Surgical decompression for symptomatic Chiari II malformation in neonates with myelomeningocele.

Between January, 1981, and July, 1991, 17 infants under 1 month of age were admitted to The Hospital for Sick Children with the signs and symptoms of a Chiari II malformation. These patients' presentation included swallowing difficulty (71%), stridor (59%), apneic spells (29%), aspiration (12%), weakness of cry (18%), and arm weakness (53%). Decompression of the Chiari II malformation was performed in all patients, with a time interval between onset of symptoms and surgery ranging from 1 to 121 days. Fifteen patients (88%) remain alive, all of whom have shown a complete recovery. The mean foll

[Surgical orbital decompression in Basedowian optic neuropathy. Apropos of 5 cases].

Surgical orbital decompression in 5 patients with basedowian optic neuropathy restored the visual acuity and visual field in all cases. The place of surgery among the different therapeutic options proposed for this serious complication of Basedow's disease is discussed, as well as the advantages over other decompression techniques of the procedure used : the cutaneous approach to a trans-ethmoidonasal decompression.

Lateral lumbar spinal canal stenosis: classification, pathologic anatomy and surgical decompression.

The lateral region of the lumbar spinal canal is subject to pathologic alterations which encroach upon the neural structures located within. In order to further understanding and facilitate communication among specialists in diverse disciplines, a subdivision of the lateral region is proposed to comprise three zones: entrance zone, mid-zone and exit zone. The anatomical boundaries of these zones are defined and the pathologic conditions of each are described, and serve as the basis for techniques of surgical decompression when clinically required.

Surgical techniques of anterior decompression and fusion for spinal cord injuries.

Many patients who have static or only slowly improving neurologic deficits and significant compression of the spinal cord and nerve roots can benefit from anterior decompression. The improvement ranges from partial root recovery to very dramatic improvement in upper as well as lower extremities in the patient with quadriparesis. Intrinsic damage or contusion of the spinal cord cannot be reversed by decompression. Patients with motor sparing preoperatively attain a better functional improvement than those patients who have only slight distal sensory function initially. The same can be said of p

Development and validation of a surgical robot system for orbital decompression surgery.

PURPOSE: Orbital decompression surgery, which expands the volume of the orbit by removing sections of the orbital walls with a drill and saw, is an important treatment option for thyroid-associated ophthalmopathy. However, it is often limited by physical factors such as a narrow operating space and instability of the manual holding of surgical instruments, which constrains doctors from accurately executing surgical planning. METHODS: To overcome these limitations, we designed a surgical robot comprising position adjustment, remote center of motion, and end-effector with a rapid surgical instr

Use of a novel ultrasonic surgical system for decompression of the facial nerve.

OBJECTIVE: The middle cranial fossa approach has been used to explore and decompress the facial nerve in patients with Bell's palsy and facial nerve tumors. Unfortunately, this approach is technically challenging and has a significant risk of injury to the facial nerve and to the cochleovestibular organs. One way to minimize the risk may be with the use of the Sonopet Omni ultrasonic aspirator (Synergetics Inc., St Charles, MO) instead of an otologic drill. METHODS: In this prospective study using cadaveric temporal bones, a total of 17 temporal bone specimens were used. Seven cadaveric tempo

[Surgical decompression of the orbit in a case of post-traumatic retrobulbar hematoma].

The authors present a case of 34-years-old man with retrobulbar hematoma caused by blunt injury accompanied by partial palsy of the oculomotor nerve and ischemia of the optic nerve and retina. Fractures of the orbit were excluded using computer tomography. Surgical decompression of the orbit was performed. The authors obtained quick and marked improvement with normal visual acuity and limitation of adduction.

[Surgical decompression of the optic nerve in intracanal injuries. Indications and results].

Fifteen patients with traumatic optic neuropathy (anatomical section excluded) underwent surgical anterior decompression of the optic nerve, via the trans ethmoid-sphenoid approach. Improvement of visual function assessed by visual acuity and visual field measurements, was obtained in 10 patients. Results remained stable for a follow-up period of at least 6 months. There was a delay of several weeks before improvement in some of the patients. Thus a long follow-up period seems recommended. Trans ethmoid-sphenoid optic nerve decompression is a reliable procedure with low morbidity, and represen

Surgical decompression of the facial nerve in the treatment of chronic cluster headache.

