OBJECTIVE: Midazolam is a relatively short-acting water-soluble benzodiazepine that provides anxiolysis and anterograde amnesia and can be given orally with few adverse effects. We evaluated the benefit and safety of oral midazolam for sedation of young children during voiding cystourethrography or nuclear cystography.
SUBJECTS AND METHODS: For 3.5 years, a highly selected group of 98 children, ages 23 months to 9 years (mean, 4 years), were given oral midazolam 0.6 mg/kg 20-30 min before cystourethrography or nuclear cystography. These children either had been frightened by a previous cathet

RATIONALE: Spontaneous rupture of the urinary bladder (SRUB) is a rare, easily missed cause of urinary ascites that mimics intrinsic renal failure through peritoneal reabsorption of urinary solutes ("reverse peritoneal dialysis"). Rapid recognition prevents unnecessary dialysis and life-threatening complications. This case underscores key diagnostic pointers (especially the ascites/serum creatinine (SCr) ratio and the decisive role of computed tomography [CT] cystography).
PATIENT CONCERNS: A previously healthy 45-year-old man presented 5 days after heavy alcohol intake with severe lower-abdo

A 68-year-old woman presented with a rare huge right occipital cystic lesion manifesting as progressive left hemiplegia. Cranial computed tomography revealed a 4 x 7 cm right occipital lobe thin-walled cyst with midline shift and compression of the ipsilateral ventricle. The patient underwent a single burr hole operation for cystography and delineation of the cyst anatomy, then a separate right parieto-occipital craniotomy with complete cyst evacuation, corticotomy, and ventriculostomy. The presumptive diagnosis was arachnoid cyst. The symptoms had completely resolved by 4 months after surgery

A case of ectopic ureterocele is reported. A 2-year-old girl was admitted for recurrent episode of urinary tract infection associated with fever. IVP showed bilateral duplex kidney, although no excretion of dye was noted from the right upper kidney. IVP also revealed a large filling defect at the bladder neck which was diagnosed as ureterocele by cystoscopy. Cystography demonstrated VUR to the right lower kidney. A complete duplication of ureter with ectopic ureterocele on the right, and incomplete duplication of ureter on the left were found at operation. She underwent complete removal of the

The role of a cystogram to assess the vesico-urethral anastomosis (VUA) after robot-assisted laparoscopic radical prostatectomy (RARP) has been debated. Early catheter removal without cystogram was reported to be associated with a trend towards an increased risk of acute urinary retention (AUR). In two cohorts we studied the effects of VUA leakage on cystogram and functional outcome after RARP. Cohort A contained 1390 consecutive men that routinely underwent a cystogram after RARP. Transurethral catheter (TUC) was removed in the absence of VUA leakage or minimal leakage on subsequent repeat cy

AIM: Ultrasound and biological tools are used to predict high-grade vesicoureteral reflux, but other markers are needed to better select patients who need voiding cystography. Our aim was to determine whether studying Escherichia coli virulence factors would help to predict vesicoureteral reflux in patients with their first acute pyelonephritis.
METHODS: We included children presenting with E. coli-related acute pyelonephritis or cystitis. Vesicoureteral reflux was assessed by voiding cystography. Virulence factors were identified by multiplex polymerase chain reaction. Statistical analysis w

Retrograde intra-vesical plain film or CT cystography may not be feasible for patients with hypersensitivity towards iodinated contrast agents. This study evaluates whether retrograde gadoxetic acid-enhanced pelvic and liver MRI can detect intraperitoneal bladder rupture in a mouse model. Pelvic and liver MRI were obtained using a 7T micro-MRI scanner in 8-to-10-week-old female ICR mice (n = 28) before and after surgically induced bladder rupture. An angio-catheter was inserted into the urethra for intra-vesical injection of gadoxetic acid (0.3 mmol/kg) during MRI. Intraperitoneal contrast lea

BACKGROUND: Imaging techniques of bladder support can be of diagnostic value to differ causes of urinary incontinence like misbuilding of the lower urinary tract, hypermobility of the bladderneck and urethral wall pathology, which is valuable prior to surgery for incontinence.
METHOD: Literature about imaging techniques, especially, voiding cystourethrography, ultrasonography and MRI were studied and their diagnostic value evaluated.
CONCLUSION: Dynamic ultrasonography is the first line imaging method for studying bladder support. Bladderneck hypermobility, as a sign of defect in the adjunct

INTRODUCTION: Trauma to the genitourinary system includes blunt and penetrating injuries to bladder and ureters. These are rare injuries and are overlooked as other abdominal and pelvic injuries often take priority. Delayed diagnosis can lead to significant morbidity and mortality. Computed tomography has taken a central role in the imaging of the ureters and bladder.
METHODS: This article reviews the anatomic relationships, mechanisms of injury, and clinical presentation to help physicians determine when bladder and ureteral injuries should be suspected and further imaging should be pursued.

