Tendoscopic debridement for stage I posterior tibial tendon dysfunction.

Our objective is to assess whether the tendoscopic synovectomy is effective to control the stage I posterior tibial tendon dysfunction. Our study is a retrospective one. The participants, six patients with stage I posterior tibial tendon dysfunction, were treated with tendoscopy with synovectomy for the past 3 years. The results show that this is a safe procedure and we could achieve similar effectiveness as the traditional open procedure. There was no complication found. None of our patients have progressed to stage II or above posterior tibial tendon dysfunction. In conclusion, tendoscopic d

Arthroscopic ankle debridement and fusion: indications, techniques, and results.

The use of the arthroscope in arthritis of the ankle has been well described and there is no question that it is a significant part of the armamentarium for the orthopaedic surgeon dealing with these patients. Unfortunately, those patients with advanced arthritis and loss of joint space do not respond well to traditional arthroscopic debridement, removal of loose bodies, debridement and drilling of osteochondral lesions, and removal of anterior osteophytes. These procedures should only be used on those patients with minimal to no degenerative arthritis. Arthroscopic ankle arthrodesis is becomi

Arthroscopic versus open debridement of penetrating knee joint injuries.

Arthroscopic debridement of penetrating knee joint injuries has become a common treatment method. A comparative study was undertaken to compare this method with open joint debridement. Fourteen penetrating knee joint injuries (fourteen patients) were treated by arthroscopic examination and debridement and were compared to sixteen penetrating knee joint injuries (fifteen patients) treated by open debridement. There were no resultant infections or operative complications in either group. Of note, the arthroscopic debridement group had a shorter postoperative hospital stay [mean of 1.6 days compa

Chronic Morel-Lavallée Lesion Treated With Percutaneous Ultrasonic Debridement.

Morel-Lavallée lesion is defined as an internal degloving injury. A trauma involving shearing forces causes separation of the superficial fascia from the overlying subcutaneous tissue and skin, creating a potential space for hemolymphatic fluid to accumulate. This leads to the development of a self-resolving or potentially chronic collection. Classically, lesions arise in the lateral thigh. Imaging features depend on the duration of the lesion. Acutely, ultrasound and MR imaging will demonstrate a hematoma-like mass that is parallel to the underlying fascia. As the lesion evolves over time, ca

Current and Future Directions in Fluorescence Imaging-Guided Debridement.

Significance: Sterility and reduction of the bioburden are crucial for healing in chronic wounds such as diabetic foot ulcers. Although there are methods for measuring bioburdens, such as semiquantitative analysis of swab/biopsy samples, microbiological sampling, and molecular diagnostics, these tools are less accessible owing to costs or not being as quick as other methods. These methods are also dependent on clinical assessment by the clinician, and high bacterial burden may appear asymptomatic. Recent Advances: Autofluorescence (AF) imaging is a novel technology for identifying and quantify

Arthroscopic Debridement for Treatment of Chronic Dynamic Scaphoid Instability.

Background  There is a paucity of information on minimal surgical intervention for the treatment of chronic dynamic scaphoid instability (CDSI) that will achieve an acceptable mid-term result. Purpose  We hypothesize that by following a debride-first-then-wait protocol, some patients with CDSI can be treated with arthroscopic debridement alone and avoid a reconstructive procedure. Patients and Methods  We performed a retrospective, longitudinal study from January 2008 to December 2018 of all patients diagnosed with CDSI and treated with arthroscopic debridement of the scapholunate interosseous

Surgical debridement of infected pubic symphysitis supports optimal outcome.

INTRODUCTION: Infected pubic symphysitis (IPS) is a rare bacterial infection of the pubic symphysis that causes subpubic pain, disability and ultimately permanent immobility. Due to difficult diagnosis, patients present with long-standing complaints and consult several doctors. To date, no validated treatment protocol exists and most patients are treated conservatively with antibiotics. This study was aimed to assess the results after careful surgical debridement and pathogen-specific antibiotic treatment in IPS. MATERIALS AND METHODS: A chart review of eight patients with proven IPS was perf

Lateral Epicondylitis Debridement and Repair Using Knotless Suture Anchor.

