Decompressive Craniectomy for Spontaneous Intracerebral Hemorrhage: A Systematic Review and Meta-analysis.

BACKGROUND: Spontaneous intracerebral hemorrhage (sICH) is a devastating disease with high mortality and morbidity, and the application of decompressive craniectomy (DC) in sICH is controversial. We conducted a systematic review to verify the effects of DC on improving outcome in sICH. METHODS: Through searching several electronic databases, we screened eligible publications. Respective risk ratio (RR) and its 95% confidence interval (CI) were calculated, data were synthesized with a fixed-effect model, and sensitivity analyses and subgroup analyses were performed. Publication bias was measur

Paradoxical herniation after decompressive craniectomy provoked by lumbar puncture or ventriculoperitoneal shunting.

OBJECT: Two patients who underwent decompressive craniectomy after head trauma deteriorated secondary to paradoxical herniation, one after lumbar puncture and the other after ventriculoperitoneal shunting. They motivated the authors to investigate further provoked paradoxical herniation. METHODS: The authors reviewed the records of 205 patients who were treated at a single hospital with decompressive craniectomy for head trauma to identify those who had had lumbar puncture performed or a ventriculoperitoneal shunt placed after craniectomy but before cranioplasty. Among the patients who met th

Surgical treatment of elevated intracranial pressure: decompressive craniectomy and intracranial pressure monitoring.

Surgical techniques that address elevated intracranial pressure include (1) intraventricular catheter insertion and cerebrospinal fluid drainage, (2) removal of an intracranial space-occupying lesion, and (3) decompressive craniectomy. This review discusses the role of surgery in the management of elevated intracranial pressure, with special focus on intraventricular catheter placement and decompressive craniectomy. The techniques and potential complications of each procedure are described, and the existing evidence regarding the impact of these procedures on patient outcome is reviewed. Surgi

Cerebral microdialysis and PtiO2 to decide unilateral decompressive craniectomy after brain gunshot.

Decompressive craniectomy (DC) following brain injury can induce complications (hemorrhage, infection, and hygroma). It is then considered as a last-tier therapy, and can be deleteriously delayed. Focal neuromonitoring (microdialysis and PtiO2) can help clinicians to decide bedside to perform DC in case of intracranial pressure (ICP) around 20 to 25 mmHg despite maximal medical treatment. This was the case of a hunter, brain injured by gunshot. DC was performed at day 6, because of unstable ICP, ischemic trend of PtiO2, and decreased cerebral glucose but normal lactate/pyruvate ratio. Outcome following decompressive craniectomy for malignant swelling due to severe head injury.

OBJECT: The aim of this study was to assess outcome following decompressive craniectomy for malignant brain swelling due to closed traumatic brain injury (TBI). METHODS: During a 48-month period (March 2000-March 2004), 50 of 967 consecutive patients with closed TBI experienced diffuse brain swelling and underwent decompressive craniectomy, without removal of clots or contusion, to control intracranial pressure (ICP) or to reverse dangerous brain shifts. Diffuse injury was demonstrated in 44 patients, an evacuated mass lesion in four in whom decompressive craniectomy had been performed as a s

Transcranial contrast imaging of cerebral perfusion in stroke patients following decompressive craniectomy.

AIM: Contrast-enhanced transcranial triggered B-mode technology can be used to examine cerebral perfusion. However, this technique is still faced with methodological problems, especially the difficulty of overcoming the temporal bone window. The aim of the present study is to evaluate a deficit in cerebral perfusion after administration of the contrast agent SonoVue (TM) in acute stroke patients following decompressive craniectomy. METHODS: Ten stroke patients (aged 39 to 59 years, mean age 57 years), in whom a decompressive craniectomy due to a malignant space-occupying infarction or intrace

Early Versus Late Cranioplasty Following Decompressive Craniectomy: A Propensity-Matched Postoperative Outcome Analysis.

OBJECTIVE: Postoperative complications following cranioplasty occur in nearly 30% of patients requiring decompressive craniectomy (DC). Controversy exists regarding the procedure's timing, particularly whether performing cranioplasty closer to or further from the craniectomy reduces morbidity. Both strategies offer distinct benefits and associated risks; however, the literature remains inconclusive regarding postoperative complication rates. METHODS: We performed a retrospective cohort study using the TriNetX database. Patients were identified as undergoing early (0-90 d) or late (91 d-1 y) c

Research Progress on the Impact of Cranioplasty After Decompressive Craniectomy on Brain Function.

Decompressive craniectomy is a surgical procedure used to treat cerebral hemorrhage, traumatic brain injury, and brain herniation caused by elevated intracranial pressure. The primary purpose of decompressive craniectomy is to reduce intracranial pressure and alleviate compression on brain tissue. It is typically used in patients with brain tissue swelling or refractory intracranial hypertension. Cranioplasty is a surgical procedure performed to repair cranial defects. It not only restores the normal anatomic structure of the skull and improves the appearance of the patient's head but also hel

Functional outcome after late cranioplasty after decompressive craniectomy: a single-center retrospective study.

