Irrigation performance of a 2-mm rigid cystoscope designed for neonatal cystoscopy: a bench study.

PURPOSE: Neonatal and infant cystoscopy is limited by narrow urethral caliber and the lack of sufficiently small cystoscopes with adequate irrigation. We developed a 2-mm (6 Fr) rigid cystoscope prototype and evaluated irrigation performance in a benchtop open-flow system. METHODS: The prototype incorporated a 0.9-mm optical channel, a 0.7-mm working channel, and two irrigation lumens (0.48 mm each). Irrigation performance was assessed under gravity and cuff-assisted irrigation, including temperature-controlled experiments. The maximum cuff pressure of 300 mmHg was used as an upper-bound benc

AI-Supported Prediction of Vesicoureteral Reflux in Children Based on Cystoscopic Configuration of the Ureteric Orifice.

Background: Vesicoureteral reflux (VUR) is a common pediatric urological disorder traditionally diagnosed by voiding cystourethrography (VCUG). Although ureteric orifice (UO) morphology during cystoscopy correlates with VUR severity, its assessment remains subjective. To our knowledge, artificial intelligence (AI) has not previously been applied to cystoscopic images for VUR prediction. We aimed to develop and evaluate the first AI model for predicting VUR from pediatric cystoscopic images and videos. Methods: In this retrospective single-center study, cystoscopic videos from children with and

Pilot study to assess the feasibility of self-administered, low-dose methoxyflurane for cystoscopic procedures.

INTRODUCTION: Methoxyflurane (MEO F) (Penthrox™) is an inhaled, self-administered, non-opioid analgesic approved by Health Canada for the short-term relief of moderate to severe acute pain associated with trauma or interventional medical procedures. In this pilot study, we evaluated the feasibility of using MEOF as an anesthetic agent in 11 patients undergoing outpatient cystoscopic procedures. METHODS: The average duration of the procedure was 24 (range 20-35) minutes and this included 10 minutes of administration time of the drug and five minutes of wait time before the procedure. The avera

Prospective Randomized Trial of Single-Use vs Reusable Cystoscope for Ureteral Stent Removal.

Background: Reusable cystoscopes are associated with risks of cross-contamination, need for chemical reprocessing, mechanical breakdown, maintenance costs, and labor for cleaning. A sterile, single-use cystoscope may reduce or eliminate the source of these infections while also decreasing exposure to reprocessing chemical agents. In this multicenter, randomized trial, we sought to compare single use with reusable endoscopes for ureteral stent removal. Methods: A three-center, randomized, dual-arm postmarket clinical trial was performed to compare the single-use cystoscope (Ambu A/S) with stand

Feasibility and Safety of a Simple Non-cystoscopic Double-J Tube Removal Technique in Children.

Objective: Double-J tube placement is an important procedure during upper urinary tract surgery. A primary drawback is the requirement of a second double-J tube removal under a cystoscope. Therefore, a simple and feasible alternative is required to remove the double-J tube without cystoscopy. The present study reported the feasibility and safety of a simple non-cystoscopic double-J tube removal technique. Method: We retrospectively analysed children who underwent pyeloplasty and ureterovesical reimplantation between June 2015 and August 2021. A simple device (a catheter with a suture) was used

A novel waterjet technology for transurethral cystoscopic injection of viable cells in the urethral sphincter complex.

AIMS: In a recent preclinical study, we noticed that injection of cells in the urethral sphincter by needle through a cystoscope under visual control frequently yielded in misplacement or loss of cells. We, therefore, investigated if a needle-free waterjet device delivers viable cells under defined settings, including injection volume and pressure, fluid velocity and transportation media, precisely through the urothelium and connective tissue close to the sphincter muscle without full penetration of the sphincter apparatus. METHODS: Mesenchymal stromal cells (MSCs) were prepared for needle-fr

False-Positive Cystoscopic Diagnosis of Ureteral Obstruction After Hysterectomy Due to a Nonfunctional Kidney.

BACKGROUND: An estimated 0.2% of the population may have a unilateral nonfunctional kidney. This pre-existing condition may lead to the mistaken presumption that a ureteral injury has occurred when performing cystoscopy after hysterectomy. CASES: Two cases are presented in which cystoscopy to confirm ureteral patency after hysterectomy was performed in patients with pre-existing unknown nonfunctional kidneys. These false-positive findings resulted in additional operative time, additional cost, and unnecessary surgical exploration. CONCLUSION: Cystoscopy to confirm ureteral patency after hyst

Cystoscopic removal of intravesical synthetic mesh extrusion with the aid of Endoloop sutures and endoscopic scissors.

Synthetic mesh is commonly used in urogynecologic surgical procedures for correction of stress urinary incontinence and pelvic organ prolapse. Bladder mesh extrusion is a recognized complication and frequently requires extensive mesh resection. We present a case of intravesical polypropylene mesh extrusion following a combined mesh kit and midurethral sling placement. A novel transurethral approach, employing Endoloop sutures with endoscopic scissors, was used to resect the protruding mesh located in close proximity to a ureteral orifice. Medline was searched for other minimally invasive treat

Cystoscopic management of an extruded coil from arteriovenous fistula embolization using arthroscopic instruments.

Pelvic arteriovenous malformation (AVM) is a rare condition in which a section of blood vessels lacks a capillary network, resulting in blood from an artery being delivered directly to a vein. This condition may be congenital or acquired. Percutaneous selective embolization can obliterate AVMs and has increasingly been used for management. Management involves transcatheter embolization with either inert glues, balloons, or fibered titanium coils that are designed to limit the flow through feeding vessels. We describe one potential complication of transcatheter embolization that involves necros

[Use of Nd:YAG laser and flexible cystoscope in outpatient treatment of recurrent superficial bladder tumors].