The nervus intermedius (NI) appears to be the main conduit for the associated symptoms of cluster headache (CH) and perhaps for the pain as well. Subtle injury of the facial nerve and NI might initiate mechanisms responsible for CH. Five patients with chronic CH unresponsive to medication underwent surgical decompression of the root exit-entry zone of the facial nerve, and in two patients the trigeminal nerve root was also decompressed. In two patients, the pain syndrome was markedly relieved for as long as two years. In one patient, initial improvement was obscured by narcotic addiction. In t

Microvascular Decompression of the Medulla: A Case Report and Review of the Surgical Management.

This case study and review of surgical management highlights the efficacy of microvascular decompression (MVD) with transposition of the vertebral artery in alleviating symptoms of vertebral artery compression syndrome (VACS). VACS is a rare condition in which the vertebral artery exerts pathological compression on the medulla, leading to a spectrum of potentially debilitating neurological symptoms. While microvascular decompression (MVD) is a well-documented treatment for cranial nerve compression syndromes, its application in relieving medullary compression due to VACS remains less explored.

The Impact of Neuronavigation on the Surgical Outcome of Microvascular Decompression for Trigeminal Neuralgia.

OBJECTIVE: Microvascular decompression (MVD) is considered the only etiological treatment for drug-resistant trigeminal neuralgia (TN). Nonetheless, despite the recent technological advances, the risks associated with MVD remain non-negligible. The aim of this study was to evaluate the impact of using neuronavigation on the surgical outcomes of patients with TN submitted for MVD. METHODS: We analyzed 30 consecutive patients (11 men, 19 women) who underwent MVD for TN between January 2019 and December 2019. Patients were divided in 2 groups according to the use of neuronavigation (group A: MVD

Surgical decompressions for cauda equina syndrome during COVID-19.

BACKGROUND: The novel coronarvirus disease (COVID-19) has had a major impact on provision of spinal neurosurgery across the world, especially in the UK, with a significant fall in operating and patient volumes, and elective clinical activities. It is unclear whether the pandemic has affected the volume of urgent spinal procedures in the UK, especially surgical decompressions for cauda equina syndrome (CES). METHODS: Therefore, we conducted a retrospective analysis of theatre records and electronic operation notes at our institution to identify all procedures performed for CES before (December

Surgical Management of the Trigeminocerebellar Artery in Microvascular Decompression for Trigeminal Neuralgia.

BACKGROUND: The trigeminocerebellar artery (TCA) is a variant branch of the basilar artery supplying the trigeminal nerve root and cerebellar hemisphere, which may contribute to trigeminal neuralgia (TGN). Despite its unique anatomic description, information regarding the preoperative diagnosis and surgical management of TCA is lacking. OBJECTIVE: To evaluate our preoperative diagnosis of TCA using 3-dimensional (3D) images and surgical management of TGN cases with TCA involvement. METHODS: We reviewed our 15 patients with TCA involvement (3.3%) among 460 consecutive patients undergoing micr

[The Value of Endoscopic Decompression in Surgical Therapy of Carpal Tunnel Syndrome].

Carpal tunnel syndrome is one of the most common diseases in hand surgery. The gold standard in therapy is the surgical release of the carpal tunnel. We provide a brief update on the relevant pathogenesis, diagnosis and therapy and discuss questions related to minimal invasive decompression of the median nerve. Together with a review of the current literature, we report on our experiences in minimally invasive carpal tunnel release via a monoportal endoscopic access in over 700 cases. In conclusion, the endoscopic technique provides superior convalescence and patient safety is comparable to op

Predictors of surgical revision after in situ decompression of the ulnar nerve.

BACKGROUND: This study was performed to identify factors associated with the need for revision surgery after in situ decompression of the ulnar nerve for cubital tunnel syndrome. METHODS: This case-control investigation examined all patients treated at one institution with open in situ decompression for cubital tunnel syndrome between 2006 and 2011. The case patients were 44 failed decompressions that required revision, and the controls were 79 randomly selected patients treated with a single operation. Demographic data and disease-specific data were extracted from the medical records. The ra

Effectiveness of surgical decompression in the treatment of a calcifying cystic odontogenic tumor.

The calcifying odontogenic cystic tumor (CCOT) is a benign lesion of odontogenic origin characterized by an ameloblastoma-like epithelium with ghost cells that may calcify. Despite broadly considered as a cyst, some investigators prefer to classify it as a neoplasm. Clinically, it occurs predominantly during the third decade of life. No difference in gender prevalence has been observed nor predilection of the lesion between maxilla and mandible. The most affected region extends from the incisor tooth to bicuspids. The classic treatment of the lesion is full excision, although a different appro