We present a case of enterovesical fistula caused by an accidental ingestion of a foreign body. A 23-year-old man presented to our hospital with pneumaturia, fecaluria, and abdominal pain but no recent possible causes of enterovesical fistula at anamnesis. Cystoscopy, cystography, and also colonoscopy were not able to detect the fistulous tract. Computer tomography (CT) revealed a fistula between bladder and bowels caused by a toothpick accidentally swallowed 2 years earlier. We tried to remove the foreign body endoscopically by cystoscopy and colonoscopy but with no success. The failure of en

A 4,000 g male infant two hours old, product of normal pregnancy and delivery was admitted in our hospital because of bladder intussusception though umbilico vesical fistula (persistent foetal bladder). The anomaly coexisted with hernia in the umbilical cord. The whole upper part of the bladder was everted and could not be brought in. The surgical correction was performed on the fourth hour. The everted bladder was reduced from inside, severed from abdominal wall and closed in two layers. Pan endoscopy revealed no lower urinary tract obstruction. The post-operative course was uneventful. We have used a simple method of ureteroneocystostomy in renal transplant patients. This entails a no-tunnel "drop-in" with a long free segment protruding into the bladder lumen. The incidence of vesicoureteric reflux was investigated in 81 transplant patients who had a follow-up period ranging from 4 months to 15 years (mean 3.7 years); 29 of these patients had recurrent urinary tract infections. Micturating cystography demonstrated reflux in 6 patients (7.4%). In these 6 patients the presence of reflux had no deleterious effect on renal function and the infections were easily controlled with

Inguinal bladder hernia is a rare clinical condition, with 1-3% of all inguinal hernias involving the bladder. Any portion of the bladder may herniate, from a small portion or a diverticulum to most of the bladder. We present a 55-year-old male with an intermittent right scrotal mass of 6 months' duration. The mass lesion protruded through the right inguinal canal before voiding and reduced after that. Scrotal sonography revealed a hypoechoic lesion in the scrotum that stretched cranially to the intra-abdominal portion of the bladder. Excretory urography showed a duplicated system in the left

We report a case of neobladder-rectal fistula that developed as an early postoperative complication of radical cystectomy and orthotopic neobladder construction procedures. A 75-year-old man underwent a radical cystectomy and orthotopic neobladder construction using Studer's method for locally invasive bladder cancer (cT2N0M0). The patient had severe watery diarrhea on postoperative day 20, and was diagnosed with a neobladder-rectal fistula based on cystography findings. We inserted a Foley catheter into the neobladder, and performed conservative treatment. Four months after development of the

We subjected to a functional and metabolic evaluation (urodynamic examination + cystography) 10 patients underwent to radical cystectomy with a ileal orthotopic reservoir (VIP) for bladder cancer. At the moment patients have a minimum 3-years follow-up and they are out of disease. The medium capacity of the reservoir is about 447 ml, with a low pressure flow, a medium pressure of ureteral closing of 62.5 cm of H2O. At the cystography neither ureteral reflux nor post miction residuum have been proved. All the patients are continent, with the exception of one patient suffering from episodes of n

The present study investigated whether ultrasonography was effective in detecting ureteric reflux in children suspected of having urinary infection. Seventeen children with febrile episodes and pyuria were enrolled. The ultrasound examination revealed ballooning of the renal pelvis during bladder contraction in 4 children, dilatation of the distal ureters in 6, and small kidney in 2. Cystography was performed on the 6 children with these ultrasound abnormalities and 1 child with two episodes of suspected urinary infection. Four children showed reflux. All of the 4 children had been found to ha

BACKGROUND: The diagnostic procedures proposed in the evaluation of sigmoidovesical fistulas complicating diverticulitis are various and their effectiveness is still not well established.
METHODS: Personal experience is based on 14 cases of colovesical fistulas secondary to sigmoid diverticulitis. Several diagnostic tools were employed: plain abdominal film (10 cases), large bowel enema (12), urography (3), cystography (2), sonography (4), and CT (5). The semeiotics of these fistulas were distinguished in direct, indirect, secondary, and related to the underlying disease.
RESULTS: The fistul

PURPOSE: We prospectively tested the safety of routine removal of the catheter as early as 2 to 4 days after laparoscopic radical prostatectomy.
MATERIALS AND METHODS: Between March 1998 and March 2001, 228 patients underwent laparoscopic radical prostatectomy for clinically organ confined prostate cancer. The last 113 consecutive patients were included in a prospective study according to gravitational cystography performed 2 to 4 days postoperatively. If no leak was seen the catheter was removed. If a leak was apparent the catheter was left indwelling for another 6 days and cystography was r