OBJECTIVE: The best time for cranioplasty (CP) after decompressive craniectomy (DC) is controversial, and there are no authoritative guidelines yet. Both complications as well as outcome may depend on the timing of CP. The aim of this single-center study was to evaluate the impact of late CP on procedural safety as well as on patient outcome. METHODS: All patients receiving CP at a tertiary university medical center between 01/2015 and 12/2022 were included retrospectively. Patients' conditions were assessed according to the modified Rankin Scale (mRS) prior to CP and 6 months after. Baseline

Posttraumatic hydrocephalus as a complication of decompressive craniectomy-same old story, new perspectives.

OBJECTIVE: Decompressive craniectomy (DC) serves as a vital life-saving intervention, demonstrating efficacy in reducing intracranial pressure (ICP). However, its efficacy hinges on meticulous surgical execution, perioperative management, and vigilance toward potential complications. The incidence of complications associated with DC plays a pivotal role in determining its superiority over medical management for patients experiencing intracranial hypertension following traumatic brain injury (TBI). METHODS: Severe cases often require more intensive therapy, prolonged mechanical ventilation, an

Optimizing Traumatic Brain Injury Recovery: Exploring the Advantages and Results of Decompressive Craniectomy.

Traumatic brain injury (TBI) causes significant mortality and morbidity across regions, imposing a substantial socioeconomic burden on societies. A major complication that can arise is uncontrolled intracranial pressure (ICP). Several strategies exist for reducing ICP in TBI patients, including head elevation, mannitol administration, and hyperventilation. Decompressive craniectomy (DC) is a therapeutic approach employed to lower ICP. This technique offers immediate and permanent relief from elevated ICP, although there are ongoing debates regarding its beneficial use and appropriate indicatio

Factors associated with post traumatic hydrocephalus following decompressive craniectomy: A single-center experience.

BACKGROUND: A decompressive craniectomy (DC) is a surgical procedure sometimes utilized to manage refractory intracranial hypertension following severe traumatic brain injury (sTBI). The previous studies have established a relationship between DC and post traumatic hydrocephalus (PTH). This study aimed to identify the factors responsible for developing shunt-amenable PTH in patients who underwent DC following sTBI. METHODS: A review of a prospectively collected database of all patients admitted with severe TBI in a tertiary neurosurgical center in North-west England between January 2012 and M

Impact of Decompressive Craniectomy on Hemorrhagic Transformation in Malignant Ischemic Stroke in Mice.

BACKGROUND: Endovascular thrombectomy has changed the management of ischemic stroke. The reperfusion can however lead to a hemorrhagic transformation (HT). Decompressive craniectomy (DC) is a surgical procedure used for malignant ischemic stroke. However, its efficacy was demonstrated before the era of endovascular thrombectomy trials. Here, we hypothesized that DC for ischemic stroke after thrombectomy could lead to a higher risk of HT. We thus evaluated this hypothesis in a mouse model of stroke induced by occlusion of the middle cerebral artery (MCAO) with or without mechanical reperfusion.

Risk Factors for Intraoperative Hypotension During Decompressive Craniectomy in traumatic Brain Injury Patients.

BACKGROUND: Decompressive craniectomy (DC) is an important therapy for treating intracranial pressure elevation following traumatic brain injury (TBI). During this procedure, about one-third of patients become complicated with intraoperative hypotension (IH), which is associated with abruptly decreasing sympathetic activity resulting from brain decompression. This study aimed to identify factors associated with IH during DC procedures and the mortality rate in these patients. METHODS: The records of adult TBI patients aged 18 years and older who underwent DC at Songklanagarind Hospital betwee

A red flag for diagnosing brain death: decompressive craniectomy of the posterior fossa.

PURPOSE: Brain death/death by neurologic criteria (BD/DNC) may be determined in many countries by a clinical examination that shows coma, brainstem areflexia, and apnea, provided the conditions causing reversible loss of brain function are excluded a priori. To date, accounts of recovery from BD/DNC in adults have been limited to noncompliance with guidelines. CLINICAL FEATURES: We report the case of a 72-yr-old man with a combined primary infratentorial (hemorrhagic) and secondary global (anoxic) brain lesion in whom decompressive craniectomy of the posterior fossa and six-hour therapeutic h

Early space-occupying cerebellar oedema requiring decompressive craniectomy following a clinically minor stroke.

We describe a patient presented with clinically a small cerebellar ischaemic stroke but required emergency decompression within 24 hours of symptoms onset after incidental finding of severe mass effect on imaging without any change in her mild clinical symptoms. Her initial multimodal acute stroke imaging, non-contrast CT of the brain and CT angiography from aortic arch to vertex were normal. CT perfusion showed a very small deficit only. The malignant mass effect was picked on an MRI scan performed routinely as part of a clinical trial, 32 hours after stroke. Our case highlights stroke evolut