Lateral epicondylitis, or tennis elbow, involves degeneration of the extensor carpi radialis brevis tendon and is often self-limiting, with surgery reserved for recalcitrant cases. Surgical management of tennis elbow consists primarily of either debridement alone or debridement with repair. Surgical repair is often performed using either a suture or a suture anchor. Good outcomes have been reported using standard repair methods; however, complications exist. Complications include potential loss of grip strength with debridement alone, as well as soft-tissue irritation caused by a prominent sut

Infected tibial nonunion in children: Is radical debridement mandatory?

UNLABELLED: Nonunion is a devastating complication of tibial fractures with a debilitating effect. Several studies reporting tibial nonunions in adults are available; however, the reports on infected nonunions in children are scarce. The purpose of this study was to evaluate the results of treating paediatric infected tibial nonunion using Ilizarov fixator without radical resection. MATERIALS AND METHODS: The study included 36 cases with infected nonunion of the tibia with 31 males and 5 females, and an average age of 11.4 years. The mean number of previous operations was 3.5. The duration be

Fixation and Grafting After Limited Debridement of Scaphoid Nonunions.

PURPOSE: To evaluate a surgical technique of treating nondisplaced waist and proximal pole scaphoid nonunions without avascular necrosis (AVN). METHODS: We performed a retrospective review of all patients with nondisplaced, scaphoid waist or proximal pole nonunions without AVN treated with the following technique. Two K-wires are positioned along the scaphoid axis to stabilize the proximal and distal poles. Debridement with a curette or burr is performed parallel to the nonunion site until the K-wires are visualized and punctate bleeding of the proximal and distal fragments is encountered. Th

Arthroscopic debridement of loose bodies from a popliteal cyst.

To our knowledge, arthroscopic removal of loose bodies from a popliteal cyst has never been reported in the medical literature. This article describes our technique for removing loose bodies from a popliteal cyst and verifies that the procedure can be performed safely and effectively.A 52-year-old man had progressively worsening right knee pain and swelling of 3 years' duration that had been treated conservatively for 6 months. On physical examination, there was boggy swelling, a palpable popliteal cyst, crepitus with range of motion, and diffuse discomfort and tenderness about the knee. Magne

Arthroscopic debridement and irrigation of periprosthetic total elbow infection.

We report on a case of arthroscopic treatment of septic arthritis of the elbow joint in a 65-year-old man with an elbow endoprosthesis. Two months after arthroplasty of the elbow joint, the patient developed acute septic arthritis of the right elbow. Methicillin-sensitive Staphylococcus aureus was identified as the causative organism. Six days after the onset of symptoms, the patient was treated with a single arthroscopic procedure of the infected periprosthetic joint, including irrigation with 5 L of Ringer's lactate solution, debridement, and partial synovectomy with a 4.5-mm curved shaver.

Anterior radical debridement and anterior instrumentation in tuberculosis spondylitis.

The conventional procedure in the treatment of vertebral tuberculosis is drainage of the abscess, curettage of the devitalized vertebra and application of an antituberculous chemotherapy regimen. Posterior instrumentation results are encouraging in the prevention or treatment of late kyphosis; however, a second-stage operation is needed. Recently, posterolateral or transpedicular drainage without anterior drainage or posterior instrumentation following anterior drainage in the same session has become the preferred treatment, in order that kyphotic deformity can be avoided. Criteria for bone debridement in massive lower limb trauma.

Adequate debridement remains the most important factor in prevention of chronic sepsis in cases of massive lower limb trauma. Despite treatment with chronic antibiotics, muscle flaps, hyperbaric oxygen, and/or free-tissue transfer, failures are common due to inadequate bone debridement. All bony fragments with marginal soft-tissue attachments and exposed bone without evidence of adequate blood flow must be removed during debridement for optimal results. Laser Doppler flowmetry (LDF) may be a useful adjunct for the assessment of bone viability during both initial and subsequent debridements. Pr
