or hypermagnesemia, is common in critically ill patients and is associated with prolonged hospitalization and increased mortality. This article reviews: (1) magnesium homeostasis, the physiologic roles of magnesium in the body and measurement of magnesium; (2) causes of dysmagnesemia in the critically ill patient; (3) the clinical and biochemical manifestations of dysmagnesemia; and (4) treatment of

Renin in critically ill patients.

The renin-angiotensin system (RAS) constitutes one of the principal mechanisms to maintain hemodynamic and fluid homeostasis. However, most research until now on RAS primarily focuses on its relationship with hypertension and its role in critically ill hypotensive populations is not well understood. With the approval of angiotensin II (Ang II) in the United States and Europe, following a phase 3 randomized controlled trial showing efficacy in catecholamine-resistant vasodilatory shock, there is growing interest in RAS in critically ill patients. Among the fundamental components of RAS, renin a

Neuromonitoring in Critically Ill Patients.

OBJECTIVES: Critically ill patients are at high risk of acute brain injury. Bedside multimodality neuromonitoring techniques can provide a direct assessment of physiologic interactions between systemic derangements and intracranial processes and offer the potential for early detection of neurologic deterioration before clinically manifest signs occur. Neuromonitoring provides measurable parameters of new or evolving brain injury that can be used as a target for investigating various therapeutic interventions, monitoring treatment responses, and testing clinical paradigms that could reduce seco

Immunoparalysis in critically ill children.

Immunoparalysis is associated with poorer outcomes in the paediatric intensive care unit (PICU) setting. We aimed to determine the group of patients with higher chances of immunoparalysis and correlate this status with increased risks of nosocomial infection and adverse clinical parameters. We conducted an exploratory study with prospective data collection in a university-affiliated tertiary medical, surgical, and cardiac PICU. Fifteen patients with multiple organ dysfunction syndrome were included over a period of 6 months. Monocyte's human leucocyte antigen (HLA)-DR expression and tumour nec

Placenta in the Critically Ill Mother.

UNLABELLED: The placenta is a temporary, multifunctional organ composed of both maternal and fetal components. It maintains homeostasis to ensure the growth of the fetus and well-being of the mother. Abnormalities in placental development have been known to be responsible for several disorders of pregnancy. Conditions coincident with pregnancy can upset the homeostasis and result in critical illness, which can greatly impact placental function and in turn affect the fetus. Decreased blood flow, acidemia, hypercarbia, and hypoxia seen in critically ill pregnant mothers can result in fetal death

Hyperferritinemia in Critically Ill Patients.

OBJECTIVE: Hyperferritinemia is frequently seen in critically ill patients. A rather rare though life-threatening condition related to severely elevated ferritin is hemophagocytic lymphohistiocytosis. We analyze ferritin levels to differentiate hemophagocytic lymphohistiocytosis from other causes of hyperferritinemia in a mixed cohort of critically ill patients. DESIGN: Retrospective observational study. SETTING: Adult surgical, anesthesiologic, and medical ICUs of a university hospital. PATIENTS: Critical care patients (≥ 18 yr old) admitted to any of the adult ICUs at Charité - Universitä

Delirium in the critically ill patient.

Delirium is an acute and transient brain dysfunction that is characterized by disturbances in consciousness, affecting both its content (i.e., attention) and level (i.e., arousal). It affects as many as 50% of those admitted to an intensive care unit (ICU). Once believed to be an inconsequential outcome of critical illness, it is now recognized that delirium is harmful in both the short- and long-term. Despite occurring frequently in critically ill patients, delirium often goes unrecognized. Well-validated delirium screening tools, designed for use in the ICU, should be used to reliably detect

The microbiome of the critically ill patient.

PURPOSE OF REVIEW: Advances in the understanding of the human microbiome outside of the ICU have led investigators to consider the role of the microbiome in critical illness. The picture that is being elucidated is one of dysbiosis occurring at multiple sites in the critically ill patient. This review describes the changes that occur in the various microbiomes of a critically ill patient, the implications of these changes and shows how advances in the understanding of dysbiosis may lead to microbiome-targeted therapies. RECENT FINDINGS: Critically ill patients undergo dysbiosis at several org

Encephalopathy in critically ill patients.

Potentially reversible encephalopathies do add to the morbidity and mortality in critically ill patients. They are commonly seen in patients above 50 years between the 6th to 20th day of illness. Wernicke's encephalopathy, osmotic demyelination, hypoglycemia and other less common conditions like non-convulsive status, cerebral venous thrombosis and acute disseminated encephalomyelitis should be looked for in the appropriate setting as to facilitate early treatment.

Shock in the critically ill neonate.

Shock is a clinical disorder that challenges caregivers in the neonatal intensive care unit. Critically ill neonates may develop shock due to a variety of causes but the predominant cause of shock in neonates is sepsis. This article provides the neonatal nurse with basic knowledge of the pathophysiology and the types of shock seen in the critically ill neonate. Treatment and supportive care of the neonate in shock is determined by the underlying cause of shock with the ultimate goal of treatment being adequate perfusion of tissues to deliver oxygen to the cells and remove metabolic waste produ