Nd:YAG laser irradiation and flexible cystoscopy were used in the outpatient management of 16 cases (21 sessions) with superficial bladder tumors. Irradiation of tumors was accomplished in all cases except 3 in which the visual field was affected by bleeding after the cup biopsy of tumors. Tumors at the bladder neck were easily irradiated through the flexible cystoscope. No tumor recurred at or near the previously irradiated site. This method provided a safe and cost-effective means to treat superficial bladder tumors in selected cases.

Multimodal Large Language Models for Cystoscopic Image Interpretation and Bladder Lesion Classification: Comparative Study.

BACKGROUND: Cystoscopy remains the gold standard for diagnosing bladder lesions; however, its diagnostic accuracy is operator dependent and prone to missing subtle abnormalities such as carcinoma in situ or misinterpreting mimic lesions (tumor, inflammation, or normal variants). Artificial intelligence-based image-analysis systems are emerging, yet conventional models remain limited to single tasks and cannot produce explanatory reports or articulate diagnostic reasoning. Multimodal large language models (MM-LLMs) integrate visual recognition, contextual reasoning, and language generation, off

Clinical Implications of Glomerulation After Cystoscopic Hydrodistention in Patients With Interstitial Cystitis and Bladder Pain Syndrome.

Interstitial cystitis (IC) and bladder pain syndrome (BPS) have several clinical phenotypes. However, these subtypes may have similar symptoms, such as bladder pain, frequency, urgency, and functional somatic syndrome. A global consensus has been reached that bladders with characteristic Hunner's ulcer lesions are classified as IC, whereas those without Hunner's ulcer are classified as non-ulcer IC/BPS. To diagnose Hunner's IC, office cystoscopy without anesthesia is usually adequate for visualizing the presence of Hunner's lesions. However, the diagnostic accuracy of cystoscopic hydrodistenti

The Cystoscope Sheath as a Platform for Performing Retrograde Intrarenal Surgery in a Transplanted Kidney with Complex Renal Anatomy.

BACKGROUND: Endourology is a widely used means by which to manage urolithiasis. Patient anatomy can oftentimes limit what can be accomplished with current technology. CASE PRESENTATION: This is a case of a patient with renal and ureteral stones within a transplant kidney. Her anatomy would not allow for a standard retrograde ureteroscopic approach. We describe a method by which to overcome this difficult scenario by using a rigid cystoscope as a platform by which a ureteroscope was passed to allow for stone removal. CONCLUSION: For this difficult case, we effectively used our instruments to

Cystoscopic injections of dextranomer hyaluronic acid into proximal urethra for urethral incompetence: efficacy and adverse outcomes.

OBJECTIVES: To determine whether dextranomer/hyaluronic acid would be more efficacious or would produce fewer complications when using the material in a standard proximal-urethra cystoscopically-directed injection technique. Injectable periurethral bulking agents are an alternative to stress incontinence surgery. Dextranomer, a highly hydrophilic dextran polymer, solubilized in a base of nonanimal stabilized hyaluronic acid, has been approved as an injectable agent for the treatment of childhood vesicoureteric reflux (Deflux, Q-Med AB, Uppsala, Sweden), and in Europe for women with stress urin

Inverted papilloma of urinary bladder: is long-term cystoscopic surveillance needed? A single center's experience.

OBJECTIVES: To review all cases of urinary bladder inverted papilloma (IP) in our institution and determine the need for cystoscopic surveillance. IP is an uncommon benign tumor of the urinary tract. Its multiplicity, recurrence, and association with transitional cell carcinoma (TCC) suggest possible malignant potential, leading to conflicting clinical conclusions regarding the need for surveillance. METHODS: All consecutive patients from January 1991 to December 2004 with IP were included in this study. A single pathologist performed the histologic review. The patients had undergone cystosco

Cystoscopic submucosal indocyanine green marking in robot-assisted radical nephroureterectomy: a multicenter cohort study.

BACKGROUND: Distal ureter and bladder cuff dissection can be difficult during robot-assisted radical nephroureterectomy (RA-RNU) for upper tract urothelial carcinoma (UTUC), particularly in the deep pelvis where tactile feedback is limited. We evaluated cystoscopic submucosal indocyanine green (ICG) marking around the ureteral orifice as a navigation aid for this step. METHODS: Adults who underwent RA-RNU with formal bladder cuff excision for clinically localized UTUC (≤T3, M0) at three centers between January 2019 and November 2024 were retrospectively analyzed. Cystoscopic retrieval of a migrated IUD with bladder stone formation during pregnancy termination: a Case Report.

BACKGROUND: Uterine perforation with intravesical migration is an exceedingly rare complication of intrauterine device (IUD) use. Its non-specific symptomatology often leads to delayed diagnosis, while the presence of a uterine scar, such as from a prior cesarean section, may significantly increase the risk. CASE PRESENTATION: A 38-years-old woman (gravida 5, para 2) with a history of two cesarean sections presented at 8 weeks of gestation for termination of pregnancy. A V-shaped copper IUD had been placed over 10 years earlier. Notably, she reported a 1-year history of intermittent urinary f

Case report: Radiographic and cystoscopic manifestations of chronic urogenital Schistosomiasis in an asymptomatic Zimbabwean man with HIV.

While undergoing evaluation for loss of consciousness and traumatic injuries after a single-vehicle collision, a 55-year-old Zimbabwean man residing in Maryland was incidentally noted to have circumferential bladder wall calcification and thickening on CT imaging. Cystoscopy revealed extensive 'sandy patches', confirmed by histopathology to represent calcified eggs of Schistosoma haematobium. Genitourinary symptoms were absent, ova were not identified on urine microscopy, and serum eosinophil level was normal, consistent with the primary infection having occurred at least 20 years prior.
