Microvascular decompression: a contemporary update.

BACKGROUND: Microvascular decompression (MVD) is the gold-standard surgical treatment for cranial nerve compression disorders, including trigeminal neuralgia (TN), hemifacial spasm (HFS), and glossopharyngeal neuralgia (GPN). This review synthesizes historical milestones, recent advances, and evolving techniques in MVD, with a primary focus on these conditions. METHODS: A comprehensive literature review was conducted using databases such as PubMed, SpringerLink, Google Scholar, BioMed Central, Scopus, and ScienceDirect. Studies published between 1970 and 2024 were analyzed, emphasizing surgic

Transconjunctival Orbital Decompression in Cherubism.

Cherubism is a rare fibro-osseous disease characterized by the progressive expansion of the mandible and maxilla during childhood. Orbital involvement occurs in a subset of patients and is clinically manifested as upward displacement of the affected eye. The bony changes tend to spontaneously diminish or even regress after puberty. The authors report a case of a young adult female who underwent craniofacial correction of the maxilla and mandible changes but continued to have unilateral scleral show. CT scans of the orbits revealed a residual mass in the right orbit, associated with superior di

Spontaneous decompression of sigmoid volvulus.

OBJECTIVES: Spontaneous decompression is an uncommon outcome of sigmoid volvulus (SV). The aim of this study was to evaluate the clinical presentation, diagnosis, treatment, and follow-up of spontaneously decompressed SV. METHODS: We utilized the data of our 1,063 SV patients, the most comprehensive monocenter SV series in the world. To obtain the worldwide data on the spontaneous decompression of SV, we researched the last 56-years' literature in Web of Science and PubMed databases. RESULTS: The incidence of the spontaneous decompression was 0.1% (1/1,063) in our SV series, whereas it was 1

Microvascular decompression for intractable singultus.

Intractable singultus due to cerebrovascular disease is very rare. We report a case of intractable singultus that improved after microvascular decompression and present a literature review. The patient was a 58-year-old man with a 30-year history of persistent singultus. Its frequency and duration gradually increased and it was resistant to multiple medical treatments. Microvascular decompression to relieve pressure on the anterolateral surface of the lower medulla oblongata from the vertebral artery resulted in the resolution of singultus. Patients with intractable idiopathic singultus who fa

MICROVASCULAR DECOMPRESSION FOR TRIGEMINAL NEURALGIA.

BACKGROUND: Trigeminal Neuralgia (TGN) is the most frequently diagnosed type of facial pain. In idiopathic type of TGN it is caused by the neuro-vascular conflict involving trigeminal nerve. Microvascular decompression (MVD) aims at addressing this basic pathology in the idiopathic type of TGN. This study was conducted to determine the outcome and complications of patients with idiopathic TGN undergoing MVD. METHODS: In a descriptive case series patients with idiopathic TGN undergoing MVD were included in consecutive manner. Patients were diagnosed on the basis of detailed history and clinica

Protective mechanisms in hypobaric decompression.

BACKGROUND: To reduce bubble formation and growth during hypobaric exposures, a denitrogenation or nitrogen "washout" procedure is performed. This procedure consists of prebreathing oxygen fractions as close to one as possible (oxygen prebreathe) prior to depressurization before ascending to the working altitude or low spacesuit pressures. During the NASA prebreathe reduction program (PRP), it was determined that the addition of a light arm exercise to short, individually designed, performance-based heavy exercise (dual cycle ergometry) during an abbreviated 2-h prebreathe (F1O2 - 1.0) reduced

Large meningoencephalocele after orbital decompression.

A 71-year-old woman with a history of thyroid eye disease was seen for evaluation of a skull-base mass noted on neuroimaging. She had previously under-gone bilateral orbital decompressions and strabismus surgery and had no neurologic symptoms. Bony defects along the skull base and cerebrospinal fluid leaks are known risks of orbital surgery. This is the first report of a large, asymptomatic meningoencephalocele after orbital decompression surgery.

[Decompression surgery for endocrine orbitopathy].

