Bilateral cleft lip and palate: improved maxillary and dental development.

BACKGROUND: Maxillary growth deficiency is still a problem in children with repaired cleft lip-cleft palate, and little progress has been made on research for its prevention. An increase in the early diagnosis of maxillary growth deficiency prompted this study to obtain an early indication of the efficacy of a new protocol for better dental and maxillary development. METHODS: A new method for repair of the hard palate in infants with bilateral cleft lip-cleft palate that made innovative use of mucosa of the nasal septum and involved less extensive surgery was coupled with the repair of the li

[Deep bite correction in adult patients].

Excessive occlusal force caused by deep bite is an etiology of alveolar bone resorption as well as premature loss of permanent teeth. Deep bite correction in adult patients should comprise of intrusion and/or proclination of anterior teeth in order to prevent skeletal alteration and relapse. The objective of the article was to present indication and mechanics of intrusion and proclination of anterior teeth with a case report of the patient who had a problem of masticatory system especially temporomandibular joint, cuspid protection system and deep bite.

Presenting novel 2D echocardiographic views for pulmonary valve assessment in tetralogy of fallot patients.

BACKGROUND: Tetralogy of Fallot (TOF) is considered one of the most common surgically repaired congenital heart conditions. Continuous improvements in surgical techniques have aimed at preserving the pulmonary valve (PV). Therefore, evaluating PV morphology and the Right Ventricular Outflow Tract (RVOT) is clinically significant as it is enlightening in selecting therapeutic strategies. This study aims to clarify the practical use of 2D transthoracic echocardiography (TTE) in detecting 2-leaflet valves versus 3-leaflet valves and compare the assessment of PV by 2D TTE with surgical assessment.

[Alveolar bone necrosis as a consequence of surgical intervention for hypophyseal adenoma].

Tumours of the pituitary gland, originating from its anterior lobe, may produce hormones and therefore generate a situation of pituitary hyperfunction or cause the typical problems of endocranial expansive masses which compress residual glandular tissue. Surgery is necessary in order to remove a pituitary tumour and is currently performed in 96% of cases using a trans-sphenoidal route. The surgical approach entails making an incision in the upper vestibular fornix from cuspid to cuspid. The stripping of a full-thickness flap allows the front wall of the sphenoid sinus to be reached, passing th

Cystic management in a 10-year old child.

Cysts are among the most common lesions found in the oral cavity. These may be developmental in origin or secondary to chronic infections. Untreated, cysts may expand causing local tissue destruction and deformities. Basically, two treatment methods are employed. Enucleation and/or Marsupialization. The following case involves a child treated at the College of Dentistry, University of the Philippines, for a large cystic lesion involving the permanent cuspid and the deciduous dentition. The lesion was accidentally discovered on a panoramic radiograph. The child underwent extraction of deciduous

An alternative treatment for the severely resorped maxillary lateral incisor: a sequela of ectopic eruption.

At times dental crowding, maxillary arch inadequacy, or an abnormal lingual or horizontal position restricts the eruption of the maxillary cuspid. This ectopic eruption can cause the canine to compress the blood supply to the periodontal tissues of the adjacent lateral incisor. If undiagnosed and untreated, physiological and chemical changes are induced, and the lateral root undergoes resorption. When the resorption is severe enough to endanger the retention of the lateral, an endeosseous stabilizer can be placed through the root and extended into the bone to stabilize the tooth artificially.

[Influence of functional therapy on the gingival recession in patients with wedge shaped defects. A 2.5-year longitudinal study of the etiology of periodontal atrophy].

In 20 patients a free way space of greater than or equal to 1.0 mm and a cuspid protected guidance was formed. A previous study has shown, that prematurity and defective contacts reduce the free way space step by step and induce an artificial clenching. The front teeth was relieved in intercuspal position. The increase sof gingival recessions at the upper jaw was over five times significant slighter in treated patients than one in the 22 men of the control group. There was no significant, differences in the lower jaw. The recessions are represented as a sequence of a chronic biomechanical over

[Acute dysfunction of mitral bioprosthesis: Doppler expression of a musical holosystolic murmur. Apropos of 2 cases].

Two women who had been fitted with a porcine mitral valve seven years previously suddenly developed acute dysfunction of the bioprosthesis with regurgitation. In both patients physical examination revealed an intense, vibrating, musical holosystolic murmur sounding like a "goose cry" and located at the apex. Pulsed doppler showed major, jet-like, central orothetic mitral regurgitation. A harmonic graph was recorded when the doppler window was positioned upstream of the porcine valve leaflets. The finding of such a doppler signal in this clinical context suggests tearing or perforation of one c

Ectopic eruption of transposed mandibular permanent lateral incisors beneath primary first molars. Case report.

Mandibular permanent lateral incisors showing bilateral transposition and ectopic eruption were seen beneath primary first molars on bitewing radiographs in a female aged 9 years. This report chronicles management of the case during the mixed dentition period. Appropriate space maintenance after extraction of mandibular primary first molars, cuspids and lateral incisors on both sides facilitated eruption and repositioning of the permanent lateral incisors without the need to sever supracrestal gingival fibres.

A multi-disciplinary approach to congenitally missing anterior teeth.

Congenitally missing teeth are not an unusual occurrence. What makes this case study unique is the combination of restorative challenges offered. The patient presented with the permanent upper right cuspid and lower anterior right and left central incisors congenitally missing, and with an underdeveloped upper right lateral incisor. Distinctive existing occlusion and tooth alignment difficulties precluded a simple approach of merely extracting the residual primary teeth and replacing the missing teeth prosthetically. Using orthodontics to create ideal interocclusal and interarch space was crit

Numerical abnormalities of permanent dentition - a case report.

Tooth number abnormalities may occur under the influence of genetic or environmental factors which intervene in the tooth formation stages (induction and proliferation), most commonly the permanent dentition being affected. The result is the appearance of dental anomalies with numerical deficiency (hypodontia, anodontia), or with dental excess (hyperodontia, over-teeth). In this paper we report a case of a 15-year-old boy with associated abnormalities of permanent dentition: mesiodens and palatal impacted cuspid at the maxillary dental arch and incisor anodontia at the mandibular dental arch.

[Craniofacial characteristics of the Class II division 1 malocclusion patients with mouth-breating: a posteroanterior].

OBJECTIVE: To evaluate the craniofacial characteristics of the Class II malocclusion patients with mouth-breating by posteroanterior cephalometry. METHODS: To measure craniofacial width of the 12 Class II malocclusion patients with mouth-breathing, and to compared these measures with corresponding measures in a group of normal children. RESULTS: The width of the maxillary base bone (J-J) was less than that in normal children significantly (P < 0.01). The mouth-breathing children's upper and lower arch width (at first molar and cuspid) were comparatively narrower, and lateronasal width (Lap-L

Transmigration of a maxillary canine. A case report.

In the oral cavity, transmigration is defined as a tooth that crosses the mid-line by more than half its length. Following extensive literature review, it was found that, although well documented with respect to mandibular canines, there were only two publications detailing maxillary cuspid transmigration. This report presents a case of transmigration where the left maxillary canine gradually crossed the mid-line and migrated to the right side in a patient with hemifacial microsomia and cleft palate. The Mupparapu classification of the migratory pattern of the mandibular canines is discussed.

Secondary alveolar bone grafting--five-year periodontal and radiographic evaluation in 100 consecutive cases.

A prospective study of secondary alveolar bone grafting based on the Norwegian (Oslo) team approach has been conducted by the CLP Unit at the Princess Margaret Hospital for Children in Perth, Australia, since 1982. The 5-year results of 100 consecutive cases of uni- and bilateral cases are presented according to periodontal and radiographic parameters of tooth support. The vast majority of cases grafted (98%) resulted in the eventual eruption of the cleft side cuspid/lateral incisor within acceptable parameters of tooth support. The results confirm the predictability of this safe procedure whe

Fiber-reinforced composite bridge and splint. Replacing congenitally missing teeth.

The congenital absence of permanent teeth, also known as partial anodontia, is a relatively common problem. The patient missing one or both lateral incisors is a frequent occurrence. The treatment of missing laterals involves both functional and esthetic considerations. Options include: cuspid lateralization; implant placement; conventional three-unit bridge; Maryland bridge; and a fiber-reinforced composite bridge (direct or indirect). Direct fiber-reinforced composite bridges provide a minimally invasive restorative option that allows for future implant placement. Selection of appropriate fi

Direct bonding on impacted teeth.

In eighteen patients twenty-three impacted teeth were surgically exposed and provided with Cuspid-Pull Brackets by the direct-bonding technique. Light orthodontic traction was applied after 10 minutes. Only three brackets were lost during the experiment. The experiment showed that sufficient bonding could be established on impacted teeth without pumicing the tooth surface and that phosphoric acid in minimal amounts causes no harm to the adjacent tissues but assists in preventing bleeding from the surrounding tissues. This made it possible to obtain the necessary dryness of the exposed tooth su

Resorption of the lateral maxillary incisor: assessment by CT.

Computed tomography of the maxillary alveolar arch was performed in three children with delayed eruption of one or both maxillary cuspids. Computed tomography demonstrated close contact between the ectopically localized and nonerupted canine and the lateral incisor. The lesion was unilateral in two patients and bilateral in one. In two patients resorption of the lateral incisor root was demonstrated. Moreover, CT permitted exact determination of the extent of the lesion by differentiating between superficial cementodentine resorption limited to the cementodentine (superficial resorption) and r