BACKGROUND: Endocrine orbitopathy is a common feature of autoimmune thyroid disease. The increase of pressure within the eye socket leads to proptosis and compression of the optic nerve. This results in cosmetic impairment as well as loss of vision. Apart from the medical treatment surgical decompression of the crowded orbit offers a valid therapeutic option. PATIENTS AND METHODS: 142 patients (264 orbits) were treated at the interdisciplinary orbital center, Johannes Gutenberg University Hospital, Mainz, Germany. The medial orbital wall was removed, followed by lipectomy. Indications for dec

Minimally invasive cervical stenosis decompression.

Cervical stenosis and cervical spondylotic myelopathy are common disorders seen by many spine surgeons. Presentation can range from asymptomatic to myelopathy or myeloradiculopathy to lower extremity and gait problems attributed to lumbar disease. Various surgical procedures are practiced in the surgical management of this disease. Posterior cervical laminectomy is a familiar procedure and is the treatment of choice for multilevel disease. With the development of newer instruments and access devices, it has become possible to treat this problem through a minimally invasive approach.

Endonasal bilateral simultaneous orbital decompression.

OBJECTIVE: To study the morbidity of the operation, to investigate it's safety and effectiveness in order to establish if it is reasonable to offer it as treatment to patients in one sitting instead of a staged procedure. Finally, to indentify the patient's long term satisfaction and to establish best practice. DESIGN: Retrospective case series study and prospective telephone interview. SETTING: Teaching General Hospital, Scotland, UK. SUBJECTS: 14 patients with dysthyroid orbitopathy who underwent the operation over a 4 year period. RESULTS: Proptosis improved in all but one of the cases.

[Endoscopic decompression of optic canal].

OBJECTIVE: To evaluate the effectiveness of endoscope in the treatment of optic nerve injury. METHODS: Thirteen patients with fracture at the inner and lower walls of optical canal underwent endoscopic decompression. The posterior 1/3 of the middle concha, the posterior ethmoid sinus and sphenoid sinus were opened, chips of bone and old hematocele were scavenged, and the optic ring of the optic canal was polished; however, the sheath of the optic nerve remained intact. In 2 cases with the dura broken at the cranial part of optic nerve, the broken site was covered with mucosa of middle concha,

Hemifacial spasm: endoscopic vascular decompression.

Sixty patients with primitive hemifacial spasm were treated by means of a minimally invasive retrosigmoid approach in which endoscopic and microsurgical procedures were combined. Intraoperative endoscopic examination of the cerebellopontine angle showed that for 56 of the patients vessel-nerve conflict was the cause of hemifacial spasm. The most common offending vessel was the posterior inferior cerebellar artery (39 patients), next was the vertebral artery (23 patients), and last was the anterior inferior cerebellar artery (16 patients). Nineteen of the patients had multiple offending vascula

Orbital decompression for thyroid orbitopathy.

BACKGROUND: Severe thyroid orbitopathy may result in optic neuropathy, corneal exposure and disfiguring proptosis. Orbital decompression has most commonly been performed for optic neuropathy, but with improved techniques, more patients are undergoing decompression for other indications. PURPOSE: This report evaluates the results and morbidity of orbital decompression for thyroid orbitopathy performed by one surgeon. METHODS: The records of 33 patients (53 orbits) undergoing orbital decompression for thyroid orbitopathy were analysed for changes in visual acuity and colour vision (where the i

[Microvascular decompression in trigeminal neuralgia].

30 cases of trigeminal neuralgia (TN) were operated between 1988-1995. 26 underwent microvascular decompression (MVD) and 4 others had partial rhizotomies. 87% of the patients became and remained pain-free, while 13% had only partial relief of pain. Permanent side-effects included unsteadiness of gait in 1 and hearing loss on the operated side, in another. There was no serious morbidity or mortality. In the same period, 119 patients underwent retrogasserian glycerol injections for TN. MVD is recommended as an effective procedure in those under 65 years of age, in whom there is no medical contr
